Abstract

Instrumento de agarre bipolar con dos mordazas prensoras (15, 16) móviles la una contra la otra, eléctricamente aisladas entre sí, cada una conectable con un polo de una fuente de tensión de alta frecuencia, que dispone cada una de dos elementos de apriete (37) con una superficie de apriete (38), distanciados el uno frente al otro, que forman entre sí una ranura longitudinal (36), y con un dispositivo de corte que comprende un elemento cortante, desplazable en las ranuras longitudinales de las mordazas prensoras (15, 16), con un filo cortante (42), donde el elemento cortante (41) está dispuesto en la ranura longitudinal (36) de una mordaza prensora (15), caracterizado porque el filo cortante (42) está alojado en la ranura longitudinal (36) entre los elementos de apriete (37) de esta mordaza prensora (15) y no sobresale por encima de sus superficies de apriete (38) en sentido hacia la otra mordaza prensora (16), y porque los elementos de apriete (37) de esta mordaza prensora (15) son elásticamente desplazables en contra del movimiento de cierre de las mordazas prensoras (15, 16) en tal medida, en relación con el elemento cortante (41), que el filo cortante (42) del elemento cortante (41) sobresale por encima de las superficies de apriete (38).

Description

Instrumento de agarre bipolar.
La invención se refiere a un instrumento de agarre bipolar con dos mordazas prensoras móviles la una contra la otra, eléctricamente aisladas entre sí, que pueden conectarse cada una con un polo de una fuente eléctrica de tensión de alta frecuencia, que comprenden cada una dos elementos de apriete, distanciados el uno frente al otro formando entre sí una ranura longitudinal, con una superficie de apriete, y con un dispositivo de corte que comprende un elemento cortante con un filo cortante, desplazable en las ranuras longitudinales de las mordazas prensoras, donde el elemento cortante está dispuesto en la ranura longitudinal de una mordaza prensora.
Con instrumentos de agarre bipolares es posible coagular tejidos en la zona sujeta, de modo que en esta zona se pueden contener hemorragias.
Es conocido coagular con instrumentos de este tipo por ejemplo determinadas zonas de vasos sanguíneos y separar a continuación los vasos en esta zona coagulada mediante un corte, el peligro de producirse hemorragias se ha minimizado por la coagulación precedente. Para ello se requieren dos pasos de procedimiento completamente distintos que normalmente se llevan a cabo también con instrumentos diferentes, es decir, por un lado la coagulación con un instrumento de agarre bipolar, y por otro lado la separación del tejido coagulado mediante un dispositivo de corte.
También se conocen instrumentos de agarre bipolares en los cuales está integrado un dispositivo de corte (folleto de la empresa CIRCON, TRIPOLAR Cutting Forceps, patente estadounidense 5 458 598). En este instrumento, las mordazas prensoras del instrumento de agarre están divididas a través de ranuras longitudinales en dos superficies de apriete dispuestas una al lado de otra, y en las ranuras longitudinales se puede introducir en sentido longitudinal del instrumento un elemento de corte provisto de un filo cortante que, después de la coagulación, separa las partes de tejido sujetas entre las superficies de apriete. En este instrumento conocido deben estar previstos mecanismos de accionamiento distintos para cerrar las mordazas prensoras para coagular, y para desplazar hacia delante el elemento cortante del dispositivo de corte; este instrumento, configurado preferentemente como instrumento de vástago tubular, tiene un diseño relativamente complicado y, además, el usuario de este instrumento debe manipular dos mecanismos de accionamiento separados para coger el tejido y para cortarlo a continuación.
En el documento WO 96/05776 se describe un instrumento de agarre bipolar en el cual un filo cortante está dispuesto en una ranura longitudinal entre dos elementos de apriete de una mordaza prensora, después de la coagulación es posible desplazar este filo cortante de tal manera que se corte el material sujeto entre las mordazas prensoras. En este documento no se describe con más detalle cómo se debe mover el elemento cortante para separar el tejido, pero es de suponer que para ello se necesita un desplazamiento manual intencionado del elemento cortante, o un desplazamiento mediante un mecanismo de accionamiento por separado.
El objetivo de la invención consiste en configurar un instrumento de agarre bipolar del tipo genérico de tal modo que se simplifique el manejo, especialmente de forma que el usuario sólo necesite un mecanismo de accionamiento para sujetar el tejido y para cortarlo a continuación.
Este objetivo se logra de acuerdo con la invención en un instrumento de agarre bipolar del tipo inicialmente descrito de tal manera que la cuchilla está alojada en la ranura longitudinal entre los elementos de apriete de esta mordaza prensora y no sobresale por encima de las superficies de apriete de esta mordaza prensora en sentido hacia la otra mordaza prensora, y porque los elementos de apriete de esta mordaza prensora pueden moverse elásticamente en relación con el elemento cortante en contra del movimiento de cierre de las mordazas prensoras hasta que el filo del elemento cortante sobresalga por encima de las superficies de apriete.
Al iniciar el movimiento de cierre de las mordazas prensoras, el elemento cortante está alojado de tal manera en la ranura longitudinal de una mordaza prensora que el filo del elemento cortante se encuentra en el interior de la ranura longitudinal, por lo que no entra en función. En este caso, el instrumento funciona como un instrumento de agarre bipolar convencional, en el cual las superficies de apriete entran en contacto con el tejido a coger sin que se note la presencia del elemento cortante. En esta fase del movimiento de cierre es posible coagular el tejido de forma convencional aplicando una tensión de alta frecuencia, una vez finalizada la coagulación también es posible retirar el instrumento de agarre bipolar, igual que un instrumento de agarre bipolar convencional.
Si en este instrumento de agarre bipolar el movimiento de cierre continúa después de la coagulación, las mordazas prensoras se mueven con tanta fuerza la una contra la otra que los elementos de apriete de la mordaza prensora, que lleva el dispositivo de corte, se desplazan elásticamente en contra del sentido de movimiento de cierre de las mordazas prensoras, por consiguiente se libera el filo cortante del dispositivo de corte que, debido al desplazamiento elástico de los elementos de apriete, sobresale por encima de la superficie de apriete. Por lo tanto, el instrumento es ahora un instrumento de corte en el cual el elemento cortante puede penetrar hasta dentro de la ranura longitudinal en la otra mordaza prensora cortando el tejido entre las mordazas prensoras. Este movimiento de corte sigue inmediatamente a continuación del movimiento de cierre normal de las mordazas prensoras, el usuario sólo debe manejar un mecanismo de accionamiento, con una ligera presión sólo se sujeta y aprieta el tejido a coagular mientras que, con un cierre fuerte de las mordazas prensoras, se cortan adicionalmente las partes de tejido sujetas entre las mordazas prensoras.
En una primera forma de realización preferida está previsto que tanto el elemento cortante como los elementos de apriete están configurados como una sola pieza con la mordaza prensora, y que el elemento cortante es una parte rígida de la mordaza prensora mientras que los elementos de apriete están unidos con las partes restantes de la mordaza prensora a través de zonas elásticamente deformables. En especial, las zonas elásticamente deformables pueden estar constituidas por nervios elásticamente deformables con una altura reducida.
En otra forma de realización está previsto que la mordaza prensora está formada de dos partes, con una primera parte rígida que comprende el elemento cortante y el apoyo de la mordaza prensora en el instrumento de agarre, y con una segunda parte, que comprende los elementos de apriete, apoyada de forma elásticamente móvil en la primera parte.
Por ejemplo, los elementos de apriete pueden estar apoyados de modo elásticamente orientable en la primera parte.
Resulta favorable si la segunda parte está configurada en forma de U con dos alas paralelas, que constituyen los elementos de apriete, y con un nervio que une estas alas cerrando la ranura longitudinal hacia el extremo distal de la mordaza prensora, y si los extremos libres de la segunda parte están apoyados de forma giratoria en el primer elemento. Así se obtiene una configuración muy estable de la segunda parte de la mordaza prensora que soporta las superficies de apriete.
La ranura longitudinal en la otra mordaza prensora, que no dispone de un dispositivo de corte, está preferentemente cerrada en ambos extremos, por lo que el dispositivo de corte penetra durante el proceso de corte en esta ranura longitudinal cerrada en todos los lados, lo que ayuda también a que la otra mordaza prensora forme un conjunto muy rígido.
La siguiente descripción de formas de realización preferidas de la invención sirve junto con el dibujo para una explicación más detallada. En las figuras se muestra:

El instrumento quirúrgico representado en el dibujo está configurado como instrumento 1 de vástago tubular, con un vástago 2 alargado tubular con un extremo distal 3 y un extremo proximal 4.
En el extremo proximal 4, el vástago 2 está unido con un conjunto 5 de mangos que comprende un mango 6 fijo y, unido con el mismo de forma pivotante, un mango 7 articuladamente unido con una barra 8 de tracción y empuje que penetra por el vástago 2 y que, mediante basculación del mango 7, puede desplazarse en relación con el vástago 2 en sentido longitudinal hacia delante y hacia atrás.
En el vástago 2 está insertado en su extremo distal 3 un soporte 9 que lleva en su extremo distal dos brazos de alojamiento 10, que discurren en paralelo y sobresalen en sentido distal, y que encierran entre sí un espacio intermedio de alojamiento 11. En este espacio está insertado un cuerpo 12 de aislamiento de un material eléctricamente aislante, como por ejemplo cerámica, configurado en lo esencial en forma de U que cubre el lado interior de los brazos de alojamiento 10.
La barra 8 de tracción y empuje penetra tanto por el soporte 9 como por el cuerpo de aislamiento 12 y termina en la zona del espacio intermedio de alojamiento 11.
En la zona de sus extremos libres, los brazos de alojamiento 10 disponen de orificios de alojamiento 13 alineados entre sí para un eje 14 de soporte que discurre transversalmente al sentido longitudinal del eje longitudinal del vástago del instrumento y penetra por el espacio intermedio de alojamiento 11 en su extremo distal abierto.
En este eje 14 de soporte están apoyadas de forma orientable una al lado de otra e independientemente entre sí dos mordazas prensoras 15, 16 cuyos extremos proximales 17, 18 penetran en el espacio intermedio de alojamiento 11. Cada extremo proximal 17, 18 está articuladamente unido con un brazo 19 ó 20, respectivamente, cuyos otros extremos están también unidos articuladamente con el extremo distal de la barra 8 de empuje y tracción, por lo que estos brazos 19, 20 forman junto con los extremos proximales 17 y 18 una articulación cuádruple a través de la cual es posible bascular las mordazas prensoras 15 y 16 alrededor del eje de giro, constituido por el eje 14 de soporte, mediante desplazamiento hacia delante y hacia atrás de la barra 8 de empuje y tracción: durante el avance de la barra 8 de empuje y tracción se distancian las mordazas prensoras, es decir, las mordazas prensoras se abren, mientras que durante el retroceso de la barra 8 de empuje y tracción se cierran las mordazas prensoras 15, 16.
Las mordazas prensoras 15 y 16 están configuradas de forma distinta. Una de las dos mordazas prensoras, denominada a continuación mordaza prensora 16 inferior, está formada por tres partes y comprende una sección proximal 21, que se extiende hasta inmediatamente delante del eje 14 de soporte, una sección 22 central que se encuentra en lo esencial en el área del eje 14 de soporte, y una sección 23 distal cuyo lado superior 24, en lo esencial plano y dirigido hacia la otra mordaza prensora 15, constituye una superficie de apriete provista de nervios transversales 25 para aumentar el agarre.
La sección central 22 se compone de un material eléctricamente aislante, por ejemplo de cerámica, y une la sección proximal 21 con la sección distal 23 aislando estas dos secciones compuestas de metal y por lo tanto eléctricamente conductivas. La sección central 22 está en contacto plano tanto con la sección distal 23 como con la sección proximal 21, de modo que en la zona de solapamiento se obtiene una estructura estratificada, asimismo, la sección central 22 se apoya con su borde posterior 26 y con una superficie de contacto 27 delantera en la sección proximal 21 y en la sección distal 23, respectivamente, por lo que la sección proximal 21 está unida de forma rígida y eléctricamente aislada con la sección distal 23.
La sección central 22 lleva lateralmente un casquillo 28, configurado como una sola pieza con esta sección, que penetra por orificios de soporte 29 y 30 en las dos mordazas prensoras 15 y 16 y aloja en su interior el eje de soporte 14 que así está eléctricamente aislado frente a la mordaza prensora 15 y el extremo proximal 18 de la mordaza prensora 16. El eje de soporte 14 es de metal y está unido de forma eléctricamente conductiva con los brazos de alojamiento 10 del soporte 9, conectado a su vez eléctricamente con el vástago 2. Este lleva una conexión eléctrica 31 a través de la que se puede establecer una unión eléctrica con una fuente de tensión de alta frecuencia no representada en el dibujo.
El eje 14 de soporte está en contacto inmediato con la sección distal 23 de la mordaza prensora 16, por lo que en esta zona se establece una unión eléctricamente conductiva entre la sección distal 23 y el eje 14 de soporte. Así es posible unir la sección distal 23 de la mordaza prensora 16 con un polo de la fuente de tensión de alta frecuencia.
De manera no representada en el dibujo, el otro polo se une con la barra 8 de empuje y tracción y a través de los brazos 19 y 20 se conectan tanto la sección proximal 21 de la mordaza prensora 16 como la mordaza prensora 15 completa al segundo polo de la fuente de tensión de alta frecuencia.
La sección central 22, fijada adicionalmente frente a la sección proximal 21 mediante una clavija 33 introducida en un taladro 32 en la sección proximal 21, separa la sección distal 23 de la mordaza prensora 16 eléctricamente de la sección proximal 21 de la mordaza prensora 16, de modo que las dos mordazas prensoras 15 y 16 están eléctricamente aisladas entre sí y conectadas cada una con un polo de la fuente de alimentación.
La otra mordaza prensora, denominada a continuación mordaza prensora 15 superior, se compone completamente de metal y está formada como una sola pieza en el ejemplo de realización según las figuras 1 a 4. Una sección proximal 34 está configurada de manera similar a la sección proximal 21 de la mordaza prensora 16 inferior y pasa a una sección distal 35 que dispone de dos elementos de apriete 37 idénticos que forman entre sí una ranura longitudinal 36 y discurren en paralelo. Ambos elementos de apriete 37 tienen en su lado dirigido hacia la mordaza prensora 16 inferior una superficie de apriete 38, enfrentada a la superficie de apriete 24 de la mordaza prensora 16 inferior, prevista igualmente de nervios transversales 39.
Ambos elementos de apriete 37 están unidos con la sección proximal 34 a través de nervios 40 en forma de cinta con una altura de construcción reducida, estos nervios están configurados tan planos que en esta zona se facilita una basculación elástica de los elementos de apriete 37 en forma de brazos alrededor de un eje de basculación que discurre en paralelo al eje 14 de soporte.
En la ranura longitudinal 36, que queda libre entre los dos elementos de apriete 37, penetra un cuerpo cortante 41, unido rígidamente con la sección proximal 34, cuyo borde inferior está configurado como filo cortante 42. Si los elementos de apriete 37 no están deformados, es decir, cuando no están desplazados elásticamente frente a la sección proximal 34, el filo cortante 42 se encuentra completamente en el interior de la ranura longitudinal 36 y no sobresale hacia abajo por encima de las superficies de apriete 38 de los dos elementos de apriete 37 (véase la representación en las figuras 2 y 3).
Cuando los elementos de apriete 37 se desplazan elásticamente hacia arriba, es decir, en sentido de apertura de las mordazas de apriete, el filo cortante 42 sobresale hacia abajo entre las superficies de apriete 38 y puede entrar en función.
En la mordaza de apriete 16 inferior está configurada también una ranura longitudinal 43, alineada con la ranura longitudinal 36 en la mordaza de apriete 15 superior y cerrada en ambos extremos, la longitud de la ranura longitudinal 43 está dimensionada de tal forma que el filo cortante 42 del cuerpo cortante 41 puede penetrar en la ranura longitudinal 43.
El instrumento configurado de esta manera sirve al mismo tiempo como instrumento de agarre bipolar y como instrumento de corte.
Con la barra 8 de empuje y tracción en su posición avanzada, las mordazas prensoras 15 y 16 están distanciadas entre sí y las tijeras de agarre se encuentran en su posición abierta. En esta posición es posible colocar las superficies de apriete 24 y 38 de las mordazas prensoras en el tejido a tratar, por ejemplo un vaso sanguíneo 44, y coger el mismo (figura 3). Mediante aplicación de una tensión de alta frecuencia en las dos mordazas prensoras 15 y 16, y debido al contacto del vaso sanguíneo con las dos mordazas prensoras 15 y 16, es posible coagular el vaso sanguíneo 44 en la zona de contacto de modo de por sí conocido de tijeras de agarre bipolares.
La coagulación tiene lugar en una posición de las mordazas prensoras 15, 16 en la que, frente a la posición abierta, están desplazadas a una posición intermedia, pero aún no están completamente cerradas. En esta posición intermedia, la presión de contacto de la mordaza prensora 15 superior en el vaso sanguíneo 44 será aún tan baja que los elementos de apriete 37 no basculan elásticamente ni se deforman en la zona de los nervios 40, por lo que el cuerpo cortante 41 con el filo cortante 42 se mantiene en la ranura longitudinal 36, es decir, el filo cortante 42 queda sin efecto
(figura 3).
Durante el avance del movimiento de cierre de las mordazas prensoras 15 y 16, retirando la barra 8 de empuje y tracción, las mordazas prensoras 15 y 16 presionan fuertemente la una contra la otra, de modo que los elementos de apriete 37 se doblan en la zona de los nervios 40 delgados, elásticamente deformables, es decir, los elementos de apriete 37 basculan hacia arriba en sentido contrario a la posición de cierre y el filo cortante 42 del cuerpo cortante 41 puede sobresalir hacia abajo sobre las superficies de apriete 38, entrar en contacto con el vaso sanguíneo 44 y cortar el mismo durante el avance del cierre de las mordazas prensoras 15 y 16, el cuerpo cortante 41 penetra con el filo cortante 42 en la ranura longitudinal 43.
Con el mismo movimiento de cierre del mango 7 orientable, el cirujano puede coger primero el tejido a tratar y cortar, coagular el mismo mediante aplicación de una tensión de alta frecuencia y finalmente, girando aún más el brazo 7, separar el tejido en la zona coagulada, para ello sólo se requiere un mecanismo de accionamiento y el cirujano puede realizar este procedimiento con una única palanca basculando la misma según las necesidades para que el instrumento adopte consecutivamente las posiciones de trabajo descritas.
El ejemplo de realización según la figura 5 es similar al de las figuras 1 a 4, las piezas que se corresponden llevan los mismos símbolos de referencia.
Existe sólo una diferencia en la zona de la mordaza prensora 15 superior, formada de dos partes en el ejemplo de realización según la figura 5. En este ejemplo de realización, los dos elementos de apriete 37 no están unidos con la sección proximal 34 de la mordaza prensora 15 a través de nervios elásticos 40, sino que están apoyados de forma orientable, a través de una clavija 45 de soporte, en el cuerpo cortante 41, es decir, en la zona de transición del cuerpo cortante 41 a la sección proximal 34. En este ejemplo de realización, los extremos distales de los elementos de apriete 37 están unidos entre sí a través de un nervio transversal 46 y los dos elementos de apriete 37 forman junto con el nervio transversal 46 un componente uniforme.
Un elemento elástico 47, representado en el dibujo sólo esquemáticamente, unido por un lado con el cuerpo cortante 41 y por otro lado con el nervio transversal 46, mantiene los elementos de apriete 37 en una posición en la que las superficies de apriete 38 sobresalen hacia abajo del filo cortante 42 en el cuerpo cortante 41, es decir, el filo cortante 42 está completamente alojado en la ranura longitudinal 36 y no entra en función. Es posible bascular elásticamente los elementos de apriete 37 en contra del efecto de este elemento elástico 47, de modo que el filo cortante 42 sale de la ranura longitudinal 36 y entra en función como se ha descrito anteriormente.
En este ejemplo de realización, los elementos de apriete 37 están unidos de forma eléctricamente conductiva con la sección proximal 34 a través de la clavija de soporte 45, por lo que también en este caso las superficies de apriete 38 están en contacto con un polo de la fuente de tensión de alta frecuencia.

Instrumento de agarre bipolar con dos mordazas prensoras (15, 16) móviles la una contra la otra, eléctricamente aisladas entre sí, cada una conectable con un polo de una fuente de tensión de alta frecuencia, que dispone cada una de dos elementos de apriete (37) con una superficie de apriete (38), distanciados el uno frente al otro, que forman entre sí una ranura longitudinal (36), y con un dispositivo de corte que comprende un elemento cortante, desplazable en las ranuras longitudinales de las mordazas prensoras (15, 16), con un filo cortante (42), donde el elemento cortante (41) está dispuesto en la ranura longitudinal (36) de una mordaza prensora (15), caracterizado porque el filo cortante (42) está alojado en la ranura longitudinal (36) entre los elementos de apriete (37) de esta mordaza prensora (15) y no sobresale por encima de sus superficies de apriete (38) en sentido hacia la otra mordaza prensora (16), y porque los elementos de apriete (37) de esta mordaza prensora (15) son elásticamente desplazables en contra del movimiento de cierre de las mordazas prensoras (15, 16) en tal medida, en relación con el elemento cortante (41), que el filo cortante (42) del elemento cortante (41) sobresale por encima de las superficies de apriete (38).
2. Instrumento de acuerdo con la reivindicación 1 caracterizado porque tanto el elemento cortante (41) como los elementos de apriete (37) están formados de una sola pieza con la mordaza prensora (15), y porque el elemento cortante (41) es una parte rígida de la mordaza prensora (15), mientras que los elementos de apriete (37) están unidos con las demás partes (34) de la mordaza prensora (15) a través de zonas (40) elásticamente deformables.
3. Instrumento de acuerdo con la reivindicación 2 caracterizado porque las zonas elásticamente deformables (40) están constituidas por nervios de altura reducida que se pueden doblar elásticamente.
4. Instrumento de acuerdo con la reivindicación 1 caracterizado porque la mordaza prensora (15) está formada de dos partes, con una primera parte rígida que comprende el elemento cortante (41) y el soporte de la mordaza prensora (15) en el instrumento de agarre, y con una segunda parte que comprende los elementos de apriete (37) y que está apoyada de forma elásticamente móvil en la primera parte.
5. Instrumento de acuerdo con la reivindicación 4 caracterizado porque los elementos de apriete (37) están apoyados de forma elásticamente orientable en la primera parte.
6. Instrumento de acuerdo con la reivindicación 5 caracterizado porque la segunda parte está configurada en forma de U, con dos alas paralelas, que constituyen los elementos de apriete (37), y con un nervio (46) que une las mismas y cierra la ranura longitudinal (36) hacia el extremo distal de la mordaza prensora (15), y porque la segunda parte está apoyada de forma giratoria en la primera parte con los extremos libres de las alas.
7. Instrumento de acuerdo con una de las reivindicaciones anteriores caracterizado porque la ranura longitudinal (43) en la otra mordaza prensora (16), que no comprende un dispositivo de corte, está cerrada en ambos extremos.
