Abstract
Prótesis intervertebral.Caracterizada porque comprende: dos piezas separadoras con forma general de cuña, presentando cada pieza una cara externa para contacto con una vértebra y otra cara de plano inclinado oblicua a la cara externa, presentando dichas piezas en la cara o zona del plano inclinado de la cuña un conjunto de entrantes conjugados que permite que una pieza entre, parcialmente, en la otra; un conjunto empujador de las plazas separadoras con una zona esferoide en contacto con el plano inclinado de dichas piezas; una camisa de confinamiento para impedir el movimiento de la esfera en una dirección perpendicular a la del conjunto empujador; de tal manera que el avance del conjunto empujador provoca un avance de la zona esferoide sobre la zona de plano inclinado de las piezas separadoras, provocando el alejamiento de las piezas separadoras entre sí y modificando la altura de la prótesis.
Description
Prótesis intervertebral.
La presente invención hace referencia a una
prótesis intervertebral.
Más en particular, la presente invención hace
referencia a una prótesis intervertebral destinada a asegurar una
distancia mínima entre vértebras y que puede ser implantada mediante
técnicas endoscópicas. Estas prótesis pueden resultar necesarias en
el caso de daño del disco intervertebral, si se quiere evitar una
operación de tipo abierto.
Si bien las prótesis intervertebral endoscópica
presenta la ventaja de evitar intervenciones peligrosas, presentan
la desventaja de que deben pasar por un canal endoscópico, lo que en
la práctica obliga a disponer mecanismos para desplegar la prótesis
en el espacio intervertebral.
Resulta conocida una prótesis de este tipo que
consiste en un cilindro de titanio que presenta un mecanismo en su
interior que permite la salida de un conjunto de patillas. Las
patillas quedan enclavadas una vez desplegada la prótesis y es
prácticamente imposible extraer la prótesis una vez desplegada. Por
otra parte, la prótesis resulta completamente rígida, lo que es
innatural y afecta a la comodidad de movimientos del paciente. Por
último, la prótesis no es regulable en altura, es decir, no se puede
regular la separación entre vértebras asegurada por la prótesis.
En conclusión, con esta prótesis, la extracción
por medios endoscópicos es potencialmente peligrosa y daña tejidos
del paciente, lo que puede provocar neuritis, entre otras
complicaciones.
Sin embargo, la necesidad de recolocar/extraer
no es infrecuente por cuanto el cirujano no dispone de medios que le
den a conocer, exactamente, cuál es la posición en la que queda la
prótesis.
Es un objetivo de la presente invención dar a
conocer una nueva prótesis intervertebral para uso endoscópico que
no presenta los inconvenientes antes citados, siendo regulable en
altura incluso una vez instalada, que permite la extracción en caso
de colocación incorrecta y que permite un cierto grado de
elasticidad en la separación mínima de vértebras.
Más en concreto, la presente invención consiste
en una prótesis intervertebral, del tipo destinada a asegurar una
distancia mínima entre dos vértebras conjuntas, caracterizada porque
comprende:
- dos piezas separadoras con forma general de
cuña, presentando cada pieza una cara externa para contacto con una
vértebra y otra cara de plano inclinado oblicua a la cara externa,
presentando dichas piezas en la cara o zona del plano inclinado de
la cuña un conjunto de entrantes conjugados que permite que una
pieza entre, parcialmente, en la otra;
- un conjunto empujador de las piezas
separadoras con una zona esferoide en contacto con el plano
inclinado de dichas piezas.
- una camisa de confinamiento para impedir el
movimiento de la esfera en una dirección perpendicular a la del
conjunto empujador;
de tal manera que el avance del conjunto
empujador provoca un avance de la zona esferoide sobre la zona de
plano inclinado de las piezas separadoras, provocando el alejamiento
de las piezas separadoras entre sí y modificando la altura de la
prótesis.
El conjunto empujador preferentemente estará
formado por una parte esférica o esferoide y una parte empujadora de
esta última. Dichas partes podrán ser piezas independientes entre sí
o bien encontrarse unidas en una única pieza.
La separación o altura definida por la prótesis
objeto de la presente invención viene definida por las caras
externas de dos piezas separadoras. A su vez, la separación entre
las caras externas depende del grado de penetración de una zona
esferoide en contacto superficial con las zonas de plano inclinado.
El grado de penetración de la zona esferoide entre las piezas
depende del avance de la parte empujadora. Los entrantes conjugados
de las piezas separadoras tienen una doble función: guían el
movimiento relativo entre piezas separadoras y aseguran una
distancia mínima entre piezas separadoras en forma de cuña. El
número de las mismas podrá ser variable. La camisa evita la salida
de la esfera de la zona de trabajo. Este sistema tiene la ventaja de
evitar enclavamiento y ser fácilmente regulable y reversible. La
reversibilidad facilita enormemente la colocación de la prótesis.
Por otra parte, la zona esferoide o parte esférica o esferoide,
puede utilizarse como elemento de absorción de impactos,
fabricándola en un material que presente ciertas propiedades
elásticas, tales como el teflón o el nailon, que permita una cierta
absorción del impacto, sin comprometer en demasía la separación
mínima entre vértebras. También podrá disponerse un vaciamiento para
mejorar las propiedades elásticas.
Preferentemente, la camisa de confinamiento
presenta un canal de entrada roscado para la parte empujadora, y la
parte empujadora presenta un roscado conjugado con el roscado del
canal de entrada. De este modo preferente se consigue
simultáneamente tanto una posibilidad ilimitada de regulación de la
altura o distancia de separación como un enclavamiento de dicha
distancia frente a fuerzas que tiendan a "cerrar" la
prótesis.
En una realización preferente, la prótesis
presenta un elemento elástico de recobro que une las piezas
separadoras entre sí. El elemento elástico de recobro (por ejemplo
una malla con propiedades elásticas o una goma o toroide elástico)
facilita la vuelta a la posición de las piezas separadoras en caso
de recolocación.
También podrá disponerse un elemento elástico
que actúe sobre la parte esferoide o esférica y que ejerza una
fuerza contraria a la separación de las piezas separadoras entre sí.
Dicho elemento podrá situarse en el espacio entre piezas
separadoras.
También de manera preferente, la camisa de
confinamiento presenta:
- un canal para introducción del conjunto
empujador;
- una tapa situada en el extremo opuesto al
citado canal; y
- dos paredes enfrentadas entre sí que unen la
tapa con el canal y que definen entre sí un espacio para las piezas
separadoras, abierto en la dirección de separación de las piezas
separadoras.
De una manera especialmente ventajosa, las
piezas separadoras presentan en las paredes laterales de sus
entrantes, a su vez, entrantes y salientes conjugados para guiado
del movimiento relativo de las piezas separadoras entre sí, lo que
facilita el guiado y minimiza la posibilidad de apertura variable de
las piezas separadoras a lo largo de toda su longitud.
Asimismo de manera ventajosa, las piezas
separadoras presentan un sistema de tope a su movimiento relativo.
Preferentemente el sistema de tope está compuesto por al menos
saliente o salientes en una cara de una pieza separadora que
interfiere con un tope que une las paredes de uno de los citados
entrantes conjugados.
Para una mejor compresión de la invención, se
adjunta a título de ejemplo explicativo pero no limitativo, unos
dibujos de unas realizaciones de la presente invención.

según la presente invención.

realización de una pieza separadora perteneciente a una realización
de una prótesis según la presente invención, conjugable con la pieza
separadora de las figuras 15 a 17.

perspectiva desde otro punto de vista, de la pieza separadora de la
figura 18.
La figura 20 es un vista en planta superior de
la pieza separadora de las figuras 18 y 19.
Las figuras 21 a 32 muestran una tercera
realización de prótesis intervertebral.
En las figura 1 a 6 se ha representado una
primera realización de prótesis intervertebral de implantación
endoscópica según la presente invención.
La prótesis intervertebral representada en las
figuras está destinada a colocarse en un espacio intervertebral
mediante técnicas endoscópicas. Su finalidad es asegurar una
distancia mínima entre vértebras contiguas (función que en un
individuo sano realiza el disco intervertebral). Dado que su
implantación es endoscópica, esto significa que la prótesis debe ser
capaz de pasar por el interior de una cánula endoscópica
(generalmente 5-9 mm), y poder desplegarse en el
interior del paciente. Las herramientas para desplegar la prótesis
han de ser también de tipo endoscópico. En las figuras 2, 3, 5 y 6
también se muestran herramientas de este tipo, de control
manual.
La prótesis mostrada en las figuras 1 a 6 está
formada por dos piezas separadoras (1), (2) en forma de cuña. Cada
pieza separadora (1), (2) presenta una cara externa. La separación
entre las caras externas de las piezas separadoras (como la marcada
como -26- en la realización de las figuras 1 a 6, o la marcada como
-103- en la realización de las figuras 7 a 20) define la distancia
mínima de separación intervertebral que asegura la prótesis. Cada
pieza separadora, al tener forma de cuña, de fine una cara o zona de
plano inclinado (19), (29). Las caras o zonas de plano inclinado de
cada pieza separadora presentan entrantes conjugados. Los entrantes
definen zonas de ranura y zonas de pared (11), (22), (13), (15),
(24) asimismo conjugadas, de tal manera que las zonas de pared de
cada pieza separadora quedan introducidas en las ranura de la otra
pieza separadora. Esto hace que cada pieza separadora entre,
parcialmente en la otra, permitiendo un grado de recogimiento máximo
de la prótesis en relación con la distancia de separación en
posición extendida. Asimismo, el sistema actúa como guía para el
movimiento relativo entre piezas separadoras.
El número de ranuras/zonas de pared a realizar
podrá ser variable.
La prótesis también comprende una esfera (4) que
se sitúa en el espacio que dejan las caras en plano inclinado de las
piezas separadoras, estando en contacto con dichas caras (19), (29).
El empuje de la esfera sobre los planos inclinados (19), (29) hace
que las piezas separadoras se separen entre sí, como puede
apreciarse al comparar las figuras 5 y 6. La esfera (4) además,
puede actuar como absorbedor de choques si se realiza en un material
con ciertas propiedades elásticas, tal como por ejemplo nailon o
teflón. No obstante, no es necesario que las propiedades elásticas
de la bola sean excesivamente destacadas, puesto que se ha de
asegurar una separación dimensional mínima y una cierta
estabilidad.
En conjunto de piezas separadoras (1), (2) y
esfera, queda confinado por una camisa (3), que no obstante presenta
aberturas para permitir el movimiento de despliegue de las piezas
separadoras (1), (2). Funciones principales de la camisa son impedir
que las piezas del conjunto, especialmente la esfera (4), realice
movimientos transversales y agrupar y proteger el conjunto de piezas
para facilitar su instalación. Tanto en el ejemplo de realización
mostrado en las figuras 1 a 6, como en el ejemplo mostrado en las
figuras 7 a 20, y tal como se representa en la figura 14, la camisa
(3) presenta forma cilíndrica con sendas aberturas en su pared para
permitir el movimiento de las piezas separadoras, de manera tal que
la camisa queda constituida por una tapa (32) que ejerce de
confinamiento distal para las piezas separadoras, una zona de canal
(33) que sirve de zona de introducción de herramientas de manejo de
la prótesis en general (7), y de partes empujadoras (5) de la esfera
(4) en particular. Ambas zonas quedan unidas por dos paredes (33),
(34) enfrentadas entre sí, lo que define la abertura para movimiento
de las piezas separadoras. El canal (33) de las realizaciones
mostradas está roscado interiormente (31).
En efecto, en la realización mostrada, la esfera
(4) es, a su vez, empujada por una parte empujadora (5) que
permanece en el interior de la prótesis una vez instalada. En la
realización mostrada, la parte empujadora es una pieza roscada (en
particular, un espárrago roscado) que presenta una cara de empuje de
la espera y otra cara con un elemento de recepción de un elemento
roscador (7), por ejemplo un rebaje de recepción de una llave Allen,
u otro tipo de unión. La parte empujadora (5) presenta un roscado
conjugado con el canal (33) de la camisa, de tal manera que el
avance de la parte empujadora (5) se realiza a través del paso de
rosca. Esto resulta ventajoso, por cuanto permite y/o favorece tanto
posibilidades casi infinitas de selección de la distancia de
separación definida por las piezas separadoras, como una
reversibilidad de la prótesis, lo que permite ajustar la prótesis e
incluso extraerla con seguridad. Además, el roscado ejerce de tope
que impide el cierre de la prótesis una vez instalada.
Para facilitar la reversibilidad, puede
disponerse de un elemento elástico de recobro (8) que una las piezas
separadoras (1), (2) entre sí. Este elemento podría ser, por
ejemplo, una malla con propiedades elásticas (por ejemplo una malla
en material elástico, una malla cortada, etc.) o incluso una o
varias gomas o toroides rodeando las piezas separadoras. En el caso
las gomas o toroides, resultará preferente realizar surcos de
asentamiento del elemento elástico en las piezas separadoras (1),
(2), En el caso de presentar mallas, podrían colocarse elementos de
anclaje (salientes), en caso de considerarse necesario.
Como se ha representado esquemáticamente en las
figuras, la prótesis objeto de la invención, debido a su estructura,
puede ser manipulada fácilmente a través de un espacio endoscópico
mediante las herramientas adecuadas (7), (8), (9), (10). El mando
(10) podrá ser manual o semiautomático, por ejemplo. Básicamente,
una herramienta Allen de tipo endoscópico (7) con un mando adecuado
para su giro controlado (10) puede ser suficiente. Opcionalmente,
también pueden incluirse soportes tubulares (8), (9) con anclaje de
bayoneta para bloqueo u otros elementos, según técnicas
conocidas.
En las figuras 7 a 20 se ha representado una
realización alternativa a la de las figuras 1 a 6 que se diferencia
principalmente en la forma de las piezas separadoras (1), (2). En
las figuras, elementos similares a los representados en las figuras
1 a 6 han sido representado con idénticos numerales, y, por lo
tanto, no serán descritos en profundidad.
Las piezas separadoras (1), (2) de las figuras 7
a 20 siguen el mismo esquema general que las de las figuras 1 a 6.
Presentan forma de cuña y presentan entrantes o ranuras conjugados
que definen un conjunto de entrantes y zonas de pared conjugados
(101), (102), (201), (202), (203) similar.
Una de las diferencias consiste en que las
paredes laterales de los entrantes, es decir, de las paredes (101),
(102), (201), (202), (203), presentan a su vez, entrantes y
salientes conjugados (106), (206), (107), (207), (108), (208),
(109), (209), (110), (210). Esto mejora el guiado entre piezas
separadoras, impidiendo el giro según un eje perpendicular al del
movimiento de separación de las piezas separadoras (1), (2).
Otra de las diferencias que se pueden apreciar
es la existencia de un sistema de tope máximo a la separación entre
piezas separadoras (1), (2), lo cual resulta interesante, entre
otros casos, si se desea prescindir del elemento elástico de recobro
(8). En particular, como se aprecia en la figura, el sistema de tope
está compuesto por unos salientes (104), (105) en una pieza
separadora (1) que interfieren con topes (204), (205) situados en
las paredes de alguna ranura conjugadas. En este caso, existen dos
salientes (104), (105), mientras que los topes han sido obtenidos
mediante la inclusión de un pasador. Por supuesto, otros sistemas de
tope también son posibles.
Los salientes (104), (105) pueden ser utilizados
asimismo para implementar un sistema elástico de recobro que
comprenda un fleje o bayoneta que sea deformado por empuje por los
salientes durante el movimiento de separación de las piezas
separadoras, de tal manera que el fleje ejerza una fuerza que tienda
a cerrar las piezas separadoras. Dicho fleje podrá ser, por ejemplo,
una lámina metálica curvada que sea aplanada por los topes al ser
empujada por éstos. Dicho sistema no ha sido mostrado por las
figuras. Por supuesto, son posibles otras maneras de colocación del
fleje, con independencia de los salientes (104), (105).
Las figuras 21 a 32 muestran una tercera
realización de la prótesis intervertebral objeto de la presente
invención. Dado que sus principio de funcionamiento y componentes
principales coinciden con los anteriormente descritos, ésta no será
descrita en detalle, explicándose únicamente algunas soluciones
diferentes a las antes mostradas. Aquellos elementos iguales o
equivalentes a los anteriormente mostrados han sido indicados en los
dibujos con idénticos numerales.
Uno de los puntos de diferencia con las
realizaciones anteriormente mostradas es que el dispositivo
empujador de las piezas separadoras es una pieza única con una zona
o parte esferoide (4) y una parte empujadora similares a las
anteriormente mostradas. La realización como pieza única presenta la
ventaja de que cuando desenrosca la parte empujadora (5) la parte
esferoide (4) sigue de manera solidaria, a la parte empujadora, sin
necesidad de colocar, por ejemplo, un elemento elástico de recobro,
tal como por ejemplo un muelle, en el espacio entre piezas
separadoras, ejerciendo una acción sobre la parte esferoide
contraria a la separación de las piezas separadoras.
Asimismo, para mejorar las propiedades elásticas
de la parte esferoide (4) se ha procedido a realizar un vaciamiento
o taladro (401) en la esfera. De esta manera la parte esferoide (4)
puede absorber impactos por compresión del espacio
intervertebral.
Asimismo, puede observarse que la superficie o
cara externa de las piezas separadoras (1), (2), presenta salientes
(199) para favorecer la integración ósea de la prótesis con las
vértebras. Para favorecer esta integración, también podrán
disponerse entrantes y/o utilizar materiales porosos adecuados para
el fin de la integración ósea.
Asimismo, las piezas separadoras (1), (2),
presentan asimismo y de manera adicional otro conjunto de entrantes
y salientes conjugados (1004), (1005), (2002), (2003), (2004),
(2005) que impiden aún más el giro relativo entre piezas
separadoras. El giro impedido por este sistema de entrantes y
salientes puede observarse en las figuras 27 y 28 (en el que los
entrantes y salientes han dejado de actuar). Este giro no es
impedido por el sistema de tope (204), (205) que en este caso
interfiere con las paredes de ranuras (221), (241) realizadas en las
paredes (22), (24) pertenecientes al sistema de zonas de ranuras y
zonas de pared conjugadas de las piezas.
Asimismo, las piezas presentan formas
redondeadas (2001) para recibir de manera más adecuada la esfera o
parte esferoide (4), de tal manera que ésta presenta una mayor área
de contacto con las piezas separadoras.
La prótesis objeto de la presente invención es
muy versátil. Típicamente puede desplegarse desde una distancia de
separación de 6,3 mm hasta una distancia de separación de hasta 15
mm.
En cuanto a los materiales, éstos serán
compatibles con su aplicación sanitaria, pudiéndose utilizar, por
ejemplo, metales como el titanio o materiales sintéticos aprobados
para el uso médico quirúrgico, como por ejemplo poliéster éster
cetona (PEEK) con hidroxiapatita.
En las figuras los elementos se han representado
de manera esquemática en aras a facilitar la comprensión conceptual.
En particular, se ha sido especialmente esquemático en lo referente
a las herramientas de manejo de la prótesis, puesto que estas
dependerán no sólo de la realización concreta de la prótesis, sino
del diseño y forma del resto del instrumental que se utilice.
Obviamente, son numerosas las variantes posibles a lo mostrado, en
especial en lo referente a los detalles de fabricación.
Si bien la invención se ha descrito con respecto
a ejemplos de realizaciones preferentes, éstos no se deben
considerar limitativos de la invención, que se definirá por la
interpretación más amplía de las siguientes reivindicaciones.
Claims (20)
1. Prótesis intervertebral, del tipo destinada a
asegurar una distancia mínima entre dos vértebras conjuntas,
caracterizada porque comprende:
- dos piezas separadoras con forma general de
cuña, presentando cada pieza una cara externa para contacto con una
vértebra y otra cara de plano inclinado oblicua a la cara externa,
presentando dichas piezas en la cara o zona del plano inclinado de
la cuña un conjunto de entrantes conjugados que permite que una
pieza entre, parcialmente, en la otra;
- un conjunto empujador de las piezas
separadoras con una zona esferoide en contacto con el plano
inclinado de dichas piezas;
- una camisa de confinamiento para impedir el
movimiento de la esfera en una dirección perpendicular a la del
conjunto empujador;
de tal manera que el avance del conjunto
empujador provoca un avance de la zona esferoide sobre la zona de
plano inclinado de las piezas separadoras, provocando el alejamiento
de las piezas separadoras entre sí y modificando la altura de la
prótesis.
2. Prótesis, según la reivindicación 1,
caracterizado porque el conjunto empujador está formado por
una parte esférica o esferoide y una parte empujadora de esta
última.
3. Prótesis, según la reivindicación 1 ó 2,
caracterizada porque la camisa de confinamiento presenta un
canal de entrada roscado para la parte empujadora, y la parte
empujadora presenta un roscado conjugado con el roscado del canal de
entrada.
4. Prótesis, según cualquiera de las
reivindicaciones 1 ó 3, caracterizada porque la prótesis
presenta un elemento elástico de recobro que une las piezas
separadoras entre sí.
5. Prótesis, según la reivindicación 4,
caracterizada porque dicho elemento elástico de recobro es
una malla con propiedades elásticas rodeando las piezas
separadoras.
6. Prótesis, según la reivindicación 4,
caracterizada porque dicho elemento elástico de comprende una
goma elástica.
7. Prótesis, según la reivindicación 4,
caracterizada porque dicho elemento elástico es un fleje.
8. Prótesis, según cualquiera de las
reivindicaciones 1 a 7, caracterizada porque la camisa de
confinamiento presenta:
- un canal para introducción del conjunto
empujador;
- una tapa situada en el extremo opuesto al
citado canal; y
- dos paredes enfrentadas entre sí que unen la
tapa con el canal y que definen entre sí un espacio para las piezas
separadoras, abierto en la dirección de separación de las piezas
separadoras.
9. Prótesis, según cualquiera de las
reivindicaciones 1 a 8, caracterizada porque la citada camisa
presenta forma general cilíndrica.
10. Prótesis, según cualquiera de las
reivindicaciones 1 a 9, caracterizada porque las piezas
separadoras presentan en las paredes laterales de sus entrantes, a
su vez, entrantes y salientes conjugados para guiado del movimiento
relativo de las piezas separadoras entre sí.
11. Prótesis, según cualquiera de las
reivindicaciones 1 a 10, caracterizada porque las piezas
separadoras presentan un sistema de tope a su movimiento
relativo.
12. Prótesis, según la reivindicación 11,
caracterizada porque el sistema de tope está compuesto por al
menos saliente o salientes en una cara de una pieza separadora que
interfiere con un tope que une las paredes de uno de los citados
entrantes conjugados.
13. Prótesis, según cualquiera de las
reivindicaciones 1 a 12, caracterizada porque la parte
empujadora es un elemento roscado que presenta una parte de empuje
de la parte esférica o esferoide y otra cara con un elemento de
recepción de un elemento roscador.
14. Prótesis, según cualquiera de las
reivindicaciones 1 a 13, caracterizada porque la parte
esférica o esferoide está realizada en un material elástico.
15. Prótesis, según cualquiera de las
reivindicaciones 1 a 14, caracterizada porque la parte
esférica o esferoide está realizada en teflón.
16. Prótesis, según cualquiera de las
reivindicaciones 1 a 15, caracterizada porque está realizada
en nailon.
17. Prótesis, según cualquiera de las
reivindicaciones 1 a 16, caracterizada porque la esfera
presenta un vaciamiento para mejorar sus propiedades elásticas.
18. Prótesis, según cualquiera de las
reivindicaciones 1 a 17, caracterizado porque las caras
externas de las piezas separadoras presentan salientes para
favorecer la integración ósea.
19. Prótesis, según cualquiera de las
reivindicaciones 1 a 18, caracterizado porque la cara externa
de las piezas separadoras es curva.
20. Prótesis, según cualquiera de las
reivindicaciones 1 a 19, caracterizado porque el dispositivo
presenta además un elemento elástico que actúa sobre la parte
esférica o esferoide y que ejerce una fuerza de efecto contrario a
la separación de las piezas separadoras.
