Abstract
El uso de cistina y/o cisteína para preparar productos medicinales para la perversión y el tratamiento de estrés oxidativo que resulta de un tratamiento de hemodiálisis en pacientes que padecen fallo renal crónico o que padecen enfermedad renal en fase terminal, mediante una administración oral.
Description
El uso de cistina o cisteína para la prevención
y el tratamiento de estrés oxidativo provocado por hemodiálisis en
pacientes con enfermedad renal aguda o crónica.
La presente invención se refiere al uso de
cisteína y su forma oxidada de disulfuro (cistina) para preparar
productos médicos para la prevención y el tratamiento del estrés
oxidativo que resulta del tratamiento hemodiálisis en pacientes que
padecen fallo renal crónico o enfermedad renal en fase terminal.
El estrés oxidativo es definido como un
desequilibrio entre los sistemas fisiológicos de protección
antioxidante y la producción aumentada de radicales de oxígeno o
nitrógeno por las células del sistema inmune. El resultado puede
ser el deterioro de la estructura molecular de proteínas, azúcares y
líquidos junto con un deterioro de las funciones celulares que
perjudica también a las funciones de los órganos vitales del cuerpo.
El estrés oxidativo se ha observado que es particularmente evidente
en pacientes que padecen de fallo renal y están experimentando
hemodiálisis. Este fenómeno es atribuido a una bioincompatibilidad
entre las células sanguíneas en circulación del paciente y las
membranas de diálisis, junto con otros factores como un estado
urémico crónico. Esta bioincompatibilidad conduce a una producción
excesiva de especies reactivas de oxígeno (ROS) por el sistema
inmune y, al mismo tiempo, a una reducción de la capacidad
antioxidante del cuerpo debida a pérdidas de moléculas
antioxidantes como glutatión (GSH), vitamina A, vitamina C y
vitamina E a través de los filtros de las membranas de
diálisis.
Una de las consecuencias de esta respuesta
inmune anormal es un estado de estrés oxidativo, que conduce a una
mayor susceptibilidad a una infección debido a la respuesta inmune
defectuosa, a la amiloidosis y la aterosclerosis acelerada debida a
la activación continua de las células inmunocompetentes. Esto activa
una respuesta inflamatoria, con la consecuente liberación continua
de citoquinas y enzimas proteolíticas lisosomales y una
estimulación de la producción de radicales libres; en la práctica,
el estrés oxidativo se convierte en un procedimiento
auto-replicante y genera un estado de inflamación
crónica.
La consecuencia principal del estrés oxidativo
son las complicaciones cardiovasculares que son la causa de muerte
principal en pacientes que padecen fallo cardiaco crónico. A un
nivel local, estas complicaciones se manifiestan por alteraciones
del endotelio, acumulación de lípidos, formación de coágulos y
oclusión del lumen.
A un nivel sistémico, el estrés oxidativo
crónico estimula la síntesis de proteínas en fase aguada por el
hígado a expensas de la síntesis de otras proteínas como albúmina y
trasferrina: el resultado es la malnutrición, que se ve exacerbada
por la descomposición catabólica de proteínas musculares y un
apetito reducido.
El fallo renal crónico (CRF) es una enfermedad
progresiva del hígado; cuando el hígado ha perdido toda su
capacidad de purificar la sangre, los pacientes no pueden sobrevivir
y tiene que ser sometidos al procedimiento de diálisis. Este estado
es definido como enfermedad renal en fase terminal (ESRD).
Se describen a continuación ejemplos de
tratamiento del estrés oxidativo administrando productos con
actividad antioxidante. El producto usado más a menudo para la
prevención y el tratamiento del estrés oxidativo en la actualidad
es N-acetilcisteína (véase, por ejemplo, Kidney
Int., Vol 64 (2003), pág. 82-91; Current
Med. Chem., 2003, 10 pág. 1241-53). En
particular, el documento WO 01/02004 describe el uso de
N-acetilcisteína mediante inyección intravenosa
antes y/o durante el tratamiento de hemodiálisis.
Según Nakanishi et al., Kidney
Int., 2003 Mar; 63(3) 1137-40, la
concentración en plasma de cisteína y homocisteína aumenta en
pacientes con fallo renal crónico que experimentan diálisis.
Consecuentemente, no parece lógico aumentar adicionalmente la
concentración de cisteína en estos pacientes.
Santangelo F., Current Med. Chem., 2003,
10 pág. 2599-2610, ha examinado recientemente los
usos terapéuticos de profármacos de cisteína como
N-acetilcisteína y sus esteres, tiazolidinas,
\gamma-glutamilcisteína y ésteres de glutatión,
exponiendo particularmente la importancia de la concentración de GSH
intracelular. Sin embargo, nunca se ha descrito ni sugerido en este
examen la administración oral de cisteína o cistina en lugar de sus
derivados.
El documento
DE-A-3414491 describe mezclas orales
de aminoácidos, que pueden contener adicionalmente cisteína o
cistina, para el tratamiento de la insuficiencia renal aguda o
crónica. Las composiciones mejoran el resultado de la terapia para
pacientes de diálisis.
La presente invención incluye la administración
oral de cistina y/o cisteína para la prevención y el tratamiento
del estrés oxidativo que resulta del tratamiento de hemodiálisis en
pacientes que padecen de fallo renal crónico o de enfermedad renal
en fase terminal (ESRD).
Es preferido el uso de cisteína, pero podrían
añadirse también otras sustancias que se conocen que tienen
propiedades antioxidantes como taurina, ácido lipoico y vitaminas A,
C y E.
Por lo tanto, esta invención se refiere al uso
de cistina y/o cisteína para preparar productos medicinales para la
prevención y el tratamiento del estrés oxidativo que resulta del
tratamiento de hemodiálisis en pacientes que padecen fallo renal
crónico o enfermedad renal en fase terminal (ESRD), mediante una
administración oral.
Estas composiciones se prepararan según métodos
convencionales bien conocidos en la tecnología farmacéutica, como
los descritos en la publicación "Remington's Pharmaceutical
Handbook", Mack Publishing Co., N.Y., USA.
Esta invención incluye la administración oral de
una cantidad eficaz de cistina y/o cisteína antes y/o después de un
tratamiento de hemodiálisis en pacientes que padecen fallo renal.
Esta cantidad dependerá de diversos factores como la gravedad del
estado y el peso del paciente. Sin embargo, una dosis unitaria
contendrá generalmente 200 a
1000 mg de cistina y/o cisteína. Las composiciones según la invención se administraran normalmente antes y/o después del tratamiento de hemodiálisis. Estas composiciones son administradas preferentemente en la forma de composiciones con una dosis oral unitaria.

Según un aspecto adicional de la invención, la
cistina o cisteína puede estar asociada con antioxidantes orales no
tóxicos, en particular vitaminas A, C y E, licopeno, ácido lipoico,
ácido ascórbico y taurina. La vitamina E y la taurina son
preferidas.
Según la invención, las composiciones pueden
estar en la forma de comprimidos, cápsulas, preparaciones orales,
polvos, gránulos, pastillas, polvos reconstituibles, jarabes,
soluciones o suspensiones. Las composiciones sólidas pueden
contener excipientes convencionales, por ejemplo, aglutinantes como
celulosa, manitol y lactosa; diluyentes como carbonato de calcio,
fosfato de calcio y lactosa; agentes de compresión; lubricantes como
estearato de magnesio; disgregantes como almidón,
polivinilpirrolidona y derivados de almidón; colorantes y agentes
para dar sabor.
Las composiciones líquidas pueden contener
excipientes convencionales, por ejemplo, agentes suspensores como
sorbitol, metilcelulosa, hidroxietilcelulosa y carboximetilcelulosa;
conservantes, por ejemplo, p-hidroxibenzoato de
metilo o propilo o ácido sórbico y agentes convencionales para dar
sabor o colorantes si se desea.
La administración oral de cistina y/o cisteína
ofrece considerables ventajas en términos de costes, facilidad y
seguridad de administración en comparación con el uso de
formulaciones intravenosas y/o de infusión de
N-acetilcisteína, que requiere personal
experimentado para la administración antes del tratamiento de
diálisis e incluye también el riesgo de contaminación durante la
diálisis.
Claims (3)
1. El uso de cistina y/o cisteína para preparar
productos medicinales para la perversión y el tratamiento de estrés
oxidativo que resulta de un tratamiento de hemodiálisis en pacientes
que padecen fallo renal crónico o que padecen enfermedad renal en
fase terminal, mediante una administración oral.
2. Uso según la reivindicación 1, en el que la
cistina y/o cisteína es para ser administrada en dosis unitarias
que varían en el intervalo de 200 a 1000 mg.
3. Uso según la reivindicación 1 ó 2, en el que
la cistina y/o cisteína es para ser administrada antes y/o después
de un tratamiento de hemodiálisis.
