Abstract

L'implant fémoral pour prothèse de hanche du type de ceux présentant une tige dont l'extrémité proximale est conformée pour le montage d'une manière démontable, d'une embase apte à recevoir directement ou d'une manière rapportée, une tête fémorale, est remarquable en ce que l'extrémité de la tige 1 et l'embase 2 présentent des agencements complémentaires aptes à assurer leur positionnement angulaire relatif variable, lesdits agencements étant conformés pour ne pas gêner leur passage au niveau du grand trochanter.

Description

Implant fémoral pour prothèse de hanche.
L'invention concerne plus particulièrement les pro theses totales de la hanche, posées sans ciment. Notamment l'implant est du type de ceux présentant une tige destinée à être engagée dans le canal médullaire du fémur, et dont l'ex truité proximale est conformée pour le montage et l'adaptation d'une embase.
D'une manière connue, l'embase reçoit directement, ou d'une manière rapportée, une tête fémorale.
Pour l'essentiel une prothèse doit assurer une bonne implantation osseuse adaptée aux différents morphotypes variés de l'anatomie et de la physiologie.
En outre, l'implant doit pouvoir être posé facilement en évitant des destructions osseuses importantes ou des fractures péropératoires.
Généralement, les implants du type de ceux indiqués, nécessitent une resection osseuse importante du grand trochanter, qui peut casser.
L'invention s'est fixée pour but de remédier à ces inconvénients.
Pour résoudre le problème posé d'éviter notamment toute destruction osseuse, l'extrémité de la tige et l'embase présentent des agencements complémentaires aptes à assurer leur positionnement angulaire relatif variable, lesdits agencements étant conformés pour ne pas géner leur passage au niveau du grand trochanter.
Avantageusement, les agencements sont constitués, en ce qui concerne la tige, par une portée polygonale formée concentriquement à la base d'un plot de centrage et, en ce qui concerne l'embase, par une empreinte en creux crantée apte à coopérer avec la portée polygonale, ladite empreinte étant formée concentriquement à un chambrage interne apte à recevoir ledit plot de centrage.
Un autre problème que se propose de résoudre l'in- vention est d'avoir un remplissage parfait du fût diaphysaire et une bonne stabilité de la tige en évitant tout phénomène de basculement et tout risque de descellement susceptible de résulter de contraintes importantes exercées sur un point donné.
Ces problèmes sont résolus en ce que l'empreinte en creux crantée de l'embase est établie à partir d'un plateau d'appui apte à coopérer, après engagement de ladite empreinte sur la partie polygonale de la tige, avec une collerette formée dans la partie proximale de ladite tige, au niveau de la corticale antérieure et postérieure de la diaphyse fémorale.
Il en résulte, compte-tenu de la position en retrait de l'embase par rapport au diamètre extérieur de la collerette, la supression du risque d'avoir des fragements osseux susceptibles de s'interposer pendant la mise en place de ladite embase.
Un autre problème que se propose de résoudre l'inventicn est d'avoir un très bon accrochage dans le tissu spongieux et dans la corticale.
Dans une première forme de réalisation, un tel problèse est résolu en ce que la tlge-est de profil tronconique et présente un filet auto-taraudant à double pas.
Dans une autre forme de réalisation, la tige est de profil tronconique et présente des cannelures longitudinales profilées aptes à faciliter la réhabilitation osseuse.
Avantageusement, le corps de la tige est soumis à un traitement de surface apte à améliorer l'ancrage et la réhabilitation osseuse.
L'invention est exposée ci-après, plus en détail, à l'aide des dessins annexés dans lesquels
- La figure I est une vue de face de l'implant, selon un exemple de réalisation, et avant montage de l'embase sur la tige.
- La figure 2 est une vue de face montrant l'implant mis en place dans le canal médullaire du fémur.
- La figure 3 est vue en coupe transversale considérée selon la ligne 3-3 de la figure 2.
- La figure 4 est une vue en coupe transversale considérée selon la ligne 4-4 de la figure 1.
Afin de rendre plus concret l'objet de l'invention, on le décrit maintenant d'une manière non limitative en se référant aux exemples de réalisation des figures des dessins.
D'une manière connue, l'implant comprend une tige
(1) et une embase rapportée (2). La tige (1), au niveau de son extrémité proximale, est agencée pour recevoir, l'embase (2) dont une partie présente un col latéralisé (2d) apte à recevoir, directement ou d'une manière rapportée, une tête fémorale (3).
Dans la forme de réalisation illustrée, la partie supérieure de la tige (1) présente un plot de centrage (la) pour le montage de l'embase (2) au moyen d'un chambrage interne (2a). Le plot (la) et le chambrage interne (2a) sont de profils corplémentaires, généralement tronconiques.
Le plot de centrage (la) présente concentriquement à sa base, une portée polygonale (lb), sur laquelle peut être engagée et centrée, avec capacité d'orientation angulaire, une empreinte en creux crantée (2b) formée concentriquement en bout de l'alésage (2a) de l'embase. D'une manière préférée, la portée (lb) est octogonale, tandis que l'empreinte complémentaire (2b) présente une série de dents régulièrement décalées angulairement sur la circonférence, d'environ 15 degrés.
Il est ainsi possible de bloquer en-rotation l'embase (2) par rapport à la tige, selon différentes positions angulaires, pour permettre, si nécessaire, un réglage de l'antéversion du col fémoral. La possibilité de ce réglage évite les risques de luxation si l'échancrure est insuffisante.
Ces agencements (lb-2b) de blocage en rotation de l'embase (2) recevant la tête fémorale par rapport à la tige selon différentes positions angulaires, permettent de dimensionner avantageusement lesdites embases de manière à respecter au maximum le grand trochanter aussi bien dans la découpe osseuse que dans le risque de fracture. A cet effet, lremprein te crantée en creux (2b) est établie à partir d'un plateau d'appui (2c), apte à coopérer, après engagement de ladite empreinte (2b) sur la portée polygonale de centrage (lb), avec une collerette (lc). Cette collerette est formée dans la partie proximale de ladite tige, au niveau de la corticale antérieure et postérieure de la diaphyse fémorale.
Après introduction de l'implant dans le canal médullaire du fémur, la collerette (lc) prend appui sur la partie.
réséqueé de l'os en évitant, dans ces conditions, tout risque de basculement tout en améliorant la stabilité de latige (1).
Ce diamètre de la collerette est limité pour ne pas engendrer un sacrifice osseux au niveau du grand trochanter, lors de la mise en place de la tige.
De même, comme le montre la figure 2, après positionnement des embases (2) sur la tige (1), le plateau d'appui (2c) desdites embases est situé à l'intérieur du diamètre de la collerette, supprimant ainsi, tout risque d'infiltration de fragments osseux dans le canal médullaire, pendant la mise en place de l'embase (2). La collerette (2c) peut présenter des échancrures pour s'adapter à un organe de manoeuvre en vue de la mise en place de la tige.
La fixation de l'embase (2) par rapport à la tige (1) peut s'effectuer par tout moyen connu et approprié. Par exemple, au moyen d'une vis (4) engagée dans l'embase notamment en alignement axial avec alesage interne (2a), pour être vissé- dans un trou taraudé, formé dans l'épaisseur du plot de centrage (la).
En ce qui concerne la tige (1) en tant que telle, cette dernière est conformée pour être implantée sans ciment en étant apte à assurer un très bon accrochage dans le tissu spongieux et dans la corticale.
Dans la forme de réalisation illustrée figure 2, la tige (1) est de forme générale tronconique et présente un filet auto taraudant (ld) à double pas pour augmenter la rapidité de pose. A la figure 1, la tige (1), de forme générale tronconi que, présente des cannelures longitudinales (le), profilées pour faciliter la réhabilitation osseuse.
Avantageusement, quelle que soit sa forme de réalisation, la tige (1) est soumise à un traitement de surface pour améliorer l'ancrage et la réhabilitation osseuse, tel qu'un revêtement à composante biologique.
Bien évidemment, la tige (1) et l'embase (2) sont exécutées en plusieurs longeurs et diamètres en fonction de l'anatomie du patient.

Claims (7)

    R E V E N D I C A T I O N s
  1. -1- Implant fémoral pour prothèse de hanche du type de ceux présentant une tige dont l'extrémité proximale est conformée pour le montage d'une manière démontable, d'une embase apte à recevoir directement ou d'une manière rapportée, une tête fémorale, caractérisé en ce que l'extrémité de la tige (1) et l'embase (2) présentent des agencements complémentaires aptes à assurer leur positionnement angulaire relatif variable, lesdits agencements étant conformés pour ne pas géner leur passage au niveau du grand trochanter.
  2. -2- Implant selon la revendication 1, caractérisé en ce que les agencements sont constitués, en ce qui concerne la tige (1), par une portée polygonale (lb) formée concentriquement à la base d'un plot de centrage (la) et, en ce qui concerne l'embase (2), par une empreinte en creux crantée (2b) apte à coopérer avec la portée polygonale (la), ladite empreinte (2b) étant formée concentriquement à un chambrage interne (2a) apte à recevoir ledit plot de centrage (la).
  3. -3- Implant selon la revendication 2, caractérisé en ce que l'empreinte en creux crantée (2b) de l'embase (2) est établie à partir d'un plateau d'appui (2c) apte à coopérer, après engagement de ladite empreinte sur la portée polygonale de la tige, avec une collerette (lc) formée dans la partie proximale de ladite tige, au niveau de la corticale antérieure et postérieure de la diaphyse fémorale.
  4. -4- Implant selon la revendication 3, caractérisé en ce que la collerette (lc) présent des échancrures pour s'adapter à un organe de manoeuvre en vue de la mise en place de la tige dans le canal médullaire du fémur.
  5. -5- Implant selon la revendication 1, caractérisé en ce que la tige (1) est de profil tronconique et présente un filet autotaraudant à double pas.
  6. -6- Implant selon la revendication 1, caractérisé en ce que la tige (1) est de profil tronconique et présente des cannelures longitudinales profilées aptes à faciliter la réhabilitation osseuse.
  7. -7- Implant selon l'une quelconque des revendications 5 et 6, caractérisé en ce que le corps de la tige est soumis à un traitement de surface apte à améliorer l'ancrage et la réhabilitation osseuse.
