UN IMPLANTE DE FUSION PARA SU INSERCION ENTRE VERTEBRAS OPUESTAS INCLUYE UN CUERPO QUE TIENE UNA SUPERFICIE INTERNA QUE DEFINE UNA CAMARA INTERIOR HUECA PARA CONTENER AHI UN MATERIAL DE INJERTO. EL CUERPO INCLUYE UN MECANISMO DE TRANSFERENCIA DE ESTRES PARA TRANSFERIR EL ESTRES DESDE LAS VERTEBRAS QUE RODEAN EL IMPLANTE AL MATERIAL DEL INJERTO CONTENIDO EN EL INTERIOR DEL IMPLANTE. Description
Implante de fusión.
La presente invención se refiere a implantes para
su inserción entre vértebras opuestas con el objeto de estabilizar
la columna vertebral. Más particularmente, la presente invención da
a conocer una jaula o cuerpo de fusión que se implanta
quirúrgicamente para estabilizar vértebras adyacentes de la columna
vertebral, a la vez que induce el crecimiento óseo entre las
mismas.
Las jaulas de fusión o los dispositivos de fusión
intercorporal proporcionan un medio para abrir el espacio discal
colapsado entre vértebras opuestas, para liberar presión en los
nervios y/o la médula espinal, colocando un separador o separadores
en el espacio discal.
Las jaulas de fusión utilizadas actualmente son
dispositivos cilíndricos o rectangulares que tienen una parte
externa roscada o dentada para acoplarse con las placas vertebrales
extremas y, de este modo, impedir el deslizamiento de la jaula.
Generalmente, este tipo de jaulas son huecas. Se pueden llenar con
injerto para inducir la fusión de las dos vértebras. Este tipo de
dispositivos proporcionan un elevado potencial para eliminar las
extensas incisiones que se requieren para la instrumentación
posterior, y abren la puerta a una cirugía mínimamente invasiva.
La técnica de implante vertebral, tal como las
jaulas de fusión, se ha desarrollado en gran medida. Los recientes
desarrollos se han dirigido a diversos aspectos del diseño de la
jaula. Por ejemplo, la patente USA 5.055.104, de Ray, publicada el 9
de octubre de 1991, da a conocer un procedimiento para implantar
quirúrgicamente jaulas de fusión roscadas entre vértebras lumbares
adyacentes por aproximación anterior. El dispositivo que se da a
conocer en dicha patente consiste en una hélice sólida de alambre
con retención de forma, que es hueca dentro de la hélice y presenta
aberturas entre las espiras adyacentes de la jaula.
La patente USA 4.011.602, de Rybicki y otros, da
a conocer un dispositivo expansible para su fijación al tejido óseo.
El dispositivo incluye un mecanismo ajustable para expandir el
elemento de cuerpo contra el tejido en el que se implanta, hasta tal
punto que proporcione un esfuerzo compresivo capaz de mantener un
ajuste ajustado y estable. Sin embargo, una vez ajustado, el
dispositivo es sólido y fijo, tal como el dispositivo dado a conocer
en la patente de Ray mencionada anteriormente.
La patente USA 5.489.308, de Kuslich y otros,
publicada el 6 de febrero de 1996, también da a conocer un implante
espinal para su utilización en la estabilización de la columna
vertebral. El implante incluye orificios a través del mismo,
colocados de tal modo que se desprenda hueso hacia el interior del
implante a medida que se hace rotar el implante.
Todas las patentes anteriormente mencionadas, así
como muchas otras patentes de la técnica, abordan aspectos tales
como la aproximación anterior, la expansión y posterior fijación, y
medios para colocar automáticamente astillas de hueso dentro del
espacio hueco interior para promover la fusión. Sin embargo, quedan
diversos aspectos de máxima importancia que no han sido abordados
por la técnica anterior.
Por ejemplo, los dispositivos de fusión
utilizados actualmente son dispositivos cilíndricos o rectangulares
que son anchos y voluminosos. El tamaño es un factor extremadamente
importante en cirugía mínimamente invasiva, en la que el dispositivo
se coloca dentro del cuerpo utilizando una cánula para minimizar el
tamaño de la incisión y, en consecuencia, el periodo de recuperación
en el hospital. En este procedimiento, es deseable practicar una
incisión lo más pequeña posible. Se utiliza una cánula para dirigir
el implante al destino deseado, debiendo ser el implante menor que
el núcleo interior de la cánula para permitir la inserción del mismo
a través de ella. Los implantes espinales actuales requieren una
incisión mayor debido a su forma ancha y voluminosa, requerida por
su estructura, que no se dirige a minimizar la incisión.
Otro problema de las jaulas de implante
utilizadas en la actualidad consiste en una falta absoluta de
comprensión de la repartición de carga, la ley de Wolf y la calidad
de la fusión. Las patentes citadas anteriormente, así como otras,
dan a conocer un dispositivo fijo que se inserta entre las placas
vertebrales extremas y se rellenan con injerto. Anteriormente se ha
asumido que si el injerto se coloca adecuadamente, tendrá lugar la
fusión. Sin embargo, la calidad del hueso está relacionada con el
esfuerzo. La ley de Wolf afirma que el hueso crece a lo largo de las
líneas de esfuerzo. Las jaulas según la técnica anterior,
anteriormente mencionadas, mantienen los cuerpos vertebrales
separados y actúan únicamente como separadores sólidos. En
consecuencia, el material de injerto del interior de la jaula
resorbe y nunca se ve tensionado. Sin esfuerzo, el material de
injerto del interior de la jaula no proporciona eficazmente calidad
de fusión. Si tiene lugar alguna fusión, lo que resulta difícil de
determinar, en el mejor de los casos, en jaulas metálicas utilizando
rayos X o la mayoría de las otras técnicas de diagnóstico, la
calidad de fusión será, en el mejor de los casos, pobre.
En segundo lugar, tal como se ha mencionado
anteriormente, el tamaño de la jaula es un aspecto importante. El
futuro de la cirugía espinal se basará en las técnicas quirúrgicas
mínimamente invasivas, tal como se ha mencionado anteriormente. El
cirujano implanta la jaula o jaulas en el disco utilizando una
cánula. Esto permite realizar una incisión pequeña en la piel,
minimizándose el trauma del tejido blando y reduciéndose el periodo
de recuperación y permanencia en el hospital. Para liberar
adecuadamente la presión sobre los nervios y la médula espinal, el
disco colapsado se debe abrir hasta un estadio tan cercano al
precolapsado como sea posible. Con los diseños de jaula actuales
descritos anteriormente, la jaula se diseña para abrir el espacio
discal hasta 16 mm, requiriendo dicha jaula una cánula de entre 18 y
20 mm. Una cánula de este tipo requiere una incisión grande. Sería
deseable proporcionar un medio que permita a una jaula de 16 mm
descender a lo largo de una cánula menor, tal como una cánula de 12
mm o de 14 mm.
Además, es deseable permitir que la jaula
comparta el esfuerzo con el hueso circundante. Una dinámica de este
tipo no sólo permitiría que la jaula actuara como un separador
necesario, sino que también permitiría que el hueso se viera
tensionado, mejorando de este modo el éxito de la fusión y la
calidad de la misma.
Además, sería deseable proporcionar al cirujano
un medio para abrir fácilmente el espacio discal. Los dispositivos
actuales requieren instrumentación para abrir el espacio discal, lo
que a menudo resulta ineficaz o difícil de realizar.
Según la presente invención, se da a conocer un
implante de fusión para su inserción entre vértebras opuestas. El
implante incluye un cuerpo que tiene una superficie interior que
define una cámara interior hueca para alojar un material de injerto.
Además, el cuerpo incluye medios de transferencia de esfuerzo para
transferir esfuerzo desde las vértebras que rodean el implante al
material de injerto alojado dentro del implante.
La presente invención también da a conocer el
hecho de que el implante de fusión incluye una parte del cuerpo que
consiste en una estructura hueca que tiene un eje longitudinal
central. La estructura incluye, por lo menos, una parte del cuerpo
flexible para permitir la contracción y la expansión alrededor del
eje.
El implante de fusión se puede implantar entre
vértebras opuestas insertando el implante en un espacio discal
colapsado entre vértebras adyacentes, incluyendo el implante un
cuerpo colapsado. A continuación, el cuerpo se expande para separar
las vértebras adyacentes.
Otras ventajas de la presente invención se
comprenderán fácilmente a medida que la misma se entienda mejor
haciendo referencia a la siguiente descripción detallada,
considerada en relación con los dibujos adjuntos, en los que:
la figura 1 es una vista en perspectiva de una
jaula de implante de fusión conformada según la presente
invención;
la figura 2 es una vista en perspectiva de una
segunda realización de la presente invención;
la figura 3 es una vista en perspectiva de una
tercera realización de la presente invención;
la figura 4 es una vista en perspectiva de una
cuarta realización de la presente invención; y
la figura 5 es una vista en perspectiva de una
quinta realización de la presente invención.
En los dibujos se muestra de forma general en
(10) un implante de fusión para su inserción entre vértebras
opuestas. Las diversas realizaciones de la presente invención se
muestran mediante números con signo prima, que indican las partes
similares entre las distintas realizaciones.
Generalmente, el implante (10) incluye una parte
de cuerpo, indicada de forma general con (12). La parte de cuerpo
(12) se puede fabricar en distintos materiales, tal como materiales
compuestos de dos o más metales, plásticos u otros materiales bien
conocidos en la técnica. Este tipo de dispositivos pueden ser
mecanizados o moldeados, dependiendo del material en el que se
realice el implante (10). Por ejemplo, el implante (10) puede estar
realizado en un material metálico que tenga cualidades tipo muelle,
tal como se describe más adelante. Se puede conformar el metal y, a
continuación, mecanizarse para obtener las formas y tolerancias
descritas más adelante. Alternativamente, el cuerpo (12) se puede
realizar en un material o compuesto plástico que se puede moldear
hasta obtener la forma deseada.
El cuerpo (12) incluye una superficie exterior
(14) y una superficie interior (16). La superficie interior (16)
define una cámara interior hueca (18) para contener un material de
injerto (no mostrado) en su interior. Este tipo de formas generales
de jaulas son bien conocidas en la técnica. Este tipo de formas se
ejemplifican en las patentes USA 5055104, 4011602 y 5489308,
mencionadas anteriormente. La superficie exterior puede incluir un
revestimiento poroso, tal como se conoce en la técnica, para
permitir el crecimiento del hueso. Alternativamente, la superficie
exterior también puede ser estructurada para mejorar la fijación del
hueso.
Con el objeto de maximizar las probabilidades de
que tenga lugar la fusión, se han utilizado diversos materiales para
rellenar los interiores de las jaulas. Cada material se selecciona
en base a su disponibilidad y para que proporcione la mayor
probabilidad de una buena fusión. Estos materiales incluyen el
autoinjerto (o material extraído del paciente durante la operación
quirúrgica), aloinjerto, que es hueso extraído de otro paciente, así
como aloinjerto procesado, que se puede presentar en pequeñas
porciones y tratar de diversas maneras. En lugar de ello, con el
deseo de reducir el tiempo de fusión, se pueden utilizar materiales
biológicamente activos. Estos materiales incluyen hueso tratado con
proteínas morfogénicas, sulfato de calcio y otros sustitutos
artificiales de hueso.
Una característica de la presente invención
consiste en que el cuerpo (12) incluye medios de transferencia de
esfuerzo para transferir esfuerzo desde las vértebras que rodean el
implante (10) al material de implante alojado dentro del implante
(10). Es decir que, a diferencia de las jaulas rígidas de la técnica
anterior, que actúan únicamente como separadores, la presente
invención da a conocer una construcción de cuerpo de jaula que
comparte la carga con las vértebras circundantes. Esto se consigue
mediante una flexión de, por lo menos, una parte del cuerpo (12)
como respuesta al esfuerzo aplicado por las vértebras circundantes.
De este modo, los medios de transferencia de esfuerzo según la
presente invención incluyen, por lo menos, una parte flexible del
cuerpo (12), que puede desviar o transferir de otro modo el esfuerzo
aplicado por las vértebras circundantes al material de injerto
contenido dentro de la cámara interior hueca (18).
En la figura 1 se muestra una primera realización
de los medios de transferencia de esfuerzo según la presente
invención. El cuerpo (12) incluye una parte superior (20) y una
parte inferior (22) para contactar con las placas vertebrales
extremas superior e inferior, adyacentes a las mismas, tras la
colocación del dispositivo. Las partes superior e inferior (20),
(22) son desviables y, en consecuencia, transfieren esfuerzo desde
los cuerpos vertebrales adyacentes al material de injerto contenido
en la cámara hueca (18). Las partes superior e inferior (20), (22)
actúan como partes tipo muelle en una dirección axial respecto a un
eje longitudinal (24), definido por la cámara interior hueca (18),
para proporcionar un medio para la flexión hacia afuera de la
jaula.
El cuerpo (12) puede presentar un grosor uniforme
en toda su extensión o puede presentar un grosor variable en, por lo
menos, un lugar. El cuerpo (12) puede presentar unas partes
laterales (26), (28) relativamente más delgadas, que pueden ser
flexibles respecto al resto del cuerpo (12). Las partes laterales
(26), (28) del cuerpo se muestran como partes opuestas del cuerpo en
la figura 1. Estas partes laterales (26), (28) del cuerpo también
pueden deflectar como respuesta al esfuerzo aplicado al cuerpo (12)
por las vértebras circundantes y, de este modo, transferir dicho
esfuerzo a la cámara interior hueca (18) y al material de injerto
contenido en ella. De este modo, el grosor de pared puede variar a
lo largo de toda la jaula tal como se desee.
El implante (10) según la presente invención se
puede conformar de tal modo que proporcione al médico una serie de
implantes con un grosor de pared variable que proporciona de este
modo a dichos implantes diversos tamaños y flexibilidades. Las
partes superior e inferior (20), (22) o las partes laterales (26),
(28) o una combinación de las mismas se pueden calibrar según el
peso de un paciente variando el grosor de pared seleccionado para
dicho paciente. A los pacientes de mayor tamaño, que ejercerán un
esfuerzo mayor sobre un implante, se les puede insertar implantes
(10) con partes de pared más gruesas, que sigan siendo flexibles
pero que puedan soportar las cargas aplicadas. Similarmente, a los
pacientes con un cuerpo menor y que cargan un peso menor se les
puede insertar implantes (10) con grosores de pared del cuerpo
menores para permitir la flexibilidad en condiciones de menor
esfuerzo.
Las figuras 2 a 5 muestran diversas subespecies
de la presente invención, en las que el cuerpo (12') a (12'''')
incluye una parte de cuerpo contraible. Más específicamente, tal
como se muestra en la realización de la figura 2, el cuerpo (12')
incluye un primer y un segundo extremos abiertos (30), (32) que
definen el eje longitudinal (24') entre ellos. Una longitud del
cuerpo (12') se extiende alrededor del eje longitudinal (24') y
entre los extremos abiertos (30), (32). En esta realización, los
medios de transferencia de esfuerzo proporcionan la contracción y la
expansión del cuerpo (12) alrededor del eje longitudinal (24) y a lo
largo de la longitud del mismo. Esta contracción del cuerpo (12)
también proporciona un implante (10) que se puede contraer hasta
obtener un tamaño menor para su suministro a un lugar quirúrgico a
través de una cánula de dimensiones minimizadas, tal como se
describe más adelante en mayor detalle.
Haciendo particularmente referencia a la
estructura mostrada en la figura 2, el cuerpo (12') consiste, en su
totalidad, en un material flexible que permite una contracción y una
expansión tipo muelle del cuerpo (12') alrededor del eje
longitudinal (24'). Alternativamente, una parte del cuerpo (12')
puede ser de un material flexible respecto al resto de la parte de
cuerpo (12'), tal como se ha descrito anteriormente con referencia a
la primera realización. El aspecto crítico consiste en que por lo
menos una parte del cuerpo (12') sea flexible, para permitir la
contracción y expansión del cuerpo (12') alrededor del eje
longitudinal (24'), tal como se describe más adelante.
Tal como se ha mencionado anteriormente, este
tipo de materiales flexibles son bien conocidos en la técnica, y
pueden ser materiales tipo muelle, tales como metales y compuestos
metálicos, así como plástico y compuestos plásticos.
El cuerpo (12') incluye unos bordes separados
(34), (36) opuestos que se extienden a lo largo de la longitud del
cuerpo (12'). La combinación del cuerpo flexible (12') y la sección
transversal sustancialmente en forma de C del cuerpo (12'),
incluyendo los bordes separados (34), (36), permite que el cuerpo
(12') se enrolle sobre sí mismo de un modo similar a un trozo de
papel enrollado. El diámetro efectivo de la sección transversal del
dispositivo se puede contraer fácilmente desde 16 mm a un diámetro
tan pequeño como 8 ó 10 mm. El implante se puede insertar a través
de una cánula tan pequeña como de 12 ó 14 mm. Esta reducción del
diámetro de la sección transversal tiene una importancia
significativa para reducir la herida quirúrgica desde un tamaño que
requeriría cicatrización o cirugía plástica, hasta un tamaño que no
daría lugar a ninguna cicatriz.
La ranura definida por el espacio entre los
bordes (34), (36') en combinación con la forma del cuerpo (12') se
puede variar según las necesidades. Por ejemplo, en la realización
mostrada en la figura 2, el cuerpo (12') tiene una forma cilíndrica,
presentando los bordes (34), (36) la misma distancia entre sí a lo
largo de toda su longitud, dando lugar, de este modo, a una ranura
recta. La ranura recta da lugar a una contracción de igual distancia
del cuerpo (12') cilíndrico, manteniéndose de este modo la forma
cilíndrica de la parte del cuerpo durante la contracción del cuerpo
(12') alrededor del eje longitudinal (24').
En la figura 3 se muestra una construcción
alternativa, en la que la parte de cuerpo (12'') tiene una forma
cónica. Para restablecer la anatomía natural de la columna
vertebral, son preferentes las formas cónicas. Es decir, la anatomía
natural de la columna vertebral, habitualmente, presenta cierto
ángulo en el espacio entre las vértebras, particularmente en la zona
lumbar. La zona lumbar de la columna presenta una curvatura natural,
por lo que resultan unos espacios discales en los que las
superficies de las vértebras no son paralelas entre sí. Una jaula
cónica puede restablecer la anatomía natural de la columna
vertebral, creando un ángulo entre las superficies exteriores (14')
opuestas del cuerpo (12''), que están en contacto con las placas
vertebrales extremas. Los bordes (34'), (36') se separan entre sí a
lo largo de la longitud del cuerpo (12''). Colapsando el cuerpo
(12'') hasta que los bordes (34'), (36') se tocan, la jaula cónica
se vuelve prácticamente un cilindro, pudiéndose insertar, en
consecuencia, a través de una pequeña cánula, de un modo similar a
la jaula cilíndrica mostrada en las figuras 1 y 2.
En la figura 4 se muestra otra realización de la
presente invención, en la que se muestra una parte de cuerpo
(12''') cilíndrica en combinación con una ranura (30) en ángulo,
definida por los bordes (34''), (36''). Esta realización ejemplifica
la variabilidad de la presente invención para su utilización en
diversas situaciones.
Similarmente, la figura 5 muestra una parte de
cuerpo (14'''') de forma sustancialmente cilíndrica, que incluye
bordes paralelos (34'''), (36'''). Sin embargo, la parte de cuerpo
(14'''') incluye unas partes laterales (38), (40) opuestas
sustancialmente planas. Esto proporciona a la parte de cuerpo
(14'''') una forma sustancialmente rectangular. Estas formas se
prefieren para impedir el rodamiento o rotación del dispositivo
dentro del espacio discal. El dispositivo (10'''') también puede
estar construido con una forma más oblonga, llenando de este modo
eficazmente el espacio que, antes, habría requerido dos dispositivos
según la técnica anterior. Por ejemplo, la patente USA 5.192.327,
concedida a Brantigan, da a conocer implantes oblongos para
vértebras que requieren diversos implantes para producir eficazmente
un resultado deseado. La presente invención puede utilizar un único
implante de diversas formas que permite la implantación de un único
implante en lugar de múltiples implantes.
Tal como se muestra en la figura 2, el cuerpo
(12') puede incluir un mecanismo limitador de la contracción para
limitar la contracción del cuerpo (12') alrededor del eje
longitudinal (24'). El mecanismo limitador de la contracción puede
incluir una parte de reborde (44) que se extiende desde uno o desde
los dos bordes (34), (36') (mostrándose en la figura 2 el reborde
extendiéndose a partir del borde (36'). Tal como se muestra en la
figura 2, el borde (34) contacta y hace tope contra dicha parte de
reborde (44) durante la contracción del cuerpo (12') alrededor del
eje longitudinal (24') mencionado, limitando de este modo la
contracción del cuerpo (12') alrededor del eje longitudinal
(24').
La parte de reborde (44) actúa también como
mecanismo limitador para la cantidad de carga que soportará el
cuerpo (12') y será transferida al material de injerto contenido en
su interior. La cantidad de contracción del cuerpo (12') alrededor
del eje longitudinal (24'), en respuesta al esfuerzo transferido por
las placas vertebrales extremas circundantes, está controlada por la
parte de reborde (44), ya que la parte de reborde (44) únicamente
permite a la jaula compartir carga hasta un cierto punto y, a
continuación, actuar como un separador si el injerto no se fusiona
completamente o es de baja calidad. Dicho de otro modo, el esfuerzo
será transferido por el cuerpo (12') a medida que el cuerpo se
contrae entre el espacio expandido entre los bordes (34), (36') y el
estado contraído, en el que los bordes (34), (36') están en contacto
o el borde (34) está en contacto con el reborde (44). El reborde
(44) proporciona una superficie más amplia para el contacto entre
los bordes (34), (36'). Una vez se aplica un esfuerzo suficiente al
cuerpo (12') como para que los bordes (34), (36') estén en contacto,
no se transfiere más esfuerzo a no ser que el cuerpo (12') incluya
partes flexibles adicionales, tal como se detalla con referencia a
la realización mostrada en la figura 1. En este estado contraído al
máximo el cuerpo (12') actúa únicamente como un separador, sin
transferir ya más esfuerzo. Sin embargo, por lo menos, el separador
mantiene la separación suficiente para permitir la protección de
las partes de nervio y médula espinal adyacentes.
Todos los cuerpos (12) a (12'''') incluyen medios
para fijar el implante (10) a (10'''') entre las vértebras
opuestas. Todas las realizaciones incluyen modelos dentados en forma
de roscas (46), o dientes, o similares. Dichas roscas se pueden
conformar de acuerdo con diversas formas o diseños, tal como se
muestra en la técnica anterior. Dichas roscas pueden presentar
formas helicoidales, rectas, en ángulo o acuñadas, o diversas otras
formas o diseños bien conocidos en la técnica.
Todas las realizaciones contraibles de la
presente invención se muestran con orificios separados (48),
estando dispuesto cada par de orificios alrededor de la ranura
mostrada en las figuras 2 a 5. Los orificios están conformados para
ser engranados por parte de un instrumento (no mostrado), incluyendo
pinzas y similares, que se puede utilizar para forzar el cuerpo
(12') a (12'''') hacia su estado contraído, para su inserción dentro
de la cánula. Similarmente, se puede utilizar una placa de retención
(no mostrada) si se requieren una contracción y una expansión para
la implantación, y no se desean para la utilización a largo plazo.
La fijación se puede fijar mediante ranuras, roscas o dientes tras

La presente invención puede estar realizada en un
metal con memoria, tal como el nitinol. Este tipo de metales se

vértebras. El estímulo, tal como calor, corriente eléctrica o frío,
se puede aplicar al implante, con lo que el implante retornará a su
forma expandida, expandiendo de este modo el espacio discal para
aliviar los nervios pinzados, o similar. A continuación, el implante
mantendrá la separación, aliviando de este modo al

Durante la operación, si se debe utilizar el
implante colocándolo directamente a través de una incisión y sin
ninguna cánula, se puede utilizar el implante que no requiere
ninguna ranura. El implante se coloca mediante incisión y

Habiendo realizado la incisión apropiada y habiendo extraído el
material discal suficiente como para crear un espacio para la jaula
o jaulas, si se desean dos jaulas más pequeñas en lugar de una única
jaula, se insertan los implantes en el espacio, de tal modo que los
dientes (46) a (46'''') de la superficie exterior por encima del
implante retienen el implante (10) a (10'''') en la posición
adecuada.

Para utilizar una cánula más pequeña y/o para
utilizar la capacidad del implante de abrir un espacio discal, se
utiliza un implante que presenta una ranura en su interior, tal como
se muestra en las figuras 2 a 5. El implante se contrae y se dispone
en la cánula para su colocación en el espacio discal, tal como se ha
descrito anteriormente. Una vez el implante abandona la cánula y se
introduce en el espacio discal, el implante se abre de forma natural
hasta recuperar su forma original o se induce a recuperar su forma
original por estímulo, en caso de que el implante esté realizado en
un material con memoria, tal como se ha descrito anteriormente.
Como con los dispositivos según la técnica
anterior, el implante contiene un material de injerto para inducir
la fusión. Gracias a la capacidad del implante de contraerse y
expandirse, el esfuerzo aplicado por las placas vertebrales extremas
sobre el cuerpo (12) a (12'''') de los implantes (10) a (10'''') se
transfiere al material de injerto, transfiriéndose de este modo la
carga de las placas vertebrales extremas al material de injerto
contenido, con lo que el dispositivo aprovecha y aplica la ley de
Wolf, proporcionando una excelente calidad de fusión.
Teniendo en cuenta lo anterior, la presente
invención da a conocer un implante que se puede utilizar únicamente
como separador, transfiriendo las tensiones desde las placas
vertebrales extremas adyacentes al injerto contenido dentro de los
implantes, o también como un implante contraible y expansible.
La invención se ha descrito a título ilustrativo,
y se debe entender que la terminología empleada se ha utilizado a
título descriptivo más que limitativo.
Evidentemente, es posible llevar a cabo muchas
modificaciones y variaciones de la presente invención a la luz de
las descripciones anteriores. Por lo tanto, se debe entender que la
invención se puede utilizar, dentro del alcance de las
reivindicaciones adjuntas, de forma distinta a la específicamente
descrita.
Claims (14)
1. Implante de fusión (10) para su inserción
entre vértebras opuestas, comprendiendo dicho implante (10):
un cuerpo (12), que incluye una superficie
interior (16) que define una cámara interior hueca (18) para alojar
un material de injerto, caracterizado porque dicho cuerpo
(12) incluye medios de transferencia de esfuerzo para transferir
esfuerzo desde las vértebras que rodean dicho implante (10) al
material de injerto alojado dentro de dicho implante (10),
incluyendo dicho cuerpo (12), opcionalmente, medios de fijación (46)
para fijar dicho implante (10) a las vértebras opuestas;
siendo capaz el implante de mantener la
separación entre dos vértebras.
2. Implante (10), según la reivindicación 1, que
incluye unas partes superior e inferior (20, 22) para contactar con
los cuerpos superior e inferior de las vértebras, siendo dichas
partes superior e inferior (20, 22) desviables y definiendo dichos
medios de transferencia de esfuerzo.
3. Implante (10), según las reivindicaciones 1 ó
2, en el que el cuerpo (12) mencionado incluye un primer y un
segundo extremos abiertos (30, 32), que definen un eje longitudinal
(24) entre los mismos, y extendiéndose una longitud de dicho cuerpo
(12) alrededor de dicho eje longitudinal (24) y entre los
mencionados extremos abiertos (30, 32), proporcionando los
mencionados medios de transferencia de esfuerzo una contracción y
una expansión del cuerpo (12) mencionado alrededor de dicho eje
longitudinal (24) a lo largo de su mencionada longitud.
4. Implante (10), según la reivindicación 3, en
el que el cuerpo (12) mencionado consiste en un material flexible
que permite una contracción y una expansión tipo muelle de dicho
cuerpo (12) alrededor del eje longitudinal (24) mencionado.
5. Implante (10), según la reivindicación 3 ó la
reivindicación 4, en el que el cuerpo (12) mencionado incluye unos
bordes separados (34, 36) opuestos, que se extienden a lo largo de
la longitud mencionada de dicho cuerpo (12), estando separados
típicamente dichos bordes separados (34, 36) por una distancia
constante a lo largo de la longitud mencionada de dicho cuerpo (12),
o divergiendo el uno del otro a lo largo de la longitud mencionada
de dicho cuerpo (12).
6. Implante (10), según la reivindicación 5, que
incluye además medios limitadores de la contracción para limitar la
contracción de dicho cuerpo (12) alrededor del eje longitudinal (24)
mencionado.
7. Implante (10), según las reivindicaciones 5 ó
6, en el que, por lo menos, uno de los bordes (34, 36) mencionados
incluye una parte de reborde (44) que se extiende a partir del
mismo, siendo capaz el otro de dichos bordes (34, 36) de contactar y
hacer tope contra dicha parte de reborde (44) durante la contracción
del cuerpo (12) mencionado alrededor del eje longitudinal (24)
mencionado, siendo por ello capaz de limitar la contracción de dicho
cuerpo (12) alrededor de dicho eje longitudinal (24).
8. Implante (10), según una de las
reivindicaciones 3 a 7, en el que el cuerpo (12) mencionado
consiste en un metal con memoria, que se puede doblar desde un
estado original hasta un estado contraído, y que es capaz de
retornar a dicho estado original tras la aplicación de un
estímulo.
9. Implante (10), según cualquiera de las
reivindicaciones 1 a 4, o según la reivindicación 8 en lo que
depende de las reivindicaciones 3 ó 4, en el que el cuerpo incluye
medios de fijación para fijar el implante mencionado a las vértebras
opuestas, y en el que dichos medios de fijación incluyen partes
roscadas (46) en las superficies exteriores opuestas de dicho cuerpo
(12), incluyendo dicho cuerpo (12) unas paredes laterales flexibles
(26, 28) opuestas entre dichas partes roscadas (46) que definen los
medios de transferencia de esfuerzo mencionados.
10. Implante (10), según la reivindicación 1, en
el que el cuerpo (12) consiste en una estructura hueca que tiene un
eje longitudinal (24) central, incluyendo dicha estructura, como
mínimo, una parte del cuerpo flexible para permitir la contracción y
expansión alrededor de dicho eje (24).
11. Implante (10), según la reivindicación 10, en
el que el cuerpo (12) mencionado consiste en un material flexible
que permite la contracción y expansión tipo muelle de dicho cuerpo
(12) alrededor del eje longitudinal (24) mencionado.
12. Implante (10), según la reivindicación 11, en
el que una zona del cuerpo (12) mencionado incluye aberturas a
través de la misma para permitir que un material de injerto,
presente en el interior de dicho cuerpo (12), contacte con el hueso,
presente en el exterior de dicho cuerpo (12), tras la inserción de
dicho implante (10).
13. Implante (10), según cualquiera de las
reivindicaciones 1 a 12, en el que el cuerpo (12) mencionado es
colapsable y expansible, siendo dicho cuerpo (12) automáticamente
capaz de expandirse tras su liberación de un instrumento de
inserción, de modo que se ensancha un espacio discal entre placas
vertebrales extremas opuestas.
14. Implante (10), según cualquiera de las
reivindicaciones 1 a 13, incluyendo una superficie exterior (14)
del cuerpo (12) mencionado, incluyendo dicha superficie exterior
(14) un revestimiento poroso para permitir el crecimiento hacia
adentro del hueso y/o una superficie exterior estructurada para
mejorar la fijación del hueso.
