Description

GELENKPROTHESE
Die Neuerung betrifft eine Gelenkprothese, beispielsweise für ein Knie- oder Hüftgelenk, welche aus einem gewölbten, beispielsweise pfannenförmigen Element für die Zusammenarbeit mit einem anderen Element des Gelenks besteht, welches gewölbte Element mittels einer Zementschicht in einer Aussparung des Knochens befestigt werden kann, und das an der Zementseite mit Abstandsorganen versehen ist, die an der Außenfläche des Elements herausragen.
Eine solche Gelenkprothese, bei einem Hüftgelenk auch wohl als "acetabulumcup" bezeichnet, dient als Ersatz des natürlichen Gelenks, wozu der Knochen in geeigneter Weise ausgehöhlt wird, mit einer Zementschicht beschichtet wird, wonach des pfannenförmige Element in den noch nicht ausgehärteten Zement gepreßt wird. Die außenseitigen Abstandhalter sorgen für den richtigen Abstand zwischen dem Element und dem darunter befindlichen Knochen und sorgen ebenfalls für die endgültige Haftung mit dem Zement, nachdem dieser ausgehärtet ist.
Bekannte acetabulumcups sind unter anderem in der US-Patentschrift 4 566 138 beschrieben. Solche Cups bestehen aus einem Körper aus einem Kunststoffmaterial, das außenseitig mit einer Metallschicht beschichtet ist. Die Abstandsorgane sind durch Ultraschallverschweißung auf dieser Metallschicht befestigt. Die Abstandsorgane sind aus Akrylmaterial hergestellt, vorzugsweise Polymethylmethakrylat (PMMA), womit eine gute Haftung des Akrylzements erhalten wird.
Die Neuerung bezweckt die Verbesserung der Befestigung zwischen den Abstandhaltern und dem pfannenförmigen Element, womit die Metalldeckschicht bei Abstandsorganen aus 0 Akrylmaterial vermieden werden kann.
Die erfindungsgemäße Prothese ist dadurch gekennzeichnet, daß jedes Abstandsorgan einen zylindrischen Verankerungsteil aufweist, an den ein verjüngtes Teil anschließt, womit das pfannenförmige Element mit jeweils einer
Blendbohrung versehen ist, deren Tiefe größer als die Länge des Verankerungsteils ist und dessen Durchmesser für die Bildung einer Preßpassung mit dem Verankerungsteil geeignet ist.
Dank der Verwendung einer Preßpassung ist die Montage des Abstandhalters in dem pfannenförmigen Element äußerst einfach und preisgünstig. Wegen des Umstands, daß das Verankerungsorgan vollständig in die Blendbohrung gebracht wird, wird durch die Elastizität des Materials des pfannenförmigen Elements das Verankerungsorgan in größerem oder geringem Ausmaße umfaßt, so daß eine formschlüssige Verbindung zustande kommt.
Dieser Effekt wird dadurch verbessert, daß die Härte des Materials der Abstandsorgane größer gehalten wird als die Härte des Materials des pfannenförmigen Elements.
Die vorgenannten und anderen Kennzeichen der Neuerung werden im einzelnen in der nachfolgenden Figurenbeschreibung eines Ausführungsbeispiels dargelegt. In der Zeichnung zeigt:

In den Figuren ist mit der Ziffer 1 das pfannenförmige Element angegeben, das in der gezeigten Ausführungsform eine kugelförmige Gestalt hat, mit einem nach außen zeigenden Flansch 2 an dessen offenem Außenrand. In dem pfannenförmigen Körper ist ein Hohlraum 3 ausgespart, in den das Gegenelement des Gelenks aufgenommen werden kann. Dieses Gegenelement kann beispielsweise der kugelförmige Endteil der mit dem Oberschenkel verbundenen Hüftgelenkprothese sein, kann aber andererseits ein Knochenelement sein, das mit dem Oberschenkel an der Kniegelenksseite verbunden ist, während das gewölbte Element 1 an der Oberseite des Unterschenkels befes-
tigt ist.
Das pfannenförmige Element ist in den bestehenden Hohlraum des Knochens B aufzunehmen (siehe Figur 2), der wohl oder nicht in der geeigneten Weise ausgekratzt wird, um die richtige Abmessung zu erhalten. Zwischen das pfannenförmige Element 1 und den Schenkel wird ein Zement 4 eingefügt, der beispielsweise aus Akrylmaterial besteht.
Um den Abstand zwischen dem pfannenförmigen Element 1 und dem Knochen B auf der gesamten Außenfläche des pfannenförmigen Elements 1 gleich groß zu halten, ist das pfannenförmige Element in der bekannten Weise mit Abstandsorganen 5 versehen. Diese Abstandsorgane sind in regelmäßigem Abstand auf um die Außenfläche des pfannenförmigen Elements 1 angebracht, was in Figur 1 schematisch dargestellt worden ist.
Nach der Neuerung werden die Abstandselemente 5 in Blendbohrungen 6 des pfannenförmigen Elements 1 eingesetzt. Zu diesem Zweck ist jedes Abstandselement im wesentlichen zylindrisch ausgeführt und weist zu diesem Zweck ein Verankerungsteil 7 sowie ein daran anschließendes verjüngtes Teil 8 auf.
Das verjüngte Teil 8 ist in der gezeigten Ausführungsform das aus dem pfannenförmigen Element 1 herausragende Teil und dient als Abstandsorgan. Selbstverständlich ist im rahmender Neuerung das verjüngte Teil 8 kleiner als auszuführen, beispielsweise als Nut, die an das Verankerungsteil 7 anschließt, wobei das aus dem pfannenförmigen Element 1 herausragende Teil einen gleichen Durchmesser wie den des Verankerungsteils 7 hat. Auch andere Formen des herausragenden Teils können angewandt werden.
Nach der Neuerung ist der Durchmesser der Blendbohrung 6 derart gegenüber dem des Verankerungsteils 7 gewählt, daß eine Preßpassung möglich wird.
Dank der Maßnahme, daß die Tiefe d der Bohrung 6 größer als die Länge a des Verankerungsteils 7 (siehe Figur 3), wird die Durchmesseränderung bei A zwischen dem Verankerungsteil 7 und dem verjüngten Teil 8 unter der Oberfläche O des pfannenförmigen Elements 1 liegen.
Da die Preßpassung in einem einigermaßen flexiblen
Material des pfannenförmigen Elements 1 erfolgt, wird der Randteil 9 der Blendbohrung über den Durchmesserrand A hinweg zurückwirken, was in vergrößertem Maßstab in Figur 3 dargestellt ist.
Dieser Effekt kann noch verbessert werden, indem das Abstandsorgan in einem Polymethylmethakrylat (PMMA) ausgeführt wird, während das pfannenförmige Element in Polyethylen ausgeführt sein kann. Von diesen Werkstoffen ist die Härte des Akrylmaterials erheblich größer als die des Polyethylens, wodurch das Zurückfedern "des Materials über den Rand A hinweg" leichter erfolgt.
Experimentell ist festgestellt worden, daß die Haftung des Abstandselements 5 in dem pfannenförmigen Element in der erfindungsgemäßen Weise erheblich stärker als die bekannten Methoden ist.
Für die Erleichterung der Einführung des Verankerungsteils 7 in die Blendbohrung 6 ist es zu bevorzugen, deren Endrand bei 10 abzuschrägen.
Das pfannenförmige Element 1 kann nach Anbringen der Abstandsorgane 5 in der bekannten Weise in dem Hohlraum des Schenkels B angebracht werden, indem zunächst eine Zementschicht 4 angebracht wird, danach wird das pfannenförmige Element 1 in den Hohlraum gepreßt, wobei die Abstandsorgane 5 des Akrylmaterials an dem Zement 4 des Akrylmaterials haften.
Die Neuerung ist nicht auf die vorstehend beschriebene Ausführungsform beschränkt.

Claims (4)

SCHÜTZANSPRUCHE
1. Gelenkprothese für beispielsweise Knie-, Hüftoder Schultergelenk, welche aus einem gewölbten, beispielsweise pfannenförmigen Element für die Zusammenarbeit mit einem anderen Element des Gelenks besteht, welches gewölbte Element mittels einer Zementschicht in einer Aussparung des Knochens befestigt werden kann, und an der Zementseite mit Abstandsorganen versehen ist, die an der Außenfläche des Elements herausragen, dadurch gekennzeichnet, daß jedes Abstandsorgan einen zylindrischen Verankerungsteil aufweist, an den ein verjüngtes Teil anschließt, womit das pfannenförmige Element mit jeweils einer Blendbohrung versehen ist, deren Tiefe größer als die Länge des Verankerungsteils ist und dessen Durchmesser für die Bildung einer Preßpassung mit dem Verankerungsteil geeignet ist.
2. Prothese nach Anspruch 1, dadurch gekennzeichnet, daß die Härte des Materials der Abstandsorgane größer gehalten wird als die Härte des Materials des pfannenförmigen Elements.
3. Prothese nach Anspruch 1 oder 2, dadurch ge- kennzeichnet, daß die Abstandsorgane aus Akrylat hergestellt sind, vorzugsweise Polymethylmethakrylat (PMMA).
4. Prothese nach einem der vorigen Ansprüche, dadurch gekennzeichnet, daß das gewölbte Element vollständig aus Polyethen hergestellt ist.
