Abstract
本発明は、特発性肺線維症(IPF)、過敏性肺炎、又はびまん性汎呼吸細気管支炎のような間質性肺感染症の治療のための生物学的および薬理学的に高い活性を有するペプチドの使用に関する。当該感染症の治療に用いられる本発明のペプチドは、当該肺感染症の病因に重要な役割を果たすと思われる保存性の高い特異的アミノ酸配列を少なくとも一つ含有する。当該アミノ酸配列を含む公知の天然ペプチドVIP (Vasoactive Intestinal Peptide)およびPACAP (Pituitary Adenylate Cyclase-Activating Polypeptide)は、人間の樹脂状細胞の成熟を阻害し、肺感染症の治療に有用である。

間質性感染症は異種起源の肺における慢性的な炎症反応の一群をいう。間質性肺感染症には異なる病態のものがあり、例えば特発性肺線維症(IPF)、過敏性肺炎、又はびまん性汎呼吸細気管支炎がある。当該疾病の病態生理学的機能は、通常の細胞組織損傷と進行性の瘢痕に正常細胞の増殖を引き起こす過度の組織修復機能が組み合わさったところに特徴がある。当該反応は度重なる組織破壊と細胞外マトリックス成分構造の分解をともなう著しい蛋白分解活性が特徴である。当該機能で細胞性免疫反応が改変し、間葉組織の増殖誘導が高度に生じる。

腫瘍壊死因子アルファのようなサイトカインやさらにトランスフォーミング増殖因子ベータ1(TGFbeta 1)のような増殖因子はこれら機能と関連している。TGFbetaは、間葉系組織の増殖を高度に刺激し細胞性免疫機能を改変させることから最も重要な増殖因子と思われる。それについてTGFbetaは産生増加における動物モデルでは重篤な肺線維症を引き起こすことが知られている。当該増殖因子における著しい産生増加は人間の肺線維症の場合にも認められる。TGFbetaは、インターフェロンガンマの産生を阻害し、インターフェロンガンマに相関する免疫反応を抑制し、免疫抑制性CD8+リンパ球の誘導、を含む免疫系の改変効果を有することも知られている。実際に進行性の線維症を有する患者には元来の組織損傷とは関連しない細胞性免疫の改変があることが以前より知られている。
さらに最近の研究では、特発性肺線維症における進行性瘢痕には“ヘルパーTタイプ2”と称する反応を導くサイトカインバランスの改変があることが確認されている。この反応ではインターロイキン4(IL-4)、IL-10およびIL13のようなTh2サイトカインの増加、並びに“ヘルパーT1”と称する反応の原理的メディエーターであるインターフェロンガンマの産生減少あるいは完全な消失が特徴である。線維症に起こる慢性的な炎症反応と対照的に、例えば細菌感染の結果として起こるような肺間質の急性炎症では、タイプ1ヘルパーTとタイプ2ヘルパーTの両方を産生するサイトカイン、例えばインターフェロンガンマ、IL-12およびIL-4といったサイトカインが同時に産生することが特徴である。さらに、TGFbetaの産生が増加し、創傷治癒の活性化が促される。
一般には特発性肺線維症は進行した病状の場合に診断されるために、細胞性機能疾患を早期に識別できることはほとんどない。創傷治癒の度合いは、次々に細胞外マトリックス成分と他の細胞成分の代謝速度の増加をもたらす炎症メカニズムによって直接的な影響を受ける。一般に、感染によって引き起こされる慢性的な炎症では病理学的に過剰な創傷治癒が引き起こされる。炎症を引き起こす物質がわかっている場合ですら、器官線維症の有効な治療のための医薬品は今のところ市場に存在しない。病理学的な機能組織の改変により極めて重要な器官制御系がゆっくりと破壊され多くの人が亡くなっている。この過程を線維症という用語で述べ、線維増殖性メカニズムにより発生し制御されている。今日では唯一の治療法は器官移植であり、大きなリスクと多額の費用をともなう。
線維増殖性反応は体内のあらゆる器官で起こる。肺における気体交換組織について言えば、肺線維症の用語で知られており、肝臓の場合には、肝硬変と称され、腎臓においては当該反応は糸球体硬化症と称される。上述のすべての疾患は線維増殖性疾患と述べることができる。線維症においては、無傷の組織では連続的に結合組織や支持組織に置き換わっていく。この過程は通常では創傷治癒の役割を担うこれら組織細胞の病理的な加速化された増殖に基づくものである。それ故に線維増殖性疾患は創傷治癒の制御不能な加速状態を伴う疾患であるといえる。線維症においては機能的器官組織が器官機能の完全な消失に至るまで置き換わっていく。
今日、次々に線維増殖性創傷治癒を引き起こしうるような肺の損傷が、150を超える異なるメカニズムで引き起こされると知られている。
これらメカニズムの中では、慢性的な感染症、有機物や鉱物性粉塵の曝露、薬物、自己免疫性のメカニズムが挙げられる。それでも線維化の過程は当該メカニズムには相関しない。小さな炎症は線維化過程の劇的な加速をもたらすのかも知れない。慢性的なウイルス疾患は、免疫抑制的な治療であるとしても、おそらく最も高頻度に進行性線維症を引き起こすであろう。
よく似た状況はあいきを伴い又は伴わない慢性的な細菌や真菌の感染症にあてはまり、隣接する肺胞での慢性的な創傷治癒過程に直接的に影響を与える気管支末端の慢性的な炎症を引き起こす。細胞スケールでは、樹枝状細胞のような抗原を提示する細胞は上述の疾患の発病において重要な役割を果たしている。それらは極めて強く、例えば、慢性的な免疫賦活を引き起こしうるTリンパ球を活性化する。それ故に上記疾患の治療のための物質には樹枝状細胞の過剰な活性を阻害することが必要である。
本発明では、VIP、PACAP、ならびに特発性肺線維症、過敏性肺炎、又はびまん性汎呼吸細気管支炎のような間質性肺感染症の治療のためのVIPおよびPACAPと同じ生物活性を有する物質について、前臨床/細胞のおよび臨床の関連について初めて記述する。
VIPおよびPACAPは中枢神経系の異なる部位、例えば海馬や大脳皮質といった特異的な大脳部位、ならびに末梢神経節で合成される。VIPは免疫細胞によっても放出される。

は、樹枝状細胞の成熟を非常に有効に阻害し、特発性肺線維症、過敏性肺炎、又はびまん性汎呼吸細気管支炎を患う患者に投与された時に非常にすぐれた効力を有することを見出した。当該アミノ酸配列を含有し、全体で10-60アミノ酸、好ましくは10-38アミノ酸、最も好ましくは10-28アミノ酸又は10-27アミノ酸を含み、かつ当該アミノ酸組成も含むVIPおよびPACAPと同様の特性を有する物質の使用が好ましい。
Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 4)

本文における“同等な生物学的活性”とは、上記示されたペプチド又はポリペプチドの関連する特性、好ましくはVIP又はPACAPの関連する特性と比較した、生物学的、生理学的、あるいは治療学的な機能の活性をいう。
本文における“誘導体”とは、VIP又はPACAPのような対応するペプチドから多少とも直接的に由来するペプチド物質、およびもとのペプチドの生物学的な特性を変えることなく付加、欠失、変異、修飾により改変したペプチド物質をいう。例えばVIPの適応可能な誘導体としては、WO 8905857, WO 9106565, EP 0663406 and WO 9729126 に挙げられる(Fmoc保護化VIP)。この用語はもとのペプチド又はポリペプチドからなる発明およびリポソームのような親油性物質と結びついた発明によれば、上記示されたペプチドおよびポリペプチドの複合体にも関する。VIP-リポソームは例えばWO 9527496 や WO 9735561に公表され、生物活性や蛋白分解に対する保護に関して改良された特性を有している。加えて、この用語はフラグメントや短縮化フラグメントのような改変フラグメントに関する。
本文における“アナログ”とは、その構造や構成が本発明のペプチドやポリペプチドとは異なる物質をいうが、むしろVIPとは異なるものであるが、生物学的特性には変化のないものである。VIPアナログは天然に存在し又は合成できるペプチドであり、ならびに非ペプチドでもありうる。本発明によれば、VIPアナログはペプチドである。公表されたVIPアナログの例としてはEP 0325044 (環状ペプチド), EP 0225020 (直鎖状ペプチド), EP 0536741 (環状VIP改変体), EP 0405242, EP 0184309 and EP 0613904が挙げられる。この用語はVIP又はPACAPのほかにVIPと近似した構造を有するVIP又はPACAP相同体をもいう。本発明によればPACAPそれ自体、ならびにその短縮化体であるPACAP-27はかかる相同体といえる。好ましいVIP/PACAP相同体は鉱石共通アミノ酸配列(one ore more consensus amino acid sequences)を含むペプチドである。上記示されたペプチドの例としてはヒスチジン イソロイシン ペプチド、ヒスチジン メチオニン ペプチド、“ヒト成長ホルモン放出因子”(GRF)、PACAP、セクレチン、およびグルカゴンがある。
本文における“安定型”とは、血中および血清中で安定性向上と半減期延長の両方を得るために改変された誘導体又はアナログ、もとのペプチドをいう。当該安定型は酵素活性によりフラグメント化されたペプチドの場合に好適である。取りうる安定型としては環状VIP又は環状PACAPのような環状ペプチド又はポリペプチド、融合ペプチド、好ましくはFc融合蛋白、あるいはペグ化VIP又はPACAPのようなペグ化ポリペプチドが挙げられる。そのようなポリペプチドの合成方法はその技術分野では知られている。ポリペプチドおよび蛋白は化学基を付加することにより蛋白分解から保護することができる。そのような付加により蛋白分解酵素が蛋白構造に物理的に接触するのを妨げ分解を防止する。ポリエチレングリコールはそのような蛋白分解からの保護を示す構造を有するものである(Sada et al., J. 本文における“融合蛋白”とは、好ましくはVIP又はVIP誘導体又はアナログである本発明の、特に安定型での、PACAPのようなポリペプチドからなる物質をいい、さらにはペプチド又は蛋白も加えられる。そのような蛋白としては好適には免疫グロブリン、さらに好適にはそのフラグメント、最も好適にはIgG分子のFc部、特にはIgG1分子のFc部が挙げられる。FcおよびVIP融合蛋白はWO 200024278にて公表されており、血中および血清中において半減期の改善されたものが提供されている。さらにFc-PACAPおよびFc-PACAP-27を例として引用することができる。
本発明による物質は薬物の合成、又は個体の病理学的な特性の評価のための診断方法に用いることができる。
本文における“個体”とは好ましくは哺乳動物、特には人間をいう。本物質は通常薬剤的に許容される基剤又は溶剤を含む医薬品組成物および処方物に用いられる。本発明で述べられた物質の処方および使用の方法は“Remington’s Pharmaceutical Sciences”, Mack Publishing Co., Easton PAにより知ることができる。
本文における“薬剤的に許容される補助担体”とは不活性で毒性のない固体又は液体の賦形剤をいう。
吸入剤とするには、本物質はエアロゾルとして利用可能であることが好ましいといえる。エアロゾルの合成方法についてはその技術分野では述べられている。吸入器を用いて投与され、本発明のペプチド又はポリペプチド、例えばVIPまたはPACAPを含有するエアロゾルは、肺症状を直接的に治療する必要がある場合に望ましいものである。
本発明の投与単位は一日当たりに必要な本物質の量または望ましい投与量となるその約数量を含むことができる。
[併用療法]
本発明の化合物は、単物質の剤型、またはカルシウムチャンネル阻害剤(ジルチアゼム)、免疫抑制物質(プレドニゾロン)、肺におけるブドウ球菌、シェードモナス菌、ブルクホルデリア、ヘモフィルス、レンサ球菌またはその他の細菌に対して特異的に効果のある抗生剤やバクテリオファージのような抗菌薬、ベータアドレナリンレセプター阻害物質やアンジオテンシンレセプターアンタゴニスト、アンジオテンシン変換酵素阻害剤(ラミプリル)、抗増殖性物質(アトルバスタチン)、エンドセリンレセプターアンタゴニスト(Bosentan, Altrasentan, Sitaxsentan, Enrasentan, BMS 193884, Darusentan, TBC 3711, BSF 208075, BSF 302146, SPP 301)、その他の抗増殖性物質(D-24851, Imatinib mesylate)といった薬物と組み合わせて患者に投与することができる。さらに本発明は少なくとの上述の薬物の一つとともに公表されたペプチドの併用療法にも関する。
本発明の化合物は、単物質の剤型、またはカルシウムチャンネル阻害剤(ジルチアゼム)、免疫抑制物質(プレドニゾロン)、肺におけるブドウ球菌、シェードモナス菌、ブルクホルデリア、ヘモフィルス、レンサ球菌またはその他の細菌に対して特異的に効果のある抗生剤やバクテリオファージのような抗菌薬、ベータアドレナリンレセプター阻害物質やアンジオテンシンレセプターアンタゴニスト、アンジオテンシン変換酵素阻害剤(ラミプリル)、抗増殖性物質(アトルバスタチン)、エンドセリンレセプターアンタゴニスト(Bosentan, Altrasentan, Sitaxsentan, Enrasentan, BMS 193884, Darusentan, TBC 3711, BSF 208075, BSF 302146, SPP 301)、その他の抗増殖性物質(D-24851, Imatinib mesylate)といった薬物と組み合わせて患者に投与することができる。さらに本発明は少なくとの上述の薬物の一つとともに公表されたペプチドの併用療法にも関する。
驚くべくことに、特許請求の範囲にも明示するように、特にVIPおよびPACAPと併用する本ペプチドおよびポリペプチドはヒト樹枝状細胞の成熟阻害剤であり、特発性肺線維症、過敏性肺炎、又はびまん性汎呼吸細気管支炎を患う患者の治療に有益な効果を有することを見出した。
[実施例]
図1-11はヒト樹枝状細胞の成熟に対するVIPの効果を示す。CD83は成熟した樹枝状細胞の特徴的な表面蛋白である。上述の蛋白の割合が低いことは細胞が成熟しておらず、このため生物学的作用を行う能力のないことを示す。1 μmol のVIP(図5)の添加、または特に9.1 μmolのVIP (図9)の添加により表面分子CD83の顕著な減少をもたらす。上述の減はM1値 38.22−9.1 μmolの添加後測定時−により示され、未処理細胞(図1)のM1値 81.26と比較される(蛍光活性化セル分析)。この方法は対応する抗原(この場合はCD83)に対する蛍光標識した抗体を利用して細胞表面を調べるものである。いわゆるMHCI(図2,6,10参照)またはMHCII(図3,7,11参照)のような他の表面分子ではVIPと同量を処理しても極めてわずかな阻害しか認められないことからCD83分子に対するVIPの効果は特異的なものである。このことは、未処理細胞MHCIのM1値が85.59であることに比較して9.1 μmol VIP 処理MHCIのM1値が70.57であること、ならびに、未処理細胞MHCIIのM1値が83.94であることに比較して9.1 μmol VIP 処理MHCIIのM1値が77.73であることからわかる。
図1-11はヒト樹枝状細胞の成熟に対するVIPの効果を示す。CD83は成熟した樹枝状細胞の特徴的な表面蛋白である。上述の蛋白の割合が低いことは細胞が成熟しておらず、このため生物学的作用を行う能力のないことを示す。1 μmol のVIP(図5)の添加、または特に9.1 μmolのVIP (図9)の添加により表面分子CD83の顕著な減少をもたらす。上述の減はM1値 38.22−9.1 μmolの添加後測定時−により示され、未処理細胞(図1)のM1値 81.26と比較される(蛍光活性化セル分析)。この方法は対応する抗原(この場合はCD83)に対する蛍光標識した抗体を利用して細胞表面を調べるものである。いわゆるMHCI(図2,6,10参照)またはMHCII(図3,7,11参照)のような他の表面分子ではVIPと同量を処理しても極めてわずかな阻害しか認められないことからCD83分子に対するVIPの効果は特異的なものである。このことは、未処理細胞MHCIのM1値が85.59であることに比較して9.1 μmol VIP 処理MHCIのM1値が70.57であること、ならびに、未処理細胞MHCIIのM1値が83.94であることに比較して9.1 μmol VIP 処理MHCIIのM1値が77.73であることからわかる。

Claims (12)
- 特発性肺疾患、過敏性肺炎、又はびまん性汎呼吸細気管支炎に直接的または間接的に関係する肺疾患の患者の治療のための薬物を合成するための物質の使用であって、
当該物質が以下のアミノ酸配列を含むペプチドまたはポリペプチドである、前記使用:
Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 1)。 - 請求項1に記載の使用であって、
当該ペプチドまたはポリペプチドがさらに以下のアミノ酸配列を含むもの、:
His-Ser-Asp (SEQ ID number 14); Phe-Thr-Asp (SEQ ID number 13)。 - 請求項1に記載の使用であって、
当該ペプチドまたはポリペプチドが以下のアミノ酸配列を有するもの、:
(A)n-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-(B)m、
ここで、A, B は任意の天然アミノ酸、AおよびBはそれぞれ互いに独立であり、またn, mは0-25から置換する値であり、n,およびmはそれぞれ互いに独立である。 - 請求項3に記載の使用であって、
(A)nがn>2の場合に以下の配列を有するもの、:
(X)o-Phe-Thr-Asp-(Y)p;、
ここで、X, Yは任意の天然アミノ酸、XおよびYはそれぞれ互いに独立であり、またo, pは0-11から置換する値であり、oおよびpはそれぞれ互いに独立である。 - 請求項4に記載の使用であって、
(X)o がo>2の場合に以下の配列を有するもの、:
(X’)q-His-Ser-Asp-(X’’)r、
ここで、X’, X’’は任意の天然アミノ酸、X’およびX’’はそれぞれ互いに独立であり、またr, qは0-4から置換する値であり、rおよびqはそれぞれ互いに独立である。 - 請求項3に記載の使用であって、
当該ペプチドまたはポリペプチドの配列が以下の群に属するもの、:
(i) Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 4)
(ii) Phe-Thr-Asp-X1-X2-X3-X4-X5-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-Asn-Ser-Ile-Leu-Asn (SEQ ID number 5);
(iii) Phe-Thr-Asp-Asn-Tyr-Thr-Arg-Leu-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-Asn-Ser-Ile-Leu-Asn (SEQ ID number 6);
(iv) Phe-Thr-Asp-Ser-Tyr-Ser-Arg-Tyr-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 7);
(v) His-Ser-Asp-X1-X2-Phe-Thr-Asp-Asp-X3-X4-X5-X6-X7-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 8);
(vi) His-Ser-Asp-Ala-Val-Phe-Thr-Asp-Asn-Tyr-Thr-Arg-Leu-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 9);
(vi) His-Ser-Asp-Gly-Ile-Phe-Thr-Asp-Ser-Tyr-Ser-Arg-Tyr-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu (SEQ ID number 10);
(vii) His-Ser-Asp-X1-X2-Phe-Thr-Asp-Asp-X3-X4-X5-X6-X7-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-X8-X9-X10-X11-(X12) (SEQ ID number 11);
(viii) His-Ser-Asp-Ala-Val-Phe-Thr-Asp-Asn-Tyr-Thr-Arg-Leu-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-Asn-Ser-Ile-Leu-Asn (VIP, SEQ ID number 1);
(ix) His-Ser-Asp-Gly-Ile-Phe-Thr-Asp-Ser-Tyr-Ser-Arg-Tyr-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-Ala-Ala-Val-Leu (PACAP-27, SEQ ID number 2);
(x) His-Ser-Asp-X1-X2-Phe-Thr-Asp--X3-X4-X5-X6-X7-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu X8-X9-X10-X11-X12- X13-X14- X15- X16-X17- X18- X19-X20- X21- X22 (SEQ ID number 12);
(xi) His-Ser-Asp-Gly-Ile-Phe-Thr-Asp-Ser-Tyr-Ser-Arg-Tyr-Arg-Lys-Gln-Met-Ala-Val-Lys-Lys-Tyr-Leu-Ala-Ala-Val-Leu-Gly-Lys-Arg-Tyr-Lys-Gln-Arg-Val-Lys-Asn-Lys (PACAP-38, SEQ ID number 3);
