Abstract
Implant semi-rigide pour la fusion intersomatique des vertèbres. L'invention concerne un dispositif destiné à être inséré entre les corps vertébraux de deux vertèbres adjacentes afin d'obtenir une fusion osseuse. Il agit comme un écarteur semi-rigide permettant de maintenir ou restituer la hauteur de l'espace discal, et d'y insérer une greffe osseuse. L'implant à la forme d'un " U " horizontal. Les 2 branches horizontales du " U " sont perforées par 2 larges trous (3). La partie fermée du " U " à la forme d'un hémicylindre (1) sans aucun effet de surface. Les extrémités des 2 branches horizontales du " U " (4) sont arrondies pour limiter les risques de lésion des éléments neurologiques en cas d'ablation de l'implant. Les faces inférieures et supérieures des 2 branches (2) sont rugueuses ou légèrement crantées afin d'éviter une migration postérieure des implants. Le dispositif selon l'invention est particulièrement destiné à la réalisation d'une fusion osseuse entre le corps de deux vertèbres adjacentes.

La présente invention concerne un implant vertébral intersomatique, c'est-à-dire un implant destiné à être inséré entre les corps vertébraux de deux vertèbres adjacentes dans le but d'obtenir une fusion osseuse. II agirait comme un écarteur semi-rigide permettant de maintenir ou restituer la hauteur de l'espace discal, et d'y insérer une greffe osseuse.
La présente invention concerne un implant vertébral intersomatique, c'est-à-dire un implant destiné à être inséré entre les corps vertébraux de deux vertèbres adjacentes dans le but d'obtenir une fusion osseuse. II agirait comme un écarteur semi-rigide permettant de maintenir ou restituer la hauteur de l'espace discal, et d'y insérer une greffe osseuse.
Les cages intervertébrales présentent toutes l'inconvénient d'êtres rigides, donc de transmettre intégralement les contraintes et de constituer, dans certaines conditions, un obstacle à une bonne reconstitution osseuse. De plus leurs formes cloisonnées diminuent d'une part le volume de greffons osseux implantés entre les deux vertèbres greffées et d'autre part réduisent les surfaces de fusion entre les deux cages. La rigidité de ces cages ne permet pas une adaptation automatique aux forces de contraintes en lordoses du rachis lombaire L'implant objet de la présente invention, apporte une solution à ces inconvénients. Cette présente invention est innovante essentiellement pour 2 raisons 1) D'une part, ces implants ne sont pas rigides, gardant une certaine élasticité, ce qui devrait permettre une accélération de la prise des greffons et de la fusion intervertébrale 2) D'autre part, ces implants sont ouverts, en forme de "U" horizontal, permettant donc d'insérer les greffons à vue au centre de l'implant et entre les deux implants La finalité de l'invention est de laisser le maximum de place pour la greffe, et d'obtenir une fusion plus rapide et plus complète du fait de l'absence de rigidité.
L'implantation se fait de deux manières: - Soit par un procédé de chirurgie classique par voie postérieure, libération de l'espace intervertébral et mise en place de deux implants de part et d'autre du fourreau dural (fig.2), la partie fermée du "U" venant se bloquer sur le ligament commun vertébral antérieur.
- Soit par voie endoscopique et donc selon une voie antérieure. La position des implants sera alors inversée, la partie fermée du "U" venant s'adosser sur le ligament commun vertébral postérieur (fig.3) après excision discale.

Les caractéristiques de la présente invention sont les suivantes 1) Géométriquement l'implant à la forme d'un U horizontal -Les 2 branches horizontales du U sont perforées par 2 trous oblongs (3) afin d'agrandir les surfaces osseuses de fusion intervertébrales. Ces trous permettent aussi la fixation d'un porte-implant -La partie fermé du U à la forme d'un hémicylindre (1) sans aucun effet de surface .
- Les parties postérieures des 2 branches horizontales du U (4) sont arrondies pour limiter les risques de lésion des éléments neurologiques en cas d'ablation de l'implant. -Les faces inférieures et supérieures des 2 branches (2) sont rugueuses ou légèrement crantées afin d'éviter une migration postérieure des implants.
- Les parties inférieure et supérieure sont identiques et plus longues que larges 2) L'implant est réalisé en titane ou en tout autre matériau biocompatible résistant à l'écrasement, recouvert ou non d'hydroxyhapatite, ce qui permet la réalisation de contrôle postopératoire par scanner ou TRM, et aussi d'obtenir une semi-rigidité de l'implant lors des contraintes axiales verticales. L'implant maintient l'espace discale tout en réalisant une stabilisation dynamique du fait de la semi-rigidité de l'implant. Cette élasticité contrôlée de ces implants va permettre une activation de la fusion osseuse par mise en contrainte permanente de la greffe osseuse intervertébrale.
3) L'absence de cloisonnements postérieur et latéral des implants permet la visualisation complète de la zone à greffer lorsque les écarteurs sont implantés de façon définitive dans l'espace intervertébral. La réalisation de la greffe osseuse peut donc se faire implants en place.

1) Implant semi-rigide de contention interne et de fusion intersomatique des vertèbres, constitué par un corps en forme de U horizontal avec une partie antérieure hémicylindrique (1) sans aucun effet de surface, une partie supérieure et inférieure (2) identique plus longue que large, perforées par 2 trous oblongs (3) afin d'agrandir les surfaces osseuses de fusion intervertébrales et de permettre la fixation d'un porte implant, l'implant ainsi configuré ne comportant pas de cloisonnement latéral ni postérieur, ce qui augmente ainsi le volume de greffe osseuse implanté dans l'espace intervertébral
2) Implant semi-rigide de contention interne et de fusion intersomatique selon la revendication 1 caractérisé en ce que les extrémités postérieures des parties supérieure et inférieure (4) sont arrondies pour limiter les risques de lésion des éléments neurologiques en cas d'ablation de l'implant
3) Implant semi-rigide de contention interne et de fusion intersomatique selon les revendications 1 à 2 caractérisé en ce qu'il est réalisé en alliage de titane forgé, revêtu ou non d'hydroxyhapatite, résistant à l'écrasement et donnant une élasticité des parties supérieure et inférieure permettant une activation de la fusion osseuse par mise er contrainte permanente de la greffe osseuse intervertébrale.
