Abstract
により、シナプス形成能を有するニューロン、オリゴデ
ンドロサイト、アストロサイト等を誘導することができ
る中枢神経系前駆細胞や、その調製方法や、かかる中枢
神経系前駆細胞等を用いた、シナプス形成促進・抑制物
質のスクリーニング方法又は神経損傷若しくは神経機能
改善治療薬等を提供すること。 【解決手段】 サイトカインの存在下に培養した脊髄由
来の神経幹細胞からなる中枢神経系神経前駆細胞を脊髄
損傷後一定期間をおいた後に損傷部に移植する。これに
より、損傷部にシナプス形成能を有するニューロン、オ
リゴデンドロサイト、アストロサイトを誘導することが
でき、誘導されたニューロンとホストのニューロンとの
間にシナプス形成が生じさせ、障害脊髄機能の改善を図
る。

リーニング方法などに関する。

る治療法は現在のところ、外傷を受けた直後に生じる化
学的な二次的損傷を最小限とどめることを目的としたス
テロイド大量投与療法と、残存機能を最大限に活用する
ことを目的としたリハビリテーション療法や筋移行術等
の手術療法などが行われている。ステロイド剤の中では
メチルプレドニゾロンの大量投与が脊髄損傷に伴う神経
症状の改善に有効であると報告されている(J.Spinal D
isord. 5(1), 125-131, 1992)が、ステロイド剤の大量
投与は全身的副作用も強く発現し、コントロールが難し
いことに加えて、感染症を伴う脊髄損傷では感染防御機
能低下をきたすという問題点がある。また、さらに現在
ステロイド大量投与療法の有効性についてさえ議論され
ている。現在、脊髄損傷に対する治療法は、急性期の組
織傷害を最小限にとどめるための治療法と残存機能を最
大限に活用する治療法が行われているが、傷害により失
われたニューロンの再生や断列した軸索の再伸長を認め
ない成人脊髄においては、損傷脊髄の再生を目的とした
治療法は未だ確立されていない。

S)中の損傷部位に、治療上有効な量を移植する方法
(特表2000−503983号公報)や、同種の単核
貪食細胞(単球、マクロファージ等)を、損傷または疾
患部位に、あるいはその近傍の中枢神経系(CNS)に
投与することにより、哺乳動物CNSにおける神経軸索
再生を促進する方法(J. Mol. Med. 77, 713-717,199
9、J. Neurosci. 19(5), 1708-16, 1999、Neurosurgery
44(5), 1041-5, 1999、Trends. Neurosci 22(7), 295-
9, 1999)(特表平11−513370号公報)などが
ある。また、明確な機序は不明であるが、spinal cord
homogenateによるvaccinationや髄鞘蛋白質であるmyeli
n basic proteinに特異的なT細胞を投与することによ
り、脊髄損傷後の運動維持の回復を促進させたという報
告もなされている(Neuron 24, 639-647, 1999、Lancet
354, 286-287, 2000)。
移植実験としては、マウスES細胞より分化させた中枢
神経系前駆細胞(CNS−NPC)を脊髄損傷モデルラ
ットに移植し機能改善が得られたとの報告がある(Nat.
Med. 5, 1410-1412, 1999)。しかし、この方法はES
細胞を用いている点で倫理的な問題があり、またCNS
−NPCへのES細胞からの分化誘導についても未だ充
分には確立されているとはいえず移植部位での奇形種の
発生が危惧されている。また、脊髄再生を目的とし胎児
脊髄を移植する実験はラットや猫の脊髄損傷モデルを用
いて行われており、移植による損傷脊髄機能の改善が報
告されている(J. Neurosci. 18, 763-778, 1998、Brai
n Pathol. 5, 451-457, 1995他)が、臨床応用へは至っ
てはいない。この原因の一つとして、一度の移植に対し
複数の胎児からの脊髄が必要となるためにドナーとなる
胎児脊髄の確保が困難であることが挙げられる。
経系の損傷は極めて治療困難な疾患で、前記のように現
在まで有効な治療法がなく、新たな治療法の開発が強く
望まれている。また近年交通事故の増大や高齢化に伴
い、脊髄損傷に罹患する患者数は増加する傾向にあり、
大きな社会問題となっている。本発明の課題は、損傷又
は機能が失われた脊髄に移植することにより、シナプス
形成能を有するニューロン、オリゴデンドロサイト、ア
ストロサイト等を誘導することができる中枢神経系前駆
細胞や、かかる中枢神経系前駆細胞の調製方法や、かか
る中枢神経系前駆細胞等を用いた、シナプス形成促進物
質若しくは抑制物質のスクリーニング方法又は神経損傷
若しくは神経機能改善治療薬等を提供することにある。
に、本発明者らは、文献(Science 255, 1707-1710,199
2)記載の方法によりラット胎生14.5日目の胎児脊
髄組織を培養することによって獲得した中枢神経系前駆
細胞(CNS−NPC)を、脊髄損傷モデルラットに損
傷後9日目で損傷部に直接注入することにより、損傷部
に移植細胞由来のニューロン、オリゴデンドロサイト、
アストロサイト等を誘導することができ、さらに、かか
る移植細胞由来のニューロンの軸索にミエリンが形成さ
れ、シナプスを形成することによって脊髄機能が改善さ
れることを見い出し、本発明を完成するに至った。

ては、脊髄内、特に脊髄が損傷しているヒト等の脊椎動
物の脊髄内で、シナプス形成能を有するニューロン、好
ましくは該シナプス形成能を有するニューロンに加え
て、オリゴデンドロサイト、アストロサイト等を誘導す
ることができる脊椎動物由来の中枢神経系前駆細胞であ
れば特に制限されるものではなく、上記脊椎動物として
は、ヒト、ラット、ネズミ、ネコ、サル、ヤギ、ウサ
ギ、イヌ、ウシ、ヒツジ、ゼブラフィッシュ、メダカ、
サメ、カエル等の脊椎動物を具体的に挙げることができ
るが、これらに限定されるものではない。そして、中枢
神経系前駆細胞がヒト中枢神経系前駆細胞の場合、無制
限に移植細胞を獲得することが可能で、かつドナー不足
を解消することができるという点から、中絶した胎児由
来の脊髄から調製したものがより好ましい。
内で、シナプス形成能を有するニューロンを誘導するこ
とができる中枢神経系前駆細胞や、シナプス形成能を有
するニューロンの他に、オリゴデンドロサイト及び/又
はアストロサイトを誘導することができる中枢神経系前
駆細胞の調製方法としては、脊髄由来の神経幹細胞をサ
イトカインの存在下で培養する方法であれば特に制限さ
れるものではなく、サイトカインの存在下で培養した脊
髄由来の神経幹細胞を損傷脊髄内に導入・移植すること
により、損傷脊髄内でニューロン、オリゴデンドロサイ
ト、アストロサイトに誘導することができる。また、損
傷脊髄の由来と神経幹細胞の由来は同一あるいは相異な
っていてもよいが、ヒト由来の損傷脊髄やヒト脊髄由来
の神経幹細胞を用いることが好ましい。例えばヒト脊髄
由来の神経幹細胞をラット損傷脊髄内に導入・移植する
ことができる。そして、ヒト脊髄由来の神経幹細胞を用
いる場合は、中絶したヒト胎児における脊髄由来の神経
幹細胞を用いることが好ましい。
在下で培養する培養方法としては、特に制限されない
が、採取した脊髄を常法に従ってトリプシン処理した後
ピペッティング等により細胞を分散し、これらの細胞
を、神経幹細胞の選択的培養法であるニューロスフェア
(Neurosphere)法(Science 255, 1707-1710, 1992)
により、37℃で、7〜10日間浮遊培養する方法を好
適に例示することができる。この浮遊培養法により、神
経幹細胞を多く含む細胞集団であるニューロスフェア
(Neurosphere)と呼ばれる細胞塊が得られるが、この
マリモのようなニューロスフェアをピペッティング等に
より細胞ひとつひとつに分け、再び同条件で浮遊培養し
てニューロスフェアを得る継代培養を2〜4回繰り返す
ことにより、移植用の神経幹細胞を充分量確保すること
ができる。浮遊培養用の培養液としては、無血清のDM
EM/F12培地が好ましく、また上記培養液に用いる
サイトカインとしては、IL−12、IL−1α、IL
−1β、IFN−γ、TNF−α、FGF−2、GM−
CSF、IL−4等を具体的に例示することができ、ま
た、これらのサイトカインから選ばれる1種又は2種以
上の組み合わせであってもよいが、中でもFGF−2
(塩基性繊維芽細胞増殖因子)が好ましい。また、サイ
トカインと併せて、EGF(上皮増殖因子)や、NGF
(神経成長因子)や、PDGF(血小板由来増殖因子)
や、神経ペプチドや、白血病阻害因子などを用いてもよ
い。
シナプス形成促進物質又はシナプス形成抑制物質をスク
リーニングすることができる。かかるシナプス形成促進
物質又は抑制物質のスクリーニング方法としては、例え
ば、少なくとも脊髄内で、本発明の中枢神経系前駆細胞
又はかかる細胞から誘導されるニューロンと、被検物質
とを接触させ、前記中枢神経系前駆細胞から誘導された
ニューロンにおけるシナプス形成の程度を評価する方法
を挙げることができ、前記中枢神経系前駆細胞又はかか
る細胞から誘導されるニューロンと被検物質との接触方
法としては、中枢神経系前駆細胞と被検物質との混合物
を損傷脊髄内に移植する方法や、被検物質を経口投与し
た後、中枢神経系前駆細胞を損傷脊髄内に移植する方法
や、中枢神経系前駆細胞を損傷脊髄内に移植し、誘導さ
れたニューロンに被検物質を注入する方法などを具体的
に例示することができる。また、シナプス形成の程度を
評価する方法としては、電子顕微鏡的観察や、シナプト
フィジーに対する免疫組織学的解析による方法などを例
示することができる。そして、かかるスクリーニング方
法により得られるシナプス形成促進物質としては、例え
ば、BDNF、NT−3、NGF等を具体的に挙げるこ
とができ、シナプス形成抑制物質としては、セマフォリ
ン、Nogo、MAG等を例示することができるが、本
発明におけるシナプス形成促進物質やシナプス形成抑制
物質は、シナプス形成促進作用やシナプス形成抑制作用
が従前知られていない物質を意味する。
薬としては、前記中枢神経系前駆細胞を有効成分とする
ものや、前記中枢神経系前駆細胞及び上記シナプス形成
促進物質を有効成分とするものであればどのようなもの
でもよい。かかる中枢神経系前駆細胞やシナプス形成促
進物質を神経損傷又は神経機能不全疾患治療薬として用
いる場合は、薬学的に許容される通常の担体、免疫抑制
剤、結合剤、安定化剤、賦形剤、希釈剤、pH緩衝剤、
崩壊剤、可溶化剤、溶解補助剤、等張剤などの各種調剤
用配合成分を添加することができる。またかかる治療剤
は、例えば溶液、乳剤、懸濁液等の剤型にしたものを注
射の型で非経口的に脊髄損傷部位等の局所に投与するこ
とができる。
治療方法としては、上記神経損傷又は神経機能改善治療
薬を脊髄内に導入する方法や、前記中枢神経系前駆細胞
を脊髄内に注入・移植する方法を挙げることができ、か
かる治療方法により、中枢神経系前駆細胞から誘導され
たニューロンにおけるシナプス形成が生起し、神経損傷
又は神経機能疾患の改善を図ることができる。また、本
発明のニューロン、オリゴデンドロサイト又はアストロ
サイトのいずれかの脊髄内誘導方法としては、本発明の
中枢神経系前駆細胞を脊髄に直接注入して移植する方法
であり、これにより損傷部脊髄組織内に中枢神経系を構
成する主な細胞であるニューロン、オリゴデンドロサイ
ト、アストロサイトを誘導することが可能となる。ま
た、本発明は、本発明の中枢神経系前駆細胞を脊髄に移
植することにより誘導されたニューロンに形成されたシ
ナプスに関する。かかるシナプス形成によって、損傷に

に限定されるものではない。 参考例1(ラット胎児脊髄由来の移植用細胞の調製) スプラーグ−ダウリー(Sprague-Dawley)ラットの胎生
14.5日目の胎児より脊髄を採取し、かかる脊髄を常
法に従ってトリプシン処理した後ピペッティングにより
細胞を分散し、これらの細胞を、神経幹細胞の選択的培
養法であるニューロスフェア法により培養した。上記培
養は、増殖因子として塩基性繊維芽細胞増殖因子(FG
F−2)を含む無血清DMEM/F12培地を使用し、
37℃で1週間浮遊培養し、神経幹細胞を多く含む細胞
集団であるニューロスフェアと呼ばれる細胞塊を得た。
このニューロスフェアをピペッティングにより細胞一つ
ひとつに分け、再び同条件で浮遊培養してニューロスフ
ェアを得た。かかる継代培養を2〜4回繰り返し、移植
用の神経幹細胞を充分量獲得した。得られた細胞を赤色
の蛍光を発する蛍光物質であるブロモデオキシウリジン
(Bromodeoxyuridine;BrdU)でラベルした。
幹細胞の移植) 脊髄損傷モデル成体ラット(SDラットの雌、体重20
0〜230gを使用)は、Holtzらの方法(Surg.
Neurol. 31, 350-360,1989)に従い重錘圧迫法を用いて
作製した。具体的には第4、5頸椎(C4、C5)椎弓
切除後、35gの重錘を第4、5頸椎高位で脊髄背側か
ら脊髄上に15分間静置することにより作製した(図
1:参考写真1参照)。損傷後9日目に、脊髄損傷部に
生じた空洞内へマイクロシリンジを用いて手術用顕微鏡
下に、参考例1で得られた神経幹細胞を5〜10×10
6個/ml含む培養液30μlを注入することにより移
植した。
ルムアルデヒドで還流固定し、移植部脊髄を取り出して
組織学的検討を行った。その結果を図2(参考写真2参
照)に示す。図2aは、培地だけを移植した脊髄損傷動
物の損傷部位を示し、損傷により空洞形成が生じている
ことがわかる。図2b−1は、BrdUで前標識した神
経幹細胞を移植した脊髄損傷動物の損傷部位を示し(ス
ケールバー250μm)、b−2はb−1の拡大図であ
る(スケールバー100μm)。図2cは、ニューロン
に分化したドナー細胞(茶色:ニューロンマーカーのH
u、灰色:BrdU)を示し、図2dは、オリゴデンド
ロサイトに分化したドナー細胞(茶色:オリゴデンドロ
サイトマーカーのCNP、灰色:BrdU)を示し、図
2eは、アストロサイトに分化したドナー細胞(茶色:
アストロサイトマーカーのGFAP、灰色:BrdU)
を示す。これらの結果から、移植部に移植細胞由来のニ
ューロン、オリゴデンドロサイト、アストロサイトが存
在していることが確認できた。ニューロン、オリゴデン
ドロサイト、アストロサイトの確認には、それぞれ抗H
u抗体、抗2′3′-cyclic nucleotide 3′-phosphohydr
olase抗体、抗Glialfibrillary acidic protein抗体を
用いた。また、ニューロン、オリゴデンドロサイト、ア
ストロサイトに分化した細胞が、移植した神経幹細胞に
由来することは、ブロモデオキシウリジン標識により確
認した。
を取りこれを口にもっていくという動作について機能評
価を行った(図3:参考写真3参照)。図3aはペレッ
ト検索テスト(pellet retrieval test)の様子を示し
ており、2.5cmの直方体のボックスを4列3段に並
べ、鉄の棒で仕切ることにより、前足でしかボックス中
に置かれた小さなペレット状の餌を取れない装置を作製
した。各ボックスに5個ずつペレット状の餌を入れ、1
5分間で取れた餌の数を記録した。かかるテストのプロ
トコールとしては、餌を制限しながら1週間プレトレー
ニングを行い、その後前記重錘圧迫法による手術を行
い、移植後4週間再度同様なプレトレーニングを行い、
5週目に2日間連続してテストするというものである。
結果を図3bに示す。図3bに示されるように、脊髄損
傷のない対照動物群(ope(-);n=10)80.30±
0.84、脊髄損傷を加えて移植を行っていない群(SC
I;n=8)47.12±5.76、脊髄損傷を加えて
培地だけを移植した群(SCI+med;n=9)50.11
±4.19、脊髄損傷を加えて神経幹細胞を移植した群
(SCI+TP;n=13)67.85±2.02であり、移
植を行わなかった対照群に比べ移植群で、統計学的に有
意な機能改善が認められた(Mann-Whitney U-test)。
かかる前肢の巧緻運動の結果から、移植により機能改善
が認められることが確認できた。
脊髄由来CNS−NPCの移植実験における移植細胞由
来ニューロンのホスト神経回路網への導入の確認) ニューロンで特異的にEYFP(enhanced yellow fluo
rescent protein)を発現するトランスジェニックラッ
ト由来の移植細胞を調製し、参考例2と同様の方法で移
植を行い、移植後5週間目に移植部脊髄を取り出した。
上記EYFPを発現するトランスジェニックラットは、
文献(Sawamoto et al. J.Neurosci. inpress)記載の
方法に準じて作製した。すなわち、神経系において発現
するTα−1チューブリン遺伝子の1.1kbのプロモ
ーター因子の制御下にあるFYFPcDNAを文献(J.
Neurosci. 14, 7319-7330, 1994)記載の方法により精
製し、このcDNAをラット受精卵の前核にマイクロイ
ンジェクションした後、かかる受精卵を仮親のSDラッ
トに戻し、トランスジェニックラットを作製した。かか
るトランスジェニックラットは、ラットの尾よりゲノム
DNAを抽出し、導入したEYFPcDNAに特異的な
プライマーを用いてPCR法により同定した。移植細胞
由来のニューロンがホスト脊髄内で分化・生存している
ことの確認は、移植部脊髄を抗EYFP抗体で染色する
ことにより行った。結果を図4a〜dに示す(参考写真
4参照)。図4aは、すべてのEYFP発現細胞が、H
u陽性のニューロンであることを示している(スケール
バー5μm)。図4bは、移植後にホスト脊髄内でドナ
ー細胞が分裂し、ニューロンへと分化した状態を示して
いる(スケールバー5μm)。図4cから、EYFP陽
性のドナー由来のニューロンが、ホスト脊髄内の長軸方
向へ軸索を伸ばしていることが観察される(スケールバ
ー50μm)。図4dから、EYFP陽性のドナー由来
のニューロンの周辺にSynaptophysin陽性のシナプス小
胞の集積がみられる(スケールバー5μm)。
た後に、電子顕微鏡で検索した結果を図4e〜hに示す
(参考写真4参照)。図4eは、免疫電顕的解析で、E
YFP陽性のドナー由来のニューロンの軸索の一部が、
ホスト脊髄内で一部有髄化していることを示している
(スケールバー0.1μm)。図4fは、EYFP陽性
のドナー由来のニューロン(*印)がシナプス前細胞と
して、ホストのニューロンとシナプス形成していること
を示している(スケールバー0.5μm)。図5gは、
EYFP陽性のドナー由来のニューロンがシナプス後細
胞として、ホストのニューロン(*印)とシナプス形成
していることを示している(スケールバー0.2μ
m)。図5hは、損傷レベルのホストの運動ニューロン
とEYFP陽性のドナー由来のニューロンがシナプス形
成していることを示している(スケールバーh−1;2
μm、h−2;0.5μm)。図4e〜hから、EYF
P陽性の細胞すなわち移植細胞由来のニューロンの軸索
にミエリン形成が認められ、移植細胞由来のニューロン
とEYFP陰性のニューロンすなわちホストのニューロ
ンとの間にシナプスが形成されていることが確認され
た。これにより、移植に用いた神経幹細胞が、脊髄内
で、シナプス形成能を有するニューロンの他に、オリゴ
デンドロサイト及び/又はアストロサイトを誘導するこ
とができる中枢神経系前駆細胞であることが確認され
た。
Cの脊髄損傷モデルラットへの移植実験) ヒト胎生9週目の中絶胎児より脊髄を採取し、増殖因子
としてFGF−2と上皮成長因子(EGF)を使用し、
さらに白血病阻害因子を添加した培養液を用いる以外
は、参考例1と同様にして移植用細胞を充分量獲得し
た。また、ラット脊髄損傷モデルは、圧迫圧を35gと
したTatorの方法(J. Neuropathol. Exp. Neurol.
58:489-498,1999)によりラット脊髄損傷モデルを作製
し、損傷後9日目に上記得られた移植細胞を脊髄損傷部
に生じた空洞内へマイクロシリンジを用いて手術用顕微
鏡下で注入した。なお、かかるラット脊髄損傷モデルに
おいては、移植日の前日より連日、免疫抑制剤としてシ
クロスポリン(Cyclosporine A)を体重1gあたり10
μg腹腔内に投与した。移植後5週間目における脊髄移
植部を抗ヒト細胞核特異抗原に対する抗体で染色したと
ころ移植細胞は移植部に生着していることが確認できた
(図5:参考写真5参照)。
前駆細胞を損傷脊髄に移植することによってシナプス形
成能を有するニューロン、オリゴデンドロサイト、アス
トロサイトを誘導することができ、障害された脊髄機能
に改善が認められることがラット脊髄損傷モデルを用い
た実験で確認された。ヒト胎児脊髄由来の培養中枢神経
系前駆細胞を脊髄損傷モデルラットに移植し生着させる
ことも可能となった。これらの技術をさらに発展させる
ことで、脊髄損傷に対する脊髄再生を目的とした新規の
治療法が開発されることが期待される。

- 【請求項1】 脊髄内で、シナプス形成能を有するニュ
ーロンを誘導することができる中枢神経系前駆細胞。 - 【請求項2】 脊髄内で、シナプス形成能を有するニュ
ーロンの他に、オリゴデンドロサイト及び/又はアスト
ロサイトを誘導することができる中枢神経系前駆細胞。 - 【請求項3】 脊髄が、損傷脊髄であることを特徴とす
る請求項1又は2記載の中枢神経系前駆細胞。 - 【請求項4】 脊髄が、ヒト脊髄であることを特徴とす
る請求項1〜3のいずれか記載の中枢神経系前駆細胞。 - 【請求項5】 脊髄由来の神経幹細胞をサイトカインの
存在下で培養することを特徴とする、脊髄内で、シナプ
