Abstract
Die Erfindung betrifft eine Platte zum Stabilisieren distaler Radiusfrakturen, umfassend einen langgestreckten Schaft(1) mit sich anschließendem distalen, anatomisch vorgeformten Plattenteil(5), wobei die Umhüllende des Plattenteils eine im wesentlichen Dreieckform aufweist, sowie im Schaft als auch im distalen Plattenteil angeordneten, mindestens am distalen Plattenende konusartig ausgeführten Gewindebohrungen(2,9) mit Gewindelängsachsen, welche im distalen Plattenteil überwiegend nicht parallel verlaufen und wobei zwischen Schaft und Plattenteil eine Kröpfung(6) ausgebildet ist. Erfindungsgemäß ist die Dreieck form des Plattenteils oder die jeweilige Umhüllende ungleichseitig, wobei die dem Schaft abgewandte Seite des Dreiecks eine Vielzahl von konischen Gewinde bohrungen aufweist. Der Bohrungsdurchmesser dieser konischen Gewindebohrungen ist kleiner als der Durchmesser der Schraubenlöcher im Schaft gewählt und die Gewindelängsachsen der Vielzahl der Bohrungen im Plattenteil schließen zur Schaftfläche einen Winkel alpha ein, welcher von 90° abweicht.
Description
Platte zum Stabilisieren distaler Radiusfrakturen
Beschreibung
Die Erfindung betrifft eine Platte zum Stabilisieren distaler Radiusfrakturen, umfassend einen langgestreckten Schaft mit sich anschließendem distalen, anatomisch vorgeformten Plattenteil, wobei die Umhüllende des Plattenteils eine im wesentlichen Dreieckform aufweist, sowie im Schaft als auch im distalen Plattenteil angeordneten Rundlöchern, die konusartig ausgeführten Gewindebohrungen sein können, mit Gewindelängsachsen, welche im distalen Plattenteil überwiegend nicht parallel verlaufen, und wobei zwischen Schaft und Plattenteil eine Kröpfung ausgebildet ist, gemäß Oberbegriff des Patentanspruchs 1.
Distale Radiusfrakturen als Brüche des handgelenknahen Anteils der Speiche, meist infolge eines Sturzes auf die ausgestreckte oder gebeugte Hand, sind die häufigsten knöchernen Verletzungen beim Menschen. Die Speiche bricht überwiegend handgelenknah mit oder ohne Beteiligung der Gelenkfläche. Schwere Verletzungen mit Frakturen der eigentlichen Gelenkfläche ergeben sich meist bei axial einwirkenden Kräften auf das Handgelenk, die im Extremfall die Speiche in mehrere Fragmente aufspalten. Solche Frakturen betreffen z.B. Patienten, die Inline-Skating, Motorradfahren oder Snowboarding als Sportart ausführen.
Die Operationsindikation ist abhängig von der Art der Fraktur und dem Ausmaß der Dislokation. Volar-Flexions- oder Smith-Frakturen sind immer instabil und eine Indikation zu einer volaren Abstützplatten-Osteosynthese. Liegt eine dorsale und/oder radiale Trümmerzone vor, besteht die Gefahr, dass auch nach Reposition die Fragmente abkippen. Auch hier besteht eine Operationsindikation. Um eine sekundäre Dislokation zu verhindern, kommen ebenfalls vorzugsweise volare winkelstabile Platten zum Einsatz.
Eine derartige winkelstabile Platte für die distale Radiusfraktur mit anatomisch vorgeformtem distalen Plattenteil gehört zum Stand der Technik und
geht z.B. zurück auf die Königsee Implantate u nd Instrumente zur Osteo- synthese GmbH. Bei der sogenannten Platte na ch Dr. Petereit mit volarer Anwendung wird das operative Repositionsergebnis durch drei winkelstabile Schrauben dauerhaft fixiert und es sind keine i ntraoperativen Korrekturen notwendig. Diese bekannte winkelstabile Platte für die distale Radiusfraktur besitzt einen langgestreckten Schaft und einen distalen Plattenteil, wobei Schaft und Plattenteil über eine Kröpfung verbunden sind. Das im wesentlichen dreieckförmige distale Plattenteil nimmt die vorerwähnten winkelstabilen Schrauben auf. Hierfür sind konische Gewindebohrungen im Plattenteil eingebracht. Diese bekannte Platte ermöglicht allerdings kein intraoperatives Nachmodellieren aufgrund ihre r sehr starren Ausführungsform.
In dem Falle, wenn eine dorsale und/oder radiale Trümmerzone vorliegt, ist es bei den Platten nach dem Stand der Technik nicht immer und nicht ohne weiteres möglich, alle Fragmente nach Repositi on zu sichern, was einen weiteren Nachteil darstellt.
Aus dem Vorgenannten ist es daher Aufgabe der Erfindung, eine weiterentwickelte neue Platte zum Stabilisieren distaler Radiusfrakturen, umfassend einen langgestreckten Schaft mit sich anschließendem distalen, anatomisch vorgeformten Plattenteil anzugeben, wobei die zu schaffende Platte grundsätzlich eine Verringerung des Operations-Trau mas ermöglicht und eine sehr hohe Funktionalität aufweist. Die Platte selbst soll vom Operateur nach- modellierbar sein und die Möglichkeit bieten, auch komplizierte Frakturen mit mehreren Fragmenten nach Reposition winkelstabil zu fixieren.
Die Lösung der Aufgabe der Erfindung erfolgt mit einer winkelstabilen Platte gemäß den Merkmalen des Patentanspruchs 1, wobei die Unteransprüche mindestens zweckmäßige Ausgestaltungen und Weiterbildungen darstellen.
Dem Grundgedanken der Erfindung folgend, ist die Dreieckform des Plattenteils oder die jeweilige Umhüllende dieses Plattenteils ungleichseitig. Die dem Schaft abgewandte Seite des Dreiecks bzw . der Umhüllenden weist eine Vielzahl von konischen Gewindebohrungen auf, wobei der Bohrungs- durchmesser dieser Gewindebohrungen wesentl ich kleiner als der Durchmesser der Gewindebohrungen im Schaft gewählt ist.
Die Gewindelängsachsen der Vielzahl der Bohrungen im distalen Plattenteil schließen zum Schaftteii überwiegend verschiedene Winkel α ein, die von 90° abweichen.
Bei einer bevorzugten Ausgestaltung weist die dem Schaft abgewandte Seite des Dreiecks eine Unterbrechung oder eine Freifläche auf, wobei Schaft und Plattenteil eine Y-Form bilden.
An den jeweiligen Seiten des die Y-Form mitbildenden Plattenteils ist bei dieser Ausgestaltung ein Querflächenstück vorgesehen, welches jeweils mindestens zwei konische Gewindebohrungen umfasst. Die Querflächenstücke können eine unterschiedliche Länge und/oder Breite aufweisen und bezogen auf den Schaftteil abgewinkelt bzw. gekröpft ausgefü hrt werden.
Schaftseitig ist neben konischen Gewindebohrungen u nd/oder zur Aufnahme von Befestigungsschrauben ein Langloch vorhanden, um durch diese Kombination sowohl das Ausrichten der Platte zu erle ichtern, als auch andererseits eine sichere Befestigung durch Verschra ubung zu gewährleisten.
Bevorzugt weist die Querschnittsfläche des Schaftes eine Wölbung auf, so dass Irritationen der Knochenoberfläche minimierbar sind.
Zum Zwecke der optimierten anatomischen Anpassung der Radiusplatte liegen die Eckpunkte des dreieckförmigen, distalen Plattenteils nicht auf einer ebenen Fläche. Denkbar ist hier eine solche Plattenstruktur, bei der die Eckpunkte des dreieckförmigen Plattenteils auf einer gekrümmten Fläche befindlich sind.
Bei einer Ausführungsform der Platte zum Stabilisieren distaler Radiusfrakturen, insbesondere bei einer volaren Platte, befinden sich die Gewindebohrungen im Plattenteil auf der dem Schaft abgewandten Seite des Dreiecks, und zwar in einer bogenförmigen Anordnung .
Der Schaft der Radiusplatte kann tailliert ausgeführt werden. Diese Taillierung ist bevorzugt im Bereich zwischen dem vorerwähnten Langloch und der sich hieran anschließenden Befestigungsbohru ng größeren Durchmessers befindlich.
Bei einer Ausführungsform der Erfindung ist d er Bohrungsdurchmesser der Gewindebohrungen im distalen Plattenteil um etwa die Hälfte kleiner gewählt als der Durchmesser der Schraubenlöcher im Schaft der Platte.
Insbesondere bei einer Ausführungsform der Radiusplatte in Y-Gestalt ist ein leichtes Nachmodellieren der Y-Schenkel und/oder der an den Schenkelenden befindlichen Querflächenstücke ausführbar.
Die vorerwähnte Y-Form bringt bei dorsaler Anwendung im Vergleich zu herkömmlichen Platten den Vorteil mit sich, d ass das Tuberkulum listeri nicht entfernt werden muss, so dass das Operations-Trauma verringert werden kann.
Gegebenenfalls vorliegende Mehrfragmentfrakturen können insbesondere durch die Wahl von winkelstabilen Schrauben mit einem Durchmesser von im wesentlichen 2 mm sehr gut fixiert werden. Zum Einsatz kommen beispielsweise vier bis acht, vorzugsweise fün f oder sechs Schrauben mit Kortikalisgewinde im Schraubenschaft.
Die Platte selbst wird aus an sich bekanntem Implantatewerkstoffen, insbesondere Implantatestahl oder Titan bzw. aus Titanlegierungen gefertigt.
Die Erfindung soll nachstehend anhand von Ausführungsbeispielen sowie unter Zuhilfenahme von Figuren näher erläutert werden.
Hierbei zeigen:
Fig. 1 verschiedene Ansichten einer Radiusplatte volar mit geschlossenem Schenkelbereich im distalen Plattenteil und
Fig. 2, 3 verschiedene Ansichten einer Radiusplatte in Y-Form mit sich hierbei ergebendem offenen Abschnitt im Bereich des distalen Plattenteils.
Wie aus den Figuren ersichtlich, weist die distale Radiusplatte jeweils einen langgestreckten Schaft 1 auf, welcher eine Anordnung von Schraubenlöchern mit oder ohne konischen Gewindebohrungen 2 in Kombination mit einem Langloch 3 besitzt. Im Übergangsbereich zwischen Langloch 3 und dem folgenden Schraubenloch 2 ist im Schaft ein symmetrischer, taillenförmiger Rücksprung 4 vorhanden.
Im Übergangsbereich vom Schaft 1 zum distalen Plattenteil 5 ist eine Kröpfung 6 vorgesehen, so dass sich die gewünschte, den anatomischen Verhältnissen entsprechende Grundstruktur ergibt.
Der Querschnitt des Schaftes 1 ist, wie aus den Figuren ersichtlich, mit einer Wölbung 7 versehen, was Irritationen der Knochenhaut zu vermeiden hilft.
Bei der Ausführungsform nach Fig . 1 wird von einer ungleichseitigen Dreiecksgestalt des Plattenteils 5 ausgegangen.
Die dem Schaft 1 abgewandte Seite des Dreiecks, d.h. die Seite 8, weist eine Vielzahl von konischen Gewindebohrungen 9 kleineren Durchmessers als derjenige Durchmesser der Schraubenlöcher 2 im Schaft 1 auf.
Der Verlauf der konischen Bohru ngen 9 kleineren Durchmessers im distalen Plattenteil 5 liegt näherungsweise auf einer Bogenlinie.
Die Gewindelängsachsen der Vielzahl der Bohrungen im Plattenteil 5 stehen in verschiedenen Winkeln α hin zum Plattenteil, wobei α einen Betrag besitzt, der von 90° abweicht. Die mit den Ziffern 1 bis 6 versehene
Bohrungsbenennung weist die i n der Figur angegebenen Winkelmaße im Bereich von 60° bis 89° auf.
Die Stirnseitenansicht der Platte nach Fig . 1 mit erkennbarem distalen Plattenende macht deutlich, da ss die Eckpunkte dieses annähernd dreieckförmigen Plattenteils nicht auf einer ebenen, sondern auf einer gekrümmten, den anatomischen Gegebenenheiten angepassten Fläche liegen. Die Darstellung des Gewindes nach Fig . 1 betrifft die Vielzahl der Gewindebohrungen im distalen Plattenteil 5, wobei durch die konische Ausführungsform sich die notwendige Winkelstabilität bei m Eindrehen einer Schraube mit komplementärem Gewinde im Schraubenkopf ergibt.
Bei der Radiusplatte nach den Fig. 2 und 3 ist ebenfalls ein Schaft 1 mit konischen Gewindebohrungen 2 und Langloch 3 sowie Taillenrücksprung 4 vorgesehen.
Die Grundform der dortigen Pla tte entspricht jedoch einem Y.
Es weist also die dem Schaft 1 abgewandte Seite des Dreiecks eine Unterbrechung oder eine Freifläche a uf.
An den jeweiligen Seiten des di e Y-Form mit bildenden Plattenteils 10 ist jeweils ein Querflächenstück 11 vorhanden, wobei die Querflächenstücke 11, wie in der Fig. 2 ersichtlich, eine unterschiedliche Länge oder aber auch Breite aufweisen.
Die Längsachsen der Querflächenstücke 11 verlaufen gemäß Darstellung nach Fig. 2 unter einem Winkel zuein ander oder entsprechen gemäß Fig. 3 annähernd einem Kreisbogen.
Die Seitenansicht auf das Y-Plattenteil 10 gemäß Fig. 2 lässt die anatomisch angepasste gekröpfte Gestalt der beiden fingerartigen Schenkel des Y- Plattenteils 10 erkennen. Auch hier weisen die Längsachsen der Gewindebohrungen überwiegend eine nicht parallele, d.h. zueinander winklige Lage auf. Die Gestalt des Y-Plattenteils 10 ermöglicht es, je nach Art der Fraktur bzw. den vorliegenden Frakturfragmenten ein geringfügiges Nachmodellieren vorzunehmen. '
Die Y-Form bringt darüber hinaus den Vorteil, dass bei dorsaler Anwendung das Tuberkulu m listeri, welches einen Knochenvorsprung darstellt, nicht entfernt werden muss.
Bei den Ausführungsformen der Erfindungen ist eine winkelstabile Ver- schraubung sowohl im Bereich des Schaftes 1 als auch im distalen Teil 5 der Platte, d.h. na he dem Gelenkspalt möglich. Mehrfragmentfrakturen können insbesondere durch die Wahl von winkelstabilen Schrauben im distalen Plattenbereich mit einem Durchmesser von im wesentlichen 2 mm sehr gut fixiert werden . Die Ausführungsform der Radiusplatte nach Fig. 2 geht von einer nicht symmetrischen Y-Plattenteil-Konfiguration aus, wobei bevorzugt der kürzere Schenkel des Plattenteils 10 zum Schaft 1 hin versetzt ist.
Es liegt im Sinne der Erfindung, die Länge der Querflächenstücke 11 und die Anzahl der in diesen jeweils eingebrachten konischen Bohrungen 9 kleineren Durchmessers zu variieren.
Die Dicke des Plattenmaterials liegt im Bereich von im wesentlichen 1 mm bis 3 mm, wobei als Material Implantatstahl, Titan oder eine Titanlegierung Anwendung findet.

Claims
1. Platte zu m Stabilisieren distaler Radiusfrakturen, umfassend einen langgestreckten Schaft mit sich anschließendem distalen, anatomisch vorge- formten Plattenteil, wobei die Umhüllende des Plattenteils eine im wesentlichen Dreieckform aufweist, sowie im Schaft als auch im distalen Plattenteil angeordneten Rundlöchern, die konusartig ausgeführte Gewindebohrungen mit Gewindelängsachsen aufweisen, welche im distalen Plattenteil überwiegend nicht parallel verlaufen, und wobei zwischen Schaft und Plattenteil eine Kröpfung ausgebildet ist, dadurch gekennzeichnet, dass die Dreieckform des Plattenteils oder die jeweilige Umhüllende ungleichseitig ist, wobei d ie dem Schaft abgewandte Seite des Dreiecks eine Vielzahl von konischen Gewindebohrungen aufweist, weiterhin deren Bohrungsdurch- messer kleiner als der Durchmesser der Gewindebohrungen oder
Schraubenlöcher im Schaft gewählt ist und die Gewindelängsachsen der Vielzahl der Bohrungen im Plattenteil zur Schaftfläche einen Winkel α einschließen , welcher von 90° abweicht.
2. Platte nach Anspruch 1, dadurch gekennzeichnet, dass im Schaft zu sätzlich ein Langloch ausgebildet ist.
3. Platte nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Querschn ϊttsfläche des Schaftes eine Wölbung aufweist.
4. Platte nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass die Eckpunkte des dreieckförmigen Plattenteils nicht auf einer ebenen Fläche liegen.
5. Platte nach Anspruch 4, dadurch gekennzeichnet, dass die Eckpunkte des dreieckförmigen Plattenteils auf einer gekrümmten Fläche liegen.
6. Platte nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass die Gewindebohrungen auf der dem Schaft abgewandten Seite des Dreiecks annähernd auf einem Kreisbogen verlaufen.
7. Platte nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass die dem Schaft abgewandte Seite des Dreiecks eine Unterbrechung oder Freifläche aufweist, wobei Schaft und Plattenteil eine Y-Form bilden.
8. Platte nach Anspruch 7, dadurch gekennzeichnet, dass an den jeweiligen Seiten des die Y-Form mit bildenden Plattenteils ein Querflächenstück vorgesehen ist, welches jeweils mindestens zwei Gewindebohrungen umfasst.
9. Platte nach Anspruch 8, dadurch gekennzeichnet, dass die Querflächenstücke eine unterschiedliche Länge aufweisen.
10. Platte nach Anspruch 8 oder 9, dadurch gekennzeichnet, dass die Längsachsen der Querflächenstücke unter einem Winkel zueinander verlaufen.
11. Platte nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass der Schaft tailliert ausgeführt ist.
12. Platte nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass der Bohrungsdurchmesser der Gewindebohrungen im Plattenteil um etwa die Hälfte kleiner als der Durchmesser der Schraubenlöcher im Schaft der Platte gewählt ist.
