Description
(項目1) 脊椎を固定するための骨固定プレート組立体(10、210、210’)において、固定プレート(12、212、212’)の各端部(14、16)に配置された少なくとも1つの孔(24、24’60)と、前記骨固定プレート(12)と椎骨(18)との係合を維持しつつ、隣接する椎骨(18)間の移動を許容する手段(26)とを有することを特徴とする骨固定プレート組立体。
(項目2) 前記骨固定プレート(12、212、212’)が、第1端部(14)及び第2端部(16)により定められる長手方向軸線(A)と、隣接する椎骨(18)間に跨がるのに充分な前記軸線(A)に沿う長さとを有し、前記骨固定プレート(12、212、212’)は、上面(20)及び下面(22)を含み、前記下面(22)が椎骨(18)と係合可能であることを特徴とする項目1に記載の骨固定プレート組立体。
(項目3) 前記隣接する椎骨(18)間の移動を許容する手段(26)が、少なくとも1つの前記孔(24、24’、60)内に配置されるインサート手段(28、28’、70)を有し、該インサート手段(28、28’、70)は、ねじ(40)を受け入れてこれを保持しつつ、ねじ(40)及びインサート手段(26)が前記骨固定プレート(12、212、212’)に関して移動できるようにし、同時に、前記骨固定プレート(12、212、212’)に関するねじ(40)の軸線方向移動を防止することを特徴とする項目1に記載の骨固定プレート組立体。
(項目4) 前記少なくとも1つの孔(24、24’)がボアであることを特徴とする項目1に記載の骨固定プレート組立体。
(項目5) 前記ボアは、前記上面(20)、下面(22)又は上面(20)及び下面(22)の両方のいずれかに配置された少なくとも1つの内部凹部(13)を有していることを特徴とする項目4に記載の骨固定プレート組立体。
(項目6) 前記インサート手段(28、28’)は前記内部凹部(13)と係合して、前記インサート手段(28、28’)を前記骨固定プレート(12、212、212’)内にロックすることを特徴とする項目5に記載の骨固定プレート組立体。
(項目7) 前記インサート手段(28、28’、70)は、前記孔(24、24’、60)内で自由に回転することを特徴とする項目3に記載の骨固定プレート組立体。
(項目8) 前記インサート手段(28、28’、70)は、ねじ(40)を骨固定プレート(12、212、212’)内にロックするためのロック手段(42)を有していることを特徴とする項目3に記載の骨固定プレート組立体。
(項目9) 前記ロック手段(42)はねじ付きボアを有し、該ねじ付きボア内にねじ(40)を保持することができることを特徴とする項目8に記載の骨固定プレート組立体。
(項目10) 前記骨固定プレート(12、212、212’)が、該骨固定プレートの前記端部(14、16)の間に配置された少なくとも1つの孔(24’)を有していることを特徴とする項目2に記載の骨固定プレート組立体。
(項目11) 前記骨固定プレート(12、212、212’)の端部(14、16)の間に配置された少なくとも1つの孔(24’)が、前記骨固定プレート(12、212、212’)の長手方向軸線(A)に沿って配置されていることを特徴とする項目10に記載の骨固定プレート組立体。
(項目12) 前記少なくとも1つの孔(24、24’)が、前記骨固定プレート(12、212、212’)に配置された凹部(45)を備えた少なくとも1つの細長いスロット(44、44’、44”)を含んでいることを特徴とする項目1に記載の骨固定プレート組立体。
(項目13) 前記細長いスロット(44、44”)が、前記骨固定プレート(12、212、212’)の長手方向軸線(A)と実質的に平行に配置されていることを特徴とする項目12に記載の骨固定プレート組立体。
(項目14) 前記細長いスロット(44’)が、前記骨固定プレート(12、212、212’)の長手方向軸線(A)に関して実質的に鋭角に配置されていることを特徴とする項目12に記載の骨固定プレート組立体。
(項目15) 前記細長いスロット(44、44”)が、前記骨固定プレート(12、212、212’)の一端(14、16)に配置されていることを特徴とする項目13に記載の骨固定プレート組立体。
(項目16) 前記骨固定プレート(12、212、212’)が、前記骨固定プレート(12、212、212’)を椎骨(18)の曲率に一致するように前記長手方向軸線(A)に関して横方向に湾曲していることを特徴とする項目1に記載の骨固定プレート組立体。
(項目17) 前記下面(22)が、前記骨固定プレート(12、212、212’)の椎骨(18)への係合を強めるための骨係合手段(46)を有していることを特徴とする項目2に記載の骨固定プレート組立体。
(項目18) 前記骨係合手段(46)が、前記骨固定プレート(12、212、212’)の前記下面(22)の実質的に全体に配置された粗面化面を有していることを特徴とする項目17に記載の骨固定プレート組立体。
(項目19) 前記粗面化面が、前記下面(22)の一部に配置されていることを特徴とする項目18に記載の骨固定プレート組立体。
(項目20) 前記インサート手段(28)が、一端の周方向ねじ受入れ部分(32)及び他端の截頭円錐状インサート部分(34)により形成された環状凹部(30)を有し、該環状凹部(30)及び前記截頭円錐状インサート部分(34)は外方に押圧されかつ横方向スロット(36)を有し、前記インサート手段(28)を前記孔(24、24’)内に挿入すると、前記環状凹部(30)及び截頭円錐状インサート部分(34)が内方に撓んで、前記環状凹部(30)及び截頭円錐状インサート部分(34)が前記骨固定プレート(12、212、212’)を通り得るようにし、次に外方に撓んで前記インサート手段(28)を前記骨固定プレート(12、212、212’)内にロックすることを特徴とする項目3に記載の骨固定プレート組立体。
(項目21) 前記インサート手段(28)の前記ねじ受入れ部分(32)が、孔(24、24’)内に配置されたねじ(40)の挿入及び緊締中に、前記孔(24、24’)内での前記インサート手段(28)の回転を防止する道具を受け入れることができる少なくとも2つの凹部(38)を有していることを特徴とする項目3に記載の骨固定プレート組立体。
(項目22) 前記隣接する椎骨(18)間の移動を許容するための手段(26)が、少なくとも1つの前記孔(60)内に配置されたインサート手段(70)を有し、該手インサート手段(70)は、前記プレート(12、212、212’)に関してねじ(40)及びインサート手段(26)が移動できるようにしつつねじ(40)を受け入れかつ保持し、同時に前記骨固定プレート(12、212、212’)に関するねじ(40)の軸線方向移動を防止し、また、前記インサート手段(70)は、骨固定プレート(12、212、212’)に関するねじ(40)の傾斜を許容するための傾斜手段を有していることを特徴とする項目1に記載の骨固定プレート組立体。
(項目23) 前記少なくとも1つの孔(60)が、2つの実質的に半球形状の端部(62)により形成され、該端部(62)は、これらの間に延びている2つの実質的に直線状の側部により間隔が隔てられていることを特徴とする項目22に記載の骨固定プレート組立体。
(項目24) 前記インサート手段(70)の前記傾斜手段が、2つの実質的に半球形状の部分(72)を有し、該部分(62)は、これらの間に延びている2つの実質的に直線状の部分(75)により間隔が隔てられていることを特徴とする項目22に記載の骨固定プレート組立体。

(項目26) 前記インサート手段(70)が、前記ねじ(40)を前記骨固定プレート(12、212、212’)内にロックするためのロック手段を有していることを特徴とする項目22に記載の骨固定プレート組立体。
(項目27) 前記ロック手段が、ねじ(40)を保持することのできるねじ付きボア(76)を有していることを特徴とする項目25に記載の骨固定プレート組立体。
本発明によれば、脊椎を固定するための骨固定プレート組立体が、隣接する椎骨間の移動を許容すると同時に骨固定プレートと椎骨との係合を維持できる機構を有している。

Claims (2)
- 固定プレート組立体を用いて脊椎を固定するシステムであって、
上面、下面、および固定プレートを貫通する少なくとも2つの孔を有する固定プレートであって、該孔の各々は、該上面および該下面のうちの少なくとも1つに配置された、少なくとも1つの内部環状凹部を有する、固定プレートと、
該孔内に柔軟に保持される少なくとも2つのインサートであって、該インサートの各々は、ねじ付き軸線方向貫通ボアおよび少なくとも1つのフランジを備え、該少なくとも1つのフランジは、該少なくとも1つの内部環状凹部と係合するように構成されている、少なくとも2つのインサートと、
少なくとも2つの骨ねじであって、該骨ねじの各々は、該インサートの該ねじ付き軸線方向貫通ボアと螺合するように構成されるねじ山部分を備えている、少なくとも2つの骨ねじと
を備える固定プレート組立体と、
椎骨の少なくとも対に対して配置され得るように構成されている固定プレートであって、該少なくとも2つの孔のうちの1つを一方の椎骨上に整列させ、該少なくとも2つの孔のうちのもう1つを他方の椎骨上に整列させる、固定プレートと、
該少なくとも2つのインサートの各々を介して、個々に、該椎骨にインサートするように構成されている少なくとも2つの各々の骨ねじと
を備え、
該骨ねじのねじ山部分と該インサートの該ねじ付き軸線方向貫通ボアとを螺合することによって、該骨ねじは、該インサートと該プレートとの間において柔軟性を維持しつつ、該インサート内にロックされ得、該椎骨に関して固定された状態にて該プレートを維持しつつ、該椎骨間の移動を許容する、システム。 - 実質的に、上記された骨固定プレート。
