Description
Aparato para la ablación de masas de tejidos.
La presente invención se refiere, en general, a
electrodos de radiofrecuencia para ablación de tejido y, más
particularmente, a un electrodo de RF mejorado que tiene una
agrupación dispersa de cables para abladir grandes volúmenes de
tejido.
El hígado es un depósito común para metástasis de
muchos cánceres, incluyendo los de estómago, intestino, páncreas,
riñón y pulmón. En cáncer colorrectal, el hígado es el lugar inicial
de dispersión en más de un tercio de pacientes, y está implicado en
más de dos tercios en el momento de la muerte. Mientras que
pacientes con metástasis colorrectal no tratada al hígado no
sobreviven ni cinco años, pacientes que experimentan resección
quirúrgica tienen aproximadamente un 25-30% de
posibilidades de sobrevivir cinco años. Por desgracia, sólo un
número limitado de pacientes son candidatos para resección
quirúrgica.
Se usa, también, criocirugía para el tratamiento
de metástasis hepáticas. La criocirugía, que se basa en un
procedimiento de congelación-deshielo a células a
las que se ha dado muerte no selectivamente, se ha encontrado que es
igualmente eficaz a la resección quirúrgica pero es más protectora
de tejido. Aunque es una mejora sobre resección de tejido quirúrgico
abierto, la criocirugía sufre todavía de desventajas. Es un
procedimiento quirúrgico abierto, requiere colocación de hasta cinco
sondas relativamente grandes y sólo se puede aplicar a un número
limitado de lesiones. Aunque están siendo desarrolladas sondas
percutáneas, actualmente sólo son capaces de tratamiento de lesiones
menores. Lesiones típicas comunes a metástasis colorrectal, sin
embargo, son relativamente grandes. Por lo tanto, se mantienen las
perspectivas para crioterapia percutánea.
Varios investigadores han usado hipertermia de
radiofrecuencia con colocación de electrodos externos, para el
tratamiento de cánceres de hígado. Se sabe que las células tumorales
son más sensibles al calor que las células normales, y que la
hipertermia regional externamente aplicada, suministrada con radio
frecuencia, tiende a abladir el tumor al tiempo que protege de daño
significativo al tejido normal. Aunque esta terapia mejora la
respuesta a quimioterapia sistémica, tiene beneficio incierto para
supervivencia a largo plazo. Una limitación de la hipertermia es que
es difícil calentar los tumores hasta una temperatura letalmente
alta. Además, las células tumorales tienden a ser termoresistentes
si sobreviven a tratamientos anticipados.
Se ha usado, también, hipertermia láser
percutánea para cáncer de hígado principal y metastásico. Se
introducen fibras láser a través de agujas, bajo guía de
ultrasonidos. Las lesiones generadas por láser están representadas
por focos hiperecoicos en las imágenes por ultrasonidos en tiempo
real, que se pueden usar para supervisar el tamaño de la lesión. Los
sistemas de fibra única de baja energía, que no requieren un sistema
de enfriamiento a lo largo de la fibra, pueden generar áreas de
necrosis limitadas a aproximadamente 15 mm de diámetro. Tales
diámetros pequeños son insuficientes para la inmensa mayoría de
lesiones encontradas clínicamente, lo que requiere, así, colocación
de múltiples fibras y tiempos de procedimiento prolongados.
Se ha propuesto, también, hipertermia de
radiofrecuencia (RF), usando un generador electroquirúrgico estándar
y una aguja fina parcialmente enfundada en plástico, para el
tratamiento de tumores de hígado y de otros tumores sólidos. En un
sistema, el aparato fue capaz de generar lesiones de aproximadamente
1x2 cm en un hígado de cerdo. A fin de producir volúmenes de
tratamiento mayores con una única aguja, se han empleado altas
corrientes y temperaturas, pero produce tejido calcinado y
carbonizado, sin agrandar el volumen de tejido a tratar. Para tratar
una lesión mayor, se necesitarían múltiples pases de aguja en
localizaciones diferentes. En ensayos preliminares, este sistema
estableció un 75% de supervivencia en 40 meses.
Se puede ver, por lo tanto, que el tratamiento de
tumores de hígado y de otros tumores sólidos principales y
metastásicos en cualquier lugar del cuerpo, sigue siendo
problemático. La cirugía es eficaz, pero sólo un pequeño porcentaje
de pacientes afectados son candidatos. La crioterapia ha tenido
resultados mejorados, pero la población de pacientes en que se puede
aplicar es esencialmente la misma que para cirugía. Los métodos
percutáneos tienen la virtud de ser menos invasivos, así que se
pueden usar apropiadamente para un espectro mayor de pacientes, pero
los métodos percutáneos actuales sufren todos de una capacidad
limitada para abladir un gran volúmen de tejido en un único
procedimiento con una única pasada de sonda.
El documento
DE-A-2.124.684 sugiere una variedad
de sondas que tienen una pluralidad de electrodos conductores que
puede comprender cables, o líquidos o gases conductores. Los diseños
sugeridos incluyen una sonda que tiene electrodos que salen en
ángulos rectos a la sonda de un punto apartado de la extremidad
distal. Se sugiere por lo visto, también, una estructura lateral a
modo de abanico.
Un objeto general de al menos algunas
realizaciones de la presente invención es proporcionar un sistema y
una sonda desplegable electroquirúrgicos mejorados en un
procedimiento percutáneo que produce un gran volumen de tejido
abladido térmicamente con un único despliegue.
Un objeto adicional es que tales sondas sean
útiles en procedimientos quirúrgicos abiertos así como en los
percutáneos.
Otro objeto es proporcionar una sonda
electroquirúrgica que proporcione tejido tratado uniformemente
dentro de una gran lesión volumétrica.
Todavía otro objeto de al menos algunas
realizaciones de la presente invención es proporcionar una sonda
electroquirúrgica percutánea que requiere sólo un pequeño agujero de
acceso pero proporciona ablación para gran tejido volumétrico.
Todavía otro objeto es proporcionar una sonda
electroquirúrgica que alivie los problemas de calcinación y
carbonización comunes con sondas de aguja única.
Estos y otros objetos serán evidentes para los
expertos en la técnica.
La presente invención proporciona un sistema de
sonda para el tratamiento por radiofrecuencia (RF) de una región
específica dentro de tejido sólido, a la que se denomina en lo
sucesivo "región de tratamiento". En un método que usa la
sonda, después de alcanzar el lugar objetivo, la pluralidad de
electrodos son desplegados dentro del tejido sólido, en una
agrupación tridimensional y, preferiblemente, en una configuración
que se adapta a o abarca todo el volumen de la región de tratamiento
o una porción tan grande del volumen de la región de tratamiento
como sea posible. Más preferiblemente, los electrodos adyacentes
están uniformemente separados entre sí (es decir, pares de
electrodos adyacentes están separados en patrón repetitivo) de
manera que la aplicación de corriente de RF a través de los
electrodos dará como resultado, en general, calentamiento uniforme y
necrosis de todo el volumen de tejido a ser tratado. Ventajosamente,
el uso de múltiples electrodos para tratar un volumen de tejido
relativamente grande permite que la energía de RF sea aplicada con
una densidad de corriente inferior (es decir, a partir de un área
total de electrodo mayor) y, por lo tanto, a una temperatura
inferior en el tejido que rodea inmediatamente el electrodo. Así, se
reduce la calcinación y carbonización de tejido (que ha sido hasta
ahora asociado con el uso de sistemas de electrodo único). El
tratamiento uniforme de un gran volumen de tejido reduce el número
de despliegues de electrodo que son necesarios para tratar una
región de tejido de cualquier tamaño dado.

Las propiedades preferidas se establecen en las reivindicaciones
dependientes y en lo que sigue. La descripción siguiente de métodos
que emplean la sonda está destinada a ayudar a la comprensión del
funcionamiento de la sonda según la invención.
Un método para emplear la sonda puede comprender
introducir los electrodos a través de tejido sólido hasta un lugar
objetivo dentro de una región de tratamiento. Los electrodos se
mantienen en una configuración radialmente restringida o aplastada a
medida que se les hace avanzar a través del tejido hasta el lugar
objetivo y son desplegados entonces más desde el lugar objetivo a la
región de tratamiento en un patrón divergente deseado. Se establece
entonces flujo de corriente de RF entre los electrodos (es decir,
bipolar) o entre los electrodos y un electrodo de retorno separado
(es decir, monopolar). El electrodo de retorno monopolar tiene un
área superficial que es suficientemente grande para disipar
cualquier efecto electroquirúrgico. Los electrodos puede ser
desplegados por una variedad de técnicas específicas. Por ejemplo,
una funda puede ser colocada inicialmente usando un obturador o
estilete hasta el lugar objetivo de manera usual. Después de retirar
el obturador o estilete, los electrodos se pueden introducir a
través de la funda y hacer avanzar desde el extremo distal de la
funda al tejido sólido. Los electrodos están dispuestos en o sobre
un miembro alargado, tal como un tubo, que los recibe de modo
alternante. Se hacen avanzar entonces los electrodos desde el tubo
o, alternativamente, el tubo puede ser retirado de modo proximal de
encima de los electrodos antes del avance de los mismos desde la
funda al tejido.
De modo similar, un método que emplea la sonda
puede comprender hacer avanzar al menos tres electrodos desde un
lugar objetivo dentro de la región de tratamiento. Los electrodos
divergen en un patrón tridimensional, preferiblemente, estando
uniformemente separados electrodos individuales para proporcionar
tratamiento volumétrico uniforme, como se describe en lo que sigue.
Se realiza entonces el tratamiento haciendo pasar corriente de RF
entre los al menos tres electrodos o entre dichos tres electrodos y
un electrodo de retorno. Preferiblemente, el método emplea más de
tres electrodos, desplegando, a menudo, al menos cinco electrodos,
empleando, preferiblemente, al menos seis electrodos, empleando,
frecuentemente, al menos ocho electrodos y empleando, a menudo, al
menos diez electrodos o más. Se apreciará que un número mayor de
electrodos individuales puede mejorar la uniformidad del
tratamiento, al tiempo que se limita la cantidad de potencia
(densidad de corriente) emitida desde cualquier electrodo único,
reduciendo, así, la temperatura en la región inmediata del
electrodo(s). Opcionalmente, se pueden evertir los al menos
tres electrodos, es decir, se pueden volver primero en una dirección
radialmente hacia fuera y, luego, en una dirección generalmente
proximal, a medida que se les hace avanzar desde el lugar objetivo.
El uso de tales electrodos múltiples evertidos proporciona una
agrupación preferida para tratar volúmenes de tejido relativamente
grandes. En particular, agrupaciones de electrodos evertidos
proporcionan corriente y calentamiento en volúmenes generalmente
esféricos que concordará más estrechamente con las geometrías
esféricas o elipsoidales del tumor típico o de otra lesión a tratar.
En contraste con esto, agrupaciones de electrodos no evertidos
efectúan, a menudo, un volumen de tratamiento cónico o irregular que
puede tener menos campo de aplicación generalizado.
Volviendo al aparato, se describirán ciertas
propiedades preferidas u opcionales.
Se pueden proporcionar, también, medios para
introducir el miembro alargado a través de tejido hasta el lugar
objetivo. Los medios pueden tener una variedad de formas, incluyendo
un montaje de funda y obturador (estilete) que se puede usar para
proporcionar la penetración inicial. Alternativamente, un elemento
de autopenetración puede estar previsto directamente sobre el
miembro alargado. Se pueden emplear, también, otros dispositivos y
técnicas usuales del tipo usado para introducir ejes y otros
miembros alargados al tejido sólido.
Los elementos de electrodo de penetración en
tejido pueden comprender cables que están recibidos dentro de un
lumen axial del miembro alargado. Por ejemplo, los cables pueden
estar en haces unos con otros sobre una de sus porciones proximales,
pero se mantienen separados y conformados sobre su porción distal,
de manera que divergen en un patrón seleccionado cuando se les hace
avanzar al tejido. Usualmente, se hace avanzar los cables
directamente desde el miembro alargado, (cuando el miembro alargado
se deja dentro de la funda o cuando se retira la funda), pero se les
podría hacer avanzar alternativamente desde la funda cuando el
miembro alargado es retirado de modo proximal desde encima de los
electrodes antes de la penetración de los mismos al tejido.
Usualmente, el miembro alargado es un tubo que
tiene un lumen axial que recibe de modo alternante el elemento de
electrodo de penetración en tejido, y los elementos de electrodo
comprenden cables individuales que pueden estar en haces, como se ha
descrito anteriormente.
Los extremos distales de los cables o de otros
elementos de electrodo están, preferiblemente, conformados de manera
que asumen una configuración radialmente restringida mientras están
presentes en el lumen axial del tubo y asumen una configuración
radialmente divergente cuando se extienden axialmente desde el tubo.
En una configuración preferida, los extremos distales de al menos
algunos de los cables están conformados de manera que asumen una
configuración evertida hacia fuera a medida que son extendidos
axialmente desde el tubo o desde otro miembro alargado. El sistema
de sonda puede incluir uno, dos o más grupos de al menos tres
electrodos que están axialmemente separados entre sí. En particular,
tales grupos axialmente separados de electrodos se pueden extender
desde el extremo distal del miembro alargado o pueden estar
distribuidos a lo largo del miembro alargado, e individualmente
extensibles para asumir la configuración tridimensional deseada.
Preferiblemente, cada grupo de cables de penetración en tejido o de
otros elementos de electrodo incluye más de tres electrodos, como se
ha descrito, en general, anteriormente.

El sistema según la presente invención estará
diseñado para introducir una pluralidad de elementos de electrodo
hasta una región de tratamiento dentro de tejido sólido del
paciente. La región de tratamiento puede estar situada en cualquier
lugar del cuerpo en el que pueda ser beneficiosa la exposición
hipotérmica. Más comúnmente, la región de tratamiento comprenderá un
tumor sólido dentro de un órgano del cuerpo, tal como el hígado, el
riñón, el páncreas, el pecho, la próstata (no accesible a través de
la uretra), y similar. El volumen a tratar dependerá del tamaño del
tumor o de otra lesión, teniendo típicamente un volumen total desde
1 cm^{3} hasta 150 cm^{3}, usualmente desde 1 cm^{3} hasta 50
cm^{3} y, a menudo, desde 2 cm^{3} hasta 35 cm^{3}. Las
dimensiones periféricas de la región de tratamiento pueden ser
regulares, por ejemplo, esféricas o elipsoidales, pero serán, más
usualmente, irregulares. La región de tratamiento puede ser
identificada usando técnicas de formación de imágenes usuales
capaces de dilucidar un tejido objetivo, por ejemplo, tejido
tumoral, tales como exploración ultrasónica, formación de imágenes
por resonancia magnética (MRI), tomografía computarizada (CAT),
fluoroscopía, exploración nuclear (usando sondas específicas de
tumor radioetiquetadas), y similares. Se prefiere el uso de
ultrasonidos de alta resolución que se puede emplear para supervisar
el tamaño y localización del tumor o de otra lesión a ser tratada, o
de modo intraoperativo o externamente.
El sistema según la presente invención emplea una
pluralidad de electrodos de penetración en tejido, típicamente en
forma de cables de metal afilados de pequeño diámetro que pueden
penetrar en tejido a medida que se les hace avanzar desde un lugar
objetivo dentro de la región de tratamiento, como se describe con
más detalle en lo sucesivo. Los elementos de electrodo, sin embargo,
pueden estar formados, también, de otras maneras, tales como hojas,
hélices, tornillos y similares. El requisito principal de tales
elementos de electrodo es que se pueden desplegar en una agrupación
tridimensional, emanando, en general, desde un lugar objetivo dentro
de la región de tratamiento del tejido. En general, los elementos de
electrodo son introducidos primero hasta el lugar objetivo en una
configuración radialmente aplastada u otra restringida y se les hace
avanzar después de eso al tejido desde un elemento de suministro en
un patrón divergente para conseguir la agrupación tridimensional
deseada. Preferiblemente, los elementos de electrodo divergen
radialmente hacia fuera desde el elemento de suministro (situado en
el lugar objetivo) en un patrón uniforme, es decir, con el
espaciamiento entre electrodos adyacentes divergiendo en un patrón
sustancialmente uniforme y/o simétrico. En las realizaciones
ilustrativas, pares de electrodos adyacentes están separados entre
sí en patrones similares o idénticos repetidos y están usualmente de
modo simétrico situados alrededor de un eje del elemento de
suministro. Los elementos de electrodo están conformados para curvar
radialmente hacia fuera y, opcionalmente, para evertir de modo
proximal, de manera que miran parcial o completamente en la
dirección proximal cuando están completamente desplegados. Se
apreciará que se puede proporcionar una amplia variedad de patrones
particulares para cubrir uniformemente la región a tratar.
Una forma preferida del elemento individual de
electrodo de una agrupación de electrodos es un único cable que
tiene una porción distal conformada que se puede extender desde el
elemento de suministro en el lugar objetivo en el tejido para
diverger en un patrón deseado. Tales cables pueden estar formados de
metales conductores que tienen una memoria de forma adecuada, tales
como acero inoxidable, aleaciones de níquel-titanio,
aleaciones de acero de resorte, y similares. Los cables pueden tener
secciones transversales circulares o no circulares, con cables
circulares que tienen típicamente un diámetro en el intervalo desde
aproximadamente 0,1 mm hasta 2 mm, preferiblemente, desde 0,2 mm
hasta 0,5 mm, a menudo, desde 0,2 mm hasta 0,3 mm. Los cables no
circulares tienen usualmente áreas en corte transversal equivalente.
Opcionalmente, los extremos distales de los cables pueden estar
rectificados o afilados para facilitar su capacidad para penetrar
tejido. Los extremos distales de tales cables pueden ser endurecidos
usando tratamiento por calor usual u otros procedimientos
metalúrgicos. Tales cables pueden estar parcialmente cubiertos con
aislamiento, aunque estarán al menos parcialmente libres de
aislamiento sobre sus porciones distales que penetran en el tejido a
tratar. En el caso de agrupaciones de electrodos bipolares, será
necesario aislar los cables de electrodo positivo y negativo en
cualquier región en la que pudieran estar en contacto entre sí
durante la fase de suministro de energía. En el caso de agrupaciones
monopolares, puede ser posible poner en haces los cables unos con
otros con sus porciones proximales teniendo sólo una capa única de
aislamiento sobre todo el haz. Tales cables en haz se pueden sacar
directamente a un suministro de energía de RF adecuado, o
alternativamente pueden estar conectados a través de otros
conductores eléctricos (intermedios), tales como cable coaxial, o
similar.
Las características de electrodo anteriormente
descritas se aplican sólo a electrodos activos destinados a tener el
efecto quirúrgico deseado, es decir, calentamiento del tejido
circundante. Se apreciará que en funcionamiento monopolar, un
"electrodo" pasivo o dispersivo debe estar previsto, también,
para completar la trayectoria de retorno del circuito que está
siendo creado. Tales electrodos, que usualmente están fijados
externamente a la piel del paciente, tienen un área mucho mayor,
típicamente, aproximadamente 130 cm^{2} para un adulto, de manera
que el flujo de corriente es suficientemente bajo para evitar
calentamiento significativo y otros efectos quirúrgicos. Puede ser
posible, también, proporcionar tal electrodo de retorno dispersivo
directamente sobre una porción de una funda o miembro alargado del
sistema de la presente invención, como se describe con más detalle
en lo que sigue (en general, cuando el electrodo de retorno está
sobre la funda, el dispositivo se denomina todavía bipolar).
El suministro de energía de RF puede ser un
suministro de energía usual electroquirúrgico de cometido general
funcionando en una frecuencia en el intervalo desde 400 kHz hasta
1,2 MHz, con una forma de onda sinusoidal o
no-sinusoidal usual. Tales suministros de energía
están disponibles a partir de muchos proveedores comerciales, tales
como Valleylabs, Aspen, Bovie y Birtcher.
La pluralidad de elementos de electrodo está
usualmente contenida por o dentro de un miembro alargado que
incorpora el elemento de suministro, típicamente una cánula rígida,
de metal o de plástico. El miembro alargado sirve para restringir
los elementos de electrodo individuales en una configuración
radialmente aplastada a fin de facilitar su introducción hasta el
lugar objetivo del tejido. Los elementos de electrodo puede ser
desplegados entonces hasta su configuración tridimensional deseada,
extendiendo los extremos distales de los elementos de electrodo
desde el miembro alargado al tejido. En el caso de la cánula
tubular, esto se puede conseguir simplemente haciendo avanzar los
extremos distales de los elementos de electrodo de modo distal hacia
delante desde el tubo, de manera que salen y se desvían como
consecuencia de su propia memoria elástica en un patrón radialmente
hacia fuera.
Un componente o elemento puede estar previsto
para introducir el miembro alargado hasta el lugar objetivo dentro
de la región de tratamiento a tratar. Por ejemplo, un montaje usual
de funda y obturador (estilete) afilado se puede usar para acceder
inicialmente al lugar objetivo. El montaje puede estar situado bajo
imágenes ultrasónicas u otras usuales, siendo retirado entonces el
obturador/estilete para dejar un lumen de acceso a través de la
funda. Los elementos de electrodo pueden ser introducidos entonces a
través del lumen de funda, típicamente mientras son restringidos en
el miembro alargado. Los elementos de electrodo son extendidos
entonces de modo distal más allá del extremo distal de la funda a la
región de tratamiento del tejido, y el miembro alargado puede ser
retirado posteriormente o dejado en su sitio. Se puede aplicar
entonces corriente de RF a través de los electrodos de forma
monopolar o bipolar. Con tratamiento monopolar, una placa dispersiva
fijada externamente al paciente está unida al otro terminal desde el
suministro de energía de RF. Alternativamente, un electrodo de
retorno que tiene un área superficial relativamente grande puede
estar previsto sobre el miembro alargado, o sobre la funda. En
funcionamiento bipolar, los elementos de electrodo individuales
pueden estar conectados alternativamente a los dos polos del
suministro de energía de RF. Alternativamente, uno o más elementos
de electrodo adicionales se pueden hacer penetrar en el tejido y
sirven como un electrodo común conectado al segundo polo.
Haciendo referencia ahora a los dibujos, en los
que partes similares o correspondientes están identificadas con el
mismo número de referencia, y más particularmente a la figura 1, el
aparato de ablación de tejido volumétrico de la presente invención
está designado, en general, como 10 e incluye una sonda 12
eléctricamente conectada a un generador 14.
En experimentos con un prototipo de la presente
invención, el inventor utilizó una unidad electroquirúrgica Bovie®
X-10 como generador 14, para generar corriente de
radiofrecuencia a energías específicas, usando la sonda 12 como el
electrodo activo y colocando la muestra de tejido sobre una placa
dispersiva o de conexión a tierra. Así, el generador 14 incluye al
menos un terminal activo 16 y un terminal de retorno 18, con una
placa dispersiva o de conexión a tierra 20 eléctricamente conectada
por el conductor 22 al terminal 18.
La sonda 12 está constituida por una pluralidad
de cables 24 eléctricamente conductores que están en haces en un
extremo proximal y conectados al terminal 16 para conducir corriente
de RF desde él. Los cables 24 están enroscados a través de un tubo o
catéter 26 eléctricamente aislado o no conductor.
Los cables 24 están formados, preferiblemente, de
alambre de resorte o de otro material que retiene memoria. Como se
muestra en la figura 1, una agrupación 28 de 10 cables está formada
con cada cable 24 arqueado desde el catéter 26 en forma general de
"U", con cada cable separado de modo sustancialmente uniforme,
como se muestra en la figura 2. Así, la agrupación 28 está formada
por una pluralidad de cables 24 que se curvan radialmente hacia
fuera desde el eje del extremo distal 26a del catéter 26. Todos los
cables 24 se extienden una longitud tal que una porción de cada
cable 24 es perpendicular al eje del tubo 26 y, preferiblemente,
siguen volviéndose a curvar hacia atrás sobre ellos mismos, de
manera que los extremos distales 24a de cable están orientados, en
general, paralelos al eje del extremo distal 26a de tubo. Como se
muestra en la figura 1, los extremos distales 24a de cable, en
general, reposan dentro de un plano ortogonal al extremo distal 26a
de tubo, y uniformemente separados entre sí.
Puesto que los cables 24 están formados por acero
de resorte, pueden ser extraídos dentro del catéter 26, para
inserción percutánea. Una vez que el extremo distal 26a del catéter
26 está en posición, cables de deslizamiento 24 a través del catéter
26 permitirán a la memoria de los cables adoptar la forma
radialmente dispuesta de la agrupación 28 mostrada en las figuras 1
y 2.
La figura 3 es una vista en corte tomada a través
de una muestra 30 de hígado, que muestra los resultados de una aguja
recta 31 de calibre 18 de la técnica anterior con 1,2 cm de metal
expuesto cuando se inserta en el hígado 30 y se hace funcionar a 20
vatios de potencia, con corriente de coagulación al 100%, durante un
periodo de 5 minutos. Como se puede ver en la figura 3, la lesión 32
producida por la aguja única tiene una forma elíptica (casi
cilíndrica) estrecha con un diámetro de aproximadamente 1,2 cm y una
longitud de aproximadamente 2 cm. La figura 3 muestra, también, los
efectos de temperaturas muy altas cerca de la extremidad de sonda
con formación de gas común con sondas electroquirúrgicas de aguja
única, dando como resultado tejido 34 calcinado y carbonizado
inmediatamente alrededor de la aguja. La calcinación y la formación
de gas asociada en el lugar de las sondas de aguja única limita
significativamente la potencia que se puede aplicar.
La figura 4 es una vista en corte a través de una
muestra 30' de hígado que muestra la lesión necrótica 32' producida
por la agrupación 28 de 10 cables de la sonda 12 de la presente
invención. La sonda 12 está situada en la muestra 30' de tejido con
el extremo distal 26a de tubo situado, en general, centralmente en
el lugar en el que se desea una lesión. Se pueden utilizar diversos
métodos, conocidos en la técnica, para situar la sonda 12, antes del
despliegue de los cables 24 (mostrados desplegados en líneas
ocultas). Preferiblemente, el posicionamiento del extremo distal 26a
de tubo es confirmado por ultrasonidos o por otras técnicas de
formación de imágenes. Una vez que el tubo 26 está situado
apropiadamente, los cables 24 se despliegan en el tejido 30',
causando la memoria del material de alambre el despliegue de cables
para tomar una forma predeterminada de agrupación.
Los solicitantes utilizaron el mismo generador 14
a una potencia de 60 vatios, con corriente de coagulación al 100%,
durante un periodo de 5 minutos. Se puede ver que la lesión
necrótica producida por la sonda 12 tiene forma aproximadamente
esférica y tiene un diámetro de aproximadamente 3,5 cm. Además, no
hay calcinación evidente, lo que indica ausencia de chisporroteo y
una distribución de temperaturas más uniforme dentro del volumen de
tejido a ser tratado. Durante los ensayos, se encontró que la
temperatura del tejido 2 cm lejos del acceso de la sonda 12 al final
de los 5 minutos fue 51,4ºC. La misma sonda 12 de 10 cables se usó
repetidamente con las mismas fijaciones y produjo lesiones
sustancialmente idénticas. Se encontró, también, que el área de
calentamiento letal se podía extender al menos otro centímetro más
allá de la lesión visible mostrada en la figura 4, después de que se
tomaran mediciones de termistor durante experimentos repetidos con
la sonda 12.
Aunque las figuras 1 y 2 muestran una agrupación
28 en forma general de "fuente" con 10 cables 24, diversos
diseños de otras agrupaciones son igualmente adecuados, utilizando
espaciamiento uniforme de los extremos distales 24a de cable desde
el extremo distal 26a de catéter para producir una lesión simétrica,
o con espaciamiento no uniforme para producir una lesión asimétrica.
Por ejemplo, como se muestra en la figura 5, se pueden formar
múltiples agrupaciones 28' separadas longitudinalmente entre sí.
Esta realización del aparato monopolar de ablación de tejido está
designada, en general, como 110 e incluye una sonda 112
eléctricamente conectada al generador 14. La sonda 112 incluye un
primer haz 124 de cables articulado a través de un tubo 126 con
extremos distales 124a de cable desplegables para formar una primera
agrupación 28'a que se extiende desde el extremo distal 126a de
tubo. Un segundo haz 125 de cables rodea al tubo 126 dentro de un
tubo exterior 127, con extremos distales 125a de cable desplegables
para formar una segunda agrupación 28'b que sobresale del extremo
distal 127a de tubo exterior. Los extremos proximales 124b y 125b de
los haces 124 y 125 de cables están eléctricamente conectados en
común al terminal activo 16.
En funcionamiento, el tubo exterior 127 está
situado con el extremo distal 127a colocado en el lugar
predeterminado para la lesión. La segunda agrupación 28'b se forma
entonces desplegando los extremos 125a de cable del segundo haz 125
de cables. El tubo interior 126 se mueve entonces axialmente, de
manera que el extremo distal 126a de tubo está separado
longitudinalmente del extremo distal 127a de tubo. El primer haz 124
de cables se despliega entonces de manera que los extremos 124a de
cable forman la agrupación 28'a longitudinalmente separada de la
agrupación 28'b.
Haciendo referencia ahora a la figura 6, un
aparato bipolar de ablación de tejido está designado, en general,
como 210 e incluye una sonda 212 eléctricamente conectada a un
generador 14. Los cables 224 están eléctricamente conectados al
terminal 16 sobre el generador 14 y terminan de modo distal en una
agrupación 228 de la misma forma que la agrupación 28 de la primera
realización. Sin embargo, el aparato 210 incluye una trayectoria de
retorno enteriza que consiste en un cable de retorno 238 revestido
con un material 236 eléctricamente no conductor, que se extiende a
través del catéter 226 dentro del haz de cables 224, y tiene un
extremo distal 238a que sobresale, en general, centralmente dentro
de la agrupación 228. El extremo proximal 238b del cable 238 está
conectado al terminal de retorno 18, para proporcionar un circuito
eléctrico cuando la sonda 212 está desplegada dentro del tejido.
Así, no es necesaria una placa dispersiva.
Haciendo referencia ahora a la figura 7, un
segundo aparato bipolar de ablación de tejido está designado, en
general, como 310 e incluye una sonda 312 con cables 324 conectados
al terminal activo 16 del generador 14. Los cables 324 sobresalen
del extremo distal 326a del tubo 326 para formar una agrupación
328.
El aparato bipolar 310 difiere del aparato
bipolar 210 de la figura 6, de dos modos. En primer lugar, un
collarín 340 está fijado al exterior del extremo distal 326a de tubo
y está eléctricamente conectado al terminal de retorno 18 por un
conductor 342, para formar un retorno eléctrico para la corriente
suministrada por los cables 324. El conductor 342 puede estar fijado
al exterior del tubo 326, o roscado por el tubo 326, al tiempo que
eléctricamente aislado de los cables 324.
En segundo lugar, los cables 324 tienen porciones
344 que están revestidas con un material eléctricamente aislante.
Las porciones 344 están separadas a lo largo de una pluralidad de
cables 324 a fin de restringir flujo de corriente desde porciones
seleccionadas de los cables 324, para crear una distribución más
uniforme de calor desde las porciones expuestas restantes de los
cables 324.
Un tercer aparato bipolar de ablación de tejido
está designado, en general, como 410 en la figura 8. El aparato
bipolar 410 incluye una sonda 412 con un conjunto de cables 424
conectado a un terminal 16' de un generador 14' de corriente, y un
segundo conjunto de cables 425 conectado al terminal opuesto 18'.
Los cables individuales de los haces 424 y 425 de cables tienen un
revestimiento eléctricamente aislador a través del tubo 426, para
impedir contacto eléctrico entre sí. Los cables 424 y 425 se
alternan, preferiblemente, por toda la agrupación 428, de manera que
circula corriente entre los cables 424 y los cables 425.
Haciendo referencia ahora a las figuras
9-14, una región de tratamiento TR dentro del tejido
T está situada debajo de la piel S de un paciente. La región de
tratamiento puede ser un tumor sólido u otra lesión en la que se
desea tratar la región por hipertermia de RF. La región de
tratamiento TR, antes de tratamiento, se muestra en la figura 9.
A fin de introducir una agrupación de electrodos
según la presente invención, un montaje usual 500 de funda y
obturador/estilete es introducido de modo percutáneo (a través de la
piel) de manera que un extremo distal de la funda se encuentra en o
dentro de un lugar objetivo TS, como se muestra en la figura 10. Se
retira entonces el obturador/estilete 504 de la funda 502, dejando
un lumen de acceso hasta el lugar objetivo, como se muestra en la
figura 11. Una sonda de suministro 510 que incorpora las propiedades
de la presente invención se introduce entonces a través del lumen de
acceso de la funda 502, de manera que un extremo distal 512 de una
cánula exterior 515 de la sonda se encuentran cerca del extremo
distal 514 de la funda 502, como se muestra en la figura 12. Los
electrodos individuales 520 son extendidos entonces de modo distal
desde el extremo distal 512 de la sonda 510 haciendo avanzar el
cable 516 en la dirección de la flecha 519, como se muestra en la
figura 13. Los electrodos 520 se hacen avanzar de manera que primero
divergen radialmente hacia fuera entre sí (figura 13), evertiendo
finalmente hacia atrás en la dirección proximal, como se muestra en
la figura 14. Si se desea, la cánula 515 de la sonda 510 es retirada
entonces de modo proximal sobre el cable 516 de electrodo, y el
cable de electrodo se fija entonces a un suministro 518 de energía
RF de manera monopolar, como se muestra, también, en la figura 14.
Se puede aplicar entonces corriente de radiofrecuencia desde el
suministro 518 de energía a un nivel y con una duración suficiente
para elevar la temperatura de la región de tratamiento TR una
cantidad deseada, típicamente, hasta una temperatura de al menos
42ºC, usualmente, hasta al menos 50ºC, durante 10 minutos o más.
Temperaturas superiores requieren, en general, tiempos de
tratamiento mucho más cortos.
Aunque el método y sistema que se acaban de
describir emplean un montaje separado 500 de funda y
obturador/estilete para introducir los electrodos de tratamiento, se
apreciará que no es necesario el uso de tal introductor separado.
Alternativamente, los electrodos se podrían introducir a través del
miembro alargado, en el caso de que el miembro alargado estuviera
provisto de un elemento de autopenetración, tal como una extremidad
afilada o una extremidad electroquirúrgica, para mejorar la
penetración en tejido. Como una alternativa adicional, un haz de
electrodos se podría introducir en cualquier forma restringida (por
ejemplo, un anillo que se puede retirar, una funda soluble, etc.),
con la restricción selectivamente liberada después de que haya
alcanzado el lugar objetivo dentro de la región de tratamiento. La
presente invención abarca, así, el uso de una variedad de sistemas
específicos para introducir una pluralidad de electrodos hasta el
lugar objetivo en tejido sólido, y para liberar y hacer diverger
después de eso los elementos de electrodo individuales a una región
de tratamiento que rodea el lugar objetivo en una agrupación
tridimensional deseada o en otra configuración o geometría.
Se puede ver, por lo tanto, que el aparato de
ablación de tejido volumétrico de la presente invención proporciona
un sistema de ablación electroquirúrgica eficaz y deseable que es
adecuado para introducción percutánea y quirúrgica abierta, produce
lesiones uniformes y produce lesiones suficientemente grandes como
para tratar a un gran espectro de pacientes.
Claims (28)
1. Un sistema de sonda para hacer penetrar una
pluralidad de electrodos en tejido sólido, comprendiendo dicho
sistema de sonda:
un miembro alargado (12) que tiene un extremo
proximal y un extremo distal; y
al menos tres elementos (24) de electrodo de
penetración en tejido sólido acoplados de modo alternante al miembro
alargado, de manera que dichos elementos de electrodo se pueden
hacer avanzar al tejido sólido después de que dicho miembro alargado
haya sido introducido a través del tejido sólido hasta un lugar
objetivo dentro del tejido sólido, estando configurados los
electrodos para extenderse de modo distal desde el extremo distal
del miembro alargado y avanzar por ello más al lugar objetivo,
curvándose radialmente hacia fuera los al menos tres electrodos
dentro del tejido sólido en un patrón tridimensional divergente,
como consecuencia de su propia memoria elástica, a medida que se les
hace avanzar dentro del tejido sólido en una dirección distal desde
el miembro alargado, para definir un volumen de tratamiento
tridimensional.
2. Un sistema de sonda según la reivindicación 1,
en el que el miembro alargado es un tubo que tiene un lumen axial
que recibe de modo alternante los elementos de electrodo de
penetración en tejido.
3. Un sistema de sonda según la reivindicación 1
o la reivindicación 2, que comprende además medios para introducir
el miembro alargado a través del tejido sólido hasta el lugar
objetivo.
4. Un sistema de sonda según la reivindicación 3,
en el que los medios de introducción comprenden un montaje (500) de
obturador y funda, en el que el obturador (504) tiene una extremidad
distal afilada y puede ser retirado de la funda (502) para dejar un
lumen de acceso hasta el lugar objetivo dentro de la funda a fin de
recibir el miembro alargado, de manera que los electrodos se pueden
extender de modo distal fuera del extremo distal (514) de la
funda.
5. Un sistema de sonda según la reivindicación 3,
en el que los medios de introducción comprenden un elemento de
autopenetración dispuesto en la extremidad distal del miembro
alargado.
6. Un sistema de sonda según cualquiera de las
reivindicaciones anteriores, en el que los elementos de electrodo de
penetración en tejido comprenden cables que se extienden a través

hacia fuera a medida que son extendidos axialmente más allá del
extremo distal del miembro alargado.
8. El aparato de las reivindicaciones 6 y 7, en
el que dichos extremos distales de cable están separados de modo
sustancialmente uniforme entre sí.
9. El aparato de la reivindicación 6, 7 u 8, en
el que dichos extremos distales de cable de sonda se curvan en arcos
que tienen sustancialmente el mismo radio.
10. El aparato de la reivindicación 6, 7, 8 ó 9,
en el que las longitudes de los extremos distales de cable de sonda,
desde el extremo distal de tubo, son sustancialmente iguales.
11. Un sistema de sonda según cualquier
reivindicación precedente, en el que el miembro alargado comprende
una cánula tubular, y los electrodos están dispuestos de manera que
los extremos distales de los mismos salen y se desvían como
consecuencia de su propia memoria elástica al hacerlos avanzar de
modo distal hacia delante desde el tubo.
12. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo
comprenden cables que tienen un diámetro desde 0,2 mm hasta 0,5
mm.
13. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo
están dispuestos para ser evertidos de modo proximal a fin de mirar
parcial o completamente en la dirección proximal cuando están
completamente desplegados.
14. Un sistema de sonda según cualquier
reivindicación precedente, en el que el volumen de tratamiento es
desde 1 cm^{3} hasta 150 cm^{3}.
15. Un sistema de sonda según cualquier
reivindicación precedente, destinado para inserción a través de
tejido hepático a fin de tratar una lesión en él.
16. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo
tienen extremidades distales afiladas para facilitar penetración a
través de dicho tejido sólido.
17. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo
están dispuestos para ser insertados a través del lumen de una funda
y ser extendidos entonces de modo distal más allá del extremo distal
de la funda a la región de tratamiento de tejido.
18. Un sistema de sonda según cualquier
reivindicación precedente, en el que los electrodos tienen sección
transversal no circular.
19. Un sistema de sonda según la reivindicación
1, destinado a tratar una lesión en tejido hepático, comprendiendo
el miembro alargado una cánula tubular que tiene un elemento de
autopenetración dispuesto en su extremidad distal destinado a
penetrar tejido hepático hasta un lugar de tratamiento en él,
comprendiendo los elementos de electrodo cables que tienen un
diámetro desde 0,2 mm hasta 0,5 mm y que tienen extremidades
distales afiladas para facilitar penetración a través de dicho
tejido hepático desde dicho lugar de tratamiento, estando dispuestos
los elementos de electrodo de manera que sus extremos distales salen
y se desvían como consecuencia de su propia memoria elástica al
hacerlos avanzar de modo distal hacia delante desde el tubo para
definir un volumen de tratamiento en el intervalo de 1 a 150
cm^{3}.
20. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo se
curvan en arcos.
21. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo se
extienden una longitud tal que una porción de cada elemento es
perpendicular al eje del miembro alargado.
22. Un sistema de sonda según la reivindicación
21, en el que dicha porción que es perpendicular al eje del miembro
alargado está situada de modo distal respecto al extremo distal del
miembro alargado.
23. Un sistema de sonda según la reivindicación
21 ó 22, en el que los elementos de electrodo siguen volviéndose a
curvar hacia atrás sobre ellos mismos.
24. Un sistema de sonda según cualquier
reivindicación precedente, en el que los elementos de electrodo son
generalmente en U.
25. Un sistema de sonda según cualquier
reivindicación precedente, que tiene al menos cinco electrodos.
26. Un sistema de sonda según la reivindicación
25, que tiene al menos ocho electrodos.
27. Un sistema de sonda según cualquier
reivindicación precedente, en el que los electrodos individuales
están uniformemente separados.
28. Un sistema de ablación de tejido que
comprende un sistema de sonda según cualquier reivindicación
precedente, en combinación con un suministro de energía (14) de
radiofrecuencia que tiene terminales activos y de retorno, de los
que al menos uno se puede conectar a dichos electrodos.
