Abstract

Description
本発明は、心臓の管状領域で又はその付近で、焼灼及び電気的活動を感知するのに特に有用な、改善された電気生理学的カテーテルに関する。
心不整脈並びに特に心房細動は、よく起こる、かつ危険な医学的病気として、特に高齢者に根強く存在する。正常の洞律動を有する患者では、心房、心室及び興奮伝導組織を含む心臓は、電気的に興奮して、同期的な、パターン化した形で拍動する。心不整脈の患者では、心臓組織の異常な領域は、正常の洞律動を有する患者のように通常の導電組織に結びついた同期的な拍動周期に従わない。それよりもむしろ、心臓組織の異常な領域では、その隣接した組織に対して迷走した状態の鼓動を伝えることにより、心周期を乱し、非同期的な心調律にする。このような異常な状態は、例えば、房室(AV)結節及びヒス束の伝導路に沿った洞房(SA)結節の領域、又は心室及び心房の心腔を形成する心筋組織のような、心臓の種々の領域に発生することが従来から知られている。
本発明は、管状領域の開口部との外側の円周の接触と、管状領域内の内側の円周の接触に適合した、焼灼及び電位感知能力を有するカテーテルを目的とする。1つの実施形態では、本発明は、肺静脈の口の周りを焼灼するとき、肺静脈内でECG信号を得ることができるように、肺静脈の口の焼灼及び肺静脈内側での電位感知のための近位電極アセンブリ及び遠位電極アセンブリの両方を有する単一カテーテルを提供する。
(1) ほぼカテーテル軸を画定する細長いカテーテル本体と、
長手方向軸を画定する第1の細長い部材、並びに、前記部材を取り囲み、近位端及び遠位端に集まる複数の突起を有する遠位電極アセンブリであって、それぞれの突起は、少なくとも1つの電極を含み、前記突起が前記部材から放射状に外向きにたわむような曲率を有する、遠位電極アセンブリと、
ほぼ放射状である部分及び前記カテーテル軸に対してほぼ横断方向であるほぼ円形である部分で構成される第2の細長い部材を有する近位電極アセンブリであって、前記ほぼ円形である部分は複数の電極を含む、近位電極アセンブリと、
前記カテーテル本体の遠位の制御ハンドルと、
前記制御ハンドルと前記遠位電極アセンブリとの間で延在する引張部材と、を含むカテーテルであって、
前記制御ハンドルが、ユーザが第1引張部材を操作して前記突起の前記曲率を変化させるよう構成される、カテーテル。
(2) 前記遠位電極アセンブリ上の前記電極が、心臓の電気的活動を感知するのに適合し、前記近位電極アセンブリ上の前記電極が焼灼に適合する、実施態様1に記載のカテーテル。
(3) 前記突起が形状記憶性を有する、実施態様1に記載のカテーテル。
(4) 前記近位電極アセンブリの前記第2の細長い部材が形状記憶性を有する、実施態様1に記載のカテーテル。
(5) 前記遠位電極アセンブリが心臓の管状領域の内側周囲方向表面に接触するよう適合し、前記近位電極アセンブリが前記管状領域の開口部と接触するよう適合する、実施態様1に記載のカテーテル。
(6) 前記遠位電極アセンブリが肺静脈の内側周囲方向表面に接触するよう適合し、前記近位電極アセンブリが前記肺静脈の口に接触するよう適合する、実施態様1に記載のカテーテル。
(7) 前記制御ハンドルが、
ハンドル本体と、
前記ハンドル本体内に実装されたコアであって、前記コアを通って延在する長手方向通路を有する、コアと、
前記ハンドル本体内に実装された近位端及び前記ハンドル本体の外側に延在する遠位端を有し、前記コア及び前記ハンドル本体に対して長手方向に移動可能なピストンと、
前記コアに固定して実装される第1固定具と、
カム受容部であって、前記カム受容部が、前記ピストン及び前記コアに対して長手方向に摺動自在であるように、前記ハンドル本体内に実装される、カム受容部と、
前記カム受容部に固定して実装される第2固定具と、
前記ピストンを取り囲む関係で、前記カム受容部より遠位に実装されるほぼ円筒形であるカムであって、前記ピストンに対する前記カムの回転が、前記カム受容部及び前記第2固定具の長手方向の動きを引き起こす、カムと、を含み、
前記引張部材の近位端は、ユーザが前記ピストン及び前記カム受容部のうちの1つを直接又は間接的に操作して前記突起の前記曲率を変化させるために、前記第1及び第2固定具のうちの1つに連結される、実施態様1に記載のカテーテル。
(8) 前記1つの固定具が、前記引張部材を近位に引き、前記突起の前記曲率を増加させるよう適合する、実施態様7に記載のカテーテル。
(9) 前記カテーテル本体と前記近位電極アセンブリとの間の中間区域と、
第2引張部材と、を更に含み、
前記第2引張部材の近位端が、前記中間区域を偏向させるための他の固定具をユーザが操作するために他の固定具に連結される、実施態様7に記載のカテーテル。
(10) 前記第2引張部材の遠位端が前記遠位電極アセンブリの遠位端において又はその付近に固定される、実施態様9に記載のカテーテル。
(11) 前記遠位電極アセンブリの前記第1の細長い部材が、管であって、前記管を通して前記引張部材の遠位部分が延在する、管である、実施態様1に記載のカテーテル。
(12) それぞれの突起が、非導電性外面であって、前記非導電性外面の上に1つ以上のリング電極が実装される、非導電性外面を有する、実施態様1に記載のカテーテル。
(13) それぞれの突起が、支持部材を有する、実施態様9に記載のカテーテル。
(14) 前記支持部材が、二チノールワイヤである、実施態様13に記載のカテーテル。
(15) 前記遠位電極アセンブリが、少なくとも3つの突起を含む、実施態様1に記載のカテーテル。
(16) 前記遠位電極アセンブリが、少なくとも5つの突起を含む、実施態様1に記載のカテーテル。
(17) 前記引張部材の近位端は、ハンドル筐体に対する前記ピストンの長手方向の動きが、前記カテーテル本体に対する長手方向の動きをもたらし、それにより前記遠位電極アセンブリを膨張させるように、前記第1固定具に直接又は間接的に固定して取り付けられる、実施態様8に記載のカテーテル。
Claims (17)
- ほぼカテーテル軸を画定する細長いカテーテル本体と、
長手方向軸を画定する第1の細長い部材、並びに、前記部材を取り囲み、近位端及び遠位端に集まる複数の突起を有する遠位電極アセンブリであって、それぞれの突起は、少なくとも1つの電極を含み、前記突起が前記部材から放射状に外向きにたわむような曲率を有する、遠位電極アセンブリと、
ほぼ放射状である部分及び前記カテーテル軸に対してほぼ横断方向であるほぼ円形である部分で構成される第2の細長い部材を有する近位電極アセンブリであって、前記ほぼ円形である部分は複数の電極を含む、近位電極アセンブリと、
前記カテーテル本体の遠位の制御ハンドルと、
前記制御ハンドルと前記遠位電極アセンブリとの間で延在する引張部材と、を含むカテーテルであって、
前記制御ハンドルが、ユーザが第1引張部材を操作して前記突起の前記曲率を変化させるよう構成される、カテーテル。 - 前記遠位電極アセンブリ上の前記電極が、心臓の電気的活動を感知するのに適合し、前記近位電極アセンブリ上の前記電極が焼灼に適合する、請求項1に記載のカテーテル。
- 前記突起が形状記憶性を有する、請求項1に記載のカテーテル。
- 前記近位電極アセンブリの前記第2の細長い部材が形状記憶性を有する、請求項1に記載のカテーテル。
- 前記遠位電極アセンブリが心臓の管状領域の内側周囲方向表面に接触するよう適合し、前記近位電極アセンブリが前記管状領域の開口部と接触するよう適合する、請求項1に記載のカテーテル。
- 前記遠位電極アセンブリが肺静脈の内側周囲方向表面に接触するよう適合し、前記近位電極アセンブリが前記肺静脈の口に接触するよう適合する、請求項1に記載のカテーテル。
- 前記制御ハンドルが、
ハンドル本体と、
前記ハンドル本体内に実装されたコアであって、前記コアを通って延在する長手方向通路を有する、コアと、
前記ハンドル本体内に実装された近位端及び前記ハンドル本体の外側に延在する遠位端を有し、前記コア及び前記ハンドル本体に対して長手方向に移動可能なピストンと、
前記コアに固定して実装される第1固定具と、
カム受容部であって、前記カム受容部が、前記ピストン及び前記コアに対して長手方向に摺動自在であるように、前記ハンドル本体内に実装される、カム受容部と、
前記カム受容部に固定して実装される第2固定具と、
前記ピストンを取り囲む関係で、前記カム受容部より遠位に実装されるほぼ円筒形であるカムであって、前記ピストンに対する前記カムの回転が、前記カム受容部及び前記第2固定具の長手方向の動きを引き起こす、カムと、を含み、
前記引張部材の近位端は、ユーザが前記ピストン及び前記カム受容部のうちの1つを直接又は間接的に操作して前記突起の前記曲率を変化させるために、前記第1及び第2固定具のうちの1つに連結される、請求項1に記載のカテーテル。 - 前記1つの固定具が、前記引張部材を近位に引き、前記突起の前記曲率を増加させるよう適合する、請求項7に記載のカテーテル。
