Description
- La présente invention est relative à un appareil pour le traitement de l'oedème des membres par le procédé de compression du membre à traiter par immersion dans un liquide de densité très élevée.
- Le brevet français n°2.267.747 décrit un appareil consistant en une cuve cylindrique verticale contenant une poche tubulaire souple fermée à une extrémité et fixée par la périphérie de son extrémité opposée ouverte sur la périphérie de la partie supérieure de ladite cuve.
- Le membre à traiter est introduit dans la poche souple et du mercure est introduit dans la cuve à l'extrémité inférieure de celle-ci tandis qu'un gaz sous pression est introduit dans la partie supérieure de la cuve, au-dessus du mercure, ce dernier exerçant ainsi une pression contrôlée perpendiculairement et uniformément sur la totalité de la surface du membre, cette pression étant plus élevée à la partie inférieure de la cuve et décroissant vers le haut de celle-ci.
- Ce procédé et l'appareil mis au point à la suite des travaux du Docteur Cartier se sont révélés extrêmement efficaces dans le traitement de l'oedème des membres supérieurs et inférieurs.
- Cependant l'appareil ainsi conçu est trop onéreux pour trouver le développement qu'il mériterait en raison de la quantité importante de mercure nécessaire et du prix élevé que celle-ci représente pour chaque appareil.
- En effet, les tailles des membres à traiter varient d'un individu à l'autre et, pour qu'un seul et même appareil puisse être utilisé pour des malades de toutes tailles, on est conduit à donner à la cuve les plus grandes dimensions possibles, c'est-à-dire un diamètre de l'ordre de 250 à 300 mm et une hauteur d'environ 1,20 m correspondant à la longueur maximale d'une jambe.
- Il en résulte que le volume de mercure nécessaire devient très important et que le prix de l'appareil devient de ce fait pratiquement inabordable.
- Le but de l'invention est en conséquence de fournier un appareil du type précité permettant d'une part d'obtenir un résultat identique en ne nécessitant qu'une très faible quantité de mercure et d'autre part de traiter les membres supérieurs et inférieurs de patients de tailles différentes, assurant ainsi une économie considérable.
- L'invention a pour objet à cet effet un appareil pour traiter l'oedème des membres en utilisant un liquide de densité élevée dont la pression agit sur le membre à traiter par l'intermédiaire d'une poche, dans laquelle ledit membre est à introduire, cet appareil comprenant une cuve cylindrique contenant une poche tubulaire souple s'étendant à l'intérieur de la cuve et étant fermée à une extrémité et fixée par la périphérie de son extrémité opposée ouverte sur la périphérie de la partie supérieure de la cuve, caractérisé en ce que l'appareil comprend une seconde poche tubulaire souple s'étendant coaxialement dans la première poche, les poches externe et interne délimitant entre elles un faible intervalle fermé, des moyens pour introduire ledit liquide de densité élevée dans ledit intervalle lorsque le membre à traiter est introduit dans la poche interne et des moyens pour réaliser dans la cuve autour de la poche externe un moulage solide, apte à être désagrégé, du membre introduit dans la poche interne, quelle que soit la taille de ce membre, la cuve étant verticale ou inclinée.
- Suivant un mode de réalisation de l'invention, lesdits moyens pour réaliser un moulage solide apte à être désagrégé comprennent une matière granulaire fluidisable disposée dans la cuve et une chambre délimitée par une paroi supérieure perforée formant le fond de la cuve et dans laquelle un fluide en mouvement est introduit sous pression au moyen d'un ajutage pour fluidiser ladite matière.
- Suivant une caractéristique de l'invention, la cuve comporte à sa partie supérieure d'une part un premier orifice de sortie pour le fluide (gaz ou liquide) de fluidisation et d'autre part un second orifice d'échappement du trop-plein de la matière granulaire fluidisée, et des moyens pour recueillir ce trop-plein.
- De préférence, l'orifice de sortie du fluide est muni d'un filtre adapté pour retenir la matière granulaire de moulage et lesdits moyens pour recueillir le trop-plein de la matière fluidisée comprennent un récipient monté rotatif sur ledit second orifice, et orientable entre une position basse de réception du trop-plein et une position haute dans laquelle la matière recueillie se déverse en retour dans la cuve.
- D'une façon avantageuse il est prévu un dispositif de fermeture dudit second orifice pour empêcher tout mouvement intempestif de retour de la matière granulaire de la cuve vers ledit récipient.
- D'une façon avantageuse il est prévu un tuyau souple s'étendant de l'extérieur à l'intérieur de ladite seconde poche, le long de la paroi ce celle-ci, pour introduire un gaz dans cette poche.
- De préférence, ledit tuyau s'étendant dans la seconde poche est relié à une source de fluide sous pression alimentant la chambre de fluidisation par l'intermédiaire d'une vanne à deux voies commandant également l'alimentation de ladite chambre.
- La matière granulaire est un produit de moulage pulvérulent, par exemple du sable, une matière plastique sous forme de grains à granulométrie contrôlée, une résine échangeuse d'ions ou autre matière analogue fluidisable au moyen d'un fluide en mouvement.
- Le liquide à densité élevée est du mercure, une boue fluide du type des boues de forages pétroliers ou autre.
- Suivant un autre mode de réalisation, la cuve est téléscopique et lesdits moyens pour réaliser un moulage solide comprennent une matière granulaire ayant une densité inférieure à 1 disposée dans un réservoir situé au-dessous de la cuve et communiquant avec celle-ci par une ouverture pouvant être fermée par une plaque perforée, des moyens étant prévus pour envoyer un liquide ayant une densité au moins égale à 1 dans ledit réservoir.
- Suivant une autre caractéristique, la cuve télescopique comporte à sa partie supérieure un orifice de sortie de trop-plein de liquide muni d'un filtre pour retenir la matière granulaire.
- Les moyens pour introduire un liquide de densité élevée dans l'intervalle fermé entre les deux poches comprennent un tuyau s'étendant à travers la paroi de la cuve, la matière de moulage et la poche externe, solidaire de celle-ci et débouchant dans ledit intervalle.
- De préférence, l'ouverture à la partie supérieure de la cuve est fermée au moyen d'un obturateur en une matière souple et élastique, comportant une ouverture centrale, la première poche, externe, étant maintenue entre ledit obturateur et la périphérie de la cuve, et la seconde poche, interne, étant fixée sur la périphérie de ladite ouverture de l'obturateur.
- La description qui va suivre en regard du dessin annexé à titre d'exemple non limitatif permettra de bien comprendre comment l'invention peut être mise en pratique.
- La figure 1 est une vue en section verticale d'un premier mode de réalisation de l'appareil suivant l'invention.
- La figure 2 est une vue en section verticale d'un second mode de réalisation de l'appareil suivant l'invention.
- En se référant à l'exemple de la figure 1, l'appareil suivant l'invention désigné dans son ensemble par la référence 1 comprend, comme connu en soi, une cuve cylindrique verticale 2 ouverte à son extrémité supérieure et ayant une hauteur qui correspond à la longueur maximale d'une jambe, c'est-à-dire environ 1,20 mètre, cette cuve contenant une poche allongée souple 3 fermée à son extrémité inférieure et fixée par la périphérie de son extrémité opposée sur la périphérie de l'ouverture de la cuve.
- Suivant l'invention, il est prévu à l'extrémité inférieure de la cuve une chambre 4 délimitée par une paroi perforée 5 disposée au-dessus du fond 6 de la cuve et communiquant avec l'extérieur par l'intermédiaire d'un ajutage 7.
- La cuve comporte également au voisinage de son extrémité supérieure, d'une part un orifice 8 pourvu d'un filtre 9 et d'autre part un second orifice 10 auquel est fixé un tuyau 11 pourvu d'un registre 12 de fermeture commandé par une manette 13 et dont l'extrémité comporte un joint rotatif 14 auquel est relié un récipient 15, l'agencement étant tel que ce récipient peut être tourné dans une position verticale basse représentée en traits pleins au dessin et une position verticale haute représentée en traits mixtes. La forme du récipient 15 est telle que celui-ci, en position haute, constitue une trémie dont le contenu peut s'écouler à l'intérieur de la cuve 2 lorsque le registre 12 est ouvert.
- Suivant le mode de réalisation représenté à titre d'exemple, l'extrémité supérieure de la cuve 2 présente un rebord périphérique radial 16 sur lequel est adapté un organe obturateur 17 qui est de préférence constitué d'une matière souple et élastique, par exemple une matière plastique mousse à cellules fermées présentant une ouverture centrale 18, un rebord périphérique 19 coopérant avec le rebord 16 de la cuve et un bossage circulaire 20 en saillie sur la périphérie de l'ouverture 18.
- La poche souple 3 est avantageusement retenue par sa périphérie entre le rebord 16 de la cuve et le rebord 19 de l'obturateur 17 et il est prévu une seconde poche souple 21 fermée à une extrémité, disposée dans la poche 3 et délimitant avec celle-ci un faible intervalle 22, la périphérie de l'extrémité de la seconde poche 21 étant fixée sur le bossage 20.
- L'intervalle 22 entre les poches externe 3 et interne 21 communique avec l'extérieur par l'intermédiaire d'un tube 23 s'étendant à travers la paroi de la cuve 2 et dont l'extrémité est solidaire de la poche externe 3, par exemple au voisinage de son extrémité inférieure.
- Une matière granulaire 24 de moulage, fluidisable au moyen d'un fluide en mouvement est disposée dans la cuve 2 autour de la poche externe 3. Cette matière peut avantageusement être constituée par exemple par du sable, une matière plastique granulaire de granulométrie contrôlée, une résine échangeuse d'ions, ou toute autre matière appropriée, dont le niveau, à l'état non fluidisé est situé au-dessous des orifices 9 et 10.
- L'appareil comporte enfin un tuyau souple 25 s'étendant vers le bas de long de la paroi de la poche interne 21 et débouchant à l'extérieur par exemple à travers l'épaisseur de l'obturateur 17. Le tuyau 25 est relié par un conduit 26 à une vanne 27 à deux voies reliée d'une part à l'ajutage 7 de la chambre 4 et d'autre part, par un tuyau 28, à une source d'alimentation en gaz sous pression (non représentée).
- Le fonctionnement de l'appareil est le suivant.
- On envoie dans la chambre 4 par exemple de l'air sous pression qui, s'échappant à travers les perforations de la paroi 5, fluidise la matière granulaire 24 entourant la poche externe 3.
- La matière granulaire ainsi fluidisée se comporte comme un liquide, permettant l'introduction du membre à traiter dans l'ensemble constitué par les deux poches interne 21 et externe 3 et au fur et à mesure de l'introduction du membre la matière granulaire en excès s'échappe par l'orifice 10 pour remplir partiellement le récipient 15, le registre 12 étant ouvert tandis que l'air de fluidisation s'échappe à travers l'orifice 8 dont le filtre 9 retient la matière.
- La fluidisation de la matière granulaire exerçant une pression sur les poches, de l'air est injecté dans la poche interne 21 au moyen de la vanne 27 et des tuyaux 26 et 25, sous une faible pression, cet air s'échappant au fur et à mesure de la progression du membre dans la poche interne.
- Lorsque le membre est complètement introduit dans la poche 21, on ferme le registre 12 afin d'empêcher tout transfert supplémentaire de la matière granulaire dans le récipient 15 et on arrête l'injection du fluide dans la chambre 4, la matière granulaire 24 se tassant alors autour de la poche 3, réalisant ainsi un moulage solide du membre à traiter.
- On injecte alors du mercure dans l'intervalle 22 entre les poches 3 et 21, jusqu'au niveau désiré.
- Le niveau de la matière granulaire de moulage tassée, indiqué par exemple par les flèches 29 se trouvant au-dessous du niveau du mercure injecté entre les poches, il est de préférence prévu un renfort annulaire 30 par exemple en tissu inextensible sur la partie de la poche externe 3 s'étendant entre le niveau du mercure et le niveau du moulage solide réalisé afin d'empêcher une déformation de la poche externe 3 et maintenir la pression du mercure à ce niveau.
- Lorsque le traitement est terminé, le registre 12 est ouvert, le membre traité étant retiré de la poche 21, éventuellement avec une nouvelle injection d'air de fluidisation dans la chambre 4 afin de faciliter le retrait du membre et après le retrait de celui-ci le registre 12 est de nouveau ouvert et le récipient 15 tourné dans la position verticale haute afin que l'excédent de matière de moulage recueilli s'écoule de nouveau à l'intérieur de la cuve 2.
- On constate que le volume de mercure nécessaire au traitement se trouve grâce à l'invention réduit au strict minimum nécessaire. En effet, le volume de mercure utilisé résulte de la compressibilité du produit de moulage et de l'espace mort résultant de la différence des volumes occupés par la matière de moulage à l'état fluidisé et à l'état tassé.
- On constate également que l'appareil suivant l'invention permet de traiter dans les meilleures conditions économiques aussi bien les bras que les jambes des malades.
- On notera enfin que le mode de transfert de la matière de moulage de la cuve de traitement au récipient 15, et inversement, pourrait être également soit une nouvelle fluidisation, soit un transfert par flottation de la matière de moulage granulaire ou pulvérulente, ce dernier moyen permettant de supprimer le joint rotatif 14. Dans ce dernier cas le dispositif de fermeture pour isoler le récipient 15 de la cuve 2 peut être une capacité gonflable à l'eau, ou autre fluide, et contenue dans le récipient 15.
- On se référera maintenant à la figure 2 qui montre un second mode de réalisation de l'appareil suivant l'invention, dans lequel les organes similaires sont désignés par les mêmes références.
- Suivant ce mode de réalisation, la cuve verticale 31 est formée d'au moins trois sections 31 a, 31 b et 31 c montées de façon étanche et coulissant télescopiquement et est fermée à son extrémité supérieure par l'obturateur 17 qui maintient les poches souples 3 et 21, comme dans l'exemple précédent.
- L'extrémité inférieure de la section 31 c de la cuve télescopique 31 est fixée autour d'une ouverture 32 ménagée dans la paroi supérieure d'un réservoir 33.
- L'ouverture 32 peut être obturée au moyen d'une plaque perforée 34 fixée sur le piston d'un vérin 35 représenté schématiquement et monté dans le réservoir 33.
- La section supérieure 31 a de la cuve 31 comporte un orifice 11 de sortie de trop-plein muni d'un filtre et relié à un réservoir 36 rempli d'eau, par l'intermédiaire d'un tuyau téléescopique 37.
- Le réservoir 33 est relié par un tuyau 38, qui débouche au voisinage de son fond, au réservoir 36 par l'intermédiaire d'une vanne à plusieurs voies 39 et d'une pompe 40 auto-amorçable.
- Le tuyau 23 relie l'espace 22 délimité entre les deux poches souples 3 et 21 à réservoir 41. Le réservoir 41 est rempli de mercure.
- La pompe est par ailleurs reliée d'une part au réservoir d'eau 36 par un tuyau 42 et d'autre part au réservoir 41 de mercure par un tuyau 43.
- Enfin, il est prévu un tube vertical ouvert 44 qui est disposé à travers l'obturateur 17 et débouche dans l'espace 22 et dont le rôle sera expliqué dans la suite.
- Dans ce mode de réalisation, la matière granulaire présente une densité inférieure à 1 afin de flotter sur l'eau.
- Le fonctionnement de ce mode de réalisation est le suivant.
- Lorsque l'appareil n'est pas utilisé, l'ouverture 32 au bas de la cuve est ouverte et la matière granulaire est rassemblée dans le réservoir 33, autour du vérin 35.
- L'espace entre la poche externe 3 et la paroi de la cuve 31 est par conséquent vide ainsi que l'espace 22 entre les poches souples 3 et 21 et la cuve peut être rétractée télescopiquement, réduisant ainsi sa hauteur, ce qui facilite l'introduction du membre dans la poche interne 21 qui peut être tirée sur le membre à la façon d'un bas.
- Lorsque le membre est en place, on étend la cuve vers le haut dans sa position d'extension totale représentée au dessin. Le tuyau télescopique 37 accompagne le mouvement de la cuve.
- On actionne alors la pompe 40, après avoir réglé la position de la vanne 39 pour injecter l'eau du réservoir 36 dans le réservoir 33.
- L'eau entrant au fond de celui-ci fait flotter la matière granulaire et la fait monter dans la cuve 31 dans laquelle elle s'accumule à partir du haut, l'eau s'échappant à travers le filtre et revenant dans le réservoir 36 jusqu'au moment où la cuve 31 est remplie complètement jusqu'en bas.
- On actionne alors le vérin 35 pour faire monter la plaque 34 et obturer l'ouverture 32. Le moulage est alors réalisé, le réservoir 33, plein d'au, est alors vidé au moyen de la pompe et du tuyau 38.
- On modifie alors le réglage de la vanne 39, qui ferme le tuyau 38, et la pompe 40 injecte dans le réservoir 41 de l'eau exerçant une pression sur le mercure qui est injecté dans l'espace 22.
- Lorsque cet espace est rempli, on laisse le mercure monter dans le tube 44 avant d'arrêter la pompe afin d'assurer une pression autour de la partie du membre au niveau de l'extrémité supérieure de la cuve par le poids de la colonne de mercure dans le tube 44 formant évent.
- Les opérations inverses sont effectuées pour libérer le membre.
- On notera enfin que la chaleur a un effet bénéfique sur le traitement de certains oedèmes, et, en conséquence, il est prévu des moyens chauffants 46a et 46b dans les réservoirs d'eau et de mercure 36 et 41.
- En outre, le domaine d'utilisation de cet appareil ne se limite pas au seul traitement de l'oedème des membres et peut être étendu aux applications suivantes: pathologie lympatique, pathologie veine-lympatique, pathologie veineuse pure, les troubles trophiques de ces pathologies, pathologie artérielle dans certaines conditions.
- Il peut être également étendu dans les domaines sportif et para-médical pour application éventuelle en traumatologie sportive, récupération physique après effort des sportifs, application éventuelle aux oedèmes saisonniers, défatigue- ment des jambes et médecine du confort.
- Dans une variante, non représentée, la cuve 2 peut être inclinée au lieu d'être verticale, cette inclinaison permettant d'une part d'obtenir une action moins énergique du mercure et d'autre part de faciliter l'introduction du membre dans la poche interne 21.
