Abstract

Le dispositif comprend un élément (2) apte à être monté en combinaison avec une tige fémorale (1), introduite dans le canal médulaire, pour assurer un remplissage diaphysaire.

Description

Dispositif de reDrise chirurgicale d'une prothèse de hanche.
L'invention concerne plus particulièrement, un ensemble fémoral pour prothèse de hanche, dans le cas d'une reprise chirugicale.
Suite à des accidents ou des complications pathologiques, il est nécessaire de remplacer l'implant prothétique, préalablement impacté, pour remplacer l'articulation naturelle. C'est le cas notamment des prothèses de hanche. Très souvent, la diaphyse du fémur est en mauvais état, de sorte que l'implant fémoral n'est plus tenu dans de bonnes conditions, avec des risques de descellements.
Généralement, dans le cas d'une reprise, on utilise une tige fémorale très longue, dite de reconstruction, afin de rétablir l'articulation.
L'importante longueur de la tige, permet de remédier au manque de diaphyse du fémur.
II apparait donc que cette technique opératoire de reprise, nécessite d'avoir un nombre important de tiges fémorales, aussi bien en longueur qu'en diamètre, pour tenter de traiter tous les cas pathologiques.
On conçoit que cette solution est onéreuse et ne permet de procéder que par approximation.
On connait également d'autres systèmes mettant en oeuvre des tiges vissées en deux parties. Cette solution est relativement complexe et n'est pas toujours pas bien adaptée, dans le cas où un ancrage métaphysaire est difficile à réaliser.
L'invention s'est fixée pour but de remédier à ces inconvénients, de manière simple, sûre, efficace et rationnelle.
Le problème que se propose de résoudre l'invention, est de diminuer d'une manière notable, le nombre de tiges fémorales de reprise.
Pour résoudre un tel problème, il a été conçu et mis au point un dispositif de reprise chirurgicale d'une prothèse de hanche, qui comprend un élément apte à être monté en combinaison avec une tige fémorale, introduite dans le canal médulaire, pour assurer un remplissage diaphysaire.
Avec ces dispositions, il est donc possible de prendre une tige fémorale sous dimensionnée et de choisir l'élément rapporté dont l'épaisseur permet d'assurer le remplissage diaphysaire.
Pour résoudre le problème posé d'avoir un appui diaphysaire convenablement réparti, I'élément est une plaque d'appui conformée pour être adaptée sur la totalité ou une partie de la section de la tige, et sur une hauteur déterminée.
La plaque est fixée au niveau de la partie distale de la tige et présente un profil interne adapté au profil externe de ladite tige.
Un autre problème que se propose de résoudre l'invention, est d'assurer un blocage en rotation de l'ensemble tige et plaque, après introduction dans le canal médulaire du fémur.
Dans ce but, la face externe de la plaque présente des agencements sous forme d'aspérités aptes à assurer un blocage antirotatoir.
Suivant une autre caractéristique, la plaque est cintrée sur une longueur correspondant très sensiblement à la moitié de la circonférence de la partie correspondante de la tige et est disposée soit du côté interne, soit du côté externe.
L'invention est exposée, ci-après plus en détail à l'aide des dessins annexés, dans lesquels:
La figure 1 est une vue en perspective des éléments du dispositif avant montage.
La figure 2 est une vue montrant l'implantation du dispositif dans le canal médulaire du fémur.
La figure 3 est une vue en coupe considérée selon la ligne 3.3 de la figure 2.
Comme le montre la figure 1, le dispositif comprend un élément fémoral (1) apte à être monté dans le canal médulaire du fémur, en combinaison avec un élément (2), conformé pour assurer un remplissage diaphysaire. L'élément fémoral (i) constitue une tige susceptible de présenter tout profil connu et approprié. La tige peut présenter un col (la) orienté angulairement. De même, le corps de la tige, qui peut être de toute section transversale, peut être lisse ou présenter des aspérités d'ancrage. Plus généralement, I'élément fémoral (1) est conformé en formes et dimensions, pour constituer une tige fémorale de reprise, parfaitement connue pour un homme du métier.
En ce qui concerne l'élément (2), ce dernier est constitué par une plaque d'appui, conformée pour être adaptée sur la totalité ou une partie de la section de la tige et sur une hauteur déterminée. Cette plaque (2) est exécutée selon différentes épaisseurs. La plaque (2) est fixée au niveau de la partie distale de la tige (1) et présente une face interne d'appui (2a), adaptée au profil externe de la tige.
La fixation de la plaque (2) peut s'effectuer au moyen de vis (3) dont les têtes (3a) sont noyées dans l'épaisseur de la plaque. Les vis (3) coopèrent avec la tige (1).
Avantageusement, la plaque (2) est cintrée transversalement sur une longueur correspondant très sensiblement à la moitié de la circonférence de la partie correspondante de la tige. La plaque (2) est disposée du côté interne (figure 2), où sont généralement concentrés les efforts. On n'exclut cependant pas de disposer la plaque du côté externe.
La face externe de la plaque présente des aspérités (2b) ou autres agencements, aptes à assurer un blocage en rotation de l'ensemble de la tige et de la plaque, après introduction dans le canal médulaire.
L'invention trouve une application particulièrement avantageuse, en cas de reprise difficile, où il est impossible d'assurer un ancrage métaphysaire dans de bonnes conditions.
Les avantages ressortent bien de la description, en particulier on souligne et on rappelle
- La possibilité d'utiliser un nombre réduit d'éléments fémoraux, la différence d'épaisseur étant compensée par les plaques, lesquelles sont réalisées dans différentes épaisseurs.
- Le remplissage diaphysaire obtenu par un appui périphérique de la plaque sur une longueur déterminée.
- La simplicité de mise en oeuvre.
- Le coût de revient réduit.

    REVENDICATIONS
  1. -1- Dispositif de reprise chirurgicale d'une prothèse de hanche, caractérisé en ce qu'il comprend un élément (2) apte à être monté en combinaison avec une tige fémorale (1), introduite dans le canal médulaire, pour assurer un remplissage diaphysaire.
  2. -2- Dispositif selon la revendication 1, caractérisé en ce que l'élément (2) est une plaque d'appui conformée pour être adaptée sur la totalité ou une partie de la section de la tige (1), et sur une hauteur déterminée.
  3. -3- Dispositif selon la revendication 1, caractérisé en ce que la plaque (2) est fixée au niveau de la partie distale de la tige (1) et présente un profil interne adapté au profil externe de ladite tige.
  4. -4- Dispositif selon la revendication 1, caractérisé en ce que la face externe de la plaque (2) présente des agencements (2b) sous forme d'aspérités aptes à assurer un blocage anti-rotatoir.
  5. -5- Dispositif selon la revendication 1, caractérisé en ce que la plaque (2) est cintrée sur une longueur correspondant très sensiblement à la moitié de la circonférence de la partie correspondante de la tige (1) et est disposée du côté interne.
  6. -6- Dispositif selon la revendication 1, caractérisé en ce que la plaque (2) est cintrée sur une longueur correspondant très sensiblement à la moitié de la circonférence de la partie correspondante de la tige (1) et est disposée du côté externe.
  7. -7- Dispositif selon la revendication 1, caractérisé en ce que la plaque (2) est fixée sur la tige (1) au moyen de vis (3) dont les têtes sont noyées dans l'épaisseur de ladite plaque.
