Abstract
【解決手段】挿管器具24は、細長い柔軟なチューブおよびチューブに取り付けられた対の部品を備え、内視鏡の外側の管路とスライド可能に嵌合するように構成される。挿管器具は、チューブの近位部に配置され、つぶれ形状と拡張形状との間で変形可能な組織受け材44をさらに備える。このチューブは、チューブの近位端が患者の腹壁および胃壁を介して外に出されるように、患者の上部消化管の内側に配置が可能であり、組織受け材44が拡張形状であるとき、組織受け材を胃壁内部に接触させて固定することができる。

医師は、患者の自然な身体開口部を介して挿入された柔軟な内視鏡を用いて、多くの医療行為を行う。通常、柔軟な内視鏡は、医師が内視鏡近位端の作動装置を用いて制御可能な、関節運動遠位端を有する柔軟なシャフトを備える。胃鏡および結腸鏡など、多くの柔軟な内視鏡は、診断および治療器具とともに対象の組織へアクセスするために不可欠な作業チャネル(生検チャネルもしくは補助チャネルとも呼ばれる)を備える。作業チャネルの直径は、内視鏡のサイズと種類により1mm〜4mmの範囲とすることができる。
管路の屈曲が内視鏡の屈曲から実質的に切り離されるように、内視鏡と協働するように構成された管路を有する案内装置とともに使用するための挿管器具、が提供される。この挿管器具は、細長い柔軟なチューブと、当該チューブに取り付けられ、内視鏡の外部管路とスライド可能に嵌合するよう構成された、対となる部材と、を備える。この挿管器具は、チューブの近位部に配置され、つぶれ形状と拡張形状との間で変形可能な組織受け材をさらに含む。チューブは、当該チューブの近位端を患者の腹壁および胃壁を介して外部に出すことが可能なように、患者の上部消化管内部に配置可能であり、組織受け材は、組織受け材が拡張形状のときに、胃壁内部に接触するように固定することが可能である。
図1に示すように、全般的に参照符号10で表される案内装置(医療装置とも呼ばれる)は、ハンドル12、ハンドル12から延びる柔軟シース14、シース14に取り付けられた柔軟管路16、およびシース14の遠位端に取り付けられたエンドキャップ18を備えることができる。ハンドル12およびシース14は、柔軟な内視鏡をその内部に受け入れる大きさである。シース14は、ポリエチレンもしくはポリプロピレンのような薄い高分子フィルムで形成され、内視鏡全体を覆うに十分な長さを有する。管路16は、押出成形のポリプロピレンのような連続した柔軟な低摩擦ポリマーの構成要素で形成することができる。

前記組織受け材は、前記組織受け材が患者の内側胃壁に接触して配置されるとき、前記拡張形状である、挿管器具。 - 請求項1に記載の挿管器具において、
前記レールは、前記案内装置の前記細長管路とスライド可能に嵌合するように構成される、挿管器具。 - 請求項1に記載の挿管器具において、
前記柔軟なチューブに接続された垂下フィラメント、
をさらに備えた、挿管器具。 - 請求項1に記載の挿管器具において、
前記柔軟なチューブの遠位端は、テーパー状の閉端部を備える、挿管器具。 - 請求項1に記載の挿管器具において、
前記柔軟なチューブと着脱可能に接続された第1の結合部材、を有する、位置決め装置、
をさらに備え、
前記位置決め装置は、前記案内装置の前記細長管路とスライド可能に嵌合するように構成される位置決めレール、を備える、
挿管器具。 - 請求項6に記載の挿管器具において、
前記位置決め装置は、患者の体内にある間、前記柔軟なチューブから外れるように構成される、挿管器具。 - 請求項1に記載の挿管器具において、
前記組織受け材は、
第1の端部、
第2の端部、および、
前記柔軟なチューブの周囲の周りに離間して設けられ、前記第1および第2の端部に接続している、複数のアーム、
を備え、
前記第1の端部は、前記柔軟なチューブに取り付けられ、
前記第2の端部は、前記柔軟なチューブを覆うように、前記第1の端部の近位にスライド可能に保持されている、
挿管器具。 - 請求項8に記載の挿管器具において、
前記複数のアームは、前記組織受け材が前記つぶれ形状のときに、前記柔軟なチューブに対して概ね平らであり、
前記アームは、前記組織受け材が前記拡張形状のときに、概ね、屈曲して前記柔軟なチューブから半径方向に広がる、挿管器具。 - 請求項1に記載の挿管器具において、
前記組織受け材は、高分子の管で形成される、挿管器具。 - 挿管システムにおいて、
案内装置であって、当該案内装置上に配置された細長管路を有する、案内装置と、
概ね柔軟なチューブであって、前記柔軟なチューブの内部を通って延在する細長チャネル、および、前記細長チャネルと接続されたレール、を有し、前記レールは、前記細長チャネルと概ね整列し、前記細長管路とスライド可能に嵌合するように構成されている、柔軟なチューブと、
前記柔軟なチューブに配置され、つぶれ形状と拡張形状との間で変形可能な、組織受け材と、
を備えた、システム。
