Description
- Die vorliegende Erfindung betrifft im Allgemeinen ein Verfahren zur Herstellung eines Stents für die Gefäßplastik.
- Der Begriff „Stent" soll im Allgemeinen ein Gerät bezeichnen, das für die üblicherweise durch Katheterisierung erzielte, endoluminale Anwendung (beispielsweise innerhalb eines Blutgefäßes) bestimmt ist, wobei es anschließend zur lokalen Stützung des Lumens an der richtigen Stelle eine Ausdehnung erfährt. Das Hauptziel ist die Verhinderung der Wiederherstellung einer stenotischen Stelle in der behandelten Region. Es sollte angemerkt werden, dass die Verwendung von im Wesentlichen ähnlichen Strukturen zum Bewirken einer Ausdehnung und Verankerung an der Stelle von Gefäßtransplantaten im Stand der Technik bereits vorgeschlagen wurde: Natürlich sollte diese mögliche Erweiterung des Anwendungsgebiets auch als im Umfang der Erfindung enthalten betrachtet werden.
- Hinsichtlich eines allgemeinen Überblicks über Gefäßstents kann üblicherweise auf die Arbeit „Textbook of Interventional Cardiology", herausgegeben von Eric J. Topol, W.B. Saunders Company, 1994, und insbesondere auf Abschnitt IV von Band II mit dem Titel „Coronary Stenting" Bezug genommen werden.
- Zahlreiche Patentschriften behandelten dieses Problem, wie zum Beispiel die US-A-4 776 337, die US-A-4 800 882, die US-A-4 907 336, die US-A-4 886 062, die US-A-4 830 003, die US-A-4 856 516, die US-A-4 768 507 und die US-A-4 503 569.
- Ein Problem in Zusammenhang mit der Einpflanzung eines Stents, das noch nicht vollständig gelöst wurde, steht in Bezug zu einer Restenose, die abhängig vom Typ des fraglichen Lumens mit mehr oder weniger großer Wahrscheinlichkeit auftrifft. Mehrere Studien haben gezeigt, dass der Hauptmechanismus, der nach einer Operation zur Implantation eines Stents eine Restenose bewirkt, in einer Neointimahyperplasie besteht, welche durch die Zellen der glatten Muskeln vermittelt wird.
- Es wurde jedoch beobachtet, dass eine Kernstrahlung, insbesondere β-Strahlung, die Neointimabildung hemmt. Die Herstellung eines Stents, der fähig ist, eine Kernstrahlung abzugeben, wurde daher bereits vorgeschlagen: auf diese Weise werden die umgebenden Gewebe nach der Einpflanzung bestrahlt, was die obenstehend erwähnte Hyperplasie hemmt.
- Zu diesem Zweck werden Atome des P32-Radionuklids mittels eines Zyklotrons auf die Oberfläche eines aus herkömmlichem Material, beispielsweise Edelstahl, gefertigten Stents geblasen, bevor dieser eingepflanzt wird (siehe „Radioactive Stents for the Prevention of Neointimal Hyperplasia" von Tim A. Fischell, aus „Endoluminal Stenting", Kapitel 18, S. 134 (1996), herausgegeben von W.B. Saunders Company Ltd., und die EP-A-0 593 136).
- Gemäß einer ähnlichen Technik (siehe „Technical and Engineering Aspects of Stents Which May Be Either Permanent or Removable" von R. Makkar et al., aus „Endoluminal Stenting", Kapitel 32, S. 230 (1996), herausgegeben von W.B. Saunders Company Ltd.) wird ein Titanstent mit Protonen bombardiert, welche eine Energie von 8 MeV haben und die Reaktion Ti48(p, n)V48 bewirken, die auch in diesem Fall zur Bildung eines radioaktiven Nuklids führt.
- Diese Verfahren erfordern die Verwendung äußerst komplizierter Ausrüstungsgegenstände, wie z.B. eines Zyklotrons, zur Beschleunigung der geladenen Partikel. Da diese Partikel an der Oberflächenschicht des Stentkörpers zurückgehalten werden, muss außerdem ein aufwendiges System zum Bewegen desselben bereitgestellt werden, um so viel wie möglich von seiner Oberfläche dem Partikelstrahl auszusetzen.
- Insgesamt sind diese bekannten Techniken daher für die Massenproduktion von radioaktiven Stents ungeeignet, obwohl sie für die Forschung zulässig sind.
- Die US-A-S 395 300 offenbart ein Verfahren zur Herstellung eines radioaktiven Stentkerns zur Verwendung bei der Behandlung von Tumoren innerhalb des menschlichen Körpers, indem eine dünne längliche Iridiumfaser einer Neutronenbestrahlung unterzogen wird.
- Die WO-95 30384 offenbart einen Stent mit einem zentralen Kern, der aus einem hinsichtlich einer fluoroskopischen Abbildung undurchsichtigen Material gebildet ist, z.B. aus Tantal oder Platin.
- Die EP-809 998 – die gemäß Art. 54(3) EPÜ für die üblichen benannten Staaten Stand der Technik ist – bezieht sich auf eine Endoprothese, die eine Körperstruktur einschließlich eines ersten Materials mit einer durchschnittlichen Ordnungszahl von mehr als 24 und eines zweiten Materials, das sich vom ersten unterscheidet, aufweist, und zwar mit einem Massenabsorptionskoeffizienten, der dahingehend wirksam ist, ein bestimmtes Verhältnis zwischen der Intensität eines einfallenden Röntgenstrahls und der Intensität eines übertragenen Röntgenstrahls zu schaffen.
- Die Aufgabe der vorliegenden Erfindung besteht im Überwinden der obenstehenden Nachteile, und die Erfindung besitzt jene charakteristischen Eigenschaften, auf die speziell in den nachfolgenden Ansprüchen Bezug genommen wird.
- Beim Stent des erfindungsgemäßen Verfahrens werden die Eigenschaften der beiden verschiedenen Materialien, aus denen dieser gebildet ist, am vorteilhaftesten ausgenützt: insbesondere das erste Material verleiht dem Stent die gewünschten mechanischen Eigenschaften und die gewünschte innere Festigkeit, während das zweite Material die Emission einer Kernstrahlung ermöglicht, und zwar mit den zuvor genannten Vorteilen in Zusammenhang mit der Wirkung zur Hemmung der Neointimabildung.
- Bei den herkömmlicherweise zur Stentproduktion verwendeten Materialien kann beispielsweise Edelstahl als erstes Material verwendet werden, während Tantal, Iridium und Gemische davon als zweites Material verwendet werden.
- Die Wahl des zweiten Materials kann durch Überlegungen hinsichtlich dessen Kompatibilität mit dem ersten Material, durch die Größe des gewünschten Aktivierungsbereichs und durch die charakteristischen Eigenschaften des Radionuklids, das nach dem Ausgesetztsein an den Neutronenfluss entsteht, bestimmt werden.
- Die Grundtechnik für die Herstellung dieses Stents bildet einen Faktor, der im Kontext der Erfindung an sich ohne Einfluss bleibt, welche ungeachtet dessen, ob die Stents aus einem Draht oder einer Mikroröhre gebildet sind, anwendbar ist.
- Das Erzielen der Radioaktivierung des zweiten Materials mittels Neutronen hat im Vergleich zu den bekannten Verfahren, welche die Verwendung geladener Partikel erfordern, erhebliche operative Vorteile.
- Zunächst werden die Neutronen in herkömmlichen Spaltreaktoren erzeugt, welche weiter verbreitet sind und besser verstanden werden als Teilchenbeschleuniger. Diese Reaktoren erzeugen auch äußerst hohe Neutronenflüsse (bis zu 1014n/cm2·s), so dass die Radioaktivierung in einem sehr kurzen Zeitraum erzielt werden kann.
- Zweitens werden Neutronen im Gegensatz zu geladenen Partikeln von den Materialien, die gewöhnlich für den Körper des Stents verwendet werden, nicht wirklich absorbiert. Daher kann die erfindungsgemäße Radioaktivierungsbehandlung durchgeführt werden, ohne den Stent zu drehen oder anderweitig zu bewegen, indem dieser jedoch einfach dem Neutronenfluss ausgesetzt wird, welcher sogar jene Teile des zweiten Materials aktivieren kann, die ihm nicht direkt ausgesetzt sind, nachdem er Teile des ersten Materials passiert hat.
- Weitere Vorteile und charakteristische Eigenschaften der vorliegenden Erfindung gehen aus der nachfolgenden detaillierten Beschreibung hervor, welche anhand eines nicht einschränkend gedachten Beispiels und unter Bezugnahme auf die angeschlossenen Zeichnungen dargelegt ist, wobei:

5 eine Ansicht eines Details eines Stents ist, und zwar gemäß einer weiteren Ausführungsform des erfindungsgemäßen Verfahrens. - Ein Stent (
1 ) besitzt einen Körper10 , der ein im Allgemeinen röhrenförmiges Gehäuse mit einer Wand umfasst, die eine geschlungene oder gitterartig geöffnete Struktur aufweist, wovon in2 ein Beispiel im vergrößerten Maßstab zu sehen ist. Der Körper10 ist dazu fähig, während des Gebrauchs von einer radial kontrahierten Position in eine radial ausgedehnte Position erweitert zu werden. Er umfasst (2 ) eine Stützstruktur12 , die aus einem ersten Material gefertigt ist, welches dieser Ausdehnung standhalten kann, ohne seine strukturelle Integrität zu verlieren. Die beispielsweise aus AISI-316L-Stahl gefertigte Stützstruktur12 wird aus einer Mikroröhre erhalten, die anschließend einem Vorgang zum Herausschneiden der Öffnungen unterzogen wird, wobei bekannte Verfahren zur Anwendung kommen, welche den Gegenstand einer recht umfangreichen Literaturmenge bilden und in diesem Zusammenhang daher keine detaillierte Beschreibung erfordern. - Eine Ta-Schicht
14 mit einer Dicke, die etwa 1/100 der Dicke des Körpers entspricht, welche üblicherweise zwischen 0,07 und 0,1 mm beträgt, wird auf der gesamten Außenfläche der Stützstruktur12 aufgetragen. Die Schicht14 kann beispielsweise unter Anwendung eines PVD (physikalischen Bedampfimgs)-Verfahrens, das unter Vakuum mit einem Apparat zur Sputterbeschichtung bewerkstelligt wird, oder unter Anwendung von galvanischen Techniken aufgetragen werden. - Durch das Aussetzen des beschichteten Stents einem Neutronenfluss, zum Beispiel innerhalb eines Spaltreaktors, werden daher radioaktive Nuklide erzeugt, und zwar hauptsächlich in Bezug auf die Tantalkerne gemäß der Reaktion Ta181(n,γ)Ta182.
- Ähnliche Reaktionen finden auch in Bezug auf die Fe58- und Cr50-Kerne statt, die im Stahl der Stützstruktur vorhanden sind, was zur Bildung der radioaktiven Nuklide Fe59 und Cr51 führt, wenn auch in einem geringeren Ausmaß als bei Tantal.

- Durch Anwendung dieser Gleichung wird der in
3 gezeigte Graph erhalten, welcher die Veränderung der Aktivität (ausgedrückt in beliebigen Einheiten) der Hauptradionuklide (Fe59, Cr51, Ta182), die sich im Anschluss an die Neutronenbestrahlung eines Stents des obenstehend beschriebenen Typs bilden, als Funktion der seit der Bestrahlung verstrichenen Zeit zeigt. - Es sollte angemerkt werden, dass nach mehreren Monaten die einzige signifikante Aktivität jene des Ta182 ist, welches eine β-Strahlung mit einer maximalen Energie von 0,43 MeV aussendet und eine Halbwertszeit von 112 Tagen besitzt. Diese Strahlung von mittlerer Energie und kurzer Dauer eignet sich besonders zur Hemmung einer Restenose.
- In Kenntnis der Zerfallsmuster des in
3 gezeigten Typs und des optimalen Werts für die medizinischen Anwendungen einer radioaktiven Aktivität ist es daher möglich, den günstigsten Augenblick für die Einpflanzung eines Stents, welcher der obenstehend beschriebenen Radioaktivierungsbehandlung unterzogen wurde, zu bestimmen. - Während des Wartens auf den passenden Augenblick kann der Stent anderen Verfahren unterzogen werden, wie z.B. einer Beschichtung mit einem höchst biokompatiblen Kohlenstofffilm oder, falls dies nicht zuvor geschah, einer Verpackung und Sterilisation.
- Durch Wiederholung der obenstehend beschriebenen Vorgänge, wobei die Tantalschicht jedoch durch eine 1 μm dicke Iridiumschicht ersetzt wird, wird im Anschluss an die Bildung des Radionuklids Ir192, dessen Veränderung der Aktivität im Zeitablauf durch Berücksichtigung der zugehörigen Linie in
3 bestimmt werden kann, ein radioaktiver Stent erhalten. - Ir192 emittiert ebenfalls eine β-Strahlung, wenngleich diese eine viel größere Energie (0,68 MeV) und eine kürzere Halbwertszeit (74,5 Tage) als Ta182 aufweist. Die Verwendung von Iridium ermöglicht die Erzielung einer größeren Eindringtiefe der Strahlung in einem kürzeren Zeitraum.

4 stellt einen Draht dar, der bei bekannten Techniken zur Herstellung eines Stents verwendbar ist, welcher Eigenschaften aufweist, die den obenstehend beschriebenen ähneln. Auch in diesem Fall weist er eine aus AISI-316L-Stahl gefertigte innere Stützstruktur12 auf, welche beispielsweise unter Anwendung einer Beschichtung durch Sputtern oder galvanischer Techniken mit einer Schicht14 aus Tantal oder Iridium überzogen werden kann, die anschließend durch Vorgänge, die den obenstehend beschriebenen ähneln, radioaktiviert wird. - Gemäß der Erfindung ist es auch möglich, die Positionen der Stützstruktur und der radioaktiven Struktur im Draht umzukehren, so dass letztere innen positioniert wird. Ein Draht dieses Typs kann beispielsweise unter Anwendung von Coextrusionstechniken des DFT (Drawn Fillet Tubing)-Typs gebildet werden.

5 zeigt eine weitere Ausführungsform der vorliegenden Erfindung. In diesem Fall ist die Struktur14 des zweiten radioaktivierbaren Materials in Form einer Mehrzahl von Einsätzen gefertigt, die in jeweiligen, an der Oberfläche der Stützstruktur12 gebildeten Vertiefungen untergebracht sind. Die Einsätze können unter Anwendung bekannter Techniken des Schweißens, Befestigens, Einschließens und dergleichen an der Stützstruktur angebracht werden. - Es versteht sich natürlich, dass die Herstellungsdetails in Bezug auf jene, die obenstehend beschrieben wurden, weitgehend abgewandelt werden können, ohne dadurch vom in den angeschlossenen Ansprüchen definierten Umfang der vorliegenden Erfindung abzuweichen, wobei das Prinzip der Erfindung dasselbe bleibt.
Claims (5)
- Verfahren zur Herstellung eines Stents mit einem Körper (
10 ) in Form eines im Allgemeinen röhrenförmigen Gehäuses, das dazu fähig ist, während des Gebrauchs von einer radial kontrahierten Position in eine radial ausgedehnte Position erweitert zu werden, wobei der Körper eine Stützstruktur (12 ) umfasst, die aus einem ersten Material gefertigt ist, welches dieser Ausdehnung standhalten kann, ohne seine strukturelle Integrität zu verlieren, wobei dieses Verfahren die Kombination zumindest eines Teils der Stützstruktur (12 ) mit einer aus einem zweiten, radioaktivierbaren Material gefertigten Struktur (14 ) vorsieht und das Verfahren dadurch gekennzeichnet ist, dass es das Auswählen des zweiten Materials aus Iridium öder Tantal und Gemischen davon vorsieht, sowie anschließend das Aussetzen des so gebildeten Körpers (10 ) einem Neutronenfluss, um das zweite Material radioaktiv zu machen. - Verfahren gemäß Anspruch 1, dadurch gekennzeichnet, dass die Verbindung der radioaktivierbaren Struktur (
14 ) mit der Stützstruktur (12 ) mittels galvanischer Techniken oder Beschichtung durch Sputtern erzielt wird. - Verfahren gemäß Anspruch 1, dadurch gekennzeichnet, dass die Verbindung der radioaktivierbaren Struktur (
14 ) mit der Stützstruktur (12 ) durch Coextrusion erzielt wird. - Verfahren gemäß Anspruch 1, dadurch gekennzeichnet, dass die Verbindung der radioaktivierbaren Struktur (
14 ) mit der Stützstruktur (12 ) durch Schweißen, Befestigen oder Einschließen erzielt wird. - Verfahren gemäß einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass der Körper (
10 ) mit einem Kohlenstofffilm bedeckt wird.
