Description

【発明の詳細な説明】
【０００１】 本発明は、脊柱の整形外科の分野に関する。より詳しくは体部間ケージの分野
に関する。
【０００２】 このようなプロテーゼは、病的脊髄分節を固定し、２つの断片の融解によって
骨の癒合を引起すためのものである。これらのケージは一般に、チタン、カーボ
ン、又は生物学的適合性の複合材料からつくられており、骨の移植片を受入れる
。
【０００３】 例として、先行技術の体部間ケージを記載しているフランス特許出願第ＦＲ２
，７６４，７９５号を参照することができよう。このケージは、骨の断片で満た
すことを目的とした中央中空空間を備えている。このケージは横断面として、そ
れぞれ中央開口部を備えた凸状の２つの幅広い面、及びそれぞれ中央開口部を備
えた２つの狭い表面を有し、前記狭い表面は、これらの狭い表面の中央開口部の
両側に分配されている斜めの歯を有する。このケージはさらに、閉じた凸状前方
末端部と、操作装置の多角形受入れハウジングを中央に有する後方末端部とを有
する。多角形案内具とねじ切りされたロッドとを備えた取付けキーが、この発明
のケージと協同作用して、椎骨間の嵌合によってこれを導入し、嵌合後回転によ
ってこれを固定する。
【０００４】 第ＦＲ２，７６１，８７９号として公告されたもう１つのフランス特許出願は
、次のようなインプラントを記載している。すなわちこのインプラントは、イン
プラント後に椎骨の篩骨と接触させるための、直径方向に向かい合ったその２つ
の側のレベルにおいて、これらの開口部の１つが、長手方向に隣接する２つの開
口部を分けるインプラントの壁の部分の正面に来るように、１つの開口部の長さ
の実質的に半分に相当するピッチだけ互いに対して角度がずらされた、複数の分
離した開口部を備えている。
【０００５】 もう１つの特許出願第ＦＲ２，７１７，０６８号は、２つの椎骨間に前方ルー
トで（ｐａｒ ｖｏｉｅ ａｎｔｅｒｉｅｕｒｅ）挿入するための体部間ケージ
について記載している。このケージは、椎骨本体と接触するための実質的に平行
な２つの枝、これらの枝の後方末端部を連結する橋、及びケージを２つの椎骨間
に挿入した後で２つの枝を角度的に間隔あけするための部品を備えている。この
ケージによって、２つの椎骨間の脊柱前彎の角度の手術前の調節が可能になる。
２つの椎骨間にケージを挿入するために、ケージホルダーが、このケージに取り
外し可能に連結される。このケージホルダーを引出した後、ねじ回しがねじを回
転させる。このねじは、ケージの２つの枝の間隔をあけるためにこれらの枝の１
つにねじで固定された、間隔あけ部品の連動後部末端部を有する。
【０００６】 同様に米国特許第ＵＳ５，８００，５５０号において、次のような体部間ケー
ジを製作することも提案された。このケージは、体部間ケージを確実に保持する
ために、ねじリードを備えた工具によって、椎骨の中に垂直に押しやることがで
きる歯付きシリンダを備えている。
【０００７】 同様にドイツ特許出願第ＤＥ４，３２７，０５４号において、支持ピストンを
取り付けることができるようにするカム軸を備えた体部間ケージを製作すること
も提案された。
【０００８】 先行技術のこれらの体部間ケージの場合に生じる問題は、骨の癒合が行なわれ
る前の一時的固定の問題である。実際、ケージが定位置に留まることは極めて重
要である。先行技術の解決法は満足すべきものではないが、その理由は、これら
の場合、椎骨間空間におけるケージの直接的又は間接的嵌合が必要だからである
。
【０００９】 先行技術の装置の大きな欠点は、特に脊柱の伸長及び／又は回転運動の際、こ
れらの装置では脊髄分節における体部間ケージの固定を完全に行なうことはでき
ないということと同時に、これらの装置では体部間ケージの切除が実際には可能
にならないということにある。
【００１０】 本発明の目的は、次のような改良された体部間ケージを提案することである。
すなわち、頸部、胸部、又は腰部ルートによって、前方又は後方外科行為による
インプラントを可能にし、骨の癒合前及び骨の癒合がひとたびなされた時のケー
ジの効率的固定を保証し、かつ骨の癒合後にケージを取り出すことができるよう
なケージである。
【００１１】 本発明によるケージの目的は、椎骨本体の伸延（ｄｉｓｔｒａｃｔｉｏｎ）を
防ぐために一定の椎骨間空間を保持することができるようにすることである。
【００１２】 このために本発明は、その最も一般的な意味において、骨断片で満たすことを
目的とした中央中空空間を備えた脊柱固定体部間ケージにおいて、隣接椎骨との
連結側面に通じている固定手段であって、前面に通じている少なくとも１つの駆
動手段によって駆動されている手段を備えており、前記固定手段がそれぞれ、少
なくとも１つの主要切刃を備えていることを特徴とする体部間ケージに関する。
【００１３】 固定手段とは、定義によれば、自由度をまったく有していないロックから成る
。これらの固定手段によってこのように固定された体部間ケージは、完全に連動
する全体を形成するために、圧縮、伸長、及び回転において同時に椎骨をロック
する。
【００１４】 好ましくは駆動手段は、回転駆動ヘッドを有しており、かつ固定手段の可逆性
を得るために二方向において回転的に可動である。
【００１５】 第一実施態様によれば、固定手段は自動ねじ立てねじから成る。
【００１６】 好ましい実施態様によれば、前記固定手段は、一般的なＳ形形状を有しており
、このＳ形形状の重心は、駆動手段の軸と一致する。
【００１７】 有利には各固定手段は、ケージの設置の際、及び場合によってはその切除の際
、骨塊体の中への固定手段の良好な浸透を可能にするために、駆動手段の軸に対
して実質的に垂直な少なくとも１つの径方向切刃を備えている。
【００１８】 有利には各固定手段はまた、ケージの切除の際に骨塊体への固定手段の良好な
浸透を可能にするために、主要切刃に対して実質的に平行な少なくとも１つの逆
方向の切刃をも備えている。
【００１９】 有利にはまた各固定手段は、固定手段のアーム間での骨の再生を可能にするた
めに開口部を備えている。固定手段と骨再生との間でこのようにして得られる協
同作用は、圧縮、伸長、及び回転において同時に椎骨の完全な保持を保証する。
【００２０】 さらにはこれらの開口部の縁部は好ましくは、固定手段の取出し及び体部間ケ

【００２４】 本発明による体部間ケージ（１）は、全体的に平行六面体の形状を有し、これ
らの様々な面、特にこれらの側面（５、５’）及び前面（６）に備えられた開口
部によってあけられている中空内部空間（４）を画定している。内部空間（４）
は、患者から採取された骨断片で満たすことを目的とするものである。これらの

（５’）に通じている２つの自動ねじ立てねじ（８、９）から成っている。

と、自動ねじ立てねじ（８、９）のねじ山と協同作用をする、すじの入った本体
とを有する。

端部から成っている。
