Abstract

Die Erfindung betrifft eine Armlagerungsschiene für Patienten mit Armlähmung, bestehend aus einer länglichen Unterlage, auf der der Arm gelagert wird. Erfindungsgemäss besteht die Schiene aus einer 40 bis 50 cm langen und ca 25 cm breiten ebenen Fläche aus Holz, Metall oder einem anderen starren Material, die an der oberen Seite mit einer elastischen, hypoallergenen Schicht mit geringer Eindringtiefe überzogen ist. Im Endbereich der Schiene ist eine knaufförmige, zur Auflagerung einer Hand geeignete, d.h. ergonomisch gestaltete, Erhebung vorgesehen. Die Schiene ist so gelagert, dass sie sich sowohl um das nicht mit der Handauflagerung versehene Ende, d.h. um das Ende im Bereich des aufgelegten Ellenbogens, drehend als auch quer zu ihrer Längsausdehnung bewegen kann.

Beschreibung
Die Erfindung betrifft eine Armlagerungsschiene für Patienten mit einer Armlähmung, die zum Beispiel durch einen Schlaganfall entstanden sein kann.
Bei solchen Patienten kann es durch den Schlaganfall zu einer Armlähmung dergestalt kommen, dass der Arm der betroffenen Seite nicht nur inaktivierbar am Körper herabhängt, sondern dass der betroffene Arm sich darüber hinaus mit leichter Anwinkelung zum Körper hin verdreht. In diesem Erkrankungszustand ist der betroffene Arm, insbesondere bei eingeschränkter Himleistung des Patienten, weitestgehend selbst für einfache Verrichtungen nicht zu gebrauchen.
Bisher war es bekannt, zur Entlastung des Patienten den betroffenen Arm starr zu fixieren.
Diese Maßnahme allein kann allenfalls das äußere Erscheinungsbild des Betroffenen verbessern, führt jedoch eher zu einer Verschlechterung der Gebrauchbarkeit der betroffenen Gliedmaße. Liegt eine eingeschränkte Gehirnleistung des Patienten vor, ist darüberhinaus nicht zu erwarten, dass der Patient sich selbst zu therapeutischen oder rehabilitatorischen Maßnahmen veranlasst.
Bislang bekannt gewordene Anordnungen mit einem motorischen Bewegungsantrieb der an der Anordnung fixierten Gliedmaße kann zwar z.B. die Abduktions- und
Adduktionsbewegung des Oberarmes zwingend herbeiführen, jedoch wird dabei weder der
Wille des Patienten, diese Bewegung von selbst auszuführen, geschult, noch kann sich der
Reflex zu derartiger Bewegung ausbilden. Außerdem sind solche Geräte nur in Kliniken und kaum im häuslichen Gebrauch einsetztbar.
Der Erfindung lag deshalb die Aufgabe zugrunde, eine Hilfsanordnung zu schaffen, mit Hilfe derer der Betroffene nicht nur angenehm von dem am Körper herabhängenden Arm entlastet ist, sondern die ihn auf Grund ihrer ergonomischen und kinematischen Gestaltung dazu veranlasst, sich selbst rehabilitatorisch zu beüben.
Die Aufgabe wird durch eine Anordnung mit den im Anspruch 1 angegebenen Merkmalen gelöst. Zweckmäßige Ausgestaltungen der erfindungsgemäßen Lösung sind Gegenstand der Unteransprüche 2 bis 11.
Die Erfindung soll an einem Ausführungsbeispiel anhand der Figuren in den Zeichnungen erläutert werden, In den Zeichnungen zeigen:

Die Armlagerungsschiene 1 besteht aus einer 40 bis 50 cm langen und ca 25 cm breiten ebenen Fläche 2 aus Holz, Metall der einem anderen starren Material, welche mit einer elastischen Schicht 3, vorzugsweise aus einem hypoallergenen Schaumstoff mit einer Dicke von 1 ,0 bis 1 ,5 cm und mit geringer Eindringtiefe, versehen ist. Die Schiene 1 hat in der
Draufsicht vorzugsweise die Form eines halben Yin Yang-Symbols und ist im breiteren
Bereich mit einer knaufförmigen Erhebung 4 versehen.
Die Erhebung 4 befindet sich bei auf der Schiene 1 aufgelegtem (im Beispiel der Figuren rechten) Unterarm im Handbereich und ist ergonomisch der Form der Innenhand angepaßt, so dass die Hand bequem auf der Erhebung abgelegt werden kann. In der Ausführungsform nach den Figuren 1 und 2 ist auf der Unterseite der Schiene eine Schicht 5, z.B. aus
Hartgummi oder Plastik, aufgetragen, die ein Rutschen erschwert, wenn die Schiene auf einer glatten Unterlage aufgelegt ist.
Bei der Ausführungsform nach den Figuren 3 und 4 ist unter der Schiene 1 an deren bei aufgelegtem Arm dem Ellenbogen zugewandten Ende ein Tragarm 6 befestigt, der über die
Schiene hinausragt und an einer senkrechten, am Sitz des Patienten befestigten Achse 7 drehbar gelagert ist. Um nicht nur eine Drehbewegung der Schiene 1 , sondern auch eine
Verschiebung quer zu deren Längsausdehnung zu erreichen, ist nach der Ausführungsform gemäß Figuren 3 und 4 zwischen der Schienel und der Drehlagerung 8 ein Drehgelenk 9 angeordnet. Dieses gestattet die Schwenkung der Schiene 1 in der horizontalen Ebene.
Durch das Zusammenspiel von Drehlagerung 8 und Drehgelenk 9 kann die Schiene 1 quer zu deren Längsausdehnung verschoben werden.
Mit der erfmdungsgemäßen Armlagerungsschiene lassen sich einige wesentliche therapeutische Effekte erzielen, die bei der Rehabilitation von Patienten mit einer
Armlähmung von Bedeutung sind.
Der gelähmte Arm hängt durch die Auflage nicht mehr unwillkürlich am Körper des Patienten herab.
Die Schiene bietet die Möglichkeit einer entlastenden Ablagerung des gelähmten Armes sowie die Möglichkeit der Lagerung in Funktionsstellung. Das ist eine medizinisch festgelegte, nach heutigem Kenntnisstand optimale Haltung eines gelähmten Armes. Durch die Auflagerung der Hand auf dem ergonomisch gestalteten Knauff 4 kommt es zu einer Entspannung der meist in Beugekontraktur vorliegenden Muskulatur. Das vorher kontrakte Handgelenk ist, das hat der praktisch Einsatz ergeben, nach ca. 10 bis 15 Minuten passiv weich in Streckung und Beugung beweglich. Der gelähmte Unterarm mit Hand laßt sich leicht vom Patienten selbst durch den gesunden Arm am Handgelenk anfassen und in eine veränderte Positionen bringen. Den Lagerungswechsel führt der Patient schnell und selbständig durch, da er die unterschiedlichen Armpositionen als angenehm und entlastend empfindet.
Beim Auflegen des Armes auf die Lagerungsschiene 1 wird diese bei der auf eine glatte
Fläche (Tisch oder Rollstuhltablett) aufgelegten, gemäß den Figuren 1 und 2 gestalteten
Schiene allmählich vom Körper des Patienten weggleiten. Das gleiche geschieht bei der
Ausführung nach den Figuren 3 und 4. Durch die Drehlagerung 8 und das Drehgelenk 9 wird die Schiene 1 sowohl um die Drehlagerung schwenkend, als auch quer zu ihrer
Längsausdehnung vom Körper des Patienten weggleiten.
Durch das träge Weggleiten der Lagerungsschiene kommt es zu einer passiven Abduktions- bewegung des Oberarmes, der wiederum, insofern der Patient den Arm (reflektorisch) wieder an den Körper herannimmt, mit aktiver Adduktion reagiert.
Hierdurch wird, als wesentlichster therapeutischer Effekt, das Schultergelenk beweglich gehalten und gleichzeitig die Schultermuskulatur aktiviert.
Durch diese Abläufe wird der gelähmte Arm ständig auch ohne Beisein eines Dritten beübt.
Die reflektorische Reaktion auf das Weggleiten ist im oben genannten Sinne insbesondere wichtig für Patienten mit eingeschränkter Himleistung.
Die erfindungsgemäße Armlagerungsschiene ist in ihrer Flächenform und in ihren
Abmessungen, insbesondere im breiteren Teil, so gestaltet, dass deren anatomisch günstigste Stellung nach der Rückführung erreicht ist, wenn sie im Bauchbereich am Körper des Patienten anliegt.
Die Schiene eignet sich bestens für den täglichen Hausgebrauch. Sie kann für den rechten und den linken Arm und für unterschiedliche Armlängen gestaltet sein. Durch einen abnehm- und waschbaren Überzug lassen sich auch hygienische Aspekte sicherstellen.

Claims

Patentansprüche
1. Armlagerungsschiene für Patienten mit Armlähmung, bestehend aus einer länglichen Unterlage, auf der der Arm gelagert wird, dadurch gekennzeichnet, dass die Schiene (1) aus einer 40 bis 50 cm langen und ca 25 cm breiten ebenen
Fläche (2) aus Holz, Metall oder einem anderen starren Material besteht, die an der oberen Seite mit einer elastischen, hypoallergenen Schicht (3) mit geringer
Eindringtiefe überzogen ist, dass im Endbereich der Schiene (1) eine knaufformige, zur Auflagerung einer Hand geeignete, d.h. ergonomisch gestaltete, Erhebung (4) vorgesehen ist, und dass die Schiene (1) so gelagert ist, dass sie sich sowohl um das nicht mit der
Handauflagerung versehene Ende, d.h. um das Ende im Bereich des aufgelegten
Ellenbogens, drehend als auch quer zu ihrer Längsausdehnung bewegen kann.
2. Armlagerungsschiene nach Anspruch 1 , dadurch gekennzeichnet, dass unter der ebenen Fläche (2) aus Holz, Metall oder einem anderen starren
Material eine das bei auf einer ebenen Unterlage, z.B. Rollstuhltablett oder Tisch, aufgelegten
Schiene (1) auftretende Rutschen einschränkende Auflage (4) vorgesehen ist.
3. Armlagerungsschiene nach Anspruch 1 , dadurch gekennzeichnet, dass die Schiene (1) an dem nicht mit der Handauflagerung versehene Ende, d.h. an dem Ende im Bereich des aufgelegten Ellenbogens, mit einem Tragarm (6) versehen ist, der um eine senkrechte Achse (7), die aη dem Sitz des Patienten befestigt ist, schwenkbar gelagert (8) ist.
4. Armlagerungsschiene nach Anspruch 3, dadurch gekennzeichnet, dass der Tragarm (6) zwischen der Schiene (1) und der senkrechten Achse (7) durch ein Gelenk (9) unterbrochen ist, um das die Schiene in waagerechter Lage schwenken kann.
5. Armlagerungsschiene nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass die Schiene (1) in der Draufsicht dem menschlichen Körper anatomisch angepasst ist.
6. Armlagerungsschiene nach Anspruch 5, dadurch gekennzeichnet, dass die Schiene in der Draufsicht die Form eines halben Yin Yang-Symbols besitzt.
7. Armlagerungsschiene nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass die elastische Schicht (3) an der oberen Seite der Schiene aus Schaumstoff besteht.
8. Armlagerungsschiene nach Anspruch 7, dadurch gekennzeichnet, dass der Schaumstoff 1 ,0 bis 1,5 cm dick ist.
9. Armlagerungsschiene nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass sie mit einem abnehm- und waschbaren Bezug versehen ist.
10. Armlagerungsschiene nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass sie für verschiedene Armlängen des Patienten ausgebildet oder entsprechend veränderbar ist.
11. Armlagerungsschiene nach einem der vorangegangenen Ansprüche, dadurch gekennzeichnet, dass sie für die Lagerung des linken oder des rechten Armes hinsichtlich ihrer Form in der Draufsicht spiegelsymmetrisch gedreht gestaltet ist.
