Abstract

Description
本発明は粘膜防御を誘導する新規のワクチン組成物、特に、経口ワクチンおよび感染に対する粘膜耐性の向上または確立した感染症の治療のための方法に関する。
気管支炎(endobronchitis)が粘膜免疫のモデルとして、特に粘膜表面におけるホスト/寄生体相関の特定のバランスを反映させるために使用されてきた。通常は無菌の気管支粘膜に、一般的には浸潤しない“無毒性の”細菌のコロニーが形成される。最も知られているのは型別不能型インフルエンザ菌(non-typical Haemophilus influenza: NTHI)である。従って慢性肺疾患を有する被験体における感染の急性発作は、コロニーを形成している細菌と気管支粘膜間の微妙に均衡のとれた関係を妨害する事象によって開始されるらしい。ホストの抑制応答はTh1 T細胞(γインターフェロンを生成する)に関係し、これは気管支粘膜内で好中球を漸増および活性化させることによって作用する。過剰な、および/または不適当な場合、この過程は咳および膿性痰の増加(確立した慢性肺疾患を有する被験体における“急性気管支炎”の特徴)となる。
発明の概要
第1の観点によれば本発明は粘膜投与可能な組成物を提供し、該組成物は粘膜表面で感染を起こす能力のある微生物の少なくとも1つを由来とする1つ以上の抗原およびTh1細胞免疫応答を誘導する能力のあるアジュバントを含有し、ここで該アジュバントは粘膜表面において感染を起こす能力を有する微生物由来ではない。
好ましくは本発明の組成物に使用されるアジュバントは、粘膜表面で感染を起こす能力を有する生物ではなく、そしてTh1タイプの細胞免疫応答を誘導することのできる微生物またはその一部である。アジュバントが細菌、例えば、限定される訳ではないが乳酸菌、ミコバクテリウム種、またはビフィドバクテリウム種から選択できるものであるのも好ましい。好酸性乳酸桿菌(L. acidphilus)、発酵乳酸桿菌(L. fermentum)、もしくはミコバクテリウム・バッカエ(M. vaccae)、またはそれらの一部でTh1細胞応答を誘導する能力があるものは更に好ましい。L. acidphilusは特に好ましい。L. acidphilus、L. fermentum、またはM. vaccaeは、Th1応答を誘導する能力を有する限り、生きたもの、または不活性化調製品として使用してもよい。好ましくはL. acidphilusおよびL. fermentumは生きた調製品として使用する。考えられるように、他の細菌も(それらが共生生物作用を有していてもいなくても)アジュバントとして好適でありえ、それらは例えば周知のアジュバント細菌、例えばラクトバシラス・カゼイ(L.casei)、ラクトバシラス・プランタルム(L. plantarum)、ラクトバシラス・ラムノサス(L. rhamnosus)、ビフィドバクテリウム・ブレーベ(Bifidobacterium breve)などである。共生生物細菌をアジュバントとして使用するのは好ましい。
第3の観点によれば、本発明は粘膜感染の治療または予防処置の方法を提供し、該方法は、それらの処置を必要とする被験体に第1の観点にかかる組成物または第2の観点にかかるワクチンを投与することを含む。
本発明の組成物およびワクチンを便宜に使用して感染に対する粘膜防御を誘導するが、存在する細菌、真菌、および/またはウィルスによる粘膜感染の処置における予防薬として使用することもできる。更に、本発明の組成物およびワクチンの好ましい使用は、粘膜表面に既に細菌がコロニーを形成している場合である。
本発明の組成物およびワクチンはいずれの粘膜表面に投与してもよく、粘膜免疫系が共通であるために所望の効果を有する。好ましい粘膜表面は上記のように口腔前庭、呼吸器管、および腸管であるが、当業者に理解されるようにいずれの粘膜表面への投与も有効である。
好ましくはワクチンは2つの過程で投与し、後に追加免疫過程を行う。
アジュバントの好ましい投与量は、アジュバントが完全体の生きた共生生物細菌の場合、約1x108から約1x1012生物体である。
好ましい実施態様の説明
例えば粘膜感染に関係する特異的抗原(例えばインフルエンザ菌のような完全体の細菌)と非特異的細菌(すなわち通常、粘膜感染と関係ないもの)とを組み合わせることによって、粘膜感染に対する防御を延長できるという驚くべき発見がなされた。特定の作用機構に拘束されることは望まないが、本発明は、使用される非特異的細菌によってT細胞による特定のサイトカイン応答の生成が誘導され、これによって伝統的なワクチン製剤の投与から予期されるのとは異なる防御が増幅されるという考えに基づいている。更に詳細に言えば、(しかしここでも特定の作用機構に拘束されることは望まないが)非特異的細菌の投与によって粘膜免疫系がT細胞応答スペクトルのTh1(すなわちIFN-γ)端に偏ると考えられる。Th1応答は粘膜のコロニー形成を最もよく制御すると考えられる。それらの生物体、およびTh1応答を刺激する能力のある他のアジュバントの使用によって、検出可能であって投与部位から離れた所で効果を有することができるTh1応答が測定される。
グラム陽性菌を共生生物アジュバントとしてグラム陰性菌と組み合わせることはこの系において相乗活性を有し、これもここで企図される。粘膜免疫系は全ての粘膜表面に共通であるため、種々の呼吸器粘膜表面が防御できる(例えば特に気管支、洞(sinus)、および中耳)。
実施例1:Th1/Th2サイトカイン応答に対する共生生物細菌の影響
共生生物細菌がTh2サイトカイン応答を抑制的に調節し、Th1サイトカイン応答を亢進的に調節するかどうかを確認するために、栄養補給針(feeding needle)を使用してC57/B16マウスに種々の数の好酸性乳酸桿菌(University of New South Wales, School of Microbiology and Immunology Culture Collection, Sydney, Australiaから入手)を2週間の間毎日、胃内投与し、その後、感作するために8μgのオボアルブミン(OVA)および水酸化アルミニウムを0.2mLのリン酸緩衝液に混合したものを腹膜内注射によって投与した。マウスに2週間の間2日毎に好酸性乳酸桿菌を更に10回投与した後、殺害した。ふるいを通して脾臓をティージング(teasing)してリンパ球を単離し、PBSで洗浄し、10x106で再懸濁した。細胞懸濁液の1mLアリコートを24ウェル平底マイクロタイタープレートのウェルに注入し、OVA(5μg/mL)で刺激した。4日間インキュベートした後、上清を回収し、標準的なELISA技術でIL-4またはIFN-γモノクローナル抗体ペアを使用してIL-4およびIFN-γの生成についてアッセイを行った。
実施例2:生きた好酸性乳酸桿菌および死滅させたNTHiの1回の管腔内投与による呼吸器管からのNTHiのクリアランスの向上
型別不能型インフルエンザ菌(NTHI)の呼吸器管からのクリアランスを向上する好酸性乳酸桿菌の能力をラットモデルで検討した。

このデータは、死滅NTHiと生きた好酸性乳酸桿菌の組み合わせがNTHi単独または好酸性乳酸桿菌単独より有効性が高いことを示している。
実施例3:好酸性乳酸桿菌を投与した免疫化ラットの肺からのNTHiのクリアランスの向上
DAラット(200-250グラム、8-10週齢、Animal Resouece Centre, Perth, WA)に、1.0mLのPBSに混合した5x1010の好酸性乳酸桿菌またはPBSのみを2日毎に7日間強制栄養によって投与し、この時点でホルマリンで死滅させたNTHi(5x109/ラット)を0.5mLのPBSに混合して管腔内(IL)投与してラットを免疫化した。ラットに2日毎に2週間投与を継続し、その後ホルマリンで死滅させたNTHi(5x108/ラット)を含有するPBS 50μLを気管内(IT)経路で追加免疫投与した。好酸性乳酸桿菌を更に7日間投与した後、ラットを5x108の生きたNTHiを含有するPBS 50μLでIT感染させた。4時間後、肺におけるコロニー形成のレベルを、実施例2に記載するようにBALおよびLHで測定した。種々の群の免疫化および給餌を表3に示す。細菌の回収を表4に示す。

好酸性乳酸桿菌を投与して死滅NTHiで免疫化したラットは、死滅NTHiで免疫化しただけのラットまたは好酸性乳酸桿菌を投与しただけのラットより、肺におけるNTHiでの感染に対して耐性が高かった。更に、好酸性乳酸桿菌を強制栄養で反復投与したラットは好酸性乳酸桿菌の1回の大量投与を行ったラット(実施例2)より感染に対する耐性が高かった。特定の作用機構に拘束されることは望まないが、このデータはクリアランスの向上が好酸性乳酸桿菌の反復投与後の腸におけるコロニー形成の増加によるものでありうることを示唆している。
実施例4:死滅NTHiおよび生きた発酵乳酸桿菌の管腔内投与によって、その後の急性NTHi感染に対する防御が向上する
NTHiのクリアランスを向上する発酵乳酸桿菌(単独で、または死滅NTHiと組み合わせて)の能力を、急性NTHi呼吸感染のラットモデルにおいて評価した。
また、LHでもB>D(p=0.041)およびC>D(p=0.015)。
このデータは、発酵乳酸桿菌は単独で、または死滅NTHiと組み合わせると、死滅NTHi単独の場合よりもその後の急性呼吸器感染に対する予防薬として高い有効性を有することを示唆している。従ってこの乳酸桿菌株も急性呼吸器感染に対して有効である。
実施例5:一定用量のNTHiと共に投与する好酸性乳酸桿菌の用量範囲の研究
特異的病原体非保有DAラット(197-230g)をCentral Animal house, University of Newcastle, NSWから入手した。6ラットの群にPBS、または一定用量の死滅NTHi(5x109)プラス種々の投与量の生きた好酸性乳酸桿菌の1回IL投与を行った(以下の表7に示す通り)。14日目にA群のラットには50μLのPBSで偽性追加免疫を行い、B-D群のラットには5x108の死滅NTHiを含有するPBS 50μLで追加免疫を行った。21日目、50μLのPBSに混合した5x108の生きたNTHiの気管内点滴注入でラットを感染させた。4時間後、ペントバルビトンの腹膜内過剰投与によってラットを殺害した。肺を10mLのPBSで洗浄して気管支肺胞洗浄液(BAL)を得た。次いで肺を10mLのPBS中でホモジナイズし、肺ホモジネート(LH)を得た。BALおよびLH中の細菌数を測定するために、BALおよびLHの連続希釈を行い、チョコレート寒天培地に既知の容量を播種した。37℃で一晩インキュベートした後、コロニーを計数し、BALおよびLHのコロニー形成単位(CFU)の総数を測定した。各群の細菌数を表8に示す。
乳酸桿菌の低濃度投与は、高濃度投与より有効性が高い。ここでも、特定の作用機構に拘束されることは望まないが、このデータは‘アジュバント効果’が乳酸桿菌のみでの効果とは異なって作用しうることを示唆している。
実施例6:NTHi免疫化の最適な投与量/投与計画の評価
死滅NTHi免疫化の最適な投与量および投与計画を決定した。評価を行った種々の投与計画を表9に示す。IL1回投与、IL1回投与後に強制栄養による投与、そしてIL1回投与後に2回の強制栄養による投与について評価した。十二指腸を暴露するための手術を伴うため、IL投与は1回しか行えなかった。動物倫理学的理由から、外科手術的介入は1回しか許されない。従ってその後の投与は強制栄養によって行った。
(i)投与計画1
特定病原菌非保有DAラット(187-213g)をCentral Animal house, University of Newcastle, NSWから入手した。ラット(6/群)に表10に示すような種々の投与量の死滅NTHiのIL1回投与を、表9の投与計画1によって行った。死滅NTHiを0.3mLのPBSに含有させた。IT追加免疫をPBS(A群)または2x107の死滅NTHi(B-D群)で行った。ラットに5x108の生きたNTHiを含有するPBS 50μLを気管内感染させた。BALおよびLHから回収した細菌を表11に示す。

1回の免疫化IL投与ではいずれの高投与量レベル(3x109および3x108)も防御免疫の提供において同等に有効であった。最も低い投与量(3x107)は有効ではなかった。
(ii)投与計画2
特定病原菌非保有DAラット(187-219g)をCentral Animal house, University of Newcastle, NSWから入手した。ラット(6/群)に表12に示すような種々の投与量の死滅NTHiのIL1回投与を、表9の投与計画2によって行った。死滅NTHiは0.3mLをPBSに含有させた。IT追加免疫をPBS(A群)または50μLのPBSに混合した2x107の死滅NTHi(B-D群)で行った。ラットに5x108の生きたNTHiを含有するPBS 50μLを気管内感染させた。BALおよびLHから回収した細菌を表13に示す。
2回投与を行うと3つの投与量の全てで、LHにおける防御の程度が同じであることは明らかである。防御のレベルはBALにおける3つの投与量でも同等であったが、この実験では一番高い濃度が統計学的に有意なだけである。
(iii)投与計画3
特定病原菌非保有DAラット(176-213g)をCentral Animal house, University of Newcastle, NSWから入手した。ラット(6/群)に表14に示すような種々の投与量の死滅NTHiのIL1回投与を、表9の投与計画3によって行った。死滅NTHiを0.3mLのPBSに含有させた。IT追加免疫を50μLのPBS(A群)または50μLのPBSに混合した2x107の死滅NTHi(B-D群)で行った。ラットに5x108の生きたNTHiを含有するPBS 50μLを気管内感染させた。BALおよびLHから回収した細菌を表15に示す。
3回の腸投与(IL1回プラス強制栄養2回)では、3つの投与レベルの全てで同様の防御が得られる。2回投与(投与計画2)に優る3回投与の利点は明らかではない。
実施例7:NTHiおよび好酸性乳酸桿菌を用いるIL1回投与での長期免疫化
好酸性乳酸桿菌の添加によって提供される防御の持続期間の向上を測定するために実験を行った。特定病原菌非保有DAラット(201-293g)をCentral Animal house, University of Newcastle, NSWから入手した。6ラットの群に、表16に示すように0.75mLのPBS、または5x109の死滅NTHIもしくは5x109の死滅NTHIおよび2.5x1010の生きた好酸性乳酸桿菌の混合物を含有するPBSをIL投与した。14日後、表16に示すようにラットに50μLのPBS、または5x108の死滅NTHIを含有するPBSでIT追加免疫を行った。IL投与の3ヶ月後、ラットを50μLのPBSに混合した5x108の生きたNTHiで気管内(IT)感染させた。感染の4時間後、ラットを殺害し、BALおよびLHを調製して総細菌数を測定したが、測定は上記の実施例に記載したように行い、コロニー形成単位(CFU)で表した。

この3ヶ月の実験期間にわたって、NTHiのみでの免疫化が最も有効であった。この期間にわたって更なる好酸性乳酸桿菌投与の効果は明らかではない。
(1)粘膜表面で感染を起こす能力のある微生物の少なくとも1つを由来とする1つ以上の抗原およびTh1細胞免疫応答を誘導するする能力のあるアジュバントを含有する粘膜投与可能な組成物であり、アジュバントが粘膜表面で感染を起こす能力のある微生物由来ではない上記組成物。
(2)抗原が細菌、カビ、またはウィルス由来である、(1)記載の組成物。
(3)抗原が完全体の(whole)微生物である、(1)または(2)記載の組成物。
(4)完全体の微生物が死滅した微生物である、(3)記載の組成物。
(5)生きた微生物が生きた微生物または弱毒化した生きた微生物である、(4)記載の組成物。
(6)抗原が1つ以上の微生物のホモジネートまたは音波処理物である、(1)または(2)記載の組成物。
(7)微生物が呼吸器管細菌性および/もしくは真菌性病原体、または通常呼吸器管にコロニーを形成し、感染を起こす可能性のある微生物である、(1)から(6)のいずれか1つに記載される組成物。
(8)微生物が型別不能型インフルエンザ菌(Heamophilus influenzae, NTHI)、緑膿菌(Pseudomonas aeruginosa)、肺炎球菌(Streptococcus pneumoniae)、白色ブドウ球菌(Staphylococcus albus)、および黄色ブドウ球菌(Staphylococcus aureus)、またはそれらのいずれかの組み合わせからなる群から選択される、(7)記載の組成物。
(9)アジュバントが微生物またはその一部であり、粘膜表面で感染を起こす能力のある生物ではなく、そしてTh1タイプの細胞免疫応答を誘導することができる、(1)から(8)のいずれか1つに記載される組成物。
(10)アジュバントが細菌である、(9)記載の組成物。
(11)細菌が乳酸菌、ミコバクテリウム種、およびビフィドバクテリア、またはそれらの組み合わせからなる群から選択される、(10)記載の組成物。
(12)細菌が好酸性乳酸桿菌(Lactobacillus acidphilus)、発酵乳酸桿菌(Lactobacillus fermentum)、およびミコバクテリウム・バッカエ(Mycobacterium vaccae)、またはそれらの一部でTh1細胞応答を誘導する能力があるものからなる群から選択される、(11)記載の組成物。
(13)細菌が好酸性乳酸桿菌である、(12)記載の組成物。
(14)細菌が生きている、(9)から(13)のいずれか1つに記載される組成物。
(15)組成物が経口投与可能な組成物である、(1)から(14)のいずれか1つに記載される組成物。
(16)1つ以上の薬剤的に許容されるキャリアー、アジュバント、溶媒、または添加剤を更に含有する、(15)記載の組成物。
(17)食品または食品サプリメントである、(1)から(15)のいずれか1つに記載される組成物。
(18)(1)から(15)のいずれか1つに記載される組成物を含有するワクチン。
(19)粘膜感染の治療または予防処置のための方法であり、それらの処置を必要とする被験体に(1)から(16)のいずれか1つに記載される組成物、または(18)記載のワクチンを投与することを含む上記方法。
(20)粘膜感染の治療または予防処置のための方法であり、それらの処置を必要とする被験体に(1)から(16)のいずれか1つに記載される組成物のアジュバント部分、次いで(1)から(16)のいずれか1つに記載される組成物の抗原部分を投与することを含む上記方法。
(21)アジュバント部分での処置がアジュバントの1回の大量注射による投与によるものである、(20)記載の方法。
(22)アジュバント部分での処置がアジュバントの反復投与によるものである、(20)記載の方法。
(23)抗原部分の投与後にアジュバントの投与を継続する、(20)または(22)記載の方法。
(24)化合物またはワクチンを粘膜表面に投与する、(19)から(23)のいずれか1つに記載される方法。
(25)粘膜表面が口腔前庭(bucal cavity)、呼吸器管、および腸管からなる群から選択される、(24)記載の方法。
(26)組成物またはワクチンを経口投与する、(19)から(25)のいずれか1つに記載される方法。
(27)ワクチンを2つの過程で投与し、次いで追加免疫過程を行う、(19)から(26)のいずれか1つに記載される方法。
(28)アジュバントが細菌であり、投与する細菌の量が約1x108から約1x1012生物である、(19)から(27)のいずれか1つに記載される方法。
(29)抗原が完全体の死滅した微生物であり、微生物の量が約1x108から約1x1012生物である、(19)から(28)のいずれか1つに記載される方法。
(30)完全体の死滅微生物とアジュバント細菌の比率が約5:1以上である、(28)または(29)記載の方法。
Claims (8)
- 以下のもの:
(1)Th1タイプの細胞免疫応答を誘導するする能力のある少なくとも1つの種の共生生物細菌の反復投与量であって、前記共生生物細菌は好酸性乳酸桿菌(Lactobacillus acidphilus)および発酵乳酸桿菌(Lactobacillus fermentum)、またはそれらの組み合わせからなる群から選択される;および
(2)粘膜表面で感染を起こす能力のある微生物の少なくとも1つを由来とする少なくとも1つの抗原であって、前記微生物は型別不能型インフルエンザ菌(Heamophilus influenzae, NTHI)、緑膿菌(Pseudomonas aeruginosa)、肺炎球菌(Streptococcus pneumoniae)、白色ブドウ球菌(Staphylococcus albus)、および黄色ブドウ球菌(Staphylococcus aureus)、またはそれらのいずれかの組み合わせからなる群から選択される、
を含む呼吸器官感染を治療するための経口投与可能な組成物であって、ここで、少なくとも1つの種の共生生物細菌の反復投与量を少なくとも1つの微生物の少なくとも1つの抗原の前に経口投与する、上記組成物。 - 抗原が、粘膜表面で感染を起こす能力のある完全体の(whole)微生物である、請求項1記載の組成物。
- 完全体の微生物が死滅した微生物である、請求項2記載の組成物。
- 完全体の微生物が生きた微生物または弱毒化した生きた微生物である、請求項2記載の組成物。
- 抗原が微生物のホモジネートまたは音波処理物である、請求項2記載の組成物。
- 共生生物細菌が好酸性乳酸桿菌である、請求項1記載の組成物。
- 共生生物細菌が生きている、請求項1記載の組成物。
- 1つ以上の薬剤的に許容されるキャリアー、アジュバント、溶媒、または添加剤を更に含有する、請求項1記載の組成物。
