Descrizione
La pelle rappresenta una efficace barriera contro gli effetti dannosi deH’ambiente proteggendo gli organi interni. I maggiori strati della pelle includono l’epidermide, il derma e l’ipoderma. L’epidermide umana à ̈ sufficientemente spessa e comprende fino a quindici strati. Essa rappresenta la prima barriera difensiva del corpo contro gli agenti nocivi di natura fisica, chimica ed ambientale, tra cui ad esempio i raggi ultravioletti, gli shock termici ed osmotici e gli inquinanti oggi sempre più diffusi nell’ambiente.
La soppressione del sistema immunitario della pelle à ̈ nota essere uno dei meccanismi attraverso cui detti agenti nocivi inducono ad esempio lo sviluppo dei tumori della pelle, principalmente relativi allo strato epidermico ed alle sue cellule.
Ad esempio la radiazione ultravioletta induce delle alterazioni a livello cutaneo sia immediate sia ritardate rispetto al tempo dell’esposizione. Oltre alle alterazioni visibili, quali la formazione delle cellule da colpo di sole note come “sun burn cells†, la produzione della melanina da parte dei melanociti ed un ispessimento dello strato corneo legato all’azione di citocheratine specifiche, si producono alcune alterazioni a livello molecolare responsabili di possibili danni a lungo termine quali: la deplezione di antiossidanti endogeni, che si verifica nelle ore immediatamente successive all’esposizione ai raggi e che induce una cascata di reazioni degenerative ritardate nel tempo (note come stress ossidativo) che interessano tutta la struttura cutanea, epidermide e derma; la reazione immunitaria che vede protagoniste le cellule di Langherans e che à ̈ mediata da citochine ed altri fattori solubili; il danno molecolare verso il DNA, in particolare attraverso la formazione della 8-oxy D guanosina.

Scopo della presente invenzione à ̈ di provvedere un mezzo per efficacemente contrastare l'azione di immunosoppressione indotta sulla pelle da agenti aggressivi quali inquinanti, disidratanti, raggi ultravioletti, shock termici ed osmotici, ed altri. In EP1328268 di proprietà della stessa Richiedente à ̈ descritta l’attività antiossidante di una miscela dei flavonoidi catechina e quercetina, basata in particolare su uno studio clinico in cui si dimostra un’elevata attività di inibizione dell’aggregazione piastrinica. In tale brevetto à ̈ prospettata anche un’applicazione di tale attività antiossidante nel contrastare l’azione di invecchiamento cutaneo da parte dei raggi ultravioletti.
Tuttavia in generale non à ̈ possibile prevedere se ad una attività antiossidante possa corrispondere per lo stesso composto, o la stessa miscela di composti, anche un’attività nei confronti di effetti di immunosoppressione, e dunque immunoprotettiva, del genere sopra discusso.
Secondo la presente invenzione si à ̈ ora sorprendentemente trovato sulla base di uno studio sperimentale che catechina e quercetina, in miscela tra loro secondo un selezionato rapporto quantitativo, sono un efficace principio attivo per combattere l’azione di immunosoppressione esercitata sulla pelle dagli agenti aggressivi sopra nominati.

L’invenzione pertanto ha per oggetto l’uso di tale miscela quale principio attivo nella preparazione di una composizione farmaceutica o dermatologica o nutrizionale o cosmetica per combattere l'azione di immunosoppressione esercitata sulla pelle da agenti aggressivi quali inquinanti, disidratanti, raggi ultravioletti, shock termici ed osmotici, e la relativa composizione caratterizzata dal fatto di comprendere quale principio attivo una miscela di catechina e quercetina in rapporto molare compreso tra circa 6:1 e 3:1 , rispettivamente.
La composizione dell’invenzione può essere formulata per uso topico sulla pelle oppure per uso sistemico, ad esempio in forma per somministrazione orale.
Si descrivono di seguito alcuni esempi di composizione secondo la presente invenzione. Essi non devono intendersi limitativi.
Negli esempi 1-5, le quantità dei componenti sono espresse come percentuali in peso all'interno di un intervallo come di seguito specificato. Per quercetina, à ̈ indicato il rapporto quercetina:catechina 1 :5 molare, rispetto alla quantità di catechina scelta entro quell'intervallo.

La Fig. 1 mostra un grafico riferito all’espressione della citochina IL-10.
La Fig. 2 mostra un grafico riferito all’espressione della citochina IL-12.
La Fig. 3 mostra un grafico riferito all’espressione del TNF-α secondo quanto di seguito più in dettaglio descritto.
La Fig. 4 mostra r'immuno-istochimica dei dimeri di timina nel campione irradiato (1 MED) e in seguito a trattamento con la miscela catechina e quercetina a 4 ore dopo irraggiamento.
STUDIO IN VITRO INTRODUZIONE E OBIETTIVO
Lo studio à ̈ stato condotto su epidermide umana ricostruita in Vitro per valutare l’azione immuno-protettiva e preventiva del danno al DNA, indotto da irradiazione UVA e UVB con una minima dose eritematosa (1MED), da parte della miscela di catechina e quercetina.
Il danno UV-indotto al DNA Ã ̈ certamente uno dei maggiori mediatori molecolari della foto-immunosoppressione (Schwarz et al., 2005).
Uno studio condotto dal gruppo di Kripke (1992) ha dimostrato con chiara evidenza che il DNA rappresenta il target preferenziale della radiazione UV nella generazione dell’immunosoppressione sistemica e che l’evento molecolare primario che media questo tipo di immunosoppressione indotta dagli UV à ̈ la formazione dei dimeri di pirimidina. In particolare i metodi che agiscono in maniera specifica su questo tipo di lesioni al DNA sono strumenti efficaci per ripristinare la funzione immunologica.
Parallelamente allo studio del danno al DNA UV-indotto sono state anche analiz zate le citochine pro-infiammatorie TNF-alfa e IL-12, e la citochina antiinfiammatoria IL-10.
L'approccio sperimentale secondo lo studio complessivamente permette di identificare in una fase precoce, e quindi di grande rilievo per l'azione dermatologica di prevenzione del foto-danno, il grado di attività delle molecole studiate verso i processi di immuno-soppressione indotti dai raggi UV, in una condizione fisiologica di epidermide vitale in seguito allo stress UV utilizzando parametri rilevanti, dinamici ed altamente sensibili come l’espressione di mediatori ed il danno al DNA a livello molecolare.
E' riconosciuta l’attività immuno-modulatrice di alcune citochine, in particolare l’interleuchina IL-12, verso le cellule immuno-competenti, ossia le cellule di Langherans. E’ stato quindi ipotizzato l’uso di questo meccanismo precoce della risposta immunitaria mediata dai cheratinociti epidermici in seguito ad irradiazione UV per valutare l’attività dei composti in esame.
IL-10 rappresenta un fattore cruciale per mantenere il fine equilibrio tra la resistenza nei confronti dei patogeni e l'infiammazione sistemica dannosa, ed à ̈ un potente inibitore della produzione di IL-12 (Haste-Amezaga et al., 1998, D’Andrea et al., 1993).
L’espressione della IL-12 nei cheratinociti epidermici dopo irradiazione à ̈ stata valutata con la tecnica RT-PCR.
La protezione dal danno al DNA à ̈ stata valutata attraverso la formazione dei dimeri di una base pirimidinica (timina) a livello degli strati epidermici basali e soprabasali che si evidenzia già dopo 4 ore dall’esposizione: essi sono riconosciuti markers del NMSC (non melanoma skin cancer) e la loro formazione viene valutata con tecnica immuno-istochimica. L’esame immuno-istochimico del tessuto trattato con catechina e quercetina evidenzia una diminuzione dell’immunomarcaggio, quindi una diminuzione dei dimeri di timina. La Fig. 4 mostra le immagini dai vetrini di immuno-istochimica dei dimeri di timina, a sinistra nel campione irradiato (1 MED) e a destra in seguito a trattamento con la miscela catechina e quercetina dell’invenzione a 4 ore dopo irraggiamento, in cui si evidenzia la ridotta colorazione delle cellule.
Con la stessa tecnica viene valutata la formazione della 8-oxy D-guanosina, marker specifico del danno al DNA indotto dagli UVA.
Per la sperimentazione à ̈ stata usata soluzione fisiologica 0.9% come sostanza inattiva di controllo, ed una miscela di catechina e quercetina in rapporto in peso 5:1, sciolta in soluzione fisiologica, quale composizione secondo l'invenzione.
Nelle figure 1-3 dei disegni allegati sono riportati i grafici relativi alle corrispondenti espressioni ottenute per un controllo non irradiato, un controllo irradiato e la composizione dell’invenzione (catechina quercetina).
Ciascuna coppia di colonne affiancate nei grafici si riferisce rispettivamente al caso di un trattamento UV di 4 ore (colonna di sinistra più chiara, 4h) e di un trattamento di 24 ore (colonna di destra più scura, 24h), irradiato secondo 1MED.
RISULTATI
I risultati della valutazione istologica in immuno-fluorescenza hanno mostrato : a) Per la 8 oxy D-guanosina bio-marker dello stress ossidativo UVA indotto dopo 4h dall'irradiazione la miscela catechina e quercetina ha mostrato un effetto protettivo, la formazione della 8oxy essendo infatti inibita rispetto al controllo IR .
b) Per i dimeri di timina: per la miscela catechina e quercetina à ̈ stata osservata l’inibizione della formazione dei dimeri di pirimidina a 4 h dopo esposizione. L’aumento di IL-10 rispetto al controllo non irradiato, sia in seguito ad irraggiamento UV sia da parte di catechina quercetina, rappresenta l’evidenza del cambiamento indirizzato verso l’immunosoppressione nell’ambiente cutaneo (Fig.1).
Tuttavia la predominanza di IL-10 rispetto a IL-12 (Fig. 2) risulta essenziale nel favorire la risoluzione della risposta infiammatoria dopo radiazione UV. In particolare la creazione di alti livelli di IL-10 e diminuzione di IL-12 a livello cutaneo à ̈ coerente con la natura immunosoppressiva dell’esposizione UV e l’esigenza cutanea di limitare la risposta infiammatoria (Kang et al., 1997).
Inoltre, la soppressione di IL-12 avviene anche ad opera del TNF-alfa, altra citochina strettamente collegata a IL-12 nella regolazione della risposta infiammatoria e produzione di IFN-gamma (Ma and Trinchieri, 2001). Il TNF-alfa riveste un ruolo chiave come mediatore del danno UV-indotto a livello cutaneo (Barr et al., 1999). L'esposizione UV porta ad un incremento del TNF-alfa, mentre il trattamento con catechina e quercetina porta ad una diminuzione del TNF-alfa.
Questo dato, illustrato in Figura 3, risulta strettamente correlato con l'incremento che catechina quercetina inducono su IL-10, in quanto IL-10 Ã ̈ un potente inibitore del TNF-alfa (Bogdan et al., 1991).
CONCLUSIONI
l risultati dello studio descrìtto hanno messo globalmente in evidenza una sorprendente azione immuno-protettiva della miscela catechina+quercetina secondo l’invenzione, con attività anche anti-infiammatoria sulla base dei dati di protezione sul danno al DNA sia UVA sia UVB indotto, dell’espressione della IL-10 e dell’espressione del TNF-alfa.
Si dimostrano pertanto efficacemente realizzati gli scopi dell'invenzione.
Claims (9)
- Rivendicazioni 1 ) Una miscela di catechina e quercetina in rapporto molare compreso tra circa 6:1 e 3:1, rispettivamente, per il trattamento della immunosoppressione indotta sulla pelle da agenti aggressivi quali inquinanti, disidratanti, raggi ultravioletti, shock termici ed osmotici.
- 2) Uso di una miscela di catechina e quercetina in rapporto molare compreso tra circa 6:1 e 3:1, rispettivamente, quale principio attivo per il trattamento della immunosoppressione indotta sulla pelle da agenti aggressivi quali inquinanti, disidratanti, raggi ultravioletti, shock termici ed osmotici.
- 3) Uso di una miscela di catechina e quercetina in rapporto molare compreso tra circa 6:1 e 3:1, rispettivamente, per la preparazione di una composizione per il trattamento della immunosoppressione indotta sulla pelle da agenti aggressivi quali inquinanti, disidratanti, raggi ultravioletti, shock termici ed osmotici.
- 4) Uso secondo le rivendicazioni precedenti caratterizzato dal fatto che detta miscela di catechina:quercetina à ̈ in rapporto molare di circa 5:1.
- 5) Composizione farmaceutica o dermatologica o nutrizionale o cosmetica per combattere l'azione di immunosoppressione provocata sulla pelle da agenti aggressivi quali inquinanti, disidratanti, raggi ultravioletti, shock termici ed osmotici, caratterizzata dal fatto di comprendere quale principio attivo una miscela di catechina e quercetina in rapporto molare compreso tra circa 6:1 e 3:1, rispettivamente.
- 6) Composizione secondo la rivendicazione 5 caratterizzata dal fatto che detto principio attivo à ̈ costituito da una miscela di catechina:quercetina in rapporto molare di circa 5:1.
- 7) Composizione secondo le rivendicazioni precedenti caratterizzata dal fatto di essere formulata per uso topico.
- 8) Composizione secondo le rivendicazioni precedenti caratterizzata dal fatto di essere formulata per uso orale.
