Conjunto para construir un implante de la cabeza de la articulación en forma de caperuza para una articulación artificial de la cadera que presenta por lo menos una caperuza (1) moldeada según la forma de la esfera natural de la articulación que puede colocarse sobre un cuello (2) resecado del fémur, así como por lo menos un medio auxiliar de fijación que puede unirse en el lado proximal con la caperuza (1) y se compone de un cuerpo (3) de armadura hueco con paredes delgadas, con una forma curvada y tal longitud que permite introducirlo a través del cuello (2) del fémur en un conducto en el fémur y fijarlo a lo largo del conducto en el hueso, estando la superficie del cuerpo (3) de armadura atravesada por numerosas perforaciones (4).

Description

Conjunto para construir un implante de la cabeza de la articulación en forma de caperuza para una articulación artificial de la cadera.
La presente invención se refiere a un conjunto para construir un implante de la cabeza de la articulación en forma de caperuza para una articulación artificial de la cadera tal como se conoce del documento EP-A-1260 200.
Ultimamente se aplican en mayor medida los llamados implantes en forma de caperuza que se colocan sobre la cabeza natural restante preparada de la articulación o sobre el cuello resecado del fémur, respectivamente, y se fijan seguidamente en esta posición. Un requisito para una fijación secundaria estable es que la materia ósea del hueso restante sea resistente. Conforme al documento DE-A-102 18 801 se propone acoplar la cabeza de la articulación en forma de caperuza en un pivote que se introduce en un hueco correspondiente fresado en el cuello del fémur. Este pivote presenta una superficie provista de una estructura reticulada tridimensional con mallas abiertas a través de las cuales pueden crecer las trabéculas óseas desde la materia ósea circundante garantizando una fijación secundaria estable.
Este tipo de tratamiento de pacientes fracasa cuando la materia ósea está debilitada, por ejemplo por osteoporosis. En especial debe suponerse que esto afecte a más pacientes femeninas que a pacientes masculinos, por lo que el objetivo de la presente invención consiste en modificar un conjunto del tipo inicialmente mencionado de tal manera que se garantice una fijación secundaria estable también en el caso de una materia ósea debilitada del hueso restante.
Este objetivo se consigue por medio de un conjunto que presenta por lo menos una caperuza moldeada según la forma de la esfera natural de la articulación que puede colocarse sobre un cuello resecado del fémur, así como por lo menos un medio auxiliar de fijación que puede unirse con la caperuza en el lado proximal y se compone de un cuerpo de armadura hueco con paredes delgadas, con una forma curvada y una longitud que permite introducirlo a través del cuello del fémur en un conducto en el fémur y fijarlo a lo largo del conducto en el hueso, estando la superficie del cuerpo de armadura atravesada por numerosas perforaciones.
Las trabéculas óseas de la materia ósea circundante pueden prender en el cuerpo de armadura y garantizar de esta manera una fijación permanente estable. La curvatura del cuerpo de armadura se selecciona de forma especialmente preferida de tal manera que el cuerpo de armadura siga in situ aproximadamente el arco de Adams del fémur en la zona del trocánter menor. De esta manera se consigue una distribución especialmente favorable de la carga en la materia ósea.
En el presente caso, el cuerpo de armadura funciona como elemento estabilizador, pero tiene además el efecto de un implante profiláctico, tal como se conoce por ejemplo del documento DE-100 07 615. No obstante, el carácter profiláctico del cuerpo de armadura no es primordial, sino más bien su carácter funcional ya que el cuerpo de armadura cumple realmente funciones de reducción de la carga, lo que no es el caso en un implante profiláctico conocido.
El cuerpo de armadura está constituido preferentemente por una estructura reticulada, moldeada en forma de tubo. La estructura reticulada proporciona una cantidad suficiente de perforaciones a través de las cuales las trabéculas óseas pueden prender en el cuerpo de armadura.
Una forma de realización alternativa del cuerpo de armadura como parte de un conjunto según una segunda propuesta de solución se diferencia del cuerpo de armadura anteriormente descrito porque se compone de un tubo hueco cuya pared exterior está constituida por una estructura reticulada tridimensional de mallas abiertas. La materia ósea puede crecer desde fuera a través de esta estructura reticulada tridimensional al interior del casquillo. Además, esta estructura reticulada tridimensional ofrece una fijación primaria del implante en el fémur.
Para la fijación de la caperuza en el cuello del fémur está previsto ventajosamente que el fondo de la caperuza esté provisto también de una estructura reticulada tridimensional de mallas abiertas. Las mallas de las estructuras reticuladas tridimensionales en el cuerpo de armadura según la segunda propuesta de solución y en el fondo de la caperuza pueden ser diferentes y pueden estar adaptadas a los tamaños de las trabéculas óseas.
La unión entre la caperuza y el cuerpo de armadura se realiza ventajosamente mediante una unión atornillada autobloqueante que garantiza que la caperuza se mantenga unida con el cuerpo de armadura de forma asegurada contra rotación.
La invención se explica más detalladamente con referencia a un ejemplo de realización según la figura única del dibujo. Esta muestra un implante de la cabeza de la articulación en forma de caperuza realizado mediante el conjunto conforme a la invención e implantado in situ en el fémur.
El fémur (6) presenta un cuello (2) resecado del fémur. La esfera natural de la articulación se ha resecado antes de la implantación. Su función la cumple ahora una caperuza (1) moldeada según la forma de la esfera natural de la articulación. Esta está colocada sobre el cuello (2) del fémur y lo envuelve mediante un collarín (7) perimetralmente continuo. En el presente caso, el fondo de la caperuza (1) está provisto de una capa de una estructura (5) reticulada tridimensional de mallas abiertas en la que prenden y través de las cuales entran las trabéculas óseas que crecen frontalmente desde el cuello (2) resecado del fémur. También el interior del collarín (7) perimetralmente continuo está recubierto de forma similar con una estructura (8) reticulada tridimensional de mallas abiertas. Las estructuras reticuladas tridimensionales en el fondo (5) y en el collarín (7) perimetralmente continuo sirven para la fijación a largo plazo de la caperuza en el cuello (2) del fémur.
Un cuerpo (3) de armadura hueco está unido con la caperuza (1). Este cuerpo (3) de armadura presenta una forma curvada que sigue aproximadamente el arco de Adams en la zona del trocánter menor (9).
El cuerpo (3) de armadura se compone en este caso de una estructura reticulada moldeada en forma de tubo que presenta en su pared exterior numerosas perforaciones (4) a través de las cuales pueden crecer las trabéculas óseas de la materia ósea circundante, aportando de esta manera a una fijación secundaria estable o facilitando la misma.
La unión estable entre la caperuza (1) y el cuerpo (3) de armadura está realizada en el presente caso mediante una unión (10) atornillada autobloqueante. Después de la exploración y la medición de las proporciones, el cirujano selecciona del conjunto de forma individual para cada paciente una caperuza (1) y un cuerpo (3) de armadura y une los mismos mediante una unión atornillada autobloqueante entre ambas partes.

Claims (5)

1. Conjunto para construir un implante de la cabeza de la articulación en forma de caperuza para una articulación artificial de la cadera que presenta por lo menos una caperuza (1) moldeada según la forma de la esfera natural de la articulación que puede colocarse sobre un cuello (2) resecado del fémur, así como por lo menos un medio auxiliar de fijación que puede unirse en el lado proximal con la caperuza (1) y se compone de un cuerpo (3) de armadura hueco con paredes delgadas, con una forma curvada y tal longitud que permite introducirlo a través del cuello (2) del fémur en un conducto en el fémur y fijarlo a lo largo del conducto en el hueso, estando la superficie del cuerpo (3) de armadura atravesada por numerosas perforaciones (4).
2. Conjunto de acuerdo con la reivindicación 1 en el cual el cuerpo (3) de armadura está constituido por una estructura reticulada moldeada en forma de tubo.
3. Conjunto para construir un implante de la cabeza de la articulación en forma de caperuza para una articulación artificial de la cadera que presenta por lo menos una caperuza (1) moldeada según la forma de la esfera natural de la articulación que puede colocarse sobre un cuello (2) resecado del fémur, así como por lo menos un medio auxiliar de fijación que puede unirse en el lado proximal con la caperuza (1) y se compone de un tubo hueco como cuerpo (3) de armadura cuya pared exterior está constituida por una estructura reticulada tridimensional con mallas abiertas, con una forma curvada y tal longitud que permite introducirlo a través del cuello del fémur en un conducto en el fémur y fijarlo a lo largo de este conducto en el hueso.
4. Conjunto de acuerdo con una de las reivindicaciones 1 a 3 en el cual el fondo de la caperuza (1) está provisto de una estructura (5) reticulada tridimensional con mallas abiertas.
5. Conjunto de acuerdo con una de las reivindicaciones 1 a 4 en el cual la unión entre la caperuza y el medio auxiliar de fijación puede realizarse mediante una unión (10) atornillada autobloqueante.
