Teknisk område
Den foreliggende opfindelse angår en fremgangsmåde til fremstilling af intravenøst indgivelige immunogammaglobulinderivater, specielt en forbedret fremgangsmåde til fremstilling af S-sulfoneret im-05 munogammaglobulin.
Teknik, hvorpå opfindelsen bygger
Immunoglobulin omfatter antistoffer mod forskellige former for sygdomme og anvendes tii profylakse og terapi af disse sygdomme, 10 men anvendelsen af immunoglobulinpræparater har været begrænset til intramuskulær injektion. Når et immunoglobuiinpræparat injiceres intravenøst, bindes det nemlig til komplementet og sænker serum-komplementkoncentrationen og fremkalder en bivirkning kaldet anti-komplementær virkning, som resulterer i, at der iagttages blodtryks-15 fald, legemstemperaturforøgelse, forstyrrelse af kredsløbssystemet og andet. Der er derfor foretaget mange undersøgelser for at nå det mål at holde immunoglobulin stabilt ved intravenøs administrering.
Den første metode er at afskære den del, som menes at binde 20 til komplementet, fra humant immunoglobulin ved anvendelse af et enzym. Fx. har A. Nisonoff foreslået at anvende pepsin som enzym (c.f. Science 132, 1770 (1970)), og en anden metode, der benytter plasminogen i serum, cathepsin og andre materialer som enzym, beskrives i japansk patentskrift 47-37529 (1972). Disse enzymbehand-25 lede immunoglobuliner har imidlertid mangler ved, at deres halveringstid er kort, og den tid, hvor de er effektive, er følgelig kort som vist af E. Merler og B. Jager in deres artikler i hhv. Vox Sang 13, 102 (1967) og Arch. Intora. Med. 119, 60 (1967).
Den anden metode er at behandle humant immunoglobulin med 30 et proteinacyleringsreagens. Fx. er det beskrevet i japansk patentoffentliggørelsesskrift 49-6119 (1974), at acylering af humant immunoglobulin gav et modificeret immunoglobulin med reduceret antikompie-ment aktivitetsniveau. Men der er risiko for, at det ved denne fremgangsmåde dannede acylerede immunoglobulin foranlediger antigenicitet 35 i menneskelegemer, og administreringen af store mængder er blevet anset for vanskelig.
Den tredje metode er at reducere disulfid-bindingerne i humant

2 immunoglobulin efterfulgt af alkylerlng. Fx. gav denne metode ifølge japansk patentoffentliggørelsesskrift 48-103723 (1973) et modificeret immunoglobulin med samme tilsyneladende molekylvægt som det umodi-ficerede immunoglobulin og et reduceret antikomplement aktivitets-05 niveau. Denne fremgangsmåde er imidlertid en to-trins-fremgangs-mlde, og operationerne er betydeligt komplicerede og er derfor ufordelagtige industrielt.
Den fjerde metode er at S-sulfonere interkæde disulfidbindin-gerne i humant immunoglobulin med tetrathionation og sulfition til 10 frembringelse af immunoglobulinderivater, som er egnede til intravenøs injektion (c.f. de japanske patentoffentliggørelsesskrifter 50-121421 (1975), 51-1630, 51-76418 og 51-112512 (1976), og det til de to førstnævnte svarende danske fremlæggelsesskrift nr. 141.432).
Denne metode er den bedste af de kendte. Men ved denne fremgangs-15 måde anvendes ustabilt tetrathionatsalt som oxidationsmiddel. Det er derfor nødvendigt med en stor mængde af tetrathionatsaltet for at fremme den fuldstændige omsætning, hvilket ofte forårsager bireaktioner, og som en mangel der må afhjælpes, yderligere fremkalder denaturering af proteinet, si tilstrækkelig reduktion af antikomple-20 mentaktivitetsniveauet inhiberes. Desuden er tetrathionationens oxidationsevne så forholdsvis stærk, at der er mulighed for, at såvel in-trakædedisulfidbindinger som interkædedisulfidbindinger brydes, og arbejdet besværliggøres af den forsigtighed, som er nødvendig for at styre omsætningen.

Forklaring af opfindelsen
Opfinderne af den foreliggende opfindelse har rettet deres forskningsarbejde mod en afhjælpning af sådanne mangler, og de har fundet ud af, at, når trithionation, som har høj stabilitet og 30 passende reaktivitet, og sulfition anvendes sammen, spaltes interkæ-dedisuffidbindinger i humant immunogammaglobulin, og de opspaltede svovlatomer S-sulfoneres (-S-SOg") samtidig til fremstilling af immuno-gammaglobulinderivater, som er egnede til intravenøs injektion, fordi de er fri for ovenstående mangler.
35 Den foreliggende opfindelse angår således en fremgangsmåde til fremstilling af intravenøst indgivelige immunogammaglobulinderivater, ved hvilken humant immunogammaglobulin i vand omsættes med to ak- 147225 3 tive stoffer til opspaltning af næsten alle interkædedisulfidbindinger og til S-sulfonéring (S-SQg) af de siledes tilgængeliggjorte svovl-atomer, og hvor det ene stof er et, der er i stand til i vand at danne sulfitioner, hvilken fremgangsmåde er ejendommelig ved, at der 05 som det andet stof anvendes et, der i vand er i stand til at danne trithionationer.
Kort beskrivelse af tegningerne
Figur 1 (a), (b) og (c) viser mønsteret i- elektroforetisk migre-10 ring af hhv. immunogammaglobulin (IG), sulfoneret immunogammaglo-bulin (GGS) opnået ifølge eksempel 1 og et andet sulfoneret immunogammaglobulin opnået ifølge referenceeksemplet på natriumdodecylsul-fonatplader. Figur 1 (d) viser forholdet mellem hvert af båndene i ovenstående mønstre og molekylvægten.

20 Bedste udførelsesform for opfindelsen
En hvilken som helst forbindelse kan anvendes som trithionat-ionkilde, hvis den kan danne trithionationer i vand, men et alkalimetalsalt af trithionat, såsom natriumtrithionat eller kaliumtrithionat, foretrækkes.
25 En hvilken som helst forbindelse kan anvendes som sulfition- kilde, hvis den kan danne sulfition i vand, men et foretrukket eksempel er svovlsyrling, natriumsulfit, natriumbisulfit, kaliumsulfit eller natriumpyrobisulfit.
Mængden af forbindelsen, som er i stand til at danne sulfition, 30 er to gange eller mere, fortrinsvis mere end 10 gange den molære mængde af de interkædedisulfidbindinger i immunoglobulinet, som skal spaltes. Mængden af forbindelsen, som er i stand til at danne trithionation, er mere end 1 mol, fortrinsvis 2 eller flere mol per mol af de interkædedisulfidbindinger i immunoglobulinet, som skal 35 spaltes. Omsætningen gennemføres i vand, og pH holdes fortrinsvis i intervallet fra 6,0 til 10,0 under omsætningen.

Reaktionstemperaturen er 50°C eller lavere, fortrinsvis i området fra 10 til 45°C. Når temperaturen overstiger 50°C, bliver im-munogammaglobulinmolekylet uønskværdigt mere modtageligt for proteindenaturering, mens temperaturer, som er lavere end 10°C, retar-05 derer reaktionsforløbet ekstremt og dermed er industrielt uanvendelige.
Reaktionstiden, under hvilken næsten alle interkædedisulfid-bindingerne i immunogammaglobulinet spaltes og S-sulfoneres, afhænger af mængden af oxidationsmiddel og reaktionstemperaturen. Den udvælges i almindelighed i området fra \ til 24 timer.
10 I overensstemmelse med opfindelsen spaltes immunogammaglobulin i næsten alle interkædedisulfidbindingerne, og disulfidet omdannes til S-sulfonatgrupper (-S-S03~) til frembringelse af H-kæden og L-kæ-den. Omsætningsproduktet adskilles ved sædvanlige oprensningsmetoder såsom dialyse, udsaltning eller søjlekromatografi. Fx. dialyse-15 res reaktionsblandingen med normal saltvandsopløsning til frembringelse af det omhandlede stof i normal saltvandsopløsning.
Den foreliggende opfindelse illustreres yderligere i de efterfølgende eksempler.
20 Fremstilling af udgangsmaterialer, der anvendes i eksemplerne 1. Fremstilling af natriumtrithionat.
Den pågældende forbindelse opnåedes ud fra natriumthiosulfat og 30% vandig hydrogenperoxid ved en fremgangsmåde, der er beskrevet i en ny serie eksperimentel kemi ("Shin Jikken Kagaku Koza"), 25 bind 8, Syntheses of Inorganic Compounds (II), p. 482, Maruzen Tokyo.
Der blev i det væsentlige ikke iagttaget nogen ændring ved henstand ved stuetemperatur i adskillige måneder. Desuden fremkaldte opvarmning af den vandige opløsning til 45°C i 4½ timer næ-30 sten ikke nogen ændringer.
2. Fremstilling af natriumtetrathionat.
Natriumtetrathionat fremstilledes ud fra natriumthiosulfat og iod efter en metode, der er beskrevet i Handbuch der Praparativen Anor-ganischen Chemie, bind 1, side 362, Ferdinand Enke Verlag, Stutt-35 gart 1960.
Produktets renhed faldt til 85% ved henstand ved stuetemperatur i en uge, og der udvikledes svovldioxidlugt. Ydermere resulte- 147225 5 rede opvarmning af den vandige opløsning til 45°C i 4,5 timer i reduktion af renheden til under 50%.
Eksempel 1
En opløsning af humant immunogammaglobulin med en koncentra-05 tion på 16% i normal saltvandsopløsning forpufret til pH 7,5 med phosphatopløsning (5 ml) kombineredes med normal saltvandsopløsning (5 ml) indeholdende natriumsulfit (0,1616 g, 6,4 gange den molære mængde af interkædedisulfidbindingerne i immunogammaglobulin) og natriumtrithionat (0,076 g, 16 gange den molære mængde af interkæ-10 dedisulfidbindingerne) og forpufret til pH 7,5 med phosphatopløsning, og omsætning udvirkedes ved en temperatur på 45°C i 4,5 time. Efter afslutning af omsætningen dialyseredes reaktionsblandingen med normal saltvandsopløsning, indtil koncentrationen af reaktanterne blev 0,1 mmol/l eller lavere, til opnåelse af 11 ml sulfoneret immuno-15 gammeglobulinopløsning med en koncentration på 7% i normal saltvandsopløsning .
Antikomplementaktivitetsniveauet CH^q bestemtes på den 5%ige opløsning i overensstemmelse med den af Kabat og Mayer beskrevne metode (Experimental Immunochemistry, side 221, 1961) og fandtes 20 at være 14,9%.
CH5q for den 5%ige opløsning af human immunogammaglobulin, udgangsmaterialet, var 90%.
Produktets anti-diphtheria titer fandtes at være 0,8 enheder/ml, hvilket var det samme niveau som for human immunogammaglobulin, 25 udgangsmaterialet.
Produktet underkastedes desuden natriumdodecylsulfonat-plade- elektroforese i overensstemmelse med en fremgangsmåde ifølge Weber og Osborne (J. Biol. Chem. 244, 446, 1969). Resultatet er anført i figur 1 (b), som klart viser, at produktet næsten er sammensat af 30 H-kæden og L-kæden, og at interkædedisulfidbindingerne er blevet sulfoneret.
I mellemtiden gennemførtes samme omsætning under anvendelse 35 af S-mærket natriumsulfit til bekræftelse af, at der indførtes 7-8,5 mol -S-SOg grupper per mol af immunogammaglobulinet.
35 Immunoelektroforesemønsteret fandtes at være helt sammenfald ende med mønsteret af det ved anvendelse af natriumtetrathionat opnåede produkt (jvf. figur 1 (c)).

Eksempel 2
Omsætningen gennemførtes pi samme mide som i eksempel 1 bortset fra, at der anvendtes 0,015 g (3,2 gange den molære mængde af interkæde sulfidbindingerne) natriumtrithionat til dannelse af sulfone-10 ret immunogammaglobulin med antikomplement aktivitetsniveau CHgg på 15,2%.

En opløsning af humant immunogammaglobulin med en koncentra-15 tion pi 16% i phosphat-forpufret normal saltvandsopløsning (5 ml) kombineredes med en anden normal saltvandsopløsning forpufret til pH 7,5 med phosphatopløsning (5 ml) indeholdende natriumsulfit (0,1616 g, 64 gange den. molære mængde af interkæde disulfidbin-dingerne i Fmmunoglobulinet) og natriumtetrathionatdihydrat (0,098 20 g, 16 gange den molære mængde af interkæde disulfidbindingerne), og omsætning udvirkedes ved en temperatur pi 45°C i 4½ time. Efter afslutning af omsætningen dialyseredes reaktionsblandingen med normal saltvandsopløsning, indtil reaktantkoncentrationen blev lavere end 0,1 mmol/l til frembringelse af 11 ml sulfoneret immunogammaglo-25 bulinopløsning med en koncentration på 7% i normal saltvandsopløsning.
Antikomplementaktivitetsniveauet CH5q for den 5%ige opløsning fandtes at være 20,2%.
Mønsteret for natriumdodecylsulfat-pladeelektroforesen af produktet er anført i figur 1 (c), og gelfiltreringsmønsteret under an-30 vendelse af "Sephadex G-200" er vist i figur 2 (b).
Industriel anvendelighed
Ved den foreliggende opfindelse anvendes forholdsvis stabil tri-thionation og sulfition som sulfoneringsreagenser, og uønskværdig 35 dekomponering af disse reagenser kan undgås. Store mængder reagenser bliver således unødvendige, og det medfører færre bireaktioner. Den foreliggende opfindelse er følgelig fordelagtig ved at
