Description

L'invention se rattache au secteur technique des implants orthopédiques et
plus particulièrement des prothèses d'épaule.
D'une manière générale et parfaitement connue pour un homme du métier, une prothèse d'épaule comprend, pour l'essentiel, un implant huméral et un implant glénoïdien. L'implant huméral est fixé dans la diaphyse humérale, tandis que l'implant glénoïdien est fixé au niveau de la glène de l'omoplate.
De nombreuses solutions techniques ont été proposées pour réaliser une prothèse d'épaule, pour tenter de reproduire, dans les meilleures conditions, l'articulation anatomique.
Quelle soit la forme de réalisation choisie, l'élément huméral présente généralement une tige agencée pour être fixée et ancrée, avec ou non du ciment, dans la diaphyse humérale. Le plus souvent, la tige présente angulairement une tête hémisphérique destinée à coopérer avec une partie complémentaire d'une cupule ou insert fixé sur une embase support, généralement en polyéthylène que présente l'implant glénoïdien.
On a également proposé une conception inverse de ce type d'articulation prothétique. Dans ce cas, la tête hémisphérique est non plus fixée au niveau de la tige humérale, mais au niveau de la glène pour coopérer avec un insert ou cupule, de forme complémentaire, fixé sur ladite tige.
Cette conception inversée de prothèse donne de bons résultats, étant donné que le deltoïde est utilisé comme élément moteur en abduction.
Pour éviter le phénomène de luxation de la prothèse contre l'acromion, compte tenu de l'absence de la coiffe des rotateurs, on a proposé des prothèses contraintes rétentives. Les résultats obtenus au niveau de la stabilité sont satisfaisants, mais ils le sont au détriment de la longévité en considérant l'usure de l'insert polyéthylène soumis à des charges importantes. Il peut en résulter un descellement de l'implant glénoïdien.
L'opérateur peut donc être en échec devant une articulation sans coiffe avec un deltoïde déficient engendrant un descellement de l'implant glénoïdien et une réserve de l'insert polyéthylène fixé sur la tige humérale.
Le problème que se propose de résoudre l'invention est de pouvoir reconstituer une articulation anti-douleur fonctionnellement statique avec pour objectif d'éviter tout affaissement de l'articulation de l'épaule.
Pour résoudre un tel problème et notamment dans le cas d'une reprise, il a été conçu et mis au point un ensemble prothétique pour l'articulation de l'épaule du type de ceux comprenant, d'une manière connue, un ensemble prothétique pour l'articulation de l'épaule constitué d'un implant huméral et d'un implant glénoïdien, l'implant huméral présentant une tige engagée dans le canal médullaire de l'humérus, tandis que l'implant glénoïdien présente une embase support fixée au niveau de la glène de l'omoplate.
Selon l'invention et pour résoudre le problème posé, la tige humérale et l'embase support, présentent une cupule ou insert hémisphérique délimitant chacun une surface d'appui et de glissement concave, de même concavité, pour recevoir, en combinaison, d'une manière congruente et en tension, un élément d'articulation rapporté.
Avantageusement, l'élément d'articulation est une sphère qui peut être en acier ou en céramique.
Avantageusement, les cupules ou inserts sont fixés d'une manière démontable sur la tige humérale et l'embase support.
Selon une autre caractéristique, la partie de la tige humérale recevant l'insert et la partie de l'embase support recevant l'insert, présente une zone d'appui et de fixation de forme concave sensiblement parallèle à la surface d'appui et de glissement correspondante et coopérant avec la sphère.
L'invention est exposée ci-après plus en détail à l'aide des figures 1 et 2 des dessins annexés qui représentent, par des vues en perspective et en coupe longitudinale, selon deux orientations différentes, l'impaction de l'implant prothétique, dans le cas d'une prothèse d'articulation d'épaule, notamment d'une prothèse de reprise, dans le cas d'une articulation avec coiffe et deltoïde détruits.
On rappelle, pour une meilleure compréhension de la suite de la description, que l'ensemble prothétique pour l'articulation de l'épaule comprend, d'une manière connue, un implant huméral et un implant glénoïdien.
L'implant huméral comprend une tige (1), de section transversale déterminée, pour être correctement stabilisée dans la diaphyse humérale en combinaison ou non avec du ciment. Cette tige (1) peut être de grande longueur pour constituer une tige de reprise. La tige (1) présente, à l'extrémité proximale, une tête humérale (la) disposée angulairement. Cette tête humérale (la), par exemple par l'intermédiaire d'une interface (2), reçoit une cupule ou insert hémisphérique (3) délimitant une surface d'appui et de glissement concave (3a).
L'implant glénoïdien présente une embase support (4), fixée par tout moyen connu et approprié, au niveau de la glène de l'omoplate. Cette embase support (4) reçoit une cupule ou insert hémisphérique (5) délimitant, de la même façon que l'insert (3), une surface d'appui et de glissement concave (5a). Les surfaces d'appui et de glissement (3a) et (5a), respectivement des inserts (3) et (5), présentent la même concavité pour recevoir, en combinaison, selon une caractéristique à la base de l'invention et d'une manière congruente, un élément d'articulation rapporté (6).
Cet élément (6) est constitué par une sphère avantageusement en acier ou en céramique. Compte tenu de l'effet de congruence recherché, les rayons de la sphère (6) et de surface d'appui et de glissement concave (3a) et (5a) sont, de préférence, d'égales longueurs. Pour assurer la stabilité de l'articulation ainsi obtenue, on obtient une tension maximum du deltoïde ou des parties molles, en augmentant, si nécessaire, le diamètre de la sphère (6) jusqu'à supprimer tout effet de pompage de l'articulation.
Les inserts (3) et (5), avantageusement en polyéthylène, sont fixés respectivement sur l'interface (2) et l'embase support (4), d'une manière démontable par tout moyen connu et approprié. A noter que la partie de la tête humérale ou de l'interface (2a) recevant l'insert (2) et la partie (4a) de l'embase support (4), présentent une zone d'appui et de fixation de forme concave, sensiblement parallèle à la surface d'appui et de glissement correspondante (3a) et (5a).
Ce type d'articulation permet d'obtenir un système mécanique reconstitué stabilisé par la rétentivité des inserts en polyéthylène (3) et (5) où les efforts de traction passent par le centre (C) de la sphère (6).
Les avantages ressortent bien de la description, en particulier on souligne et on rappelle : - la stabilité obtenue résultant de la congruence de l'insert huméral (3) et de l'insert glénoïdien (5) en combinaison avec la sphère d'articulation (6) ; - la motricité obtenue résultant de la latéralisation du point d'application des forces du deltoïde par l'augmentation du diamètre de la sphère (6) ; - la modularité obtenue par la possibilité d'adapter les inserts (3) et/ou (5), en fonction du cas pathologique à traiter compte tenu du caractère démontable desdits inserts ; - la liaison entre l'implant huméral et l'implant glénoïdien par la sphère qui constitue un élément totalement indépendant.

Claims (5)

  1. REVENDICATIONS
    - 1- Ensemble prothétique pour l'articulation de l'épaule comprenant un implant huméral et un implant glénoïdien, l'implant huméral présentant une tige (1) engagée dans le canal médullaire de l'humérus, tandis que l'implant glénoïdien présente une embase support (4) fixée au niveau de la glène de l'omoplate, caractérisé en ce que la tige humérale (1) et l'embase support (4), présentent une cupule ou insert hémisphérique (3) et (5) délimitant chacun une surface d'appui et de glissement concave (3a) et (5a), de même concavité, pour recevoir, en combinaison, d'une manière congruente et en tension, un élément d'articulation rapporté (6). -
  2. 2- Ensemble selon la revendication 1, caractérisé en ce que l'élément d'articulation rapporté (6) est une sphère. -
  3. 3- Ensemble selon la revendication 1, caractérisé en ce que les cupules ou inserts (3) et (5) sont fixés d'une manière démontable sur la tige humérale (1) et l'embase support (4). -
  4. 4- Ensemble selon la revendication 1, caractérisé en ce que la partie de la tige humérale (1) recevant l'insert (3) et la partie de l'embase support (4) recevant l'insert (5), présentent une zone d'appui et de fixation de forme concave sensiblement parallèle à la surface d'appui et de glissement correspondante et coopérant avec la sphère. -
  5. 5- Ensemble selon la revendication 1, caractérisé en ce que la zone d'appui et de fixation de la tige humérale (1) est formée angulairement par rapport à ladite tige. 10-6- Ensemble selon la revendication 1, caractérisé en ce que l'insert (3) de la tige (1) est fixé à cette dernière par l'intermédiaire d'une interface (2). -7- Ensemble selon la revendication 1, caractérisé en ce que les inserts (3) et (5) sont en polyéthylène. -8- Ensemble selon la revendication 2, caractérisé en ce que la sphère (6) est en acier. -9- Ensemble selon la revendication 2, caractérisé en ce que la sphère (6) est en céramique.
