を以下に説明する。

測定プローブを記載している。これらのデバイスでは、ヘモグロビン濃度を上昇
させるために局所組織潅流速度を高めることにより酸素濃度計の感度を向上させ
ている。米国特許第5,148,082号明細書は、センサー内に載置した半導
体デバイスにより発生させた熱により組織を温めることにより光プレチスモグラ
フィー測定中の患者組織の血流を増加させる方法を記載している。前記した加熱
素子は、熱源及び光源として機能する余り効率的でないフォトダイオードを含む
。

この特許明細書で教示される光源と検出器の間の最長分離距離でサンプリングさ
れる深さ及び横方向距離は数mmである。図1に示し、本明細書に定義するよう
に、平均サンプリング深さdavは所与のNI測定でサンプリングされる組織表
面に対して垂直な軸に沿った平均侵入深さである。図6〜8に示すヒト前腕の熱
モデルから、周囲温度に応じて、1.7mmの深さの組織温度は皮膚表面の温度
よりもせいぜい0.5℃高いことが示唆される。Wilsonら(J.Qu,B

,2(3),p.319−325(July 1997))によれば、0.5℃
の温度変化で予想される散乱変化は5mM(90mg/dl)のグルコース濃度
変化に等しい。よって、米国特許第5,551,422号明細書によりプローブ
される温度勾配のために散乱変化は正常な生理学的グルコースレベルで予想され
る信号と同じ程度である。

試薬を用いず、無痛で且つ環境にやさしい器具も要望されている。

的測定に対してわずかな効果が与えられる。

クと接触してから)この状態に達するまでに要する時間を図8に示す。図8には
、皮膚が熱伝導ディスクに触れる直前の中心軸に沿ったモデル温度が示されてお
り、それは時=0で表示されている。皮膚を熱伝導ディスクと接触させてから3
0〜600秒の範囲の時間で、組織(真皮)温度は上昇し、本質的に一定値に達
するまで温度勾配は下降する。60秒の平衡期間後、算出した温度勾配は2mm
の深さで0.2℃に達する。600秒で、温度勾配は平坦になり、時間=無限大
と表示される定常状態値に達する。

または検出される前に何回も拡散される状況に限定される)。多重散乱の条件は

媒体の境界条件が拡散理論近似を適用するために重要である。SRLS測定の場
合、光源と検出器の分離距離が輸送光学的平均フリーパス、媒体中の2つの連続
する吸収または散乱相互作用間の平均距離[1/(μa+μs’)]よりもかな
り広いという条件が必要である。低吸収係数(1cm−1)及び10cm−1の
散乱係数を有する組織の場合、近IRでの平均フリーパスは1mmである。拡散
理論近似は1mmよりも非常に大きい、通常1〜7cmの光源−検出器距離で適
用する。光源からの距離が遠い場合の測定は遠視野条件と呼ばれる。光子と組織
の相互作用部位間の平均フリーパスは通常0.01〜2mmであり、可視スペク
トルでは0.75mmが典型的な値である。本発明において、検出器距離は0.
4〜0.5mmほど短くてもよい。従って、この距離は平均フリーパスよりも短
いかまたはそれに匹敵し、拡散理論近似は当てはまらない。光源と検出器間の狭
い分離距離での測定は近視野条件を与える。
、モンテカルロシミュレーションを用いて一連のステップにおける散乱及び/ま
たは吸収の確率を計算することにより得ることができる。当該物理的量のスコア
は、シミュレートした光子の有限数の統計的不確かさの範囲内でつけられる。モ
ンテカルロ法のパワーは、光源、検出器及び組織境界条件を実質的に取り扱う能
力、並びに組織の光学特性の組合せにある。モンテカルロ法は光伝播計算におい
て偏光及び拡散効果を調和し得、これらの方法は本発明において拡散理論近似よ
りも好ましい。
使用した公有ドイメンソフトウェアプログラムは、オレゴン州ポートランドに所
在のOregon Laser Centerから入手したLihong Wa
ng及びSteven L.Jacquesによる“多層の濁った媒体のモンテ
カルロシミュレーション(Monte Carlo simulations of multi-layered turbid m
edia)”であった。
数は200,000/ランであり、光はファイバー(n=1.5)から組織(n
=1.4)に伝播した。組織層の厚さは5〜25mmに設定した。光の入射地点
から0.44mm、0.78mm、0.89mm、1.17mm、1.35mm
及び1.81mmの距離で反射した光を数個のμs’値とμa値のマトリックス
について算出した。これらの距離はファイバー23,24,25,26,27及
び28の位置にほぼ相当した。生じたlogeR(i)対loge(Ri/Rj )をグリッドとしてプロットした。一定のμa及びμsポイントを結んで、lo
geR(i)対loge(Ri/Rj)スペース中にグリッドを形成した。ここ
で、R(i)は距離iでの反射率、R(j)は距離jでの反射率を表す。空間分
解後方散乱をイントラリピド溶液、イントラリピド懸濁液中のヘモグロビン溶液
、オパールグラス、及び異なる量の散乱及び吸収顔料を配合するように重合した
プラスチックロッドの組について測定した。実験値はモンテカルロで作成したグ
リッド上に重複し、参照材料の吸収及び散乱係数をグリッドから作成した表を用
いて求めた。空間分解光散乱(SRLC)はヒト志願者の前腕の背面部分で測定
し、同じグラフにプロットした。モンテカルロシミュレーションの結果及びヒト
志願者に対する反射率の測定値の例を図9に示す。円を繋ぐ破線が実験データで
ある。実線は示した吸収及び散乱係数に対するモンテカルロ適合度を示す。この
グラフは、モデルから予測される反射率の値が実験結果に近似していることを示
す。数人の白人、東洋人及び地中海人についてのμs’値及びμa値を34℃で
測定した。幾つかの照射波長でのμs’及びμaの平均値を使用して、平均フリ
ーパス(mfp’)を計算し、表1に示す。

測定した。非糖尿病患者におけるグルコース濃度は88mg/dlであり、糖尿
病患者におけるグルコース濃度は274.6mg/dlであった。SRLS測定
は各被験者の前腕で実施した。

依存性は、(1mm粒子中の)散乱係数の温度に対する依存性よりも大きく、こ

Claims (38)
- 【請求項1】 所与の温度で身体部分の少なくとも1つのパラメーターを測
定する方法であって、 (a)前記身体部分の温度を調節するステップ、 (b)温度調節を受ける身体部分の体積は光学測定で測定される身体部分の体積
よりも大きく、温度調節を関数として前記身体部分の少なくとも1つの光学特性
を少なくとも1つの波長で測定して、光学測定値を得るステップ、及び (c)(b)の光学測定値を分析して、前記身体部分の少なくとも1つのパラメ
ーターの測定値を得るステップ を含むことを特徴とする前記方法。 - 【請求項2】 少なくとも1つのパラメーターが検体の存在であることを特
徴とする特許請求の範囲第1項に記載の方法。 - 【請求項3】 少なくとも1つのパラメーターが検体の濃度であることを特

するステップ、 (c)前記身体部分の温度を該身体部分の正常な生理学的温度よりも高い温度に
上昇させるステップ、 (d)ステップ(c)の温度で前記身体部分の少なくとも1つの光学特性を測定
するステップ、及び (e)ステップ(b)及び(d)で測定した光学特性を分析して、前記身体部分
の少なくとも1つのパラメーターの測定値を得るステップ を含むことを特徴とする前記方法。 - 【請求項14】 更に、温度を関数として散乱係数を測定するステップを含
むことを特徴とする特許請求の範囲第13項に記載の方法。 - 【請求項15】 更に、温度を関数として吸収係数を測定するステップを含

下させるステップ、 (d)ステップ(c)の温度で前記身体部分の少なくとも1つの光学特性を測定
するステップ、 (e)前記身体部分の温度を該身体部分の正常な生理学的温度より高い温度に上
昇させるステップ、 (f)ステップ(e)の温度で前記身体部分の少なくとも1つの光学特性を測定
するステップ、及び (g)ステップ(b)、(d)及び(f)の測定値を分析して、前記身体部分の
少なくとも1つのパラメーターの測定値を得るステップ を含むことを特徴とする前記方法。 - 【請求項24】 更に、光学特性の測定値を身体中の検体の濃度と相関させ
るステップを含み、前記濃度はステップ(a)、(b)、(c)、(d)、(e
)、(f)及び(g)と別個の方法で測定されることを特徴とする特許請求の範

光伝送素子及び少なくとも1つの受光素子、 (c)温度コントロールを受けた身体部分の一定体積を照射するための少なくと
も1つの光源及び少なくとも1つの検出器、及び (d)前記身体部分の光学特性を測定するための信号プロセッサー を含み、前記した温度コントロール素子は、少なくとも1つの光源及び少なくと
も1つの検出器により照射される身体部分の体積部分よりも大きい該身体部分の
体積部分の温度をコントロールし得ることを特徴とする前記装置。 - 【請求項34】 光源から検出器までの距離及び光源の波長は、組織中の侵
入深さを温度コントロールされている深さに制限するように選択されていること
を特徴とする特許請求の範囲第33項に記載の装置。 - 【請求項35】 波長が400〜2500nmの範囲であることを特徴とす
る特許請求の範囲第34項に記載の装置。 - 【請求項36】 光源の波長が600〜1300nmの範囲であることを特
徴とする特許請求の範囲第33項に記載の装置。 - 【請求項37】 発光素子と受光素子の中心間の分離距離は6mm以下であ
ることを特徴とする特許請求の範囲第33項に記載の装置。 - 【請求項38】 身体部分中の深さが2mm以下の深さに限定されているこ
とを特徴とする特許請求の範囲第33項に記載の装置。
