Definitions
- the invention relates to a probe for enteral nutrition of a subject having a jejunal jejunal tube for probing having a proximal and a distal end and a lumen between the proximal and distal ends. Moreover, the invention relates to a probe system with an enteral feeding tube and a gastric tube for gastric decompression or drainage.
- nasogastric tube To decompress the stomach, it may be necessary to place a nasogastric tube next to the feeding tube.
- the nasogastric tube In the event that the nasogastric tube is placed in connection with a feeding tube, it is known to place the nasogastric tube coaxially over the feeding tube.
- the nasogastric tube should have the largest possible inner diameter and / or the feeding tube should have the smallest possible outer diameter. Because the annulus between nasogastric tube and feeding tube should be as large as possible to be able to empty the stomach.
- Probe systems with a feeding tube and a gastric tube are from the DE 101 52 788 A1 and from the US 2003/018320 known.
- the feeding tube is placed with the help of an endoscope.
- the feeding tube is inserted into the working channel of the endoscope until the distal end of the jejunal tube is placed in the jejunum.
- the nasogastric tube is pushed onto the feeding tube until the distal end of the nasogastric tube is placed in the gastrointestinal tract.
- a guidewire for the feeding tube In order to ensure the safe feeding and the secure position of the feeding tube, it is known to provide a guidewire for the feeding tube.
- This guidewire is intended to give the feeding tube the required rigidity.
- a so-called "zebra wire" of a NiTi alloy with PTFE sheath is known.
- the endoscopic displacement of the feeding tube in conjunction with a nasogastric tube using a guide wire is problematic in that the mutual displacement of the instruments and tubes must not lead to a change in position (dislocation) of the feeding tube in the gastrointestinal tract.
- the EP 1 266 646 A2 describes a probe system with a feeding and gavage.
- the nasogastric tube has a two-layered construction with an outer layer of a soft, flexible plastic and an inner layer of a plastic which is harder with respect to the plastic of the outer layer.
- the intrinsically unstable outer layer of the stomach tube should be stabilized by the inner layer of harder plastic.
- the beats EP 1 266 646 A2 To make microrough the surface of the feeding tube to improve the lubricity.
- a catheter with a harder inner layer and a softer outer layer is made of EP 0 354 695 A2 known.
- the invention has for its object to provide a feeding probe that can be placed without the use of a guidewire, especially in conjunction with a feeding tube.
- the enteral nutrition probe according to the invention has a probe tube with a construction of at least two layers.
- the outer first layer and the second layer adjacent to the outer layer differ in the hardness of the material.
- the material of the first layer has a greater hardness than the material of the second layer.
- the harder outer layer ensures sufficient stability to be able to place the probe without further aids.
- the outer harder layer reduces the sliding friction, so that the stomach tube can be pushed over the feeding tube more easily.
- the inner layer adjacent to the outer layer ensures the necessary flexibility. With the combination of the two layers of different hardness on the one hand improved flexibility and on the other hand improved feed behavior is achieved with improved buckling strength.
- the probe tube has a three-layered construction with an outer first layer, an inner second layer adjacent to the outer layer, and an inner third layer adjacent to the inner layer.
- the inner layer is made of a material that has a greater hardness than the material of the inner layer.
- the materials of the inner and outer layers have the same hardness. It has been found that this embodiment is characterized by further improved sliding and Vorschubigenschaften.
- the material of the outer layer preferably has a Shore hardness according to ASTM D 2240 which is greater than 40 and less than 70 D, preferably greater than 50 and less than 60 D, in particular 55 D, while the material of the inner layer is preferably a Shore hardness which is greater than 70 and less than 100 A, preferably greater than 80 and less than 90 A, in particular 85 A.
- the surface of the outer layer is microroughened. With the matted or slightly rough surface, the friction can be further reduced.
- the strength of the jejunal tube can be reduced. This results in a smaller outer diameter so that the probe tube fits into the working channel of the standard endoscopes.
- the probe system according to the invention provides, in addition to the feeding tube, a gastric tube for gastric decompression or drainage.
- the nasogastric tube has a gastric probe tube that is smaller in length and larger in diameter than the jejunal tube of the feeding tube. This makes it possible to place the gastric tube in the gastrointestinal tract and the jejunal tube in the jejunum and insert the jejunal tube into the gastric tube.
- the jejunal probe tube should be more than twice the length of the gastric tube.
- the gastric probe tube also has an at least two-layered construction.
- the inner layer of the gastric tube is preferably made of a material having a higher hardness than the material of the outer layer. Since the outer layer is softer than the inner layer, the probe tube on the one hand by sufficient stability and on the other hand by sufficient suppleness to avoid injury to the mucosa during insertion of the nasogastric tube.
- the lubricity of the nasogastric tube is improved in a particularly preferred embodiment again by a micro-rough surface.
- the probe system according to the invention comprises a feeding tube and a stomach tube.
- the Figures 1 and 2 show the feeding tube while the FIGS. 3 and 4 show the gastric tube.
- the feeding tube 1 has a probe tube 2 in a length of, for example, 2700 mm, which has an open proximal end 3 and an open distal end 4. Between the proximal and distal ends 3, 4 extends the lumen 5 of the probe tube.
- the probe tube has an outer diameter of, for example, 2.6 mm and an inner diameter (lumen) of, for example, 1.8 mm.
- connection piece 6 Connected to the proximal end 3 of the probe tube 1 is a connection piece 6 for connecting a transfer system (not shown) for supplying a nutrient solution.
- the hoses of the known transfer systems have a complementary connector, which is connected to the connector 6 of the feeding probe.
- the connector 6 of the feeding probe 1 has a base body 7 with a Luer-lock connector 8. Positive and negative luer lock connectors are state of the art. To close the Luer-lock connector 8, the connector 6 has a closure cap 9, which closes the connector 8 tight. The closure cap 9 is connected to the base body 7 of the connection piece 6 via a flexible web 10.
- the connector 6 is clamped on the probe tube 2 of the feeding tube, so that the connector can be removed.
- the base body 7 of the connecting piece on a clamping piece 7a, that is placed on the proximal end of the probe tube.
- the probe tube 2 In the region of the distal end 4, the probe tube 2 has four lateral outlet openings 11 for the exit of the nutrient solution. To avoid injury, the distal end 4 of the tubing is rounded.
- Fig. 2 shows a section through the feeding probe 1.
- the probe tube 2 has a three-layer structure. It consists of an outer layer 2a of a harder material, an inner layer 2b of a softer material and an inner layer 2c of a harder material. The surface of the outer layer 2a is microroughened.
- the material of the outer layer 2a has a Shore hardness according to ASTM D 2240, which is preferably greater than 50 and less than 60 D, in particular 55 D.
- the inner layer 2c is made of a material having the same Shore hardness as the outer layer 2a.
- the inner layer 2b is made of a material with a Shore hardness according to ASTM A 2240 which is greater than 80 and less than 90 A, in particular 85 A.
- the inner and outer layers 2a, 2c have, for example, a layer thickness of 0.1 mm.
- the probe tube 2 may for example consist of elastic polymers, for example thermoplastic elastomers, polyurethane, modified polyamides or polyolefins, styrene polymers, fluoropolymers or PVC with plasticizer. It is crucial that on the one hand the required Shore hardness and on the other hand the biocompatibility of the materials is given.
- the three-layer structure of the feeding probe according to the invention leads to improved feed behavior and greater kink resistance with improved flexibility. Since the feeding tube does not make use of a guidewire, fewer steps are required when placing the feeding tube in conjunction with the stomach tube. Also, the manufacturing costs are lower without the expensive guidewire. In addition, the risk of dislocation of the feeding tube is further reduced.
- the outer diameter of the jejunal tube can be chosen so that it fits through the working channel of a standard endoscope and still allows a sufficient inner diameter for application of nutrient solution in the small intestine. Preferably, the outer diameter should be less than 2.8 mm.
- the nasogastric tube of the probe system according to the invention is described below.
- the nasogastric tube 20 has a gastric probe tube 21 in a length of 1200 mm with an open proximal end 22 and an open distal end 23. Between the proximal and distal ends 22, 23 extends the lumen 24 of the probe tube 21.
- the distal end 23 of the probe tube is closed with a terminally open olive 25, the is X-ray contrast capable.
- Lateral openings 26 are distributed circumferentially in the region of the distal end 23 of the probe tube 21.
- the inner diameter of the probe tube 21 of the stomach tube 20 is larger than the outer diameter of the probe tube 2 of the feeding tube 1, so that the probe tube of the feeding tube 20 can be pushed onto the probe tube of the feeding tube 1 to form an annular gap with a sufficient cross section.
- the outer diameter of the gastric probe tube 21 is 5.2 mm and the inner diameter is 3.9 mm.
- the connecting piece 28 has a first access 29 for receiving the jejunal probe tube 2 and a second access 30 for connection to a suction line, not shown.
- the fitting 28 of the nasogastric tube 20 is preferably a Y-adapter with a straight fitting 28a and a fitting 28b branching from the straight fitting.
- Connected to the straight connection piece 28a is a first connector 31 for connecting the connection piece 6 of the feeding probe 1 and the branching connection piece 28b to a second connector 32 for connecting the suction line.
- the connector 32 for connecting the hose is, for example, a Luer-lock connector.
- the Luer-lock connector 32 can be tightly closed with a matching cap 33, which is connected via a flexible web 34 with the branching connection part 28b.
- connection piece 6 of the feeding probe 1 has a cylindrical lower part 12, which can be fittingly placed on the connector 31 of the connecting piece 28 of the feeding tube 20.
- the lower part 12 and the connector 31 are such designed such that the connection piece 6 of the feeding probe 1 and the connector 31 of the connecting piece 28 of the feeding tube 20 form a snap connection.
- 12 grooves or grooves may be provided, for example, on the outside of the connector 31 and the inside of the lower part, which fit into each other, so that a secure, but easily releasable connection is created.
- the probe tube 21 of the stomach tube 20 has a two-layered construction.
- FIG. 4 shows a section through the probe tube 20.
- the gastric probe tube 21 may consist of the same material as the jejunal probe tube 2.
- the outer layer 21 a of the gastric probe tube 21 is made of a material having a Shore hardness according to ASTM A 2240, which is preferably greater than 70 and less than 90 A, in particular 85 A.
- the inner layer is made of a material having a Shore hardness preferably 50 to 70 D, in particular 55 D.
- the outer layer 21 a has a greater thickness than the inner layer 21 b, wherein the inner layer, for example, 0.1 to 0.2 mm is strong. Since the outer layer 21 a is softer than the inner layer 21 b, the gastric tube is less traumatic to the esophagus with sufficient stability.
- the inner surface of the inner layer 21 b may turn microroughened.
- FIG. 5 shows the feeding tube 1 in conjunction with the feeding tube 20.
- the feeding tube 1 is placed by means of an endoscope.
- the connection piece 6 is not placed on the probe tube 2.
- the endoscope is pulled out, which would not be possible with attached connector.
- the probe tube 21 of the nasogastric tube 20 is pushed over the probe tube 2 of the ⁇ Ernährungsprobe 1.
- the probe tube 2 of the feeding tube 1 is shortened at the proximal end to the correct length, and the connector 6 is placed on the proximal end of the probe tube 2 and the first connector 31 of the feeding tube 20.
- the transfer system for supplying the nutrient solution and the suction line, not shown, for the decompression of the stomach can be connected.
