Abstract

CETTE FACE 3A EST POURVUE DE STRIES 3B. LE PLATEAU COMPORTE DEUX TROUS 3C, 3D PERMETTANT SA FIXATION AU GRAND TROCHANTER 5B A L'AIDE DE VIS 7. TRAUMATOLOGIE.

Description

Lorsqu'une cassure du fémur intervient chez une personne agée, il n'est généralement pas possible d'envisager la réunification des deux parties de ce col du fait d'une décalsification avancée. Dans ces conditions on remplace purement et simplement la rotule de ltextrémité du fémur par une prothèse de hanche. L'implantation d'un tel dispositif doit être faite en un temps le plus court possible afin d'éviter pour les vieillards une longue période d'anesthésie.
Une grande partie de"ktactures du col du fémur chez les personnes agrées sont cervico-trochamt#riennes du fait de la structure même de cette partie de l'os en question. On a constaté que la surface de la fracture est approximativement une partie de cylindre.
De manière que le temps opératoire soit réduit au minimum, tout en conservant des caractéristiques mécaniques et de stabilité suffisantes pour la prothèse, on a prévu que le plateau de celle-ci comporte une face d'appui présentant la forme de la surface de fracture afin que celle-ci n'ait pas a être retouchée lors de ltopération. On gagne ainsi un temps précieux par rapport à la technique opératoire antérieure. Il est bien évident que les fractures en question se situent à des étages différents et que pour conserver la longueur normale du col du fémur on agit sur des paramètres de la prothèse telle que la longueur du pivot ou le diamètre de la sphère.
La face d'appui est avantageusement striée pour présenter une plus grande surface de contact et éviter les phénomènes de basculement.
L'aileron usuel qui unit la queue de la prothèse et son plateau est également prévu strié car étant donné l'état peu satisfaisant des os des personnes ages, on a intérêt à avoir la meilleure répartition possible des contraintes auxquelles son extrémité appareillée est soumise.
En outre le plateau possède deux trous pour permettre sa fixation au trochanter en fin d'intervention à l'aide de vis appropriées.
Le dessin annexé, donné à titre d'exemple, permettra de mieux comprendre l'invention, les caractéristiques qu'elle présente et les avantages qu'elle est susceptible de procurer
Fig. 1 est une vue en perspective d'une prothèse établie conform#ment à l'invention.
Fig. 2 en est une vue partielle à plus grande échelle.
Fig. 3 en est une coupe suivant III-III (fig. 1).
Fig. 4 illustre le montage d'une prothèse de hanche suivant l'invention après une cassure cervico-trochantérienne.
La prothese suivant l'invention illustrée en fig. 1 comprend conte à l'accoutumée une queue 1 destinée A s'engager dans le canal médullaire du fémur et un pivot 2 propre à recevoir une sphère destinée à reconstituer la rotule extrême du col du fémur. Ces deux composants 1 et 2 sont séparés par un plateau 3 qui, conformément å l'invention, présente une forme concave en direction de la queue 1. Ce plateau affecte avantageusement la forme d'une portion de cylindre dont la face 3a tournée vers la queue 1 est pourvue de stries transversales destinées à augmenter la surface d'appui. De meme l'aileron 4, qui de manière usuelle réunit la face 3a et le dessus convexe de la queue 1, est préVul/strié comme indiqué en 4a.Les stries 3b de la face 3a et de l'aileron sont préférablement semi-circulaires avec une largeur égale à environ 1,5 mn.
La queue 1 présente en section transversale une forme carrée à angle arrondi comme illustré en fig. 3, chacune de ses faces étant pourvue#d'une saignée longitudinale la débouchant au niveau de I'extrémi- té libre de ladite queue.
On a illustré en fig. 4 un fémur 5 ayant subi une fracture cervico trochantérienne dont la surface Sa est approximativement une partie de cylindre. En effet une telle cassure s'effectue naturellement à la limite correspondant un renfort ossetKentre le grand trochanter 5b et le petit trochanter Se, c'est-à-dire que la fracture s'effectue à la lisière de ce renfort.
Dans ces conditions la forme particulière suivant l'invention de la face 3a du plateau 3 permet une mise en place de la prothèse sans rectification de la surface de fracture Sa.
Bien entendu la prothèse est choisie de manière que son pivot 2 permette le montage d'une sphère 6 dont l'emplacement reconstitue exactement la longueur du col du fémur avant fracture. A cet effet et comme cela est bien connu, on utilise des sphères s1enfonçant suivant des longueurs différentes par rapport au pivot.
Afin d'augmenter l'assujettissement de la prothese par rapport au fémur on prévoit de ménager dans son plateau 3 deux trous 3c, 3d permettant la mise en place de vis 7 qui pénètrent dans le grand trochanter.
On a ainsi réalisé une prothèse de hanche dont le temps de mise en place est très réduit et qui permet d'éviter les problèmes de basculement.
Il doit d'ailleurs être entendu que la description qui procède n'a été donnée qut titre d'exemple et qu'elle ne limite nullement le domaine de l'invention dont on ne sortirait pas en remplaçant les détails d'exécution décrits par tous autres équivalents.

Claims (4)

REVENDICATIONS
1. Prothèse de hanche du genre comprenant une queue (1) destinée à être engagée dans le canal médullaire du fémur, séparée d'un pivot (2) destiné a recevoir une rotule (6) par un plateau (3), caractérisée en ce que son plateau (3) présente une face d'appui (3a) arrondie, convexe en direction de ltextrémité libre du fémur de manière que celle-ci n'ait pas à être retouchée lorsque la cassure est cervico-trochantérienne.
2. Prothèse de hanche suivant la revendication 1, caractérisée en ce que la face d'appui (3a) du plateau (3) est striée.
3. Prothèse de hanche suivant la revendication 2, caractérisée en ce que son aileron (4) en soi connu qui réunit la queue (1) au plateau (3) est strié.
4. Prothèse de hanche suivant la revendication 1, caractérisée en ce que son plateau (3) comporte deux trous (3c, 3d) pour sa fixation au trochanter (5b) à l'aide de vis (7).
S. Prothèse de hanche suivant la revendication 1, caractérisée en ce que la section de sa queue (1) est carrée avec angles arrondis, ses quatre faces comportant chacune une saignée longitudinale (la).
