Description
本開示は、外科用ステープル器具によって使用する、細長いバットレス（ｂｕｔｔｒｅｓｓ）材料を組み込むアンビルアセンブリに関する。特に、本開示は、ステープル留めの後に、外科用ステープル器具からバットレス材料を解放するバットレス解放機構に関する。
様々な外科的処置の間に、組織の２つの側を結合することが、しばしば必要となる。このことは、一般に、組織の２つの縁を互いに同じ高さに接合し、ステープル留め、または縫合などにより該２つの縁を固定することによって遂行される。場合によっては、組織を接続するステープル線または縫合線は、特に、病的な組織および相対的に弱い組織を断裂させるか、または組織を通って引き抜ける傾向があり得る。さらに、治癒する間に、漏出がステープル線または縫合線を通って起こり得る。
これらの問題を緩和するために、「バットレス材料」と呼ばれる細長い材料が、ステープル留めまたは縫合の前に組織に対して位置決めされる。バットレス材料は、ステープル線または縫合線を強化する傾向があり、かつ治癒の前の漏出を防止する傾向がある。

（概要）

外科用ステープル器具１２は、概して、本体部分１４から遠位に延びる細長い管状部材１６を有するピストルグリップの本体部分１４を含んでいる。ステープルカートリッジ１８は、細長い管状部材１６の遠位の端部２０に取り付けられる。アンビルアセンブリ１０は、細長い管状部材１６の遠位の端部２０で移動可能に取り付けられ、ステープルカートリッジ１８から離れて距離をおかれた開放位置と、アンビルアセンブリ１０がステープルカートリッジ１８と協働的な整列の閉鎖状態にある閉鎖位置との間で移動可能である。

外科用ステープル器具１２は、ステープルの顎部閉鎖と射出との両方を遂行する単一のトリガ２２によって示されているが、本バットレス解放機構はまた、射出機構と区別される、顎部を閉鎖するためのクランプ機構を利用するタイプの外科用ステープル器具によって用いられ得ることがさらに企図されている。例えば、米国特許第５，４７６，２０６号を参照されたい。該特許の内容は、参考として特に本明細書中に援用される。

アンビルカバー７８は、長手に延びるサイドカット９０および９２を含み、サイドカット９０および９２は、アンビルカバー７８をアンビル部材２６に固定するために、アンビル部材２６内に形成された側壁９４および９６と摩擦嵌めの様態で係合するように構成される。あるいは、アンビルカバーは、溶接、接着材または他の手段を用いて付着され得る。取り付け構造９８が、近位の端部８８に提供されることにより、アンビルアセンブリ１０を細長い管状部材１６の遠位の端部２０（図１）に付着させることを容易にする。
シンチトラックは概して、１つ以上のトラックを規定する本体部分を含んでいる。ここで図３および図３ａを参照すると、シンチトラック５０は、第一のトラック１０２および第二のトラック１０４を有する本体部分１００を有している。第一のトラック１０２および第二のトラック１０４が、縫合具３２の第一の側５４および第二の側５６をそれぞれ挟む（ｐｉｎｃｈ）かまたは締める（ｃｉｎｃｈ）ための寸法にされることにより、縫合具３２をシンチトラック５０に固定する。本体部分１００はさらに、以下の本明細書中でさらに詳細に論じられる事項である、解放部材６０を受け取る中央スロット１０６を含んでいる。

ここで図２に戻って参照すると、本明細書中で上記されたように、解放部材６０が、シンチトラック５０を第一の縦の位置と第二の縦の位置との間で移動させるために提供される。解放部材６０が、シンチトラック５０の中央スロット１０６内を通るように構成された遠位のくさび１２２を含むことにより、シンチトラック５０を第一の縦の位置と第二の縦の位置との間で移動させる。傾斜した面１２４が、遠位のくさび１２２から近位に延びている。遠位のくさび１２２の側１２６および１２８が、遠位のくさび１２２をシンチトラック５０に対して案内することを容易にする。図３ａの実施形態において、シンチトラック５０の底の表面は、概して水平である。しかしながら、他の実施形態において、カム表面などの１つ以上の表面が、傾斜した面１２４と協働するように形成され得、傾斜した面１２４の形状に対応し得る。
ここで図３を参照すると、解放部材６０は、遠位のくさび１２２から近位に延びるガイドバー１３０をさらに含んでいる。ガイドバー１３０は、中央部分１３２と、近位に延びるフランジ付きの部分１３４とを含んでいる。フランジ付きの部分１３４は、解放バー６０をアンビル部材２６の側壁９４および９６内に整列させることに役立つ。示されたように、ガイドリブ１３６が、ガイドバー１３０から下方に延び、そしてナイフスロット２８内を通るように構成され、その結果として、解放部材６０が、シンチトラック５０に対して長手の方向または水平の方向に移動可能である。

ここで図２〜図７を参照するが、最初に図２について参照すると、アンビル部材２６への細長いバットレス材料３０の組み立てがここで記載される。細長いバットレス材料３０の近位の端部３８が、アンビル部材２６の近位の端部６４に固定される。具体的には、近位の端部３８が、アンビル部材２６の下側１４０に対して位置決めされる。近位の縫合具３４は、第一の側７２および第二の側７４の第一の自由端１４２および第二の自由端１４４が切り抜き４４および４６を通過して、近位の縫合具３４の後部スパン７６を細長いバットレス材料３０の近位の端部３８との係合に導くように操作される。

図２〜図７を参照すると、アンビル部材２６への細長いバットレス材料３０の遠位の端部３６の組み立てがここで記載される。最初に図３および図３ａを参照すると、解放部材６０は、第一の位置すなわち最も近位の位置にある。シンチトラック５０は、シンチトラック５０の本体部分１００に形成されたスロット１０６内の、解放部材６０のくさび１２２の通行および係合に起因して、第一の位置すなわち縦に最も高い位置にある。
図２を参照すると、細長いバットレス材料３０の遠位の端部３６が、アンビル部材２６の下側１４０に対して同じ高さに位置決めされる。遠位の縫合具３２は、第一の側５４および第二の側５６の自由端１４６および１４８が、細長いバットレス材料３０の遠位の端部３６に形成された切り抜き４０および４２をそれぞれ通過するように操作される。示されたように、アンビル部材２６の遠位の端部５２は、離れて距離をおかれた一対のホール１５０および１５２を提供される。遠位の縫合具３２の自由端１４６および１４８が、ホール１５０および１５２を通過し、それによって、第一の側５４および第二の側５６が、シンチトラック５０の第一のトラック１０２および第二のトラック１０４のそばに整列される。縦の張力が、自由端１４６および１４８に付与されることにより、細長いバットレス材料３０の遠位の端部３６をアンビル部材２６の下側１４０に対して固定する。

ここで図１および図６〜図１０を参照すると、アンビルアセンブリ１０の使用がここで記載される。図６および図７を参照すると、本明細書中で上記されたように、最初の位置において、解放バー６０は、アンビル部材２６に対してシンチトラック５０を第一の位置すなわち縦に最も高い位置に維持する、第一の位置すなわち最も近位の位置にあり、それによって、遠位の縫合具３２の後部スパン５８の張力を細長いバットレス材料３０の遠位の端部３６に対して維持する。
図１を参照すると、使用において、外科用ステープル器具１２は、アンビルアセンブリ１０とステープルカートリッジ１８とが、ステープル留めされる組織（図示されない）の近くに位置決めされるように操作される。一旦外科用ステープル器具１２が適切に位置決めされると、トリガ２２が動かされることにより、アンビルアセンブリ１０を組織の近くでステープルカートリッジ１８に対して閉鎖位置に移動させる。具体的には示されないが、一方でアンビルポケットが、アンビル部材２６上に提供されることにより、対象の組織を通って、かつ細長いバットレス材料３０を通ってステープルカートリッジ１８から射出されたステープルを曲げて留める。
ここで図２に戻って参照すると、本明細書中で上記されたように、近位の縫合具３４の後部スパン７６が、ナイフスロット２８を横切って延びる。ここで図８を参照すると、ナイフバー１５４が、ステープルカートリッジ１８とアンビルアセンブリ１０とによって組織内に形成されたステープル線を切り開くために、外科用ステープル器具１２と関連づけられている。ナイフバー１５４がナイフスロット２８を遠位に通過するので、ナイフバー１５４のブレード１５６が、細長いバットレス材料３０の近位の端部３８を切り開き、近位の縫合具３４の後部スパン７６を切断する。ナイフバー１５４がスロット２８を通って遠位に前進し続けることが、ブレード１５６に細長いバットレス材料３０を切り開き続けさせる。
図８を引き続き参照すると、ナイフバー１５４がアンビル部材２６を通って遠位に前進するので、ナイフバー１５４の遠位の上部エッジ１５８が、解放部材６０の近位のエッジ１６０に接近する。図９に最も良く示されるように、ナイフバー１５４の遠位のエッジ１５８が遠位に移動し、解放部材６０の近位のエッジ１６０と係合するときに、ナイフバー１５４が解放部材６０を遠位に駆動し始め、その結果として、解放部材６０の遠位のくさび１２２が、シンチトラック５０に形成されたスロット１０６（図２および図３）を通って遠位に駆動される。
図１０を具体的に参照すると、解放部材６０がナイフバー５４によって第二の位置すなわち最も遠位の位置に駆動されるときに、遠位のくさび１２２の傾斜した面１２４がスロット１０６を通り越し、シンチトラック５０が、アンビル部材２６に対して第二の位置すなわち縦に最も低い位置まで降りることを可能にする。示されたように、シンチトラック５０が第二の位置すなわち縦に最も低い位置にあるときには、張力が、遠位の縫合具３２において解放され、その結果として、遠位の縫合具３２の後部スパン５８が、アンビル部材２６の下側１４０の下でかなりの距離を降りる。このことが、開口またはギャップ１６２を遠位の縫合具３２の後部スパン５８とアンビル部材２６の下側１４０との間に作り出す。
一旦ナイフバー１５４が最も遠位の位置に到達すると、トリガ２２への圧力が解放され得、アンビル部材１０が、ステープルカートリッジ１８に対して開放位置に移動することを可能にする（図１）。アンビルアセンブリ１０が開放位置に移動されるときに、組織をステープル留めされている、細長いバットレス材料３０の遠位の端部３６が、ギャップ１６２を通って自由に引き抜かれ、遠位の端部３６が、アンビル部材２６から離れることを可能にする。本明細書中で上記されたように、そして図２に示されたように、せん孔線４８が、細長いバットレス材料３０の遠位の端部３６で形成されており、細長いバットレス材料３０を通ってナイフブレード１５６によって形成された切断線に概ね沿って、細長いバットレス材料３０が、半分に分離することを可能にする。この態様において、アンビルアセンブリ１０は、アンビル部材２６への細長いバットレス材料３０の単純かつ簡単な組み立てを可能にし、さらに重要なことには、細長いバットレス材料３０の遠位の端部３６の、アンビル部材２６からの簡単な解放を可能にする。
様々な修正が本明細書中に開示された実施形態になされ得ることは理解されるであろう。例えば、解放部材が、近位に引き抜かれるように配置され得ることにより、シンチトラックの位置を変更し、そして縫合具とバットレスとを解放する。他の実施形態において、近位に引き抜かれる解放部材または以上で論じられた解放部材６０が、外科用器具のユーザによる係合用の、外科用器具のハンドル上の別個のアクチュエータに接続され得る。別の実施形態において、開示されたシンチトラックが、より多くのトラックまたはより少ないトラックを有し得ることにより、縫合具材料の、より長い巻きつけかまたはより短い巻きつけに適応する。さらに、縫合具トラックと解放バーとを含む開示された張力付与機構が、ステープルカートリッジ内に組み込まれ、ステープルカートリッジ内の駆動バーによって機能するように適合され得る。さらに、細長いバットレス材料を維持する縫合具における張力を解放する開示された方法および構造は、例えば、円形のステープラーなどのような外科用ステープラーの他の形態における使用を見出し得る。さらなる実施形態において、解放部材が、シンチトラックを縦に押し下げることにより、縫合具における張力を解放し、バットレスを解放する。従って、上記の説明は、限定としてではなく、単に特定の実施形態の例示として解釈されるべきである。当業者は、添付の特許請求の範囲の精神および範囲の内において他の修正を想起し得る。

Claims (16)
- アンビルアセンブリであって、
第一の側および第二の側を有するアンビル部材と、
該アンビル部材の該第一の側から該アンビル部材の該第二の側に通っている縫合具材料と、
該アンビル部材の該第一の側で移動可能に取り付けられ、かつ該縫合具と係合可能なシンチトラックであって、該シンチトラックは、該縫合具に張力をかける第一の縦の位置と該縫合具における張力を解放する第二の縦の位置との間で移動可能である、シンチトラックと、
解放部材であって、該アンビル部材の該第一の側で移動可能に取り付けられ、該シンチトラックを該第二の縦の位置に移動させるために、該シンチトラックに対して移動可能である、解放部材と
を備える、アンビルアセンブリ。 - 前記アンビル部材の前記第二の側に隣接して位置決めされた細長いバットレス材料をさらに備える、請求項１に記載のアンビルアセンブリ。
- 前記縫合具は、後部スパンと、該後部スパンから延びる第一の縫合具の側および第二の縫合具の側とを含み、該後部スパンは、前記シンチトラックが前記第一の縦の位置にあるときに、前記細長いバットレス材料を前記アンビル部材の前記第二の側に隣接して維持するために、該細長いバットレス材料と係合可能である、請求項２に記載のアンビルアセンブリ。
- 前記シンチトラックは、第一のトラックを含み、前記第一の縫合具の側は、該第一のトラックにおいて固定される、請求項３に記載のアンビルアセンブリ。
- 前記シンチトラックは、第二のトラックを含み、前記第二の縫合具の側は、該第二のトラックにおいて固定される、請求項４に記載のアンビルアセンブリ。
- 前記解放部材は、くさびを含み、前記シンチトラックは、スロットを含み、それによって、該シンチトラックは、該くさびが該スロット内に位置決めされているときに、前記第一の縦の位置にあり、該シンチトラックは、該くさびが該スロットの外にあるときに、前記第二の縦の位置にある、請求項３に記載のアンビルアセンブリ。
- 前記アンビル部材は、長手に延びるスロットを含み、前記解放部材は、該長手に延びるスロット内を移動可能なガイドリブを含む、請求項２に記載のアンビルアセンブリ。
- 前記アンビル部材は、該アンビル部材の前記第一の側と前記第二の側との間に延びる第一のホールおよび第二のホールを含み、前記縫合具材料の前記第一の側は、該第一のホールを通って延び、該縫合具材料の前記第二の側は、該第二のホールを通って延びる、請求項３に記載のアンビルアセンブリ。
- 前記細長いバットレス材料は、せん孔線を含む、請求項２に記載のアンビルアセンブリ。
- 前記細長いバットレス材料は、前記縫合具材料の前記第一の側および前記第二の側の通行に適応するために、第一の切り抜きおよび第二の切り抜きを含む、請求項３に記載のアンビルアセンブリ。
- 前記細長いバットレス材料の近位の端部を前記アンビル部材に固定する近位の縫合具材料をさらに備える、請求項３に記載のアンビルアセンブリ。
- 前記近位の縫合具材料は、後部スパンと、該後部スパンから延びる第一の側および第二の側とを含み、該後部スパンは、前記細長いバットレス材料を前記アンビル部材の前記第二の側に固定する、請求項１１に記載のアンビルアセンブリ。
- 前記アンビル部材は、該アンビル部材の近位の端部に隣接する一対のスロットを含み、該一対のスロットは、前記近位の縫合具材料の前記第一の側および前記第二の側を摩擦で固定するための寸法にされる、請求項１２に記載のアンビルアセンブリ。
- 前記細長いバットレス材料は、該細長いバットレス材料の前記近位の端部に隣接して形成された、前記近位の縫合具材料の前記第一の側および前記第二の側の通行用の第一の切り抜きおよび第二の切り抜きを含む、請求項１２に記載のアンビルアセンブリ。
- 前記アンビル部材に添付されたカバーをさらに備え、該カバーは、前記シンチトラックの視覚化を提供する遠位のウィンドウを含む、請求項４に記載のアンビルアセンブリ。
- 前記解放部材は、第一の水平の位置と第二の水平の位置との間で移動し、
該解放部材は、該解放部材が該第一の水平の位置にあるときに、前記シンチトラックを前記第一の縦の位置に維持し、
該解放部材は、該解放部材が該第二の水平の位置にあるときに、該シンチトラックが前記第二の縦の位置に移動することを可能にする、請求項１に記載のアンビルアセンブリ。
