Abstract
Description
本発明は、ω−コノペプチドと、モルヒネ、ブピビカイン、クロニジン、ハイドロモルホン、バクロフェン、クエン酸フェンタニル、ブプレノルフィンおよびクエン酸スフェンタニルからなる群から選ばれる鎮痛剤、またはその薬学的に許容される塩を含有する医薬製剤であって、該ω−コノペプチドがその効力を維持し、該鎮痛剤と物理的にも、化学的にも共存できる該医薬製剤に関する。該医薬製剤は髄腔内投与に適している。
本発明は、ω−コノペプチドおよび1種以上の鎮痛剤を患者に投与する併用(combination)薬剤療法に関する。ω−コノペプチドおよびその他の鎮痛剤が、種々の投与経路のいかなる経路で、特に髄腔内投与、特に継続的髄腔内投与される。本発明は、鎮痛剤をω−コノペプチドと併用し、併用により各薬剤の必要な投与量を減らし、そして各鎮痛剤に付随する副作用を減少させる様に、該鎮痛剤とω−コノペプチドを組合せて投与する方法の必要性を満たす。該鎮痛剤とω−コノペプチドとの組合せ投与はまた、主体における疼痛軽減において、相加的効果、或いは相乗的効果すら提供する。
ω−コノペプチドは、ω−コノトキシンとしても知られており、コヌス属に属する肉食性の海産巻き貝に見出される低分子(24−29個のアミノ酸)のジスルフィドに富んだポリペプチドである。全てのω−コノペプチドは、特定のVSCCのサブタイプへの親和性が異なるが、N−タイプ電圧感受性カルシウムチャンネル(VSCC)に結合しており、それがニューロンにおいてもっぱら見出される。神経細胞膜脱分極に応じて、N−タイプVSCCsが、開いて、そして神経伝達物質の放出を来すカルシウムの進入を許容する。N−タイプVSCCsは、脊髄の背面角のRexed板 I and IIに豊富にあり、疼痛の1次球心線維は、初めてシナプシスを合図する。ω−コノペプチドはRexed板I and IIのN−タイプVSCCsに結合し、プレシナプシス末端へのカルシウムの移送を阻止し、それによって、神経伝達物質の放出を阻止する。この場所でのN−タイプVSCCsでカルシウムの進入を阻止することにより、末梢神経傷害後に発展し、末梢神経傷害を特徴づけるシグナルを含め、疼痛シグナルが容易に伝達されず、或いは完全に阻止される。
ω−コノペプチド、好ましくはジコノチドでの処置は、神経傷害的疼痛の進行の阻止に有用である。鎮痛性のω−コノペプチドは、allodyniaモデルの様な疼痛のアヘン製剤抵抗性モデルにおいてのみならず、ラット挙尾モデル或いはラットホルマリンモデルの様な神経傷害的疼痛の従来のアヘン製剤感受性モデルの双方において、鎮痛剤として有効である。
ω−コノペプチドの希釈溶液は、メチオニン残基の酸化または還元或いは生物活性の消失で立証されるように、概して溶液中で不安定である。殊に12位にメチオニンを有するジコノチドは、そのメチオニンがスルホキシ体で存在すると、ω−コノペプチドMVIIA結合サイトへの結合において、約10倍強力でない。しかし、ω−コノペプチドは、ペプチド構造中に存在するメチオニン残基の酸化を防止することにより、溶液中で相当安定化される。例えば、ジコノチド酸化は、組成物に乳酸塩緩衝液を添加して阻止できる。更に詳細には、150mM乳酸塩緩衝液(pH4−4.5)は、ジコノチドの安定性をかなり改善する。ジコノチドペプチド濃度が約0.1mg/ml未満であるジコノチド溶液は、水、生理食塩水、或いはペプチド化学の分野で使用される種々の緩衝液のいずれにおいても、溶解させると、急速に酸化される。0.01−0.1mg/mlの範囲のジコノチド溶液は、乳酸塩(150mM、pH4−4.5)で安定化させると、45℃で数週間安定である。50μg/mlのメチオニンを含有する緩衝剤は、また150mM乳酸塩緩衝液または酸性生理食塩水(pH4−4.5)のいずれかがその溶液を緩衝するために使用されるとき、ジコノチドの安定化に有効である。
ω−コノペプチドとその他の鎮痛剤との併用療法は、単品薬剤投与に比べいくつかの利点を提供する。第1に、組合せ薬剤の投与は、個々の薬剤の投与量を少なくしうる。そしてそれは、どちらかか、或いは両方の薬剤耐性の進行を減少させ、そして薬剤相互作用の可能性を減少させる。各薬剤の減少した濃度は、また薬剤の用量依存性副作用を制限する。更に、各薬剤のより低い濃度が治療効果を達成するために必要とされるが故に、有効量(ED50)が減少される。
モルヒネ
モルヒネ((7,8-didehydro-4,5-epoxy-17-methylmorphinan-3, 6-diol)は、あらゆるタイプの疼痛にたいして強力な鎮痛作用を有するアヘンアルカロイドである。モルヒネは容易に耽溺性を引起し、従って、他の疼痛緩和剤が不適当とされた時の最終手段の薬剤である。INFUMORPH(登録商標)(硫酸モルヒネ)は、希硫酸でモルヒネを中和して合成された、かなり安定な塩である。硫酸モルヒネは、水和物の水を失い、空気、光に晒すと黒ずむ。このモルヒネ塩は、ガン患者の処置のために経口投与で、特にイングランドで使用されている。モルヒネ単独の代表的髄腔内投与量は、0.5−75mg/日の範囲、通常は2−20mg/日である。
DILAUDID(登録商標)(塩酸ハイドロモルホン、塩酸ジハイドロモルホン)は、モルヒネを多量の白金またはパラジウムを用い、酸性条件下、接触水素化と脱水素化により合成された、モルヒネの合成誘導体である。ハイドロモルホンは、モルヒネの代替物(5倍強力)であるが、殆ど同等の耽溺性と活性のより短い持続性を有する。ハイドロモルホンのモルヒネより勝る唯一の利点は、より少ない昼間鎮静あるいは眠気を与えることである。ハイドロモルホンの代表的投与量は、0.05−15mg/日である。
MARCAINE(登録商標)(塩酸ブピバカイン)は、初期局麻剤として、そして補助的脊髄鎮痛剤としての両方で、臨床的に使用されているナトリウムチャンネル阻害剤である。ブピバカインは、化学構造および性質において、メプリバカインに似たアミドクラスの局麻剤である。ブピバカインの作用持続時間はテトラカインの2−3倍である。ブピバカインの強さは、テトラカインに匹敵するが、しかし両者がメピバカインとリドカインの強さの約4倍である。ブピバカイン単独の代表的髄腔内投与量は、1−100mg/日の範囲、通常は5−15mg/日である。
ロピバカインは、運動性ロス無しの無痛覚の1つを選択とするA-beta 線維上のA-delta とC 線維への相対的親和性を持ったアミドタイプの局麻剤である。ロピバカインは、ブピバカインと比較して、効率的知覚遮断と共に運動遮断の親和性が少なく、そしてブピバカインより脂質溶解性が低い。ブピバカインと比較して、ロピバカインは、毒性が少なく、知覚および運動遮断間の運動神経に対する知覚神経に、より選択的であり、そしてより少ない溶解性を有し、より大きな脊髄体節の広がりを生じる。ブピバカインと比較して、ロピバカインは、作用の持続性は短く、2相性時間依存薬物動態を有する。(G. DURACLON(ハイドロモルホン;2,6-dichloro-N-2-imidazolidinylidenebenzenamine)は、α2アドレナリン作動性レセプター、P1プリン作動性レセプター、およびH2ヒスタミンレセプターのアンタゴニストである。クロニジンは、中枢性血圧降下剤として使用され、またたいていのアヘン性禁断症状を廃止させる。クロニジンは、急性(術後)疼痛と慢性疼痛症候群の双方を処置するために臨床で使用されている。疼痛の処置のために臨床で使用した時のクロニジンの公知の副作用は、高血圧と徐脈である。クロニジン単独の代表的髄腔内の1日の投与量は、10−400μg/日の範囲、通常は25−75μg/日である。
バクロフェン(γ-amino-β-(p-chlorophenyl)butyric acid)は、GABABレセプターの選択的アゴニストとして作用し、そして中枢神経の神経伝達物質の放出を阻止するγ−アミノ酪酸(GABA)の4−クロロフェニル誘導体である。バクロフェンは、抗痙攣(筋肉弛緩)効果のために使用され、そして脊髄損傷や多発性硬化症が原因で起きる難治性痙直のために特に処方される。バクロフェンは、また鎮痛作用を有し、ラットに非経口で、或いは髄腔内投与したとき、抗侵害受容的である。バクロフェン単独の代表的髄腔内投与量は、20−2000μg/日の範囲、通常は300−800μg/日である。
SUBLIMAZE(登録商標)(クエン酸フェンタニル(N-(l-phenethyl-4-piperidyl)propionanilide citrate)は、ヒトでのモルヒネより50倍の鎮痛活性を有するアニリド誘導体である。クエン酸フェンタニルは、主に麻酔剤の補助薬として使用され、急速な開始(4分)と作用の短期持続を有する。他の強力な鎮痛剤と同様な副作用は、特に呼吸抑制と徐脈に共通である。クエン酸フェンタニルは依存性傾向を有する。クエン酸フェンタニルは代表的には、10μg/hour の用量で髄腔内投与される。
クエン酸スフェンタニル(N- [4-(methoxymethy)-l-[2-(2-thienyl)ethyl]-4-piperidinyl]-N-phenylpropanamide 2-hydroxy-1,2,3-propanetricarboxylate)は、強力なアヘン性鎮痛剤である。スフェンタニルは代表的には、0.1−1.5μg/hour の範囲の用量で髄腔内投与される。
種々の濃度のω−コノペプチドと種々の濃度の1種以上の薬剤との組合せで含有する製剤は、想定され、その薬剤には限定的でなく、モルヒネ、メサドン、ハイドロモルホン、ブプレノルフィン、メペリジン、フェンタニル(クエン酸フェンタニル)、スフェンタニル(クエン酸スフェンタニル)、ブピバカイン、ロピバカイン、テトラカイン、クロニジン、チザニジン、デキストルファン、デキストロメトルファン、メマンチン、ケタミン、オクトレオチド、バルプレオチド、バクロフェン、ミダゾラム、ネオスチグミン、アスピリン、アデノシン、ガバペンチン、ケトロラック、オクトレオチド、ドロペリドール(またはそれらの薬学的に許容される塩)が含まれ、そしてその際にジコノチドがその効力を保持し、そして鎮痛剤と物理的、化学的に共存しうる。
本発明は、患者の疼痛軽減方法を提供する。該方法は、患者にω−コノペプチドの有効量と鎮痛剤の有効量を投与する方法であり、その際にω−コノペプチドとその鎮痛剤は共存性があり、投与の間に両者の活性を維持している。ω−コノペプチドとその鎮痛剤は互いに別々の製剤でよく、そして同時に或いは連続して投与されうる。別にω−コノペプチドとその鎮痛剤は、予め混合し、投与用の1つの製剤を形成しうる。
実施例1.ジコノチドの安定性
目的:ジコノチド製品は一般に生理食塩水(100μg/ml)に溶かし、pH4.3に調整される。遊離のメチオニン(50μg/ml)が抗酸化剤としてその溶液に添加される。この溶液は3年間2−8℃で貯蔵されると、勝れた安定性を示した。このものを臨床に使用するとき、医者がしばしば、生理食塩水を用いて、この製品を25μg/mlに希釈する。このことは遊離メチオニン濃度を4倍に減らし、そしてペプチドを更なる酸化的分解にさらす。この研究の目的は、メチオニン50μg/ml中のジコニチド25μg/mlの安定性を調べることである。
実施例2.ジコノチドの他の髄腔内投与薬剤との相容性
ジコノチドと7種の他の今日市販されている髄腔内投与薬剤(塩酸ハイドロモルホン、塩酸ブピバカイン、硫酸モルヒネ注射液、クエン酸フェンタニル注射液、クエン酸スフェンタニル、注射用バクロフェン、および注射用塩酸クロニジン)との物理的相容性が、混合前と混合24時間後の外観、色、pH、および特有事項を評価して、決定した。物理的相容性であるとみなされるためには、混合液は細粒に関して米薬局方(USP)基準を満たし、許容pH値に留まり、そして色、外観の変化を有してはならない。
ジコノチドと5つの他の髄腔内投与薬の化学的相容性を高速液クロ(HPLC)で混合状態でのジコノチドの濃度を評価して決定した。調製混合物を37℃で60日間、ポリメチルペンテンバイアル中で保存した。注射用0.9%食塩水(USP)で、0.025mg/mlに希釈したジコノチドを、高濃度、低濃度の各薬剤と合体した。混合物中の他の組成の濃度は決定しなかった。この研究からのデータは下記表2に示す。
硫酸モルヒネと混合した時のジコノチドの相容性は、100μg/mlのジコノチドを硫酸モルヒネ(INFUMORPH(登録商標))と混合し、そしてその混合物をMedtronic SYNCHROMED(登録商標)Infusion System(薬剤分配移植ポンプ)に入れ、決定した。試料は特定の間隔でそのポンプの容器から得、混合物中の両薬剤成分の濃度を決定した。表3にジコノチドの安定性を、その最初の濃度の割合として表示する。
モルヒネとジコノチドを一緒に投与した場合の相乗効果(熱板及び尾部浸漬試験法)
0.03μg/hrのジコノチドの継続的髄腔内注入によって、ラットの熱板及び尾部浸漬試験法において、熱応答時間に関して、せいぜい、適度の増加を示した。しかし、0.03μg/hrのジコノチドをモルヒネ(15μg/hr)の緩和な抗侵害受容量と共に投与すると、両試験における応答時間は、いずれの単独の場合に比べ遙かに凌がした。
ラットの後ろ足ホルマリン試験において、1:10の一定投与率でのジコノチドとモルヒネの合剤の髄腔内ボーラス投与によって、ホルマリン惹起持続的回避反応を用量依存的に抑制した。測定されたED50値は、予想の相加的鎮痛効果で予見された値とは有意に異ならない。この結果からこのモデルにおいて、モルヒネとジコノチドとの併用投与は相加的鎮痛効果を示すことを示唆する。
ラットにおける研究の結果が以下のことを示している。
・ジコノチドは、モルヒネの鎮痛効果に対する耐性の進行を阻止しない。
・ジコノチドは、モルヒネ耐性ラットにおいてモルヒネ誘導無痛覚に影響を与えない。
・ジコノチドの亜急性髄腔内注入は、続いて起こるモルヒネの無痛覚に影響しない。
1.ジコノチドは、モルヒネ耐性動物に投与したときに、モルヒネの鎮痛効果を回復しない。
2.髄腔内投与モルヒネの鎮痛効果に耐性のラットはジコノチドに交叉耐性を示さない。
皮下投与のモルヒネ(10mまたは30mg/kg)と髄腔内ボーラス注射で投与のジコノチド(0.1μg)間での急性相互作用は、慣例の機能観察の1組の装置および心拍と血圧の測定を用いてラットで調べた。皮下投与のモルヒネと髄腔内ボーラス注射のジコノチドの全体の行動的と心臓血管の効果が、それらが組合せ投与されたとき、増強もされず、減少もされなかった。これらの知見は、ジコノチドの鎮痛用量が、単独で、あるいは高用量の皮下投与モルヒネと併用して、髄腔内投与されると、よく耐えられることを示している。
ジコノチドの髄腔内投与は、ラットでのCO2吸入に対する呼吸分時量の応答を弱めなかったし、モルヒネの皮下注で引起された急性呼吸機能低下を悪化させることもしなかった。最少の副作用で顕著な無痛感覚を示すジコノチドの0.1μgの用量での髄腔内ボーラス注射がCO2に対して呼吸分時量応答を弱めなかったし、モルヒネの皮下ボーラス注射(10または30mg/kg)により引起された急性呼吸機能低下を悪化させなかった。同様に、ジコノチド鎮痛用量、0.1または0.3μg/hrで継続的に髄腔内注入によって、亜急性的に投与されると、単独でも、あるいは皮下投与されるモルヒネの鎮痛用量(200μg/hr)と併用しても、呼吸機能低下を引起さなかった。
ラットにおいて、モルヒネを皮下ボーラス注で投与すると、用量依存的に胃腸管輸送阻害を示した。モルヒネを、それ自体胃腸管運動に影響しないジコノチドの髄腔内投与量(0.3μg)と組合せ投与すると、モルヒネの用量応答カーブが有意に左にシフトした。この知見は、ジコノチドとモルヒネが相互作用し、相乗的にラットでの胃腸管輸送を減少させる。逆に、マウスに、ジコノチド(1μg)の髄腔内投与量と共にモルヒネ(3mg/kg)の皮下注投与されたとき、胃腸管輸送は、モルヒネが単独で投与されたときに観察される程度と同程度に減少した。単独投与されたときに、胃腸管輸送は、どちらか一方の化合物により、最大に減少させられる可能性のために、この知見は2つの化合物間の潜在的相互作用を決定的に除外しない。
選択的GABAB受容体アゴニストであるバクロフェンは、脊髄損傷あるいは多重硬化症による治療しにくい痙直の治療の脊髄使用が指示されている。バクロフェンは、また、実験動物で非経口または髄腔内のいずれかにより投与されると、鎮痛作用を有し、抗侵害受容的である。ラットにバクロフェンあるいはジコノチド単独で、髄腔内ボーラス注射により、後ろ足の背面の皮下に5%ホルマリン注によって引起された持続性疼痛行動を用量依存的に減少に導いた。ジコノチドとバクロフェンの組合せが髄腔内ボーラス注射により投与されると、ホルマリン誘導持続的疼痛への影響は、各化合物を単独で投与したことによる効果の合計に統計学的に同等であった。これらの結果が、ジコノチドとバクロフェンの組合せ髄腔内投与がラットの後ろ足ホルマリン試験において相加的無痛感覚を示すことを示している。
ブピバカインはナトリウムチャンネル阻害剤であり、臨床的に初期局所麻酔剤として、そして補助的脊髄鎮痛剤としての両方で使用されている。ブピバカインの髄腔内ボーラス注射により、ホルマリン誘導急性あるいは持続性後ろ足回避行動を有意に阻止しなかったが、しかし300μgのブピバカインは、即座のそして1時的な後ろ足の麻痺を示した。ジコノチドを髄腔内ボーラス注射(0.03、0.1または0.3μg)すると、用量依存的に持続的疼痛行動を抑制した。ブピバカインを、完全神経ブロックを起さない用量で、ジコノチドと一緒に髄腔内ボーラス注射すると、ジコノチド誘導無痛感覚を有意に変更しなかった。その結果から、ブピバカインは、ラット後ろ足ホルマリン試験において、無痛感覚を引起さず、ジコノチド誘導無痛感覚を変更しなかった。
α2−アドレナリンレセプターアゴニストであるクロニジンは、全身投与した場合に、中枢媒介メカニズムを介して無痛感覚を生みだす。クロニジンは急性(術後)疼痛および慢性疼痛症候群の双方を処置するために、臨床的に使用されている。ジコノチドまたはクロニジンを単独で、あるいは組合せて、髄腔内ボーラス注射すると、用量依存的にラットの後ろ足ホルマリン試験において、持続的疼痛行動を抑制した。薬剤併用の理論的相加ED50値は、観察された値と有意に異ならないし、また複合追加および混合ドーズレスポンス曲線のlog 直線部分に適合した回帰線間に有意な差がない。これらの知見は、ジコノチドとクロニジンは髄腔内経路で一緒に投与されると、相加的鎮痛効果を有するとの結論を支持する。
クロニジンは、疼痛の処置のために、臨床的に使用すると、低血圧と徐脈の副作用が知られている。クロニジンをラットに、髄腔内ボーラス注射すると、用量依存的に徐脈を示し、動脈血圧の2元的効果を有した:低容量(1−3μg)でCNS介在低血圧を示し、一方高用量(10−50μg)で血管滑平筋における末梢α2−アドレナリンレセプターの活性化による顕著な血圧応答を示す。クロニジン注射10分前に、ジコノチドの高鎮痛用量(0.3μg)を髄腔内ボーラス注射すると、意識のあるラットにおいて、髄腔内投与されたクロニジン(3μg)により引起された降圧あるいは徐脈応答を悪化させなかった。このことは、心臓血管系内において有意な薬剤相互作用が存在しないことを示している。
ジコノチドの潜在的毒性は、ラットにおいて、1500ng/kg/hrまでの用量と、犬において1200ng/kg/hrまでの用量(少なくとも患者の最大用量の30倍)で、28日間継続的髄腔内注入して評価した。
神経組織病理学的検査の結果、対照とジコノチド処理ラットおよび犬で、慢性炎症を伴う脊髄圧迫を示した。それはITカテーテルにより生じた圧迫のためである。この見解はジコノチド投入で悪化されなかったし、他の組織病理学的変化も観察されなかった。結論的には、ジコノチドを28日間継続的IT注入を受けたラットと犬で観察された行動的そして神経学的効果は、神経毒性または組織病理学を伴わないジコノチドの薬理学的活性に関連していると思われた。更に、ジコノチド処理犬には、注入部位を調べたが、ITモルヒネ投与に関して述べられているように(Yaksh, et al., Anesthesiology 2003; 99: 174-87)、カテーテル先端での肉芽腫の形成が観察されなかった。
患者の基準
患者はジコノチドの髄腔内安定投与量の女性と男性患者であった。全ての患者は、少なくとも0.2μg/hr(4.8μg/日)の投与量であった。更に、患者が痛み度合いの視覚のアナログ尺(VASPI) による40mm以上のスコアで表される亜最高疼痛リリーフを有する患者か、あるいはジコノチドにより解放されない残存疼痛やジコノチドで解放される疼痛と異なる性質の残存疼痛を有する患者のいずれかであった。
ジコノチドは100μg/mlの濃度の5mlの単用量バイアルで供給された。ジコノチドの用量は、少なくとも0.2μg/hr(4.8μg/日)であった。4週処理フェーズ間の平均用量は、1週での26.5μg/日 から4週での27.9μg/日であった。終末でのジコノチドの平均蓄積用量は731.1μgであった。患者は、4週併用処理フェーズの間、平均27日間(3から30日の範囲で)ジコノチドにさらされた。
Infumorph (保存条件無しの硫酸モルヒネ殺菌溶液;Elkins Sinn, Inc., Cherry Hill, NJ) がモルヒネ濃度10または25mg/mlのどちらかの濃度で20mlガラスアンプルで供給された。Infumorphの用量上昇スケジュールは個々の患者の全身アヘンの1日投与量に従い調節した。低用量レジメでは、100mg/日以下の経口モルヒネ相当量を受ける患者は0.25mg/日で開始し、2週、3週、4週とそれぞれ、0.5,1.0,2.0mg/日と上昇させた。中間用量レジメでは、100−300mg/日の経口モルヒネ相当量を受ける患者は0.5mg/日で開始し、2週、3週、4週とそれぞれ、1.0,2.0,3.0mg/日と上昇させた。高用量レジメでは、300mg/日以上の経口モルヒネ相当量を受ける患者は1.0mg/日で開始し、2週、3週、4週とそれぞれ、2.0,3.0,4.0mg/日と上昇させた。Infumorphの用量増加は、耐えられない逆のことが起こったとか、あるいは有意に改善された無痛感覚がえられたならば、中止した。
効果の測定は、VASPIスコア、Categorical Pain Relief Scale(CPRS), Clinical Global Impression (CGI) およびWeekly Systemic Opioid Consumputionを含む。
ジコノチドとモルヒネの組合せ髄腔内投与の効果を評価するために、VASPI (Handbook of Pain Assessment(2nd eddition) D. C. Turk and R. Melzack, Eds, Guilford Press, New York, 2001)を用いてその時点の疼痛強度を評価した。患者はベースライン(基準線)評価のために、疼痛の連続体(即ち、無痛から最悪な疼痛まで)を表す100−mmライン上の患者の現在の疼痛強度を評価した。連続体の左側は、スコア0に相当し、連続体の右側は、スコア100に相当する。各研究診察において、患者は上記と同じ連続体を用いて自己の現在の疼痛強度を評価するように要求された。試験の第1の疼痛効果の評価は、ベースライン測定で比較した併用治療の場合と単独髄腔内投与鎮痛剤(ジコノチド)の場合のVASPIにおけるパーセント変化であった。
合計24人の患者がこの試験に参加した。22人の患者(92%)が4週処置フェーズを完了し、2人の患者(8%)は副作用のため早期に中止した。
患者の基準
患者はモルヒネ1日2および20mg/日の範囲での髄腔内投与量の男性、女性の患者であった。患者のいずれかは、>40mmのVASPIスコアで表示される亜最高疼痛リリーフを有するか、あるいはモルヒネで緩和されず、モルヒネで緩和される疼痛と異なった性質(例えば侵害受容の疼痛)の残留性疼痛を有していた。
ジコノチドは100μg/mlの濃度の5ml単投与量バイアルで供給された。ジコノチド投与量は、ベースラインにおいて0.025μg/時(0.6μg/日)から出発して、研究の処理期間を通じ、毎週増量した。患者はその後毎週観察され、次のように増量したジコノチドが投与された;診察2(7日目)で0.05μg/時(1.2μg/日)、診察3(14日目)で0.10μg/時(2.4μg/日)、診察4(21日目)で0.20μg/時(4.8μg/日)、診察5(28日目)で0.30μg/時(7.2μg/日)。ジコノチドの増量投与は、耐え難い副作用が生じたとき、有意な改良鎮痛効果が得られたとき、あるいは最終投与量が0.30μg/時に達したときに、中止した。最終でジコノチドの平均蓄積投与量は、74.4μg(8.8から118.6μgの範囲)であり、そして第5週の平均投与量は0.197μg/mlであった。平均して患者は合計31.5日間(14から35日間)ジコノチドにさらされた。
Infumorph(保存料なしの硫酸モルヒネ滅菌溶液;Elkins Sinn, Inc., Cherry Hill, NJ)は10mgあるいは25mg/mlのいずれかのモルヒネ濃度の20mlガラスアンプルで供給された。Infumorphの用量は、研究の併用処理を通じて安定であり、そして2から20mg/日の範囲であった。
効果の測定は、VASPIスコア、CPRS、CGIおよび週単位全身アヘン消耗を含む。
統計的効果の分析
組合せ髄腔内投与のジコノチドとモルヒネの効果を評価するために、患者は実施例13に記載したVASPIを使用した現在の疼痛強度を評価した。試験の第1の疼痛効果の評価は、ベースライン測定で比較した併用治療の場合と単独髄腔内投与鎮痛剤(モルヒネ)の場合のVASPIにおけるパーセント変化であった。
合計22人の患者がこの試験に参加した。16人の患者(73%)が5週処置フェーズを完了し、6人の患者(27%)は副作用のため早期に中止した。
患者の基準
患者はバクロフェン(bacrofenかLIORESAL(登録商標)のいずれか)を1日22および800mg/日の範囲での髄腔内投与量の男性、女性の患者である。患者は、選択診察およびベースライン診察時、最少40mmのVASPIスコアで表示される疼痛および亜最高疼痛リリーフを有する。
全ての患者は、ベースライン診察の少なくとも7日間の間、LIORESAL(登録商標)(between 22 and 800 g/d)、全身投与アヘン、および他の付随的薬剤の安定用量でなければならない。
SynchroMed(登録商標)EL Infusion Systemの内容が除去され、PRIALT(商標)およびLIORESAL(登録商標)に置き換えられる。LIORESAL(登録商標)の用量はスクリーン期間中の最後の7日間と実験の最初の9週間を通じて、同じ用量を維持する。PRIALT(商標)の最初の用量は0.025ug/hr(0.6ug/日)である。
最初のPRIALTの用量は、ベースライン(0日)から始め、1週間0.025μg/hr(0.6μg/日)である。処置1週間後ポンプを再充填し、用量を0.05μ/hr(1.2μg/日)に増量する。処置3週後ポンプを再充填し、用量を0.10μ/hr(2.4μg/日)に増量する。処置5週後ポンプを再充填し、用量を0.15μ/hr(3.6μg/日)に増量する。処置7週後ポンプを再充填し、用量を0.20μ/hr(4.8μg/日)に増量する。最終週の間、用量を0.20μ/hr(4.8μg/日)に維持する。用量の増加は、増量中止基準に遭遇するまで行う。
LIORESAL(登録商標)の用量はスクリーニングの間に確立した安定用量を一定に保ち、変えなく、そして少なくとも22μg/日で、800μg/日を超えない。
ポンプ流量率は一定に保たれ、少なくとも12mcl/hr(288mcl/日)で1または2日以内でポンプ量が空になるまで続ける。薬物の用量は、これは両者の化合物の注入率を変えるので、ポンプの流量率を変えることにより調整されることはない。
VSPIは各臨床診察で決められる。CPRSは診察7/最初の終了診察および可能ならば延長フェーズの終了診察で決められる。CGIは、診察7/最初の終了診察および可能ならば延長フェーズの終了診察で決められる。
痙直スコア(修正Ashworth Scale:0-4正常から強直状態;痙攣scale:0-4 痙攣無しから1時間10回以上;Penn Spasm Frequency Scale, VASS(Visual Analogue of Spasticity Scale) が各臨床診察で決定される。
初期効果変数は、ベースライン診察から診察7/早期終了診察までのVASPIの百分率変化を含む。

Claims (20)
- ω−コノペプチドと、モルヒネ、ブピビカイン、クロニジン、ハイドロモルホン、バクロフェン、フェンタニル、スフェンタニルおよびブプレノルフィンからなる群から選ばれる鎮痛剤、またはその薬学的に許容される塩を含有する医薬製剤であって、該ω−コノペプチドがその有効性を維持し、該鎮痛剤と物理的にも、化学的にも共存できる該医薬製剤。
- 該ω−コノペプチドがジコノチドである請求項1に従う方法。
- 該医薬製剤が髄腔内投与に適した製剤である請求項1に記載の製剤。
- 該医薬製剤が継続的髄腔内注入に適した製剤である請求項3に記載の製剤。
- 該医薬製剤が薬剤投与用植込み式ポンプ中37℃で少なくとも7日間安定である請求項1に記載の製剤。
- 該鎮痛剤がモルヒネ、クロニジン、ハイドロモルホン、フェンタニルまたはスフェンタニルである請求項1に記載の製剤。
- 該鎮痛剤がバクロフェンまたはブピビカインである請求項1に記載の製剤。
- ω−コノペプチドの有効量と、ハイドロモルホン、ブプレノルフィン、フェンタニルおよびスフェンタニルからなる群から選ばれる鎮痛剤、またはその薬学的に許容される塩の有効量を患者に投与することからなる患者の疼痛軽減方法。
- 該ω−コノペプチドがジコノチドである請求項8記載の方法。
- 該ω−コノペプチドと該鎮痛剤が患者に投与前に混合される請求項8記載の方法。
- 該投与が髄腔内投与である請求項8記載の方法。
- 該髄腔内投与が継続的髄腔内注入である請求項11記載の方法。
- ω−コノペプチドの有効量と、ブピビカイン、クロニジンおよびバクロフェンからなる群から選ばれる鎮痛剤、またはその薬学的に許容される塩の有効量を患者に投与し、その際に該投与法が継続的髄腔内注入することからなる患者の疼痛軽減方法。

- 該投与が継続的髄腔内注入である請求項16記載の方法。
- 髄腔内アヘン誘導肉芽腫形成に敏感な患者に、ω−コノペプチドを該患者の髄腔内に投与することからなる患者の疼痛軽減方法。
- 更にブピバカイン、クロニジンおよびバクロフェンからなる群から選ばれる鎮痛剤、またはその薬学的に許容される塩の有効量を患者に投与し、その際に該投与法が継続的髄腔内注入することからなる請求項19記載の方法。
