Abstract
Description
制御送信器(controller−transmitter)と言う音響変換器および1つ以上の埋め込まれた受信刺激器(receiver−stimulator)デバイスを用いる心臓組織の刺激は、最近、本明細書における発明者らによって、上で参照された特許出願において提案されている。制御送信器は音響信号を生成し、該音響信号は受信刺激器によって受信され、受信刺激器は、次に電気信号を生成し、該電気信号は結合された組織電極を介して心臓組織または他の組織に送られる。制御送信器は、外部にあり得るが、通常は埋め込まれ、制御送信器が、埋め込み可能なペースメーカのサイズに類似する合理的なサイズを有することと、制御送信器が、バッテリから長い期間、典型的には3年以上動作可能であることと、を要求する。比較的に小さいサイズおよび比較的に長い動作期間は、受信刺激器が、制御送信器からの音響エネルギーを効率良く利用することを要求する。
以下の特許および特許公開は、印加された音響エネルギーを電気出力に変換することが可能な様々な埋め込み可能な変換器を記載しており:該特許および特許公開は、米国特許である特許文献1、特許文献2、特許文献3、特許文献4、特許文献5、および特許文献6と、米国特許出願公開である特許文献7、特許文献8、および特許文献9と、公開された独国出願である特許文献10とである。
システムおよび方法が、種々の目的のために体組織に電気信号を送るために提供される。エネルギーは、典型的には、心臓組織に刺激を与えるために、例えば、徐脈(bradycardia)のための、不整脈(tachyarrhythmia)の終結のための、心不全に対する両心室の再同期療法のための、または同様のもののための心臓ペーシングにおいて送られる。しかしながら、本発明のシステムおよび方法は、種々の他の適用、例えば、神経刺激、脳刺激、随意筋刺激、胃刺激、骨成長刺激、痛みの改善、および同様のものに対する適用を含むものにおいて使用され得る。
図1に図示されるように、本発明の原理に従って構成された例示的な受信刺激器10は、変換器アセンブリ12と、整流回路13およびフィルタ回路14を含む復調器回路構成と、組織接触電極15とを備えている。オプションとして、インピーダンス整合回路16が提供されることにより、変換器アセンブリの出力を、整流/フィルタ回路および電極と接触した組織によって表される電気インピーダンスと整合させ得る。本発明は、変換器アセンブリ12および変換器のアセンブリに対するある特定の設計を提供し、該変換器のアセンブリは、次に、高い等方性の受信刺激器動作を提供することにより、変換器または複数の変換器の出力が音響ソースに対する変換器または複数の変換器の方向からかなり独立させるものである。
Claims (28)
- 埋め込み可能な受信刺激器デバイスであって、
音響ソースから音響エネルギーを受信し、該音響エネルギーに応答して電気信号を生成する等方性変換器アセンブリと、
該電気信号を受信し、生体的刺激電気出力を生成する復調器回路構成と、
該刺激電気出力を受信し、該出力を組織に送る少なくとも2つの組織接触電極と
を備えている、埋め込み可能な受信刺激器デバイス。 - 前記音響エネルギーに応答して、前記変換器アセンブリによって生成された前記電気信号は、音響ソースに対する方向が変化するときに、わずかに±6dBだけ変化する、請求項1に記載の受信刺激器デバイス。
- 前記音響エネルギーに応答して、前記変換器アセンブリによって生成された前記電気信号は、音響ソースに対する方向が変化するときに、わずかに±3dBだけ変化する、請求項1に記載の受信刺激器デバイス。
- 前記変換器アセンブリは、圧電変換器材料を備えている、請求項1に記載のデバイス。
- 前記圧電変換器材料は、多結晶セラミック圧電材料からなる、または単結晶圧電材料からなる、請求項4に記載のデバイス。
- 前記変換器アセンブリは、円筒形圧電変換器を備え、該円筒形圧電変換器は、その対向した表面上に形成された一対の電極を有している、請求項4に記載のデバイス。
- 前記対向した電極は、外部円筒面および内部円筒面上に形成されている、請求項6に記載のデバイス。
- 単結晶圧電材料からなる前記圧電変換器は、<001>または<011>配向に切断されている、請求項6に記載のデバイス。
- 前記対向した電極は、前記円筒形の平坦な端面上に形成されている、請求項8に記載のデバイス。
- 前記変換器アセンブリは、前記<001>または<011>配向に切断された単結晶からなる変換器を備えている、請求項4に記載のデバイス。
- 前記変換器アセンブリは、PMN−xPT系の単結晶材料からなる変換器を備えており、xは、5%から50%までの範囲である、請求項1に記載のデバイス。
- 前記変換器アセンブリは、PZN−xPT系の単結晶材料からなる変換器を備えており、xは、5%から50%までの範囲である、請求項1に記載のデバイス。
- 前記変換器アセンブリは、Relaxor−PT系の単結晶材料からなる変換器を備えている、請求項1に記載のデバイス。
- 前記音響ソースは、前記単結晶圧電材料からなる前記圧電変換器材料の低い周波数共振にマッチするように選択された周波数で、前記音響エネルギーを伝送する、請求項5に記載のデバイス。
- 前記変換器アセンブリは、複数の個別の変換器エレメントを備えており、前記復調器回路構成は、前記電気信号が各変換器エレメントから出力を生成する復調器回路に向かうようにして複数の復調器回路を備え、該復調器回路の全てからの該電気信号の全てを加算することにより、前記生体的刺激電気出力を生成する加算回路をさらに備えている、請求項1に記載のデバイス。
- 前記電気信号は、並列に加算される、請求項15に記載のデバイス。
- 前記電気信号は、直列に加算される、請求項15に記載のデバイス。
- 前記電気信号は、直列−並列の組み合わせで加算される、請求項15に記載のデバイス。
- 前記変換器エレメントは、各々が音響ソースの半波長未満のサイズを有し、合計で任意の横の寸法で一波長よりも大きいサイズを有する、請求項15に記載のデバイス。
- 内部容積を囲う壁を有するシェルをさらに備え、前記変換器アセンブリおよび復調器回路構成は、該内部容積内にあり、該壁の少なくとも一部は音響的に透過するものである、請求項1に記載のデバイス。
- 前記シェル全体は音響的に透過するものである、請求項20に記載のデバイス。
- 前記シェルは、円筒形である、請求項21に記載のデバイス。
- 前記シェルは、5.0mm未満の直径と、20.0mm未満の長さを有し、より好ましくは、3.0mm未満の直径と、10.0mm未満の長さを有する、請求項22に記載のデバイス。
- 埋め込まれた受信刺激器にエネルギーを送る方法であって、
変換器アセンブリを有する受信刺激器を埋め込むことと、
音響エネルギーを、音響ソースから該埋め込まれた受信刺激器に向けることであって、該変換器は電気信号を生成し、該電気信号は、該変換器の方向が該ソースに対して変化するときに、わずか±6dBだけ変化する、ことと、
該電気信号を復調することにより、生体的刺激電気出力を生成することと、
該電気出力を組織まで運ぶことと
を包含する、方法。 - 前記変換器アセンブリは、音響エネルギーを実質的に等方性の様式で受信するように構成されている、請求項24に記載の方法。
- 前記音響エネルギーは、外部の音響ソースから前記受信刺激器に向けられる、請求項24に記載の方法。
