[047] 少なくとも1つの重度の有害事象が、1,617例のナタリズマブ-治療MS患者のうち251例(15.5%)により、そしてプラセボ-対照試験において1,135例のプラセボ-治療患者のうち214例(18.9%)により、引き起こされた。臓器系により分類された最も一般的な重度の有害事象は、神経系の障害であった(5.9%ナタリズマブ、10.2%プラセボ)。MSの再発は、この発生に顕著に寄与した(4.7%ナタリズマブ、9.0%プラセボ)。二番目に最も一般的な重度の有害事象は、感染および外寄生であり(2.4%ナタリズマブ、2.2%プラセボ)、虫垂炎および尿路感染が最も一般的であった(両群において<1%)。
[050] 3例の確認されたPMLの症例が同定され、そのうち2例が致死であった。2例は、MS患者中で生じ、そして1例はクローン病を有する患者において生じた。MS患者は両方とも、AVONEX(登録商標)に加えて、2年以上にわたってナタリズマブの投与を受けていた。クローン病患者は、18ヶ月の期間にわたって8回のナタリズマブ投与を受け、そして持続性のリンパ球減少症により示されたように、慢性アザチオプリン使用のために免疫不全状態であった。3例のPML患者のすべてが、僅かな臨床的変化を示し、それについて疾患経過の初期に患者またはその家族が気付いた。
[068] ナタリズマブ治療を中止することの結果を、プラセボ、3 mg/kgナタリズマブ、または6 mg/kgナタリズマブを毎月1回の注入を6回行うように無作為化された213人の患者が関与した、フェーズ2研究において注意深く評価した。患者を、最後の注入後7ヶ月間追跡した。その間、再発およびその他の有害事象を記録し、そしてナタリズマブの最後の投与ののち4ヶ月後および7ヶ月後にMRIスキャンを行った。プラセボ群および2群のナタリズマブ投与群の間で比較を行った。予想されたように、再発を起こした患者の比率、並びに再発の頻度は、研究薬の停止の後に、プラセボ群における比率および頻度と同程度にまで、ナタリズマブ群において上昇した。さらに、ナタリズマブ群における活発なMRIスキャンの比率が、治療の停止の後に、プラセボ群の比率と同程度にまで、徐々に上昇した。したがって、ナタリズマブ治療の停止の結果、有効性の喪失が引き起こされたが、ナタリズマブを用いた治療を受けなかった場合に予想された様な疾患活性を超える疾患活性の増加の証拠は見られず、すなわち、リバウンド効果は見られなかった。したがって、ナタリズマブ治療を中止したMS患者は、疾患活性の顕著な増加についてのリスクは増加しない。

多発性硬化症
[071] MSは脳および脊髄の慢性疾患である。合衆国のような温暖な地帯では、MSの発症率は1年あたり約1/100000から5/100000であり(US National MS Society; NMSS)、合衆国の患者数は350000から400000人であると推定されている。MSは若年成人の、主として女性の疾患であり、疾患の発症が典型的には20から40歳の間に起こる。MSの最初の臨床的兆候は、通常は、視神経(視神経炎)、脊髄(横断性脊髄炎)、または脳幹/小脳に影響する、臨床上単離される症状の形態をとる(Runmarker and Anderson, Brain 116:117-134 (1993))。結果的にMSを発症し始める患者数の推定値は大きくばらつくが、視神経炎の症例では、発病時のMRIでのMS様病変の存在は、10年以内に臨床上確実なMSを発症する確率が80%より大きいことを示す(O'Riordan et al., Brain 121:495-503 (1998);Sailer et al., Neurology 52:599-606 (1999))。

(c) ナタリズマブを用いて患者の治療を開始し;
(d) 進行性多病巣性白質脳障害の精神的症候および身体的症候について処方医師に情報を提供し;
(e) 進行性多病巣性白質脳障害の精神的症候および身体的症候について患者に情報を提供し、そして患者に対して、少なくとも1つの症候が存在する場合に医師に対して報告するよう指示をし;
(f) 進行性多病巣性白質脳障害の指標について患者をモニタリングし;そして
(g) 進行性多病巣性白質脳障害の指標が存在する場合に、ナタリズマブの投与を終了すること;
を含み、ここで、試験、情報、およびモニタリングにより、治療の安全性が向上する、
ナタリズマブを使用して、炎症性疾患または自己免疫疾患を有する患者を治療する方法。 - 疾患が多発性硬化症である、請求項58に記載の方法。
- 多発性硬化症が、再発寛解型多発性硬化症、二次進行型多発性硬化症、一次進行型多発性硬化症、および慢性進行型多発性硬化症から選択される、請求項59に記載の方法。
- 疾患が、炎症性腸疾患または関節リウマチである、請求項58に記載の方法。
- 炎症性腸疾患がクローン病である、請求項61に記載の方法。
- モニタリングにより、患者の尿、血液、および/または脳脊髄液中のJCVを検出する、請求項58に記載の方法。
- モニタリングが、連続的に患者の血液サンプルを取り出し、サンプル中のJCVに対するIgG抗体量を測定し、そしてサンプル中の抗体量を比較することを含む、請求項63に記載の方法。
- モニタリングが、サンプル中のJCVに対するIgM抗体量を測定し、そしてサンプル中のIgM抗体量とIgG抗体量を比較することをさらに含む、請求項64に記載の方法。
- モニタリングが、セロコンバージョンおよび/または患者の尿および/または血液中のJCVの増加性の力価を検出し、そして
(a) 連続的な尿サンプルおよび/または血液サンプルの比較により、セロコンバージョンおよび/またはJCVの増加性の力価が検出される場合に、患者の脳脊髄液サンプルを取りだし;そして
