La partie métaphysaire (1b) et le col (1c) sont raccordés par une collerette (1d) présentant des agencements (1d1) pour la fixation de fragments osseux, ladite partie métaphysaire étant apte à recevoir, d'une manière fixe, des embases (2) rapportées de remplacement osseux et de différentes hauteurs positionnées en appui contre ladite collerette (1d), chaque embase (2) étant destinée à constituer un appui horizontal sur une coupe osseuse réalisée à hauteur variable en fonction des traits de fracture.

L'invention se rattache au secteur technique des implants orthopédiques notamment aux tiges fémorales pour prothèse de hanche.
D'une manière parfaitement connue pour un homme du métier, une tige fémorale comprend, pour l'essentiel, une partie diaphysaire, et une partie métaphysaire prolongée par un col apte à recevoir une tête fémorale. En fonction du cas pathologique à traiter et du type de fracture, la tige fémorale est susceptible de présenter différentes caractéristiques techniques.
L'invention concerne plus particulièrement une tige pour les fractures supérieures du fémur qui peut également être utilisée pour traiter certains cas de reprise simple de prothèse et d'exérèse tumorale.
La majorité des fractures supérieures du fémur est traitée par des vis céphaliques, des plaques ou des clous d'ostéosynthèse.
Les fractures de caractère instable (fractures per-trochantériennes complexes, fractures trochantéro-diaphysaires, fractures soustrochantériennes) et concernant des patients très âgés, peuvent cependant être des indications de prothèse.
Selon l'état antérieur de la technique, dans ces indications de prothèse, on utilise généralement une tige fémorale présentant, au niveau de la partie de raccordement de la métaphyse et du col, un bloc en interne de la métaphyse, d'une hauteur donnée, apte à remplacer les fragments osseux trochantériens et à assurer un appui horizontal sur la corticale après recoupe du fémur selon deux traits orthogonaux.
2867381 2 Ce bloc métaphysaire peut présenter des trous permettant de fixer, au moyen de fils d'acier par exemple, les tubérosités osseuses de l'extrémité supérieure du fémur.
L'application de ce type de tige fémorale est donc limitée. Elle oblige à conformer le fémur au niveau du foyer de fracture au volume et à la hauteur du bloc supéro-interne de la tige.
En particulier, ce type de tige n'est pas adapté aux cas de fractures inter-trochantérienne à deux fragments pour lesquels les fragments osseux peuvent être re-fixés et le stock osseux initial conservé.
Elle impose également un centre articulaire à une hauteur donnée, en rapport avec la hauteur de l'appui horizontal instauré entre le bloc métaphysaire et la corticale interne.
Un appui imposé bas par le cas de fracture peut amener une luxation postopératoire par le raccourcissement du bras de levier articulaire qu'il entraîne.
A partir de cet état de la technique, le problème que se propose de résoudre l'invention est celui de pouvoir traiter soit le cas de fractures simples synthétisables soit le cas de fractures multiples non synthétisables imposant un remplacement des fragments par un bloc métallique, avec la contrainte de toujours restituer correctement la position du centre articulaire.
2867381 3 Pour résoudre un tel problème, il a été conçu et mis au point une tige fémorale pour prothèse de hanche présentant une partie diaphysaire, une partie métaphysaire et un col latéralisé apte à recevoir une tête fémorale.
Selon l'invention, la partie métaphysaire et le col sont raccordés par une collerette présentant des agencements pour la fixation de fragments osseux, ladite partie métaphysaire étant apte à recevoir, d'une manière fixe, des embases rapportées de remplacement osseux et de différentes hauteurs positionnées en appui contre ladite collerette, chaque embase étant destinée à constituer un appui horizontal sur une coupe osseuse réalisée à hauteur variable en fonction des traits de fracture.
Pour résoudre le problème posé de pouvoir équiper la partie métaphysaire avec une embase de différentes hauteurs, la collerette est inclinée d'environ 45 par rapport à l'axe fémoral, chaque embase présentant une face supérieure d'appui inclinée d'une manière correspondante.
Pour résoudre le problème posé d'assurer la fixation des fragments osseux dans le cadre de fractures simples notamment, la collerette déborde du côté interne de la partie métaphysaire et de chacune des faces latérales antérieure et postérieure, en se raccordant progressivement au niveau du côté externe.
Les agencements de la collerette sont constitués par des trous débouchants pour l'engagement de vis à corticale.
Pour résoudre le problème posé de rendre solidaire l'embase sélectionnée au niveau de la partie métaphysaire de la tige, la fixation de chaque embase, en appui contre la collerette, s'effectue par une vis transversale traversant l'épaisseur de la partie métaphysaire.
2867381 4 Pour résoudre le problème posé de parfaitement intégrer l'embase sélectionnée au niveau de la partie métaphysaire de la tige, chaque embase présente une section transversale en U délimitant deux joues latérales aptes à enserrer la section transversale de la partie métaphysaire et à prendre appui contre la face antérieure et postérieure de ladite partie métaphysaire. Le bord libre de chaque joue présente un profil correspondant à celui du bord externe de la partie métaphysaire. La partie commune de liaison entre les deux joues est inclinée et d'épaisseur constante.
Avantageusement, chaque embase présente au moins un trou interne pour le passage de fils de réinsertion trochantérienne.

2867381 5 D'une manière parfaitement connue, la tige fémorale, désignée dans son ensemble par (1), présente une partie diaphysaire (la), une partie métaphysaire (lb) et un col (lc) apte à recevoir une tête fémorale.
Selon l'invention, la partie métaphysaire (lb) et le col (lc) sont raccordés par une collerette (ld). La collerette est inclinée d'environ 45 par rapport à l'axe fémoral. Cette collerette (1d) déborde du côté interne de la partie métaphysaire et de chacune des faces latérales antérieure (1 b 1) et postérieure (lb2), en se raccordant progressivement au niveau du côté externe. La collerette (1d) présente des agencements sous forme de trous débouchants (ldl) pour l'engagement de vis à corticale pour, notamment, la fixation des fragments osseux, dans le cas d'une fracture simple. Par exemple, la collerette (1d) présente quatre trous débouchants (ldl).
Selon une autre caractéristique importante de l'invention, en combinaison avec la collerette (ld), la partie métaphysaire (lb) de la tige est apte à recevoir, d'une manière fixe, des embases rapportées et de remplacement osseux (2). Ces embases (2), qui peuvent être de différentes hauteurs (figures 3, 4, 5 et 6), sont destinées à constituer un appui horizontal sur la coupe osseuse réalisée en deux traits orthogonaux. Chaque embase (2), quelle que soit sa hauteur, est conformée pour enserrer la section métaphysaire de la tige et pour être positionnée en appui contre la collerette (1d).
Comme le montrent notamment les figures 7 et 8, chaque embase (2) présente une section transversale en U délimitant deux joues latérales (2a) et (2b) aptes à enserrer la section transversale de la partie métaphysaire en prenant appui contre les faces antérieure (lbl) et postérieure (lb2). Chaque embase présente une face supérieure d'appui (2c) inclinée d'une manière 2867381 6 correspondante à la collerette (ld). Le bord libre (2d) de chaque joue (2a) et (2b), présente un profil correspondant sensiblement à celui du bord externe de la partie métaphysaire.
La fixation de l'embase sélectionnée (2), au niveau de la partie métaphysaire de la tige, s'effectue au moyen d'une vis (3) traversant l'épaisseur de la partie métaphysaire, et vissée dans un trou taraudé formésdans l'épaisseur de la zone (2b) de l'embase concernée.
A noter que chaque embase présente au moins un trou (2e) pour le passage, d'une manière connue, de fils de réinsertion trochantérienne. 10 A titre indicatif nullement limitatif, la tige fémorale selon l'invention peut être équipée de trois embases de hauteurs différentes, à savoir 30, 40 et 60 mm par exemple.
Compte tenu des caractéristiques à la base de l'invention, il apparaît donc que lorsque la tige fémorale n'est pas équipée de l'embase (2) (figures 1 et 2), la collerette (1d) permet la fixation des fragments osseux par les vis à corticale.
Lorsque la tige est équipée d'une embase sélectionnée afin d'obtenir un appui horizontal, la tige est destinée à la reconstitution du fémur proximal en cas de fragments multiples non synthétisables par simples vis ou en cas de perte osseuse (reprise de prothèse, exérèse tumorale) La tige fémorale présente par ailleurs les caractéristiques suivantes données à titre indicatif nullement limitatif La partie métaphysaire est tulipée.

La partie diaphysaire est de section circulaire et pourvue de deux trous de verrouillage distal par vis dans sa version sans ciment. Dans cette version, la tige et ses embases sont avantageusement recouvertes d'un matériau ostéo-conducteur pour favoriser leur intégration à l'os.
Le col (1c) est relativement long (40 mm) et latéralisé pour stabiliser l'articulation.
L'axe du col (1c) est formé perpendiculairement à la base d'appui de la collerette (ld).
Les avantages ressortent bien de la description.

Claims (2)

  1. 8 REVENDICATIONS
    -1- Tige fémorale (1) pour prothèse de hanche présentant une partie 5 diaphysaire (la), une partie métaphysaire (lb) et un col (lc) apte à recevoir une tête fémorale, caractérisée en ce que la partie métaphysaire (lb) et le col (1c) sont raccordés par une collerette (1d) présentant des agencements (1 dl) pour la fixation de fragments osseux, ladite partie métaphysaire étant apte à recevoir, d'une manière fixe, des embases (2) rapportées de remplacement osseux et de différentes hauteurs positionnées en appui contre ladite collerette (ld), chaque embase (2) étant destinée à constituer un appui horizontal sur une coupe osseuse réalisée à hauteur variable en fonction des traits de fracture.
    -2- Tige selon la revendication 1, caractérisée en ce que la collerette (1d) est inclinée d'environ 45 par rapport à l'axe fémoral, chaque embase (2) présentant une face supérieure d'appui inclinée d'une manière correspondante.
    -3- Tige selon la revendication 1, caractérisée en ce que la collerette (ld) déborde du côté interne de la partie métaphysaire et de chacune des faces latérales antérieure et postérieure, en se raccordant progressivement au niveau du côté externe.
    -4- Tige selon la revendication 1, caractérisée en ce que la fixation de chaque embase (2), en appui contre la collerette (1d), s'effectue par une vis transversale (3) traversant l'épaisseur de la partie métaphysaire (lb) .
  2. 2867381 9 - 5- Tige selon la revendication 1, caractérisée en ce que chaque embase (2) présente une section transversale en U délimitant deux joues latérales (2a) et (2b) aptes à enserrer la section transversale de la partie métaphysaire (1) et à prendre appui contre la face antérieure et postérieure de ladite partie métaphysaire.
    - 6- Tige selon la revendication 5, caractérisée en ce que le bord libre de chaque joue (2a) et (2b) présente un profil correspondant à celui du bord externe de la partie métaphysaire (lb).
    -7- Tige selon la revendication 5, caractérisée en ce que la partie commune de liaison entre les deux joues est inclinée et d'épaisseur constante.
    - 8- Tige selon la revendication 1, caractérisée en ce que les agencements de 15 la collerette (ld) sont constitués par des trous débouchants pour l'engagement de vis à corticale.
    -9- Tige selon la revendication 1, caractérisée en ce que chaque embase (2) présente au moins un trou interne de passage de fils de réinsertion 20 trochantérienne.
    - 10- Tige selon la revendication 1, caractérisée en ce que la partie diaphysaire est de section rectangulaire pour assurer la tenue en rotation en version cimentée.
    - 11- Tige selon la revendication 1, caractérisée en ce que la partie diaphysaire est de section cylindrique pourvue de deux trous de verrouillage distal par vis ou clavette dans la version non cimentée.
