Abstract
Description
- Die Erfindung betrifft ein Mittel zur Vorbehandlung verunreinigter medizinischer oder chirurgischer Instrumente und Apparate, einen mit einem solchen Mittel befüllten Aerosolbehälter sowie ein Verfahren zur Aufbereitung medizinischer und/oder chirurgischer Instrumente und Apparate.
- Chirurgische Instrumente durchlaufen einen sogenannten Instrumentenzyklus. Nach Gebrauch im OP werden sie vom Chirurg bzw. OP-Personal in einem Behälter abgelegt und der Aufbereitung zugeführt. Bekannt ist die sogenannte Prädesinfektion als Nassentsorgungsmethode, bei der die Instrumente in einen großen Tauchbehälter gefüllt mit Desinfektionslösung abgelegt werden. Diese Tauchbehälter enthalten häufig 50 l Desinfektionslösung. Eine solche Prädesinfektion wird bspw. In Frankreich häufig angewendet.
- Die entsorgten Instrumente werden zu einer zentralen Instrumentenaufbereitung gebracht. Die Instrumentenaufbereitung erfolgt in Krankenhäusern inzwischen weitestgehend zentral, da nach dem Stand der Technik und relevanter Empfehlungen zur Krankenhaushygiene eine dezentrale Aufbereitung nicht empfohlen wird. So ist insbesondere vorgesehen, dass die zentrale Aufbereitung eine sogenannte unreine und eine reine Seite aufweisen muss, für den Zugang zur reinen Seite sind beispielsweise Schleusen vorgesehen, durch die die reine Seite betreten werden kann. Dies ist wirtschaftlich nur in einer zentralen Aufbereitung darstellbar.
- In der zentralen Aufbereitung werden die entsorgten Instrumente der maschinellen Reinigung und Desinfektion zugeführt. Nach dem Reinigen und Desinfizieren werden sie auf der reinen Seite entnommen und in der Regel zu gängigen OP-Sets verpackt, die beispielsweise die erforderlichen Instrumente für eine bestimmte Operation enthalten. Diese Sets werden dann in einem weiteren Schritt dampfsterilisiert (134°C) und für die Verwendung gelagert.
- Die nasse Entsorgung kann eine sogenannte Fixierung von Verunreinigungen an den benutzten Instrumenten verursachen, beispielsweise durch chemische Denaturierung von Proteinen. Ferner bedeutet es einen hohen Aufwand, Tauchbäder mit 50 l Desinfektionslösung und darin gelagerten Instrumenten zu einer zentralen Instrumentenaufbereitung zu fahren.
- Bekannt ist ebenfalls (Zentr Steril 2010; 18 (4): 235-243)), gebrauchte Instrumente trocken vom Gebrauchsort zur Zentralsterilisation zu transportieren. Um die Antrocknung von Rückständen während des Transports oder der Zwischenlagerung zu verhindern, wird die Oberfläche der Instrumente nach dem Gebrauch mit Lösungen zum Feuchthalten, in der Regel so genannte Schaumsprays, benetzt.
- Die im Stand der Technik bekannten Schaumsprays weisen sämtlich Enzyme auf, um den Proteinabbau einzuleiten bzw. zu unterstützen und auf diese Art und Weise eine Antrocknung von Blutresten zu verhindern. Um die Wirksamkeit der Enzyme zu gewährleisten, liegt der pH-Wert üblicherweise im neutralen Bereich.
- Der Erfindung liegt die Aufgabe zu Grunde, ein Mittel und ein Verfahren der eingangs genannten Art zur Verfügung zu stellen, dass eine Antrocknung von Rückständen vor Durchführung der zentralen Aufbereitung der Instrumente verhindert und den dauerhaften Werterhalt der Instrumente gewährleistet.
- Gelöst wird diese Aufgabe durch ein Mittel zur Vorbehandlung verunreinigter medizinischer oder chirurgischer Instrumente und Apparate, das Tenside enthält und versprühbar formuliert ist, dadurch gekennzeichnet, dass es enzymfrei ist und einen pH-Wert von 9 oder mehr aufweist.
- Zunächst seien einige im Rahmen der Erfindung verwendete Begriffe erläutert.
- Medizinische und/oder chirurgische Instrumente und Apparate sind sämtliche im medizinischen und Krankenhausbereich eingesetzten Geräte sowie Teile davon, die nach ihrer Verwendung einer Instrumentenaufbereitung unterzogen werden können.
- Das erfindungsgemäße Mittel ist versprühbar formuliert. Dies bedeutet, dass es anwendungsfertig in einer Form bereitgehalten wird, die einem Benutzer unmittelbar ein Versprühen als ein Aerosol auf die Oberflächen der Instrumente gestattet. Ein Aerosolbehälter ist ein Behälter, aus dem das Mittel bspw. Mittels eines Sprühkopfs als Aerosol versprühbar ist.
- Die Erfindung hat erkannt, dass die in Stand der Technik bekannten Schaumsprays in mehrerlei Hinsicht nachteilig sind. Sie gewährleisten insbesondere nicht einen Schutz metallischer Oberflächen (insbesondere Oberflächen aus Edelstahl und eloxiertem Aluminium) gegenüber Korrosion, insbesondere durch Chlorid-Ionen induzierter Lochfraßkorrosion.
- Das erfindungsgemäße Mittel weist hingegen einen vergleichsweise hohen pH-Wert auf, es hat sich gezeigt, dass dadurch ein wirksamer Schutz insbesondere gegen durch Chlorid-Ionen induzierte Korrosion gewährleistet wird. Erfindungsgemäß wird auf den Einsatz der im Stand der Technik bei Schaumsprays für notwendig gehaltenen Enzyme verzichtet. Die Anwesenheit von proteolytisch wirksamen Enzymen verlangt eine verhältnismäßig niedrigen pH-Wert im neutralen Bereich, da nur in diesem pH-Bereich die Enzyme ihre Wirkung entfalten können. Überraschenderweise ist trotz des Verzichts auf Enzyme und trotz der vergleichsweise hohen Alkalität des erfindungsgemäßen Mittels einerseits ein wirksamer Schutz der Oberflächen gegen unerwünschte Antrocknung von Schmutzrückständen und andererseits gegen Korrosion gewährleistet.
- Bevorzugt liegt der pH-Wert des erfindungsgemäßen Mittels bei wenigstens 10, bevorzugte Obergrenzen sind pH Werte von 13 bzw. 12. Ein pH-Wert im Bereich 10-12 ist somit besonders bevorzugt.
- Die Bestimmung des pH-Wertes erfolgt in einer Lösung von 1 Gew.-% des erfindungsgemäßen Mittels in Wasser.
- Die Tenside sind erfindungsgemäß bevorzugt amphotere und/oder anionische Tenside. Die verwendeten anionischen Tenside sind bevorzugt schaumbildend. Dies erleichtert ein Versprühen des Mittels als Schaum. Als anionisches Tensid eignen sich beispielsweise Acylglutamate, Akylsulfate und - ethersulfate. Als amphotere Tenside eignen sich beispielsweise Amphopropionate, Amphoacetate und Betaine.
- Der Tensidgehalt liegt bevorzugt zwischen 0,2 und 5 Gew.-%, weiter vorzugsweise 0,5 und 1,5 Gew.-%.
- Als Quelle von Alkalität kann das Mittel beispielsweise alkalische Phosphate aufweisen, besonders bevorzugt Polyphosphate, die gleichzeitig eine für die gleichmäßige Benetzung der Oberfläche der Instrumente wichtige Funktion als Dispergierhilfsmittel übernehmen können. Besonders bevorzugt sind Alkalitripolyphosphate wie Kaliumtripolyphosphat. Der Gehalt an alkalischen Dispergiermitteln beträgt bevorzugt 5 bis 40 Gew.-%, weiter vorzugsweise 15 bis 30 Gew.-%.
- Ein weiterer Gegenstand der Erfindung ist ein Behälter, der eine Einrichtung zur Erzeugung eines Aerosols aufweist und ein Mittel gemäß der vorliegenden Erfindung enthält. Es kann sich beispielsweise um einen Pumpspraybehälter handeln, alternativ kann es sich um eine mit einem Treibgas unter Druck gesetzte Sprühdose handeln.
- Ein weiterer Gegenstand der Erfindung ist die Verwendung eines Mittels gemäß der vorliegenden Erfindung zur Inhibierung der chloridinduzierten Korrosion bei verunreinigten medizinischen oder chirurgischen Instrumenten und Apparaten. Die Erfindung ermöglicht es, das verunreinigte Instrumente über Zeiträume von mehreren Stunden einerseits an der Oberfläche feucht gehalten werden und andererseits nicht oder allenfalls in sehr geringen Maße korrodieren. Ebenfalls Gegenstand der Erfindung ist ein Verfahren zur Aufbereitung medizinischer und/oder chirurgischer Instrumente und Apparate, mit den Schritten:
- a) Besprühen der gebrauchten Instrumente und/oder Apparate mit einem Mittel gemäß einem der Ansprüche 1 bis 9;
- b) Überführen der Instrumente und/oder Apparate zu einer zentralen Instrumentenaufbereitungsstation;
- c) Durchführen einer vollständigen Reinigung, Desinfektion und Sterilisation der Instrumente und/oder Apparate in der zentralen Instrumentenaufbereitungsstation zur Wiederherstellung eines gebrauchsfertigen Zustands der Instrumente und/oder Apparate.
- Der Zeitraum zwischen Schritt a) und Beginn des Schritts c) beträgt bevorzugt 24 h oder weniger, vorzugsweise 12 h oder weniger, weiter vorzugsweise 6 h oder weniger.
- In der zentralen Instrumentenaufbereitungsstation wird eine vollständige Reinigung, Desinfektion und anschließende Sterilisation der Instrumente und Apparate durchgeführt. Ergebnis dieser Aufbereitung sind gebrauchsfertig zur erneuten Verwendung hergerichtete Instrumente, bspw. verpackte OP-Sets. Erfindungsgemäß kann die Sterilisation auch getrennt von der vollständigen Reinigung und Desinfektion in der zentralen Instrumentenaufbereitungsstation stattfinden. Bspw. können die gereinigten und desinfizierten Instrumente verpackt und dann ggf. räumlich (und wahlweise zeitlich) getrennt von der Aufbereitung der zentralen Instrumentenaufbereitungsstation sterilisiert, bspw. dampfsterilisiert werden.
- Ausführungsbeispiele der Erfindung werden nachfolgend erläutert.
- Folgende Bestandteile werden gemäß der nachfolgenden Rezeptur angesetzt (Angaben in Gew.-%):
Einsatzrohstoff CAS-Nr. Funktion Einsatzmenge (in %) Wasser, vollensalzt 76,1 Kallumtripolyphosphat 13.845-36-8 Dispergierhilfmittel, Alkalität 23 kurzkettiges Amphopropionat 64260-45-8 Tensid amphoter 0,4 Natriumacylglutamat 29923-31-7 Tensid anionisch, Schaumbildner 0,5 - Das so angesetzte Mittel (nachfolgend als MAS bezeichnet) wird in einem Aerosolbehälter versprühfertig zur Verfügung gestellt.
- Prüfkörper mit den Abmessungen 50x50x2mm aus dem Werkstoff X46CR13, geschliffen wurden in einer Miele Maschine Typ 7883 mit folgendem Programm gereinigt:
- Vorreinigung: kalt
- Reinigung: ca. 3ml/l neodisher® LaboClean FLA; 80°C
- Neutralisation: neodisher® Z
- Zwischenspülung: kalt
- Schluss-Spülung: 75°C
- Die gereinigten Prüfkörper wurden anschließend mit einem mit Methanol getränkten Papiertuch abgewischt. Zur Abgrenzung einer Teilfläche der Prüfkörper wird je 1 Messzelle mit einem Durchmesser von 13 mm pro Prüfkörper geklebt.
- Folgende Lösungen werden angesetzt:
- Versuch 1: 0,2 ml Kochsalzlösung (0,9%ig) + 0,2ml MAS
- Versuch 2: 0,2 ml Kochsalzlösung (0,9%ig) + 0,5ml MAS
- Die Lösungen werden in Messzellen appliziert. Nach 6, 24 und 72 h Lagerung bei 20°C werden die Messzellen entfernt und mit schwachem Fließdruck werden die Prüfkörper mit lauwarmen Wasser 30 s abgespült.
- Eine Sichtkontrolle im Labor und eine mikroskopische Kontrolle ergab keine Korrosionsspuren.
- Die Prüfkörper wurden anschließend wie folgt behandelt:
- mit Methanol entfetten,
- 30 min in VE-Wasser kochen,
- 60 min in Wasser abkühlen lassen,
- 120 min an der Luft trocknen.
- Eine zweite Sichtkontrolle und zweite mikroskopische Kontrolle ergab ebenfalls keine Spuren von Korrosion.
- Da im Beispiel 2 keinerlei Korrosion festgestellt wurde, wurden ergänzend Prüfungen nach DIN 51360 (Bestimmung der Korrosionsschutzeigenschaften von wassergemischten Kühlschmierstoffen) durchgeführt. Diese Norm ist so angelegt, dass man aufgrund der Prüfung an vergleichsweise korrosionsanfälligen Graugussspänen in jedem Fall eine Korrosion in gewissen Umfang erhält.
- Folgende Materialien wurden bereitgestellt:
- · Filterpapier : d = 70mm; 589 Fa. S&S
- · Kunststoffpetrischalen: d = 90mm
- · Graugussspäne, 2g pro Filterpapier
- · 0,9%ige Kochsalzlösung
- · MAS (Beispiel 1)
- Je 2g Graugussspäne werden auf das Filterpapier eingewogen. Folgende Lösungen werden angesetzt:
- Versuch 1: 1 ml Kochsalzlösung (0,9%ig) + 1 ml MAS
- Versuch 2: 0,6 ml Kochsalzlösung (0,9%ig) + 1,4 ml MAS
- Versuch 3: 2 ml MAS
- Versuch 4: 2 ml Kochsalzlösung (0,9%ig)
- Versuch 5: 2 ml VE-Wasser
- Die Versuche 3 bis 5 sind Vergleichsbeispiele.
- Mit der Mischung jedes Versuchs werden 2 Prüfungen vorgenommen. Die jeweilige Lösung (2 ml) wird in die Petrischale gegeben. Die Petrischalen werden 2 h geschlossen gelagert. Danach werden die Späne entfernt und das Filterpapier unter fließendem Stadtwasser abgespült, in Aceton geschwenkt und bei Raumtemperatur getrocknet. Nachdem das Filterpapier getrocknet ist, wird der Korrosionsgrad durch Sichtprüfung ermittelt. Die Ergebnisse sind wie folgt:
- Versuch 1: Korrosionsgrad 2 (leichte Korrosion)
- Versuch 2: Korrosionsgrad 1 (Spuren von Korrosion)
- Versuch 3: Korrosionsgrad 0 (keine Korrosion)
- Versuch 4: Korrosionsgrad 4 (starke Korrosion)
- Versuch 5: Korrosionsgrad 3 (mäßige Korrosion)
- Man erkennt, dass MAS wirksam die chloridinduzierte Korrosion hemmen kann.
- Dieses Beispiel beschreibt einen vollständigen Instrumentenzyklus unter Einschluss eines erfindungsgemäßen Verfahrens.
- Im OP-Saal genutzte chirurgische Instrumente werden nach Gebrauch in eine Schale eingelegt, die sich im Operationssaal befindet und mit einem Schaumspray des Beispiels 1 besprüht.
- Die so vorbehandelten Instrumente werden zur zentralen Instrumentenaufbereitung transportiert. Von der sog. unreinen Seite der zentralen Instrumentenaufbereitung werden sie einer im Stand der Technik bekannten Instrumentenaufarbeitung unterzogen. Die Reinigung und Desinfektion erfolgt in einer Instrumentenspülmaschine und umfasst folgende Schritte:
- Kaltwasservorspülung, 3 min,
- Reinigung mit 0,5%iger wässriger Lösung von neodisher mediclean forte® (Alkalischer Reiniger der Chemischen Fabrik Dr. Weigert GmbH & Co. KG), 55°C, 10 min,
- Zwischenspülen mit Wasser bei Raumtemperatur, 1 min,
- Thermodesinfektion mit VE-Wasser, 93°C, 5 min,
- Trocknung.
- Die so behandelten Instrumente werden auf der sog. reinen Seite der zentralen Instrumentenaufbereitung überprüft, ggf. mit Pflegemitteln behandelt und zu Instrumentensets verpackt. Anschließend erfolgt eine übliche Dampfsterilisation.
- Die sterilisierten Instrumentensets werden in das Sterillager verbracht und dort für die nächste Verwendung bereit gehalten.
Claims (13)
- Mittel zur Vorbehandlung verunreinigter medizinischer oder chirurgischer Instrumente und Apparate, das Tenside enthält und versprühbar formuliert ist, dadurch gekennzeichnet, dass es enzymfrei ist und einen pH-Wert von 9 oder mehr aufweist.
- Mittel nach Anspruch 1, dadurch gekennzeichnet, dass es einen pH-Wert von wenigstens 10 aufweist.
- Mittel nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass es einen pH-Wert von höchstens 13, vorzugsweise höchstens 12 aufweist.
- Mittel nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass es amphotere und/oder anionische Tenside enthält.
- Mittel nach Anspruch 4, dadurch gekennzeichnet, dass es ein anionisches schaumbildendes Tensid enthält.
- Mittel nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass der Tensidgehalt 0,2 bis 5 Gew.-%, vorzugsweise 0,5 bis 1,5 Gew.-% beträgt.
- Mittel nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, dass es ein alkalisches Dispergiermittel enthält.
- Mittel nach Anspruch 7, dadurch gekennzeichnet, dass das alkalische Dispergiermittel ein Polyphosphat ist.
- Mittel nach Anspruch 7 oder 8, dadurch gekennzeichnet, dass es 5 bis 40 Gew.-%, vorzugsweise 15 bis 30 Gew.-% alkalisches Dispergiermittel enthält.
- Behälter, der eine Einrichtung zur Erzeugung eines Aerosols aufweist, dadurch gekennzeichnet, dass er ein Mittel nach einem der Ansprüche 1 bis 9 aufweist.
- Verwendung eines Mittels gemäß einem der Ansprüche 1 bis 9, zur Inhibierung der chloridinduzierten Korrosion bei verunreinigten medizinischen oder chirurgischen Instrumenten und Apparaten.
- Verfahren zur Aufbereitung medizinischer und/oder chirurgischer Instrumente und Apparate, mit den Schritten:a) Besprühen der gebrauchten Instrumente und/oder Apparate mit einem Mittel gemäß einem der Ansprüche 1 bis 9;b) Überführen der Instrumente und/oder Apparate zu einer zentralen Instrumentenaufbereitungsstation;c) Durchführen einer vollständigen Reinigung, Desinfektion und Sterilisation der Instrumente und/oder Apparate in der zentralen Instrumentenaufbereitungsstation zur Wiederherstellung eines gebrauchsfertigen Zustands der Instrumente und/oder Apparate.
- Verfahren nach Anspruch 12, dadurch gekennzeichnet, dass der Zeitraum zwischen Schritt a) und Beginn des Schritts c) 24 h oder weniger, vorzugsweise 12 h oder weniger, weiter vorzugsweise 6 h oder weniger beträgt.
