Abstract
Description
本願は米国特許願第07/866,691号(1992年4月10日出願、係属中)の一部継続出願であり、その内容をここに本明細書の一部として援用する。
免疫応答
免疫応答においては、究極の標的は抗原(バクテリアまたはその他の侵入物)である。マクロファージ等の抗原細胞は抗原を摂取し、抗原ペプチドへと断片化する。これらのペプチドの一部分は互いに結合して主要組織適合遺伝子複合体(MHC)分子を形成し、それらが細胞表面上に発現する。T−リンパ球は、MHC分子と結合した非天然ペプチドを認識することができるレセプターを有する。T−細胞は活性化されてリンフォカインまたは化学信号を分泌し、それらが免疫系の他の成分を起動させる。そうして起動される細胞の殆どはB−リンパ球である。B−リンパ球は、さまざまな体液中の抗原の一部分を認識するが、これらの抗原はMHC分子とは結合していない。T−細胞は抗原全体を認識することはできない。T−細胞上のレセプターは抗原の蛋白質フラグメント、即ち通常は8から15のアミノ酸で構成されたペプチド類を認識する。
ウイルス感染においては、ウイルスは突然変異によってその外層膜を急速に変化させることができるようで、それによって抗体による中和を防ぐ。しかしウイルスがその核の中にウイルス自体の生存プロセスに不可欠な蛋白質類を含む場合は、そうした突然変異が起る余地はない。ウイルスが細胞中で複製されると、ウイルスの蛋白質から短いペプチド鎖が離れ落ち、細胞表面に移動する。それらはT−細胞の成熟標的となるので、T−細胞は感染細胞を攻撃できることとなり、ウイルスの広がりを防止するか、あるいは自己免疫疾患の場合に起きるように体の細胞を攻撃する。
自己免疫疾患は、T−リンパ球がMHCの自己クラスII分子に結合した自己抗体を認識して活性化された場合に起きる。自己免疫疾患の基本的な特徴の1つは、免疫系が身体の自己成分と他の外部侵入物の成分とを区別することができず、したがって体の免疫系が自分自身に向かうことにある。一卵性双生児の研究で認められるように、これには遺伝的要素が明らかに関与する。多発性硬化症のケースでは、双子の一方が多発性硬化症を発症した場合、もう一方が発症するチャンスは約30%であることが示されている。
また同一人に一以上の自己免疫症が起きる傾向もある。こうした場合、この関連性は自己免疫スペクトルの同一領域中の疾患同士であることが多い。例えば、自己免疫性甲状腺炎を発症している患者の悪性貧血発症例は、相当する年令および性別のランダム集団の検査における発症例よりもかなり多い。
自己免疫現象はまた、家系に集中する傾向もある。橋本病や悪性貧血の患者は、彼らの一親等の親族内に甲状腺および胃炎の自己抗体を有する例が多い。自己免疫疾患を純然たる臓器特異性のものから臓器特異性の低いものへと順にならべると、次に示すような自己免疫疾患スペクトルが得られる。
臓器特異性から臓器非特異性へと降順にならべた場合、次のような自己免疫疾患が含まれる:橋本甲状腺炎、原発性粘液水腫、甲状腺中毒症、悪性貧血、自己免疫萎縮性胃炎、アジソン病、未熟閉経、男性不妊症、若年型糖尿病、グッドパスチュア症候群(Goodpasture's Syndrome)、交感性眼炎、水晶体性ぶどう膜炎、多発性硬化症、乾癬、自己免疫溶血性貧血、特発性血小板減少性紫斑病、特発性白血球減少症、原発性胆汁性肝硬変、活性慢性肝炎HBs−ve、特発性肝硬変、尋常性天疱瘡、腫瘍性大腸炎、シェーグレン症候群、連鎖球菌感染後の糸球体腎炎、リューマチ性関節炎、硬皮症、ワグナー肉芽腫症、多発性/皮膚筋炎。
HIVのライフサイクルには次のステップが含まれる:
付着、脱外皮、逆転写、RNA−アーゼHの分解、第2の鎖のDNA合成、核への移行、組込み、潜伏、ウイルス転写、RNAの核輸送、蛋白質合成、RNA安定化、蛋白質のグリコシル化、RNAの折畳みとビリオンの組立て、ウイルス放出、および成熟。
これら全過程において最も厄介な部分は、HIVは現在までヒトで試験されたあらゆる抗レトロウイルス剤をすり抜けることができる形に突然変異できることである。その結果、多くの研究者はHIVを打ち負かすためには、HIVといえども複数の薬剤の組合わせに対しては突然変異で十分素早く耐性を有するに至ることはできないであろうという理論のもとに、HIVに対して同時にいくつかの薬剤をぶつけなければならないと結論づけている。さらに良い方法はHIVに対して複数の標的を有する単一の薬剤を使用することであろう。
最近の非常に有望な方法の1つはプロテアーゼ抑制剤を使用するものである。プロテアーゼ抑制剤は、HIV複製の後期組込みステップで作用するものであるため、治療的な介入物として魅力的な研究対象である。逆転写酵素抑制剤がHIV感染前の培養細胞に添加した場合のみHIV複製を阻止するのに有効であるのに対して、プロテアーゼ抑制剤は慢性感染細胞からのHIV産生を抑制することができる。
ウイルス侵入阻止:HIVの侵入はHIVgp120膜蛋白質が敏感な細胞表面上のCD4分子と非常に特異的に結合することにより始まる。さらに、シンシチウムの形成およびHIVの細胞から細胞への伝播では、未感染細胞表面上でgp120とCD4の結合が起きる。CD4レセプターの可溶性リコンビナント(sCD4)またはsCD4の血清寿命半減時間を延ばすように設計されたキメラCD4−免疫グロブリンG(IgG)は、臨床的に得られるレベルで培養細胞中のHIV感染とシンシチウム形成を効果的に阻止することが示されている(非特許文献2−3)。
核酸および免疫再構築など、異なる進展段階でHIV複製を阻止するその他の標的も多く存在する。
アルツハイマー病、パーキンソン病ならびに筋萎縮性側索硬化症(ALS)の神経変性障害の原因はわかっていない。これらの障害はそれぞれ主として、ペプチド性神経伝達物質(可能性としてはサブスタンスP)が失われることにより引き起こされるものであるとされている。この喪失が今度はこれら疾患のそれぞれで見られる古典的な神経変性を創出する。この神経変性は、幼少期におけるCNSのウイルス感染に続く自然の老化と長期にわたる自己免疫崩壊が組み合わされた結果起こるものである。したがって上記の喪失は徐々に起こり、臨床症状が見られる時迄には抗原刺激が取り除かれるに伴って炎症プロセスは消失してしまう(非特許文献11)。
特発性振せん麻痺(パーキンソン病)の病因に関するフリーラジカル仮説は自己免疫メカニズムに立脚した議論と多くの類似点がある。両者とも、神経破壊に関与すると思われる細胞性および分子性のからくりを提示している。最近では、フリーラジカル仮説が特に優勢になっている。何人かの研究者が、フリーラジカルによる損傷が疾患のプロセスにひそむ大きな要因であるとする説を支持する観察結果を報告している。(非特許文献14)。
ラムノリピドはバイオ界面活性を有するグリコリピドの一群である。炭水化物と脂質の異なる組合わせ、異なる結合および異なるイオン状態によって、親水性/親油性バランスの大きく異なる多くの糖脂質(グリコリピド)がある。シュードモナス属(Psedomonas)の種々の菌株は、溶解性または不溶性の炭素源を用いて培養した場合に、ラムノリピドを細胞外に分泌する能力があることが知られている。
ラムノリピド類は全体として、また特定の種々のラムノリピド化合物としても、ある種の生物学的活性を有することは知られているが、今日まで誰もラムノリピド類を自己免疫疾患の治療に使用することは示唆しておらず、とりわけ臓器特異性(または非特異性)自己免疫疾患、エイズ、アルツハイマー病、パーキンソン病、あるいは筋萎縮性側索硬化症に対するラムノリピド類の有効性については全く示唆されていない。
非特許文献21は、化粧品、医薬品および食品に有用なグリコリピドなどを含む乳化組成物について述べている。但しそのような組成物中でラムノリピドが薬理学的活性を持つとは何ら説明されていない。
ヒラヤマ[Hirayama]らは、非特許文献22において、緑膿菌株から単離された新規なメチル−ラムノリピド類は、抗菌、殺マイコプラズマ、および抗ウイルス活性を有する生物学的活性物質であることを開示している。
特許文献4は、例えばポテト−X−ウイルス、またはタバコモザイク病ウイルスに対する抗植物性ウイルス治療法の可能性に関するものである。
ハフェルブルグ[Haferburg] らは、非特許文献23において、緑膿菌株196Aからのラムノリピドが過敏な宿主Nicotiana glutinosa Lの葉の上のタバコモザイクウイルスの局所病変の数を最大90%減少させることを示している。ムラサキツメクサ斑点ウイルスも同じ程度にラムノリピドによる影響を受けている。
特許文献6において、ピリャック[Piljac],G.らは乾癬および関連する皮膚疾患の治療におけるラムノリピド類の局所適用について開示している。
特許文献7において、ピリャック[Piljac],G.らはラムノリピド類の表面張力および界面張力特性をベースとして、異なる産業への応用の可能性を開示している。
したがって本発明の目的は、一以上のラムノリピド類を活性成分として含む組成物を投与することにより、自己免疫疾患を治療する方法を提供することにある。
本発明のさらに別の目的は、一以上のラムノリピド類を活性成分として含む組成物を、免疫回復を必要とする患者に投与することによって免疫を回復させる方法を提供することにある。
本発明の別の目的は、一以上のラムノリピド類を活性成分として含む組成物を使用して、臓器特異性および臓器非特異性自己免疫疾患を治療する方法を提供することにある。
本発明の別の目的は、一以上のラムノリピド類をそれらを必要とする患者に投与することによって、エイズを治療する方法を提供することにある。
R2はHまたは−CH(R4)−CH2−COOH;
R3は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビル;
そして R4は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビルである]が免疫活性を与え、また免疫回復および免疫調節に有効なものであり、したがって臓器特異性および臓器非特異性自己免疫疾患、エイズ、アルツハイマー病、パーキンソン病、ならびに筋萎縮性側索硬化症に有効な治療を与えるとの発見によって達成された。
(1) 式(I):
R2はHまたは−CH(R4)−CH2−COOH;
R3は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビル;
そして R4は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビルである]で表されるラムノリピド類の一以上を活性成分として含有する医薬組成物を、それらを必要とする患者に有効量投与することを特徴とする自己免疫疾患用医薬組成物。
(8) 式(I)で表わされる一以上のラムノリピドにおいて、R3が−(CH2)x−CH3(ここで、xは4〜20)である前記(1)記載の組成物。
(9) 式(I)で表わされる一以上のラムノリピドにおいて、R4が−(CH2)x−CH3(ここで、xは4〜20)である前記(1)記載の組成物。
R2はHまたは−CH(R4)−CH2−COOH;
R3は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビル;
そして R4は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビルである]で表されるラムノリピド類の一以上を活性成分として含有する、乾癬、魚鱗癬、尋常性天疱瘡、全身性エリテマトーデス、偏平紅色苔癬、エイズ、パーキンソン病、アルツハイマー病及び筋萎縮性側索硬化症からなる群から選択される疾患の治療に使用するための免疫回復用医薬組成物。
(14) 式(I)で表わされる一以上のラムノリピドにおいて、R3が−(CH2)x−CH3(ここで、xは4〜20)である前記(10)記載の組成物。
(15) 式(I)で表わされる一以上のラムノリピドにおいて、R4が−(CH2)x−CH3(ここで、xは4〜20)である前記(10)記載の組成物。
本発明は、式(I)のラムノリピド類の一以上を活性成分として含有する組成物を、それらを必要とする患者に治療有効量投与することにより、各種の自己免疫疾患を治療する方法に関する。
R2はHまたは−CH(R4)−CH2−COOH;
R3は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビル;
そして R4は(C5−C20)−飽和、モノまたはポリ不飽和ヒドロカルビルである]。
本発明の方法で使用される組成物において、好ましいラムノリピドはジラムノリピド(r1=α−L−ラムノピラノシル)である。
さらに、置換基R2がHの場合、ラムノリピドはただ1つのリピドグループを含有するだけであり、一方R2が−CH(R4)−CH2−COOHの場合は、ラムノリピドは1つのエステル結合により結合された2つのリピドユニットを有するものである。
置換基R3およびR4は直鎖、または分岐鎖C5−C20炭化水素基であり、これらは飽和あるいは一以上の不飽和部位を含むものとすることができる。好ましいのは、式−(CH2)x−CH3の直鎖飽和アルキル基で、x=4−20のものである。最も好ましいのは、直鎖飽和アルキル基でx=4−6のものである。
最も好ましいラムノリピドの実施態様は下記の式(II)の化合物である:
インビトロの試験結果、特に自己免疫疾患の治療に関するデータは、リポキシゲナーゼ、アセチルコリンエステラーゼ、ベータガラクトシダーゼ、カルボキシペプチダーゼAおよびトリプシンから選択した酵素の抑制作用と、DNA合成、細胞毒性、T−リンパ球増殖およびConA共存でのT−リンパ球増殖に対するラムノリピドの作用に関するデータを含んでいる。
インビボの試験結果、特に自己免疫疾患の治療に関するデータは、細胞の免疫抑制に対するラムノリピドの効果(オキサゾロン誘発の遅延型過敏性)と、免疫回復(シクロホスファミドとカンジダ感染での免疫抑制)、ならびに免疫調節(カンジダ・アルビカンス感染)に関するデータを含んでいる。
同じく以下に含まれるものには、いくつかの皮膚自己免疫疾患の治療に関する臨床データがあるが、これらは自己免疫疾患であると認定されている乾癬および偏平紅色苔癬に対して、コルチコステロイド類を使用する従来の治療法と比較した場合に、本発明のラムノリピドが優れた治療効果を有することを示すものである。

免疫薬理学的テスト結果では本発明のラムノリピドは、オキサゾロン誘発遅延型過敏性でみる細胞の免疫刺激性は示さないが、オキサゾロン誘発遅延型過敏性の細胞免疫抑制テストではラムノリピドによる中程度の免疫抑制作用を示した。体液の免疫刺激および免疫抑制テストでは何ら顕著な作用は示さない。カンジダ・アルビカンス感染による本発明のラムノリピドの免疫調節テストでは、濃度10mg/kgから1mg/kgの範囲のラムノリピド(II)が中程度の作用を示した。シクロホスファミドによる免疫抑制とカンジダ・アルビカンスによる感染に基づく免疫薬理学的テストでは、本発明のラムノリピドは10mg/kgまたはそれ以上の濃度であっても非常に強い免疫回復作用を示し、腹腔内投与では0.1mg/kgの濃度でも中程度の免疫回復を示した。
このラムノリピドは自己免疫疾患、アルツハイマー病、パーキンソン病、およびエイズで特定の役割を果たす種々の酵素に対して抑制効果を示した。リポキシゲナーゼ抑制剤(チェン[Chen],F.ら、Ophthalmic Research、1991)は、実験的な自己免疫性ぶどう膜の発達抑制に顕著な効果を示した。スパーニー[Spurney] ,R.F.ら(Kidney International、1991年1月)は、腎臓中のロイコトリエン産生の増加がループス腎炎の病原性に重要であろうということを示している。
ウォール[Wall],JRら(Journal of Endocrinological Investigation、1993年12月)は、β−ガラクトシダーゼとの融合蛋白質を開示しており、これは甲状腺が関与する眼障害、バセドウ病の甲状腺機能亢進の患者の29−43%、ならびに未治療の橋本甲状腺障害の患者で検出されたものである。β−ガラクトシダーゼの阻害はこの融合蛋白の生成を防ぐことができる。
ここに示した結果は、本発明のラムノリピドがある種の自己免疫疾患の進展に関与する種々の酵素に対して非常に顕著な抑制作用を有することを示しており、また非常に有望な免疫活性、特に免疫回復性を有し、同時に皮膚自己免疫疾患に対する臨床的な証明もしており、これらのことから活性成分として一以上のラムノリピドを含有する薬用組成物が、エイズ、パーキンソン病、アルツハイマー病および筋萎縮性側索硬化症などの臓器特異性および非特異性自己免疫疾患の治療に有効であることを示している。
菌株の特性
選択培地を使用して油井泥から、ラムノリピドバイオ界面活性剤を分泌する能力のある桿状のグラム(−)バクテリアを単離した。BBLRMinitekTM番号同定システムの「非発酵およびその他のグラム(−)バクテリア用インストラクション」(BBLR微生物学システム;1987)を適用することにより、このバクテリア株はタイプ番号646011の緑膿菌(P.aeruginosa)と同定された。上記の番号同定においてプロファイル番号を割り出した反応の他に、下記の増殖テストも行った:

P.aeruginosa産生株を好気的に15L(LKB)バイオリアクターで28−32℃の半連続条件(RFBC)で、または30L発酵器(Electrolux)を用いてバッチ方式で培養した。RFBC法のエアレーションは2.0−3.0L/分、ボルテックス混合は400−8,000rpmで、バイオリアクターの作業容量は泡があるためにバイオリアクター全容量の2/3を超えないようにした(LKB発酵器の場合は4.5−7.1Lとした)。バッチ法の場合は30L発酵器でエアレーションを5.0−30.0L/分、600−1,000rpmのボルテックス混合とし、Electrolux発酵器の作業容量は12Lとした。
培養培地は、グルコース9g、ペプトン5g、酵母抽出物2g、NaCl 5g、K2HPO4 0.5g、KH2 PO4 0.5g、MgSO4 2g、H2O、KNO3 3g、ゴットリーブ溶液1mLおよび水1リットルからなるものである。炭素源としてのグルコースをグリセリンに置き換えれば、結果的に二相システムとなり収率をさらに良くすることができる。代替培地としては、K2HPO4 0.5g、KH2 PO4 0.5g、MgSO4 0.2g、KNO3 2g、NaCl 1g、ゴットリーブ溶液1mL、H2O 1リットルおよび3%の不溶性炭素源(グリセリン、軽油)である。表面張力の減少(28−31mN/m)を収率および発酵終了の指標として使用した。
濃縮
発酵後、バイオマスを連続流シャープレス遠心分離器により60,000Gで室温で分離した。次いで得られた上清を下記のように処理した。
最初の容積を蒸発により当初の1/10に減らし、濃HClによりpH1.5−2.0で、+4℃で酸沈降させた。水に溶けやすい細胞外グリコリピドは、酸沈降時にpHと温度を変えることにより簡単に濃縮することができた。次に、フィルターペレットを水に再溶解させ、0.1mmフィルター、およびミリポア連続フローシステムにより106および105フィルター(PTHK000C5)で限外ろ過した。ラムノリピドは臨界ミセル濃度以上ではミセルを形成するため、限外ろ過により濃縮することができ、それら凝集物が比較的高分子の分離膜上に保持される。こうして塩、遊離アミノ酸、ペプチドおよび小蛋白質などの低分子量の不純物は容易に分離された。この方法は、大量の培地を非常に低コストで迅速に処理できるため、ラムノリピドの精製に非常に重要なものである。
濃縮はまた溶剤抽出によっても良好に行うことができ、たとえばジクロロメタンで満足に行える。このプロセスの後半で、酸による沈降処理後にCH2Cl2抽出を行う精製ステップをさらに加えることもできる。
カラムクロマトグラフィ
低圧の実験室規模の分離用(ファルマシア)吸着カラムで、アンバーライ トXAD−7、XAD−8樹脂(ローム&ハース)上でカラムクロマトグラフィを行った。平衡と水洗は水で行い、ラムノリピドは最大50%のEtOHまたはMeOHで溶離させた。その後有機溶媒を蒸発させ、ラムノリピド画分を酸で沈降させた。陰イオン交換カラムIRA−400(Clサイクル)も使用できるが、1M NaClによる溶離を行うために、あまり適当ではない。
培養液画分の上清、酸沈降物、ジクロロメタン抽出物、2回精製した沈降物および精製工程の種々の画分を、薄層クロマトグラフィ(シリカゲル60F254[ メルク] 0.2−0.5mm、および同じ担体(ケミカ)で調製された2mm厚の分離用プレート)に付した。プレート(幅0.21mm)を乾燥機中で1時間、90℃で活性化させ、続く30分間使用した。試料を、プロパノールと25%NH4OHが80:15との混合溶剤中で展開した。プレートに入れるサンプル量はサンプルのタイプと純度に応じて変えた。3時間処理後の平均Rf値は0.26であった。
スペクトル分析
薄層クロマトグラフィ後に調製物を1H NMR、13C NMRおよび質量分光分析計を使用して同定した。いくつかのデータを下に示す。

酵素阻害
ラムノリピド(II)をインビトロで広範囲の各種酵素に対してテストした。
下記の酵素は下記の各種濃度のラムノリピドにより阻害された:
−リポキシゲナーゼ 10μg/mL
−アセチルコリンエステラーゼ 1mg/mL
−α−アミラーゼ 1mg/mL
−β−ガラクトシダーゼ 100μg/mL
−カルボキシペプチダーゼA 1mg/mL
−トリプシン 1mg/mL
すべての酵素検定は、コンピュータ接続の自動マイクロプレートリーダー(Labsystem Multiskan )およびダイオードアレイ分光光度計(Perkin Elmer)により分光分析方式で行った。
−α−キモトリプシン

−エラスターゼ
−α−グルコシダーゼ
−アルカリホスファターゼ
テストは、D.A.ヴァンデンバーグ[Vanden Berghe] ら、Inst.Pasteur 84,101(1986)に基づいて行った。ラムノリピド(II)はベロ(VERO)細胞については濃度≧200μg/mLで毒性があり、MT4細胞については濃度>169.40μg/mLで細胞毒性があった。以下のものに対しては活性はなかった:
−単純ヘルペスウイルス
−コクサッキーB2ウイルス
−麻疹エドモンストンAウイルス
−ポリオIウイルス
−セムリキ森林熱ウイルス
−水疱性口内炎ウイルス、濃度≦100μg/mL
HIV I、菌株IIIBに対しては、ラムノリピド(II)は最大保護9%にとどまった。
カンジダ・アルビカンス、大腸菌、黄色ブドウ球菌および緑膿菌に対するラムノリピド(II)の最小阻止濃度(MIC)はテストした培地では>0.5%であった。
ラムノリピド(II)は、濃度0.1mg/mLで、血清成長因子が存在しない場合に、A431ヒト表皮細胞におけるDNA合成を完全に抑制した(表2)。濃度0.5mg/mLで血清が存在する場合は、ラムノリピド(II)は細胞毒性があることが判明した(表3)。このラムノリピド(II)は、フォルボール−ミリステート−アセテート(PMA)、プロテインキナーゼCアクチベーター(PKC)の細胞“抑制”作用を0.05mg/mLでも弱めた。このように、ラムノリピド(II)はA431細胞の成長を、特にPMAが存在する場合に抑制した。これは本発明のラムノリピドの調製における良好な“表皮ケラチン細胞”抑制作用を意味するものである。本発明のラムノリピドとPMAの相乗作用で、ラムノリピド(II)がPKC抑制剤となる可能性は少ない。
ラット血小板が多い血漿におけるコラーゲン誘発の血小板凝集は0.1mg/mLのラムノリピド(II)には影響されなかった。したがって、ラット血小板(PKCを含む)で起きるような細胞活性化システムへの干渉は見られなかった。
エイムス N.ブルース[Ames N.Bruce]らが記載する方法(MutationResearch,31(1975),347−364))に基づいてテストを行った。ラムノリピド(II)のテスト濃度は、10、100、200、500、1000μg/プレートとした。
テストしたラムノリピドの濃度範囲では、1ペトリ皿当たりのバクテリア復帰変異体の数は、S9の混合がない場合に見られる自然発生の復帰変異体数の範囲内であった。したがって、ラムノリピド(II)はテストした濃度範囲では変異原性はなかったと結論づけられる。
平板効率の測定と世代時間を測定した。テストはCHO細胞系で行った。世代時間(GT)平板効率(PE)は下記の通りであった:
GT=16時間
PE=90%
非同時性細胞系(CHO細胞)の集団増殖をフレッシュニーR.イアン[Freshney R.Ian]の“Culture of animal cells”,1983;(125−128)に基づいて行った。下記濃度のラムノリピドをCHO細胞の増殖培地に添加した:31.25μg/mL、62.50μg/mLおよび125.0μg/mL。適用したラムノリピド濃度では、CHO細胞の増殖能力は大きくは変化しなかった。
種々の濃度のラムノリピド(II)で処理した後のCHO細胞のコロニー形成能力を、キルビーB.J.[Kilbey B.J.] らの“Handbook of mutagenicity test procedures”,1977;(68および175)に基づいてテストした。下記濃度のラムノリピド(II)をテストした:0、8、16、32、62.5および125μg/mL。対照群と比べて上記濃度のラムノリピド(II)ではコロニー形成能力の顕著な喪失は観察されなかった。
構造的染色体異常分析(Biological Dosimetry 1986,Technical;Reports Series,20,International Atomic Energy Agency,ウィーン,59頁)。
異なる濃度のラムノリピド(II)を2時間目および16時間目に投与した(31.25、62.50、125および250μg/mL)。分裂中期(メタファーゼ)の400が明らかに観察され、各標本ごとに20の染色体(異数体)を分析した。31.25μg/mLの濃度が、無処理の対照標本に比べて多くの構造的染色体異常を引き起こしたことが明らかであった。同じインキュベーション時間での62.5μg/mL濃度のものは染色体異常の傾向が少なく、最も高濃度の2つについては対照群とほぼ同じ値であった。テストした物質では16時間のインキュベーション後に顕著に異なった状態が観察された。全体的な構造的染色体の損傷が数倍になり、異常な細胞の数も2から4倍に増加した。しかしながら、この場合も濃度が増加すると異常の数が減少する点が注目される。
いわゆるスパイラル欠陥と呼ばれる現象は、ラムノリピド(II)添加のCHO細胞の2時間インキュベーションでは認められなかった。
小核テスト(フレンチM.[French M.] およびA.A.モーリー[A.A.Morley](1986),Mutation Res.,161:193の方法の応用)。ラムノリピド(II)の濃度31.25μg/mL、62.50μg/mL、125μg/mLおよび250μg/mLでの小核テストの結果は、明らかにテスト化合物と細胞内物質の相互作用を示した。小核の数は平行対照標本に比べて増加した。ラムノリピド添加CHO細胞の16時間インキュベーション後には、二核細胞内の小核の数は4まで増加した。
この方法は、カトウ[Kato],H.,Nature(1974)252:70を応用したものである。ラムノリピド(II)のテスト濃度は、31.25μg/mL、62.50μg/mL、125μg/mLおよび250μg/mLとした。本法による試験結果では対照群と処理培養細胞群の間には顕著な差はなかった。
本法は、キルビーB.J.[Kilbey B.J.] らの“Handbook of mutagenicity test procedures”,1977:250および267を応用したものである。濃度31.25μg/mL、62.50μg/mL、125μg/mLのラムノリピド(II)へのCHO細胞の2時間および16時間曝露を30回繰り返した後の染色体異常の全体的な発生頻度およびタイプは対照サンプル群と顕著な差はなかった。
ラムノリピド(II)の溶血活性は赤血球が沈降した後、ヘモグロビンが溶液を着色した場合に見られる。もし溶血活性がなければ、赤血球は溶液を着色せずに沈降する。テストの目的のために1mLのクエン酸ウシ血液を49mLの等張液に添加した。
濃度1.0mg/mLから0.01mg/mLの範囲の一連のテストサンプルを調製した。1mLの希釈クエン酸ウシ血液を1mLの希釈テストサンプルに添加した。試験管を静かに振とうして混合させ、それぞれ0.25時間、1時間および24時間放置した。15分間で溶血を起こしたラムノリピド(II)の最小濃度は0.35mg/mLであり、1時間では0.25mg/mL、24時間では0.125mg/mLであった。
B−リンパ球増殖
方法:常法によりマウス脾臓からB細胞を単離し、DMEMに懸濁させた。106個/mLの細胞を37℃で1晩インキュベートし、細胞増殖の刺激性(プラス評点、1μg/mLリポ多糖に関して)または抑制性(マイナス評点、ブランクの対照と比較)を評価した。テスト物質はこれらの条件下で、濃度10、1、0.1、0.01、および0.001μMで評価した。1晩インキュベートした後、2uCi[3H]チミジンの取込み量を液体シンチレーション計測器を用いて測定評価した。
方法:常法によりマウス脾臓からB細胞を単離し、DMEMに懸濁させた。106個/mLの細胞を10μg/mLのリポ多糖(LPS)の存在下37℃で1晩インキュベートし、細胞増殖の刺激性(プラス評点、1ng/mLのラットインターロイキン−5に関して)または抑制性(マイナス評点、対照と比較)を評価した。テスト物質はこれらの条件下で、濃度10、1、0.1、および0.01μMで評価した。1晩インキュベートした後、2uCi[3H]チミジンを各ウェルに添加した。さらに48時間インキュベーション後に細胞を採取し、チミジン取込み量を液体シンチレーション計測器を用いて測定評価した。
方法:常法によりマウス胸腺からT細胞を単離し、DMEMに懸濁させた。5×106個/mLの細胞を37℃で1晩インキュベートし、細胞増殖の刺激性(プラス評点、3μg/mLのコンカナバリン(ConA)に関して)または抑制性(マイナス評点、ブランクの対照と比較)を評価した。テスト物質は濃度10、1、0.1、0.01、および0.001μMでこれらの条件下で評価した。1晩インキュベートした後、2uCi[3H]チミジンを各ウェルに添加した。さらに48時間インキュベート後に細胞を採取し、チミジン取込み量を液体シンチレーション計測器を用いて測定評価した。
方法:常法によりマウス胸腺からT細胞を単離し、DMEMに懸濁させた。5×106個/mLの細胞を、3μg/mLのConAの存在下で37℃で1晩インキュベートし、細胞増殖の刺激性(プラス評点、10U/mLのラットインターロイキン−2に関して)または抑制性(マイナス評点、対照と比較)を評価した。テスト物質は濃度10、1、0.1および0.01μMでこれらの条件下で評価した。1晩インキュベートした後、2uCi[3H]チミジンを各ウェルに添加した。さらに48時間インキュベート後に細胞を採取し、チミジン取込み量を液体シンチレーション計測器を用いて測定評価した。
免疫薬理学テスト
免疫刺激(細胞性)−オキサゾロン誘発遅延型過敏性
予め剃毛した5匹のマウス群の腹部表面にオキサゾロン(5%溶液0.1mL)を塗布(感作)する1時間前に、テスト物質を腹腔内投与した。7日後にオキサゾロン2%溶液25μLを右耳に塗布(誘発投与)し、賦形剤を左耳に塗布した。24時間後にそれぞれのマウスを屠殺し、耳の厚さをDyer型マイクロメーターゲージで測定した。30パーセント以上増加したものは免疫刺激活性があると考えられる。
5匹のマウス群の剃毛腹部表面に5%オキサゾロンを0.1mL塗布して感作
した。1時間後にテスト物質を腹腔内投与し、連続5日間(毎日1回)投与した。
さらに4日後に、各マウスの右耳に5%オキサゾロン25μL塗布による誘発投与を行った。24時間後に、耳の厚さをDyer型マイクロメーターゲージで測定した。
対照動物と薬剤処理動物の比較で、50パーセントまたはそれ以上の減少があったものは、顕著な免疫抑制活性があると考えられる。
6匹のマウス群に2%SRBC懸濁液を0.2mL静脈注射で投与して感作した。感作後9日目に血液を眼窩洞から抜き取り、また6匹のマウスから同量の補体不活性血清を抜き取り、単一の0.25mLサンプルを作るためにプールした。補体添加の連続2倍希釈液を10回繰りかえし調製した。血清価は、完全な溶血を示す希釈率の逆数で表わした。血清価が16以下または128以上のものは有意であると考えられ、それぞれ顕著な免疫抑制または免疫刺激活性を示す可能性がある。
免疫刺激活性を調べるために、テスト物質または賦形剤をマウス群に3日連続で腹腔内投与し、3回目(最終)注射の2時間後にSRBCに感作させた。
免疫抑制活性を調べるために、SRBCへの感作2時間後から、テスト物質または賦形剤をマウス群に3日連続で腹腔内投与した。
シクロホスファミドによる免疫抑制とカンジダ・アルビカンスによる感染
テスト物質または賦形剤を10匹のマウス群に、1、3、および5日目に腹腔内投与し、シクロホスファミド(25mg/kg p.o.)を2、4、および6日目に投与した。最後の免疫抑制剤投与の1日後に、賦形剤処理対照群が10日以内に90から100パーセント死亡率となるのに十分な量のカンジダ・アルビカンス懸濁液を、マウスに誘発投与した。薬剤処理群で生存率が30パーセント以上大きいものはすべて有意であると考えられ、免疫回復活性を示す可能性がある。
カンジダ・アルビカンス感染
テスト化合物または賦形剤を10匹のマウス群に、賦形剤処理対照群が10日以内に90から100パーセント死亡率となるのに十分な量のカンジダ・アルビカンス懸濁液を誘発投与する1時間前に、腹腔内投与した。処理群の中で生存率が30パーセント以上大きいものはすべて明らかに有意であり、免疫調節の結果と考えられる。一方20%の増加は中程度の免疫調節と考えられる。
ラット骨髄細胞における構造的染色体異常分析の結果は、ラムノリピド(II)の濃度31.5mg/kg(動物体重)では染色体の損傷を引き起こさないことを示した。対照および処理サンプル群において、分析した細胞中僅か1%だけが、染色体の個別の破壊および無動原体フラグメントを伴った染色分体が見られた。そのような結果は十分に有意であるとはいえず、テクニカル誤差であり、かつラムノリピド(II)と骨髄DNAの相互反応によるものではないと見なされる。
この検討の目的は、LD90、LD50、LD10の決定、ならびに1回の静脈投与、5日間で5用量(1日1用量)の腹腔内投与、および1日5用量の腹腔内投与によるマウスの標的臓器毒性を決定することにあり、これによって薬剤の毒性を推定し、またげっ歯目の発癌性検討のための初期用量を決定できるようなデータをげっ歯動物で得ることにある。
テストはマウスC57B1/6(動物出所:BantinおよびKingman)で実施した。このテストは、米国Health and Human Services省のNational Toxicology Programのテクニカル・レポート・シリーズに記載の方法に準拠した。
下限用量(LBD)は67.5mg/kg、上限用量(UBD)は127.5mg/kgであった。
テストに供されたラムノリピドを静脈投与した結果、マウスの平均LD50は105.0mg/kgということも示された。
範囲探索検討フェーズ(1用量)において、ラムノリピド(II)の静脈内投与における1用量毒性は7.5mg/kgであった。ラムノリピド(II)の投与4日後に、好中球の顕著な増加とリンパ球の顕著な減少が観察された。観察期間中には処理マウスには行動および外観の変化は見られなかった。
下限用量(LBD)は45mg/kg、上限用量(UBD)は125mg/kgであった。
範囲探索検討(5日間5用量、1日1用量)において、ラムノリピド(II)の腹腔内投与における1用量毒性は5mg/kgであった。ラムノリピド(II)の投与4日後に、好中球、好塩基球、好酸球の顕著な増加と、リンパ球の顕著な減少が観察された。観察期間中には処理マウスには行動および外観の変化は見られなかった。
下限用量(LBD)は45mg/kg、上限用量(UBD)は125mg/kgであった。
範囲探索検討(1日5用量)において、ラムノリピド(II)の腹腔内投与における5用量毒性は5mg/kgであった。ラムノリピド(II)の投与4日後において、ラムノリピド(II)と対照の間には統計的な差異は見られなかった。
すべての試験において、各マウスの死体解剖で得た下記組織を検査した:骨(大腿骨)、骨髄(大腿骨)−滑動部のみ、脳、結腸、十二指腸、心臓、腎臓、肝臓、肺(ホルマリン漬け)、膵臓、脾臓、筋肉、副腎結節、子宮、腸管。
結論として、すべての対照および実験動物のすべての器官に関する完全な組織病理学報告書において、実験グループには実験物質に起因すると考えられる退行性−進行性または炎症性の変化が全く認められなかったことが示された。
男性11名、女性6名の17名の患者をラムノリピド(II)で治療した。これらのうち、12名は乾癬、2名は神経皮膚炎、1名は偏平紅色苔癬、1名は脂漏性皮膚炎、そして1名は湿疹様の限局単房性皮膚炎を患っていた。
患者の年令は19から80才の範囲であった。
乾癬の患者は以前数回治療を受けており、この疾患を1.5年から25年患っている。
乾癬の最初の患者6名の実験的治療は1991年5月から6月にかけて開始した。
すべての患者は、選択した乾癬箇所にラムノリピド(II)を含むクリームを1日2回、21日間、塗布して治療した。
身体の残りの乾癬箇所はコルチコステロイドで局所治療した。
患者の治療および追跡は“二重盲検”で行った。
治療の有効性はPASIインデックスでモニタした。
最初の6名の乾癬患者で改善方向の結果が得られたため、ラムノリピドを乾癬のより広い箇所(患者の身体の乾癬病変箇所の半分まで)に適用して年末まで治療し、観察を続けた。また、閉鎖包帯も使用(48時間まで)し、その使用期間は35日まで延長された。
このような方法により、患者が病院を離れるまでに良好な結果が観察され、病状再発も少なくなった。何人かの患者は治療後10ヵ月でさえも再発は見られなかった。
研究室テストのための血液サンプルは、治療初日と最終日に12名の患者すべてから採取された。多くの研究室テストが実施されたが、本発明のラムノリピド(II)による乾癬病変部の局所治療の結果、前記と異なる試験結果は見られなかった。
患者は治療後3、6および9ヵ月後に診察し、ラムノリピド(II)を適用した同じ箇所には他のいかなる局所剤も塗らないように指示した。
追跡後の結論は、ラムノリピド(II)を適用した乾癬病変部すべてにおいて良好な結果が得られたということである。この物質の治療効果は、殆どのケースにおいて、局所コルチコステロイドが効果を現わすより早いか、または同じ時期であった。再発はラムノリピド(II)治療10ヵ月後というように極めて遅いのに対し、局所コルチコステロイド治療の患者はなお緩解期の状態にある。
再発は、乾癬局所に対するラムノリピド(II)治療の期間の長さに関係するものと思われる。
神経皮膚炎の2名の患者は1992年初めに治療した。ラムノリピド(II)を1日2回病変部に適用し、閉鎖包帯も使用(24時間以内)した。
患者の治療および観察はこの場合も“二重盲検”で行った。コルチコステロイドを適用しているその他の部分を比較用とした。
改善は極めて迅速に見られ、コルチコステロイドを適用した箇所よりも早く、また再発は局所コルチコステロイド適用した部分より少し遅れて現れた。
脂漏性皮膚炎、偏平紅色苔癬、限局単房性皮膚炎を患っていたそれぞれ1名の患者は完全に治癒した。
当然のことながら、上記の教示に照らせば本発明の多くの修正や変形が可能である。したがって、本発明は本明細書に具体的に記述した以外にも添付の請求項の範囲において実施できることはいうまでもない。
Claims (6)
- 医薬組成物の形態が、クリーム、ローション、注射液、錠剤、カプセル剤、及び経口摂取可能な溶液および懸濁液からなる群から選択される形態のものである請求項1記載の組成物。
- 式(I)で表わされる一以上のラムノリピドにおいてR1が、α−L−ラムノピラノシルである請求項1記載の組成物。
- 式(I)で表わされる一以上のラムノリピドにおいて、R3が−(CH2)x−CH3(ここで、xは4〜20)である請求項1記載の組成物。
- 式(I)で表わされる一以上のラムノリピドにおいて、R4が−(CH2)x−CH3(ここで、xは4〜20)である請求項1記載の組成物。
