Abstract

Description
Máscara nasal.
Esta invención se refiere a máscaras nasales
particularmente, aunque no solamente, para ser usadas proporcionando
una terapia de CPAP (Presión de Conducto de Aire Positiva Continua)
a pacientes que padecen Apnea de Sueño Obstructiva (OSA).
En la técnica de los dispositivos respiratorios,
existe una bien conocida diversidad de máscaras respiratorias que
cubren la nariz y/o la boca de una persona para proporcionar una
obturación continua alrededor de las áreas nasales y/u orales de la
cara de modo que puede ser proporcionado un gas con una presión
positiva dentro de la máscara para que sea consumido por el usuario
o usuaria. La utilización de tales máscaras varía desde la
respiración a elevada altitud (por ejemplo, en aplicaciones
aeronáuticas) a aplicaciones mineras y contra incendios, a diversas
aplicaciones de diagnóstico médico y aplicaciones terapéuticas.
Un requisito de tales máscaras respiratorias ha
sido el de proporcionar una obturación eficaz contra la cara del
usuario para impedir la fuga del gas que se suministra.
Ordinariamente, en configuraciones de máscara de la técnica
anterior, una buena obturación de la máscara con la cara solamente
había sido alcanzada en muchos casos con una incomodidad
considerable para el usuario. Este problema es crucial en la mayoría
de las aplicaciones, especialmente en las aplicaciones médicas, que
requieren que el usuario utilice ese tipo de máscara de modo
continuo durante horas o quizás incluso días. En tales situaciones,
el usuario no tolerará la máscara durante largos periodos y los
objetivos de diagnóstico o terapéuticos óptimos no se conseguirán, o
se conseguirán con una gran dificultad y considerable incomodidad
del usuario.
En común con diseños de la técnica anterior,
existe una incapacidad de obturar eficazmente cuando la cara del
usuario/usuaria llega a distorsionarse. Por ejemplo, como se muestra
en la máscara de técnica anterior de la Figura 1 cuando el usuario
300 está durmiendo sobre su costado, un lado 302 del equipo de
cabeza tiende a ser apretado mientras el otro lado tiende a ser
aflojado. Esto origina que el eje de la máscara 306 sea retorcido
con respecto al eje de la cabeza 308, debido al momento torsor neto
que se aplica al equipo de cabeza, que origina una fuga 310 sobre
un lado. El usuario/a 300 durmiendo sobre su costado puede
distorsionar también los contornos faciales alrededor del área nasal
312 y puede conducir a más fugas.
El documento US 5.832.918 describe un
dispositivo respiratorio que tiene una máscara abovedada dura
habitual y una tira fijada a la bóveda de la máscara.
Un objeto de la presente invención es tratar de
proporcionar una máscara nasal que supere en parte los
inconvenientes anteriormente mencionados de la técnica anterior o
que al menos proporcione a la industria una opción útil.
Consecuentemente, la presente invención
proporciona un dispositivo que tiene las características de la
reivindicación 1.
La invención se compone de la parte expuesta y
contempla también construcciones de las que se proporcionan los
ejemplos siguientes.
Una forma preferida de la presente invención se
describirá a continuación con referencia a los dibujos que se
acompañan, en los cuales:
la figura 1 es una vista en planta de una
máscara de la técnica anterior que ilustra una fuga lateral.
la figura 2 es una vista en planta de una
máscara según la realización preferida de la presente invención;
la figura 3 es un diagrama de bloques de un
sistema de presión de conducto de aire positiva, continua (CPAP)
humidificado que puede ser usado en combinación con la presente
invención;

La presente invención proporciona mejoras en el
campo de las máscaras nasales para ser usadas en la terapia de CPAP.
En particular, se describe una máscara nasal que es más cómoda para
ser usada por el usuario/a y reduce la fuga lateral en comparación
con las máscaras de la técnica anterior. Se apreciará que la máscara
nasal como se describe en la realización preferida de la presente
invención puede ser usada en el cuidado respiratorio en general o
con un ventilador pero se describirá ahora, más adelante, con
referencia a la utilización en un sistema de CPAP humidificado.
Con referencia a la figura 3, en ella se muestra
un sistema de Presión de Conducto de Aire Positiva Continua (CPAP)
humidificado en el que un paciente 1 recibe gases a presión y
humidificados a través de una máscara nasal 2 conectada a una vía
de transporte de gases humidificados o conducto 3 inspiratorio. Se
ha de entender que los sistemas de suministro podrían ser también
de VPAP (Presión de Conducto de Aire Positiva Variable) y BiPAP
(Presión de Conducto de Aire Positiva de Dos Niveles) o de otras
numerosas formas de la terapia respiratoria. El conducto
inspiratorio 3 está conectado a la salida 4 de una cámara 5 de
humidificación que contiene un volumen de agua 6. El conducto
inspiratorio 3 puede contener medios de calentamiento o hilos de
calentamiento (no mostrados) que calienten las paredes del conducto
para reducir la condensación de los gases húmedos dentro del
conducto. La cámara 5 de humidificación se forma preferiblemente a
partir de un material plástico y puede tener una base muy
conductora del calor (por ejemplo una base de aluminio) que esté en
contacto directo con una placa 7 de calentador del humidificador 8.
El humidificador 8 se proporciona con medios de control, o
controlador electrónico, 9 que pueden comprender un controlador
basado en un microprocesador que ejecute comandos de software de
ordenador almacenados en la memoria asociada.
El controlador 9 recibe entradas de fuentes
tales como medios de entrada de usuario o el indicador 10 a través
del cual un usuario del dispositivo puede, por ejemplo, establecer
un valor requerido (valor preestablecido) de la humedad o
temperatura de los gases suministrados al paciente 1. El controlador
puede recibir también entradas de otras fuentes, por ejemplo, de
temperatura y/o de sensores 11 y 12 de velocidad de la circulación
a través del conectador 13 y del sensor 14 de temperatura de la
placa de calentador. En respuesta al grado de humedad establecido
por el usuario por medio del indicador 10 de la vía de entrada del
valor de la temperatura y las otras entradas, el controlador 9
determina cuando (o a que nivel) activar la placa 7 de calentador
para calentar el agua 6 dentro de la cámara 5 de humidificación. A
medida que el volumen de agua 6 se calienta dentro de la cámara 5
de humidificación, el vapor de agua empieza a llenar el volumen de
la cámara por encima de la superficie del agua y se extrae de la
cámara 5 de humidificación a través de la salida 4 con la corriente
de gases (por ejemplo aire) proporcionada desde los medios de
suministro de gases o soplante 15 que entran en la cámara a través
de la entrada 16. Los gases exhalados desde la boca del paciente
pasan directamente al medio ambiente que le rodea en la figura
3.
El soplante 15 está provisto de medios de
regulación de la presión variables, o de un ventilador 21 de
velocidad variable, que aspiran aire u otros gases a través de la
entrada 17 de soplante. La velocidad del ventilador 21 de velocidad
variable es controlada por el controlador 18 electrónico (o
alternativamente la función del controlador 18 podría ser efectuada
por el controlador 9) en respuesta a entradas procedentes del
controlador 9 y a un valor requerido predeterminado establecido de
usuario (valor preestablecido) de la presión o velocidad de
ventilador por medio del indicador 19.
Haciendo referencia a la figura 4, en ella se
muestra la máscara nasal detalladamente, según la realización
preferida de la presente invención. La máscara nasal incluye un
cuerpo hueco 102 y una entrada 103 conectada al conducto
respiratorio 3. La máscara 2 está posicionada alrededor de la nariz
del usuario 1 con el equipo 108 de cabeza asegurado alrededor de la
parte posterior de la cabeza del paciente 1. La fuerza restringida
del equipo 108 de cabeza sobre el cuerpo hueco 102 y el resto 108 de
la frente garantiza suficiente fuerza de compresión sobre el cojín
104 de la máscara, para proporcionar una obturación eficaz contra la
cara del paciente.
El cuerpo hueco 102 se construye de un material
relativamente inflexible, por ejemplo, plástico de policarbonato.
Ese tipo de material proporcionará la rigidez requerida así como la
transparencia y es un aislador relativamente bueno. Los gases
expiratorios pueden ser expulsados a través de una válvula (no
mostrada) en la máscara, de un conducto expiratorio adicional (no
mostrado), o mediante cualquier otro método como sea conocido en la
técnica.
Haciendo referencia ahora a las figuras 4 y 5,
en ellas se muestra el equipo 108 de cabeza conectado al cuerpo
hueco 102. La presente invención en vez de las ataduras fijas o
ajustables tradicionales utiliza una aplicación de deslizamiento
entre el equipo 108 de cabeza y el cuerpo hueco 102. Esto se logra
mediante un bucle 120, que se extiende a través de sujetadores 122,
124 de equipamiento en cada lado del equipo 108 de cabeza y sobre
la parte superior del cuerpo hueco 102. El bucle 120 se aplica
alternativamente con guías 126, 128 montadas en la superficie
superior del cuerpo hueco 102. Las guías limitan al bucle 120 pero
le permiten deslizarse dentro y fuera, lo que significa que el
equipo 108 de cabeza puede moverse lateralmente, con independencia
del cuerpo hueco 102.
La ventaja de esta solución radica en que como
la cara es contorneada durante diversas posiciones de sueño, el
equipo de cabeza es capaz de moverse con los cambios de posición
mientras la máscara permanece en la posición correcta sobre la nariz
del usuario y se mantiene una obturación eficaz.
Las guías adicionales 129, 130, 131 permiten que
el usuario ajuste la posición del bucle 120, lo cual permite obtener
una presión diferente sobre la obturación que depende de la posición
del bucle 120.
Para garantizar más la comodidad del usuario y
la presión eficaz sobre el cojín 104 de máscara, el equipo 108 de
cabeza puede ser construido usando dos tiras que se extienden
alrededor de la parte posterior de la cabeza de usuario como se
muestra en la figura 4 o con una cubierta parcial de la cabeza o
cualquier otra configuración, como se conoce en la técnica. En este
caso las tiras, o la cubierta parcial de la cabeza se fabricarían
usando neopreno pero también pueden ser fabricadas usando cualquier
otro material, como se sabe en la técnica, que sea cómodo para el
usuario.
En una realización más, mostrada en las Figuras
6 y 8 de la presente invención, se ilustra usando una tira de
deslizamiento para fijar el equipo 108 de cabeza al cuerpo hueco
102. La tira 200, mostrada en la figura 8 aisladamente, se construye
de poliacetal (Delrin 500P NC010) usando técnicas de moldeo por
inyección para obtener un acabado pulido. Este material, similar a
otros derivados basados en el nailon, con su acabado pulido tiene un
rozamiento particularmente bajo muy eficaz, y por lo tanto se
desliza con respecto al cuerpo hueco 102 con muy poca
resistencia.
Como se muestra en la figura 6, el cuerpo hueco
102 incluye un cierto número de sujetadores 202 de aplicación,
dentro de los cuales, la tira 200 de deslizamiento, en uso, se
aplica elásticamente y solamente puede ser desmontada de los mismos
usando una fuerza sustancial. Esto significa que con el uso normal
la tira 200 de deslizamiento permanecerá retenida dentro de los
sujetadores 202 de aplicación. También se apreciará en la figura 6
que se proporciona un cierto número de sujetadores, para permitir
una presión procedente de diferentes ángulos para diferentes formas
de cara.
Como se muestra en la figura 8, la tira de
deslizamiento incluye una sección media 204 destinada a moverse de
modo alternativo con respecto a los sujetadores 202 que se aplican,
terminando en cada extremo con enganches 206, 208 que se fijan al
equipo de cabeza. El primer enganche 206 es un bucle completo a
través del cual el miembro 108 de cabeza se fija de modo permanente,
por ejemplo, con una tira velcro. El bucle 208 en el otro extremo,
es solamente un bucle parcial 210 diseñado de modo que una tira o
bucle del equipo 108 de cabeza puede ser deslizado fácilmente dentro
o fuera de la sección abierta 212 permitiendo una fácil retirada y
fijación de la máscara.
Se ha de tener en cuenta que en todas las
realizaciones de la presente invención la fijación del equipo de
cabeza a la máscara está diseñada para deslizarse con un rozamiento
tan pequeño como es posible al mismo tiempo que garantiza una fuerza
directa adecuada sobre el cojín de la máscara en la cara del
usuario. Como se muestra en la Figura 2 la conexión 320 de
deslizamiento permite que el equipo 322, 324 de cabeza proporcione
una fuerza uniforme sobre ambos lados de la máscara 326. Esto evita
aplicar un momento torsor sobre la máscara y retorcer la cámara al
mismo tiempo que minimiza fugas de la máscara desde la obturación
con la cara 326.
Claims (9)
1. Un dispositivo para administrar un suministro
de gases a un usuario/a (1) que comprende o incluye:
una interfaz (2) de paciente que tiene un cuerpo
hueco (102), estando destinada la interfaz a estar en comunicación
de fluido con dicho suministro de gases (15),
un equipo (108) de cabeza destinado a ser fijado
a, o alrededor de, la cabeza de dicho usuario (1), donde dicha
interfaz (2) de paciente está destinada a obturar sustancialmente
con respecto al menos a una porción de una cara y/o el sistema
respiratorio de dicho usuario (1) en al menos una orientación y
posición correctas sobre dicho usuario (1) y en el que hay una
conexión (120, 200) de deslizamiento entre dicho equipo (108) de
cabeza y dicha interfaz (2) de paciente, cuando dicha interfaz (2)
de paciente está aplicada a un usuario (1), en el que
la conexión (120, 200) de deslizamiento está
forzada por un par de sujetadores (202) o guías (126, 128, 129, 130,
131) de un cierto número de pares de sujetadores (202) o guías (126,
128, 129, 130, 131) pero puede deslizarse dentro y fuera, de modo
que el equipo (108) de cabeza puede moverse lateralmente, de modo
independiente del cuerpo hueco (102), y la conexión (120, 200) de
deslizamiento es una tira forzada por el par de sujetadores (202) o
guías (126, 128, 129, 130, 131) a moverse lateralmente.
2. Un dispositivo según la reivindicación 1, en
el que dicha interfaz (2) de paciente es una máscara nasal.
3. Un dispositivo según la reivindicación 2, en
el que dicha máscara nasal (2) comprende o incluye una porción (102)
de cuerpo que tiene una entrada (103) que recibe dicho suministro de
gases, y medios (104) de obturación fijados a, o integrados con,
dicha porción (102) de cuerpo que están destinados a obturar contra
los contornos faciales de dicho/a usuario/a.
4. Un dispositivo según la reivindicación 3, en
el que dicha conexión (120, 200) de deslizamiento está destinada a
permitir que dicho equipo (108) de cabeza se mueva sustancialmente
con respecto a dicha máscara nasal (2), al mismo tiempo que todavía
proporciona fuerza compresora suficiente sobre dichos medios (104)
de deslizamiento para garantizar que dicho suministro de gases es
administrado a dicho usuario sin fugas significativas.
5. Un dispositivo según una cualquiera de las
reivindicaciones 1 a 4, en el que la aplicación entre dicho equipo
(108) de cabeza y la conexión (120, 200) de deslizamiento está
destinada a permitir la liberación temporal de la misma.
6. Un dispositivo según la reivindicación 5, en
el que la conexión (120, 200) de deslizamiento es alargada teniendo
un primer extremo (122) y un segundo extremo (124), un primer bucle
(206) cerrado situado en dicho primer extremo (122) y un segundo
bucle parcial (208) que tiene una sección abierta situada en dicho
segundo extremo (124), por lo que en uso dicho equipo (108) está
sustancialmente fijado permanentemente a dicho primer bucle (206) y
fijado sustancialmente pero de modo desmontable desde dicho segundo
bucle (208)
7. Un dispositivo según las reivindicaciones 5 ó
6, en el que los sujetadores (202) o las guías (126, 128, 129, 130,
131) se proporcionan sobre dicha porción (102) de cuerpo.
8. Un dispositivo según la reivindicación 1, en
el que dicho equipo (108) de cabeza incluye una tira (200) de
deslizamiento que se desliza conectada con, o adyacente a, dicha
interfaz de paciente.
9. Un dispositivo según una cualquiera de las
reivindicaciones 1 a 8, en el que dicho equipo (108) de cabeza
incluye un bucle (120) de deslizamiento que se desliza conectado
con, a través de, o adyacente condicha interfaz (2) de paciente.
