Abstract

Prothèse osseuse. La prothèse est caractérisée en ce que: - la partie métaphysaire (4) présente deux faces planes, situées de part et d'autre d'un plan de symétrie, dit perpendiculaire au plan de référence sagittal anatomique, - la queue (6) est cylindrique, de surface lisse et de longueur au moins égale à 65% de la longueur totale de la prothèse, - la queue présente, à distance de son extrémité (14), deux trous transversaux (16), traversants, pratiqués dans le plan de symétrie et destinés au passage de deux vis de fixation. Application aux prothèses de hanche dites de reprise.

Description

La présente invention concerne le domaine des implants orthopédiques et,
plus particulièrement, des prothèses mises en oeuvre pour réhabiliter les
articulations du corps humain.
L'objet de l'invention est plus particulièrement dirigé vers les prothèses de hanche qui sont mises en place après résection du col du fémur, dans les
interventions destinées à réhabiliter l'articulation coxo-fémorale.
Dans le domaine technique ci-dessus, il est maintenant habituel d'implanter des prothèses de hanche qui ont fait l'objet de nombreuses propositions techniques, en vue d'assurer une réhabilitation dans les meilleures conditions et pour
des durées les plus longues possibles.
Il serait vain de vouloir établir un rappel exhaustif de toutes les propositions connues à ce jour, en raison de leur grand nombre. Il peut simplement être rappelé que les prothèses de hanche peuvent être, généralement, classées en deux catégories selon qu'elles sont implantées avec ou sans présence d'un liant
intermédiaire, tel qu'un ciment.
Le recul, maintenant attaché à l'implantation de telles prothèses, permet de constater que la ou les implantations effectuées pour un même fémur conduisent à une nécessité de revoir, tôt ou tard, l'implantation réalisée qui connaît des désordres exigeant l'extraction et le remplacement de la prothèse. Les causes de tels désordres sont nombreuses et peuvent provenir d'une implantation première non optimale, d'une dégradation du liant, de l'installation d'une infection, d'un
descellement ou d'une dégradation naturelle du matériel osseux abritant la prothèse.
Dans de telles situations et pour rétablir l'intégrité fonctionnelle de l'articulation coxo-fémorale, il est nécessaire de procéder à la mise en place d'une nouvelle prothèse, dite de reprise, qui doit, bien évidemment, être implantée dans
le fémur dont l'état a été altéré par les désordres précédents.
Dans une telle situation, il convient, certainement, de tenir compte de l'état du matériel osseux, voire des déformations du fémur ou, encore, de la dégradation de son extrémité supérieure d'appui résultant de la résection du col du
fémur.
Il est aussi nécessaire, généralement, de prendre en compte l'obligation de procéder à un nettoyage préalable du ciment de fixation de la prothèse antérieure lorsque celle-ci est du type lié ou, encore, à une remise en état du canal médullaire dont la partie interne a pu être dégradée en cas de battement d'une prothèse non liée
ou de descellement d'une prothèse liée.
Il est donc nécessaire de pouvoir disposer d'une prothèse qui puisse répondre aux exigences ci-dessus, de manière à surmonter les désordres, complications ou difficultés d'extraction attachés à une prothèse existante, afin de permettre la réimplantation d'une prothèse de reprise rétablissant l'intégrité de
l'articulation coxo-fémorale.
L'objet de l'invention est de répondre à cette exigence en proposant une prothèse de reprise permettant de tenir compte des altérations d'intégrité ou de modifications de potentialité physiques et structurelles du fémur préalablement implanté. L'objet de l'invention est de proposer une prothèse de reprise, adaptée aux aléas de réinstallation ou de rétablissement de l'intégrité, en raison des difficultés d'extraction de la prothèse précédente ou, encore, des dégâts osseux ou des
déformations naturelles du fémur.
Pour atteindre les objectifs ci-dessus, la prothèse de hanche, dite de reprise, est caractérisée en ce que: - la partie métaphysaire présente deux faces planes, situées de part et d'autre d'un plan de symétrie dit perpendiculaire au plan de référence sagittal anatomique, - la queue est cylindrique, de surface lisse et de longueur au moins égale à 65 % de la longueur totale de la tige, - la queue présente, à distance de son extrémité, deux trous transversaux, traversants, pratiqués sensiblement dans le plan de
symétrie et destinés au passage de deux vis de fixation.
Diverses autres caractéristiques ressortent de la description faite ci-
dessous en référence aux dessins annexés qui montrent, à titre d'exemples non
limitatifs, des formes de réalisation de l'objet de l'invention.
La fig. 1 est une perspective de la prothèse selon l'invention.
La fig. 2 est une vue latérale, en partie arrachée, correspondant à la
fig. 1.
La fig. 3 est une vue transversale prise selon la ligne 1m-Im de la fig. 2.
La fig. 4 est une coupe-élévation montrant l'implantation de la prothèse de reprise dans un fémur.
La fig. 5 est une élévation montrant un développement de l'invention.
Selon les fig. 1 à 3, la prothèse de hanche comprend une tige 1 qui est
liée à un col 2 formant une tête prothétique 3.
Au sens de l'invention, il peut être considéré indifférent que la tête 3 fasse partie intégrante de la prothèse ou soit constituée par une tête sphérique
rapportée comme cela est illustré par le dessin.
La prothèse est réalisée de manière que la tige 1 comporte une partie épaisse ou massive 4, dite métaphysaire, s'étendant à partir d'une base 5 de
raccordement avec le col 2 et se prolongeant par une queue 6.
La partie massive métaphysaire 4 est définie par deux faces planes 7a et 7b qui sont disposées de part et d'autre d'un plan de symétrie P-P', dont la référence géométrique est à considérer en relation avec un plan sagittal anatomique S. Bien que la tige 1 puisse être conformée pour présenter une courbure appropriée au fémur dans lequel elle doit être implantée, il est considéré, au sens de l'invention, que le
référentiel P-P' est du type plan, généralement parlant.
Les faces planes 7a et 7b sont, à partir de la base 5, légèrement convergentes en direction du plan de référence sagittal S et présentent, chacune, des stries, rainures ou analogues 8, établies longitudinalement et parallèlement entre
elles, de façon à favoriser l'ancrage ultérieur de la masse osseuse de reconstitution.
A cet égard, la partie métaphysaire peut, avantageusement, faire l'objet d'un apport d'un revêtement d'hydroxyapatite. Les stries 8 sont du type non débouchant en extrémité transversale sur le col, afin d'éviter toute migration de particules vers le haut. Par contre, en extrémité distale, les stries 8 sont "fuyantes", c'est-à-dire qu'elles tendent à se raccorder à la surface des faces 7a, 7b, de manière à faciliter
une rétraction future en cas de besoin.
La partie métaphysaire 4 est délimitée, transversalement aux faces 7a et 7b, par un bord interne 9, de forme concave, s'étendant dans le prolongement du col 2 pour se raccorder à la queue 6. De même, la partie métaphysaire est délimitée transversalement par un bord 10, dit extérieur, prenant naissance à partir d'un épaulement 11 formé par la base du col 2 et se raccordant, directement ou par l'intermédiaire d'une succession de segments plans rectilignes, à la queue 6. Dans tous les cas, l'axe général A-A' de la tige 1 fait, avec l'axe B-B' du col 2, un angle et compris entre 135 et 140 et la base 5 dudit col est orientée
perpendiculairement à l'axe B-B'.
Il peut être envisagé, comme cela apparaît à la fig. 1, de prolonger la base 5 du col 2, et à partir du bord interne 9, par une patte d'appui 12 dont la
fonction sera expliquée ultérieurement.
La partie métaphysaire 4 présente, par ailleurs, en retrait de son bord externe 10, des trous traversants 13 s'ouvrant sur les deux faces 7a et 7b et pratiqués, de préférence, de façon rectiligne et parallèlement l'un à l'autre, selon deux directions qui peuvent être appréciées comme étant perpendiculaires au plan P-P,. Selon une autre disposition de l'invention, la queue 6 présente une forme cylindrique de surface lisse et de longueur telle qu'elle confèere, à la prothèse de reprise selon l'invention, une longueur axiale comprise entre 160 et 350 mm. Cette longueur est, à chaque fois, choisie pour correspondre à environ 65 % de la longueur totale de la prothèse. La longueur axiale de la queue 6 est appréciée en étant prise entre l'épaulement 11 et l'extrémité 14 de la queue 6. Cette extrémité 14, de forme générale arrondie, correspond, de préférence, à une partie terminale 15
globalement tronconique ou en forme d'ogive.
En retrait de la partie terminale 15, la queue 6 présente deux trous 16 traversants, parallèles entre eux et qui sont pratiqués selon des directions perpendiculaires à celles des trous 13 pour être, en conséquence, orientés perpendiculairement au plan de référence sagittal anatomique S. A titre d'exemple, la prothèse de reprise selon l'invention peut être réalisée en alliage de titane TAGV selon ISO 5832-3 ou en acier implantable du type
acier inoxydable Z6 CNM 21-09 forgeable (M30NW) selon ISO 5832-9.
La prothèse de hanche décrite ci-dessus est destinée à être implantée dans un fémur F dont le canal médullaire 20 a fait l'objet d'une remise en état naturel ou d'un nettoyage destiné à éliminer les traces de liant ou de ciment correspondant à
une implantation antérieure.
Le canal médullaire 20 est traité à cet égard sur une longueur largement supérieure à celle des prothèses habituelles et, pour accéder à la partie diaphysaire du canal médullaire, il est préconisé de tailler, dans la partie métaphysaire externe du fémur, un volet V (fig. 4) offrant un accès à la partie interne métaphysaire tulipée
et surtout un accès en profondeur au canal médullaire 20.
Lorsque les opérations préalables de remise en état ont été assurées, il est aussi prévu de conférer au canal médullaire une section droite transversale supérieure à celle de la queue 6, voire d'adapter, par résection, la face interne de la corticale interne de la partie métaphysaire du fémur, pour autoriser un appui réparti de coopération avec le bord interne concave 9. Il peut aussi être envisagé de retailler le plan de résection initial, de manière à lui conférer un état favorisant un appui éventuel avec la collerette d'appui 12 qui constitue alors une butée limitant l'engagement de la tige dans le canal médullaire et permettant de transmettre des
contraintes à l'os afin que ce dernier se redynamise.
La prothèse selon l'invention est implantée, comme cela est illustré par la fig. 4, sans présence de ciment, mais en prévoyant une liaison ou un verrouillage distal consistant à exécuter, dans la partie distale externe du fémur, des trous de passage de deux vis 21 qui sont engagées, par ces trous, à l'intérieur des trous traversants 16 de la queue 6 pour être vissées directement dans la corticale interne
du fémur.
Comme cela ressort de la fig. 4, la prothèse selon l'invention est alors implantée directement dans le canal médullaire, en étant verrouillée en partie distale avec appui simultané du bord interne concave 9 sur la face interne de la corticale interne de la partie métaphysaire, appui éventuellement complété par l'arrêt ou la
butée assuré par la collerette 12.
Le volet V préalablement découpé est alors remis en place et lié, pour une consolidation osseuse ultérieure, par deux liens engagés dans les trous traversants 13, de façon à établir, par passage au travers du trait de coupe ayant permis le détachement du volet V, deux ceintures ou cerclages 22 solidarisant le volet V à la partie matéphysaire 4 dans une position stable qui est conférée par la coopération entre le bord interne et la corticale interne du fémur et par le verrouillage établi par les vis 21. Dans cette position stable, la formation du cal osseux rétablit rapidement l'intégrité entre le fémur et le volet V. Les dispositions structurelles rappelées, attachées à la prothèse de reprise ci-dessus, permettent de réaliser la réhabilitation d'une implantation prothétique antérieure connaissant des altérations ou des désordres, sans qu'une telle réimplantation ait à souffrir des désordres de l'implantation précédente. En effet, par la découpe du volet V, il est possible d'atteindre un matériel osseux de bonne qualité dans la partie distale profonde du fémur F et de réaliser un verrouillage maintenant, en raison de la grande longueur de la queue 6, une liaison ferme et résistante favorisant l'appui du bord 9 sur la face interne de la corticale interne de la partie
métaphysaire du fémur.
De cette manière, une reconstitution du matériel osseux interne au canal médullaire peut intervenir dans les meilleures conditions, de même que le rétablissement de l'intégrité de la partie métaphysaire du fémur par reconsolidation du volet V immobilisé en position stable sur la partie métaphysaire de la prothèse par
l'intermédiaire des cerclages 22.
Il doit, bien entendu, être considéré que les vis 21 peuvent être prévues
pour établir un verrouillage temporaire ou permanent.
Selon un développement de l'invention, il est prévu, comme cela apparaît aux fig. 2 et 3, de ménager, dans la face transversale proximale de l'épaulement 11, une rainure ou glissière 30 associée à un trou borgne 31, de préférence taraudé. Ces deux moyens techniques représentent des références d'adaptation et fixation temporaires pour un matériel ancillaire 32 possédant, selon la fig. 5, un barreau ou un fût rigide 33 constituant un guide gabarit permettant, au besoin au moyen d'un canon 34, le perçage en aveugle de la partie diaphysaire du fémur en exacte correspondance avec les trous 16 lorsque la prothèse est mise en place dans le
canal médullaire.
L'invention n'est pas limitée aux exemples décrits et représentés car
diverses modifications peuvent y être apportées sans sortir de son cadre.

Claims (9)

REVENDICATIONS:
1 - Prothèse de hanche, dite de reprise, du type comprenant une tige (1) liée par un col (2) à une tête (3), ladite tige comportant, à partir du col, une partie massive (4), dite métaphysaire, prolongée par une queue (6) et la tige possédant un axe général (A-A') faisant un angle (a) avec l'axe (B-B') du col, caractérisée en ce que: - la partie métaphysaire (4) présente deux faces planes (7a, 7b), situées de part et d'autre d'un plan de symétrie (P-P'), dit perpendiculaire au plan de référence sagittal anatomique (S), - la queue (6) est cylindrique, de surface lisse et de longueur au moins égale à 65 % de la longueur totale de la prothèse, - la queue présente, à distance de son extrémité (14), deux trous transversaux (16), traversants, pratiqués dans le plan de symétrie
et destinés au passage de deux vis de fixation (21).
2 - Prothèse de hanche selon la revendication 1, caractérisée en ce que la partie métaphysaire (4) comporte: - deux faces planes (7a, 7b), légèrement convergentes en direction du plan de référence sagittal (S), - un bord (9), dit interne, concave, raccordant le col à la queue, - et un bord (10), dit externe, prenant naissance à partir d'un
épaulement (11) du col et se raccordant à la queue.
3 - Prothèse de hanche selon la revendication 1 ou 2, caractérisée en ce que la partie métaphysaire (4) est raccordée, par son bord interne (9), au col avec la présence intermédiaire d'une collerette d'appui (12) s'étendant dans le plan de la
base (5) de raccordement entre le col et la partie métaphysaire.
4 - Prothèse de hanche selon l'une des revendications 1 à 3, caractérisée
en ce que la partie métaphysaire (4) comporte deux faces planes (7a, 7b)présentant des rainures longitudinales parallèles (8) non débouchantes en extrémité proximale
de la partie métaphysaire.
5 - Prothèse de hanche selon l'une des revendications 1 à 4, caractérisée
en ce que la partie métaphysaire comporte deux faces planes 7a et 7b qui sont, à
partir de la base 5, convergentes en direction de la queue.
6 - Prothèse selon l'une des revendications 1 à 5, caractérisée en ce que
la partie métaphysaire comporte, dans ses faces 7a, 7b, des stries qui sont raccordées
de façon fuyante par leur extrémité distale auxdites faces.
7 - Prothèse de hanche selon l'une des revendications 1 à 6, caractérisée
en ce que la partie métaphysaire (4) présente, en retrait de son bord externe (10), des trous traversants (13) s'ouvrant dans les deux faces planes et réservés au passage de liens (22) destinés à cercler un volet osseux (V) découpé dans la partie externe
métaphysaire d'un fémur (F) dans lequel ladite prothèse est implantée.
8 - Prothèse de hanche selon l'une des revendications 1 à 7, caractérisée
en ce que la partie métaphysaire comporte, en retrait de son bord externe, des trous
traversants d'axes perpendiculaires aux trous (16) de la queue (6).
9 - Prothèse de hanche selon l'une des revendications 1 à 8, caractérisée
en ce que la partie métaphysaire au moins définit un axe longitudinal (AA') faisant,
avec l'axe (B-B') du col, un angle (o) compris entre 135 et 140 .
