Description
- Die Erfindung bezieht sich auf chirurgische Implantate, die aus bioabsorbierbarem (bioabbaubarem) Polymer, Copolymer, Polymer-Legierung oder -Verbundwerkstoff bestehen, wobei die Implantate zur Befestigung eines Knochenblocks (Transplantat) in einem Bohrloch in einem Knochen verwendet werden, und auf ein Verfahren zum Einsetzen solcher Implantate in einen Patienten.
- Es ist in der Chirurgie allgemein bekannt, ein Knochen-Patellarsehne-Knochen-Transplantat, das aus dem Knie des Patienten entnommen wird, zu verwenden, um das schwer beschädigte vordere Kreuzband (VKB) zu ersetzen. Bei einem chirurgischen Eingriff wird ein Knochentransplantat in einem Bohrloch befestigt, das vom Kniegelenk in den distalen Oberschenkelknochen gebohrt wird, und ein weiteres Knochentransplantat wird in einem Bohrloch befestigt, das in das proximale Schienbein gebohrt wird. Die Knochenstopfen werden in Bohrlöchern mit Knochenbefestigungsschrauben, in den meisten Fällen mit sogenannten Interferenzschrauben, befestigt. Eine Schraube wird im Raum zwischen dem Bohrloch und dem Knochentransplantat angeordnet, um das Knochentransplantat im Bohrloch festzuklemmen. Das Patellarsehnenteil zwischen den Knochenblöcken wirkt als neues VKB. Die chirurgische Technik des Knochen-Sehne-Knochen-Verfahrens wird zum Beispiel in Bach, B.R., Potential Pitfalls of Kurosaka Screw Interference Fixation for ACL Surgery, American Journal of Knee Surgery, Vol. 2, Nr. 2 (1989), Seiten 76-82 (Ref. 1) beschrieben.
- Die Befestigungsschrauben, wie z.B. die Interferenzschrauben, bestehen normalerweise aus Metall, wie zum Beispiel rostfreiem Stahl oder Titan, oder aus einem bioabsorbierbaren Polymer, wie zum Beispiel Polylactid. Metallische Materialien und/oder bioabsorbierbare Polymer- und Verbundmaterialien, die zur Herstellung von Knochen-Sehne-Knochen- Transplantat-Befestigungsschrauben geeignet sind, sind in der Fachliteratur beschrieben. EP 0 651 979 A offenbart eine orthopädische Befestigungsvorrichtung mit Schließnocken, die einen länglichen Körper mit einer Reihe von Gewindespitzen aufweist, die ihn teilweise in Umfangsrichtung umgeben. - Die Verwendung von Schrauben als Befestigungsimplantate für Knochentransplantate bei Knochen-Sehne-Knochen-Verfahren wird durch verschiedene Tatsachen erschwert:
- – die Gewindegänge der Schraube können den Knochenblock während des Einsetzens der Schraube in Stücke schneiden, wenn die Schraube im Verhältnis zum Knochenblock zu groß ist und/oder wenn der Raum zwischen dem Bohrloch und dem Knochenblock zu klein ist;
- – die Gewindegänge der Schraube können die Sehne während des Einsetzens der Schraube beschädigen;
- – der Knochenblock (und die Sehne) können während des Einsetzens der Schraube mit der Schraube drehen, so dass die optimale Position des Knochentransplantats verloren geht und/oder das Knochentransplantat beschädigt wird;
- – es kann eine Abweichung des Transplantats und/oder der Schraube auftreten; und
- – die bioabsorbierbare Schraube kann bei ihrem Einführen zerbrechen.
- Komplikationen wie die oben erwähnten werden in den drei angegebenen Vorveröffentlichungen erörtert.
- Daher ware es vorteilhaft, über ein Knochen-Sehne-Knochen-Befestigungsimplantat zu verfügen, das die obigen Komplikationen vermeidet, die mit dem Einführen von Schrauben in ein Bohrloch wie bei den Vorrichtungen des Stands der Technik verbunden sind. Die vorliegenden Erfindung beseitigt überraschenderweise weitgehend diese bei der Verwendung von solchen Vorrichtungen gemäß dem Stand der Technik auftretenden Probleme. Bei der vorliegenden Erfindung wird ein bioabsorbierbares Implantat aus bioabsorbierbarem Polymer, Copolymer, Polymerlegierung oder faserverstärktem oder teilchengefülltem Verbundwerkstoff hergestellt, wobei das Implantat in ein Loch oder einen Bohrkanal geschoben wird, der in einem Knochen ausgebildet ist, um ein Knochentransplantat im Bohrloch zu befestigen, wobei das Implantat aufweist: (
1 ) mindestens einen länglichen Körper, (2 ) mindestens ein Greifelement, um das Implantat im Bohrloch festzuklemmen, und (3 ) eine Auflagefläche zur Anordnung eines Knochenblocks zwischen dem Implantat und der Wand des Bohrlochs. Das Implantat kann auch (4 ) mit einem zusätzlichen Arretiermittel ausgestattet sein, um das Gleiten des Knochenblocks aus dem Bohrloch zu verhindern. - Durch Verwendung von gemäß der vorliegenden Erfindung hergestellten chirurgischen Implantaten können die oben erwähnten, bei Implantaten für Knochen-Sehne-Knochen-Befestigungen gemäß dem Stand der Tech nik auftretenden Schwierigkeiten und funktionellen Einschränkungen vermieden werden.
- Die Erfindung wird in der nachfolgenden Beschreibung unter Bezugnahme auf die beiliegenden Zeichnungen dargestellt. In den Zeichnungen zeigen

1 gezeigt, besteht ein chirurgisches Befestigungsimplantat gemäß der Erfindung aus (a) einem länglichen Körper (1 ); (b) mindestens einem Greifelement (2 ), das das Implantat im Bohrloch (3 ) festklemmt, so dass das Greifelement (die Greifelemente) während der Einführung des Implantats zumindest teilweise im Knochen (4 ) versinkt (versinken); und (c) einer Auflagefläche (5 ), auf der das Knochentransplantat (6 ) (aus dem die Sehne (7 ) vorsteht) sich zwischen dem Implantat (1 ) und der Oberfläche des Bohrlochs (3 ) befindet. Eine enge Presseinpassung des Implantats (1 ) und des Knochentransplantats (6 ) in das Bohrloch (3 ) wird erreicht, wenn die maximale Dicke des Implantats (1 ) in Kombination mit der maximalen Dicke des Knochentransplantats (6 ) größer ist als der Durchmesser des Bohrlochs (3 ). - Zusätzlich kann gemäß
2 das Implantat (1 ) weiter ein Arretiermittel (8 ) (z.B. einen Vorsprung, einen Stachel, eine Schwelle usw.) aufweisen, das vor oder im proximalen Teil der Auflagefläche (5 ) angeordnet ist. Das Arretiermittel verhindert wirksam das Gleiten des Knochentransplantats (6 ) aus dem Bohrloch (3 ), nachdem das Knochentransplantat (6 ) mit dem Implantat (1 ) in das Bohrloch gedrückt wurde, und gleichzeitig ermöglicht das Arretiermittel es der Sehne (7 ), aus dem Bohrloch (3 ) auszutreten. - Die Greifelemente gemäß der Erfindung, wie sie zum Beispiel in den
1 und2 dargestellt sind, weisen typischerweise mindestens einen Vorsprung, Gewinde, Querrippe, Stachel, pyramidenförmige Anhebung, usw. auf, die aus der Oberfläche des Implantats vorstehen. Die Geometrie der Greifelemente ist derart, dass das Implantat (1 ) leicht in das Bohrloch (3 ) gleitet, aber nicht zurückgleitet. Gemäß der in3 gezeigten, vorteilhaften Ausführungsform der Erfindung besitzen die Greifelemente Stachel (9 ), die aus der Oberfläche des Implantats (1 ) vorste hen. Die Stachel (9 ) gemäß3 verursachen nur einen geringen Widerstand, wenn das Implantat (1 ) mit dem Knochentransplantat (6 ) in das Bohrloch (3 ) geschoben wird, aber die Stachel verhindern das Zurückgleiten des Implantats (1 ) aus dem Bohrloch (3 ) nach seinem Einführen. Da das Knochentransplantat (6 ) in dem Bohrloch (3 ) in Bezug auf das Implantat (1 ) festgeklemmt ist, wird auch ein Herausgleiten des Knochentransplantats (6 ) aus dem Bohrloch (3 ) nach dem Einführen wirksam verhindert. -
4 zeigt typische Querschnitte von Knochentransplantaten (6 ) in Bezug auf ein Bohrloch (3 ) im Knochen. Wie bei Betrachtung der in4 dargestellten Geometrien ersichtlich, hat das Implantat in besonders vorteilhafter Weise eine zylindrische Form. Die Auflagefläche und das Arretiermittel werden dann auf der zylindrischen Oberfläche des Implantats ausgebildet. -
5 zeigt eine perspektivische Ansicht eines typischen Implantats (1 ) gemäß der Erfindung mit kreisförmigen Rippen als Greifelemente (2 ) um den zylindrischen Implantatkörper herum, mit einem schwellenartigen Arretiermittel (8 ) und mit einer gezahnten Auflagefläche (5 ). -
6 zeigt eine perspektivische Ansicht eines weiteren zylindrischen Implantats gemäß der Erfindung, das Stachel als Greifelemente (2 ) auf der Oberfläche des Implantats aufweist, mit einem Arretiermittel (8 ), das mit einer Nut (8' ) für die Sehne versehen ist, und mit der Auflagefläche (5 ) in den Körper des Implantats versenkt, um den Boden eines Schlitzes (10 ) zur Aufnahme eines Knochentransplantats zu bilden. -
7 ist eine Querschnittansicht der Ebene A des in6 dargestellten Implantats und stellt den Schlitz (10 ) innerhalb des Implantats (1 ), das Knochentransplantat (6 ) innerhalb des Schlitzes (10 ), das auf der Auflagefläche (5 ) angeordnet ist, und die Greifelemente (Stachel) (9 ) auf der Oberfläche des Implantats (1 ) dar. - Gemäß der vorliegenden Erfindung kann der Knochenblock vor dem Einführen in das Bohrloch im Knochen am Implantat befestigt werden, oder der Knochenblock kann zuerst in das Bohrloch eingeführt und dann das Implantat in das Bohrloch eingeführt werden. So kann das Implantat gemäß der vorliegenden Erfindung arthroskopisch in ein Bohrloch in einem Knochen entweder zusammen mit dem Knochenblock, oder nachdem der Knochenblock zuerst im Bohrloch angeordnet wurde, geschoben werden, zum Beispiel unter Verwendung einer oder einer Kombination der direkt nachfolgend aufgelisteten Techniken:
- – das Implantat wird durch eine rohrförmige Kanüle an seinen Platz im Bohrloch geschoben;
- – es wird ein Längsloch durch das Implantat hergestellt, um einen Führungsdraht aufzunehmen, und das Implantat wird entlang des Führungsdrahts an seine Stelle im Bohrloch geschoben; und
- – ein
kleines Gewindeloch (
11 ) wird im proximalen Teil des Implantats ausgebildet, wie es in Perspektive in8 dargestellt ist, wobei die Spitze (zum Beispiel bajonettförmig) eines Einsetzinstruments im Loch (11 ) befestigt und das Instrument verwendet wird, um das daran befestigte Implantat im Bohrloch in Stellung zu schieben. - In gleicher Weise kann, wenn der Knochenblock zuerst in das Bohrloch eingeführt wird, der Knochenblock in gleicher Weise wie oben beschrieben arthroskopisch in das Bohrloch geschoben werden.
- Gemäß einer vorteilhaften Ausführungsform der Erfindung besitzt das Implantat Löcher oder eine offene Porosität, um Gewebe- oder Knochenbildung innerhalb des Implantats zu erleichtern. Solche Löcher oder Poren haben typischerweise einen Durchmesser zwischen 100 μm und 2000 μm. Die Löcher oder Poren können mit spongiösem Knochen des Patienten oder mit keramischem Knochenersatzpulver oder Granulaten (z.B. Bioglas) gefüllt werden, um ihr Füllen mit neuem Knochen zu beschleunigen. Solche neuen Knochengewebe innerhalb von Löchern oder Poren des Implantats erleichtern das endgültige Heilen des Bohrlochs und die Befestigung des Knochenblocks innerhalb des Bohrlochs, wenn das Implantat biologisch abgebaut wird und aus dem Bohrloch verschwindet.
- Durch Verwendung erfindungsgemäßer Implantate ist es daher möglich, Knochentransplantate in Bohrlöchern im Knochen wirksam zu befestigen und gegen Kräfte zu blockieren, die die Tendenz haben, die Knochenimplantate zu lockern, ohne eine zeitaufwändige und riskante Befestigung mit einer Schraube durchführen zu müssen, die den Knochenblock und/oder das Sehnentransplantat beschädigen kann, das am Knochentransplantat befestigt ist.
- Erfindungsgemäße Befestigungsimplantate können aus bioabsorbierbaren (bioabbaubaren oder resorbierbaren) Polymeren, Copolymeren, Polymer-Legierungen oder -Verbundwerkstoffen, z.B. aus Poly-α-Hydroxidsäuren und anderen aliphatischen, bioabbaubaren Polyestern, Polyanhydriden, Polyorthoestern, Polyorganophosphazenen, und anderen bioabsorbierbaren Polymeren hergestellt werden, die in vielen Veröffentlichungen offenbart sind, zum Beispiel in den finnischen Patentanmeldungen FI-952884 und FI-955547, und in der PCT-Anmeldung WO 90/04982.
- Erfindungsgemäße Implantate können aus bioabbaubaren Polymeren unter Verwendung eines Polymers oder einer Polymerlegierung hergestellt werden. Die Implantate können auch verstärkt werden, indem das Material durch Fasern, die aus resorbierbarem Polymer oder Polymerle gierung hergestellt werden, oder durch bioabbaubare Glasfasern verstärkt wird, wie zum Beispiel p-Tricalciumphosphat-Fasern, Bio-Glasfasern oder CaAl-Fasern (siehe z.B. Europäische Patentanmeldung Nr. EP-A 0 146 398). Keramikpulver können auch als Additive (Füllstoffe) in den erfindungsgemäßen Implantaten verwendet werden, um die Knochenbildung zu fördern.
- Erfindungsgemäße Implantate können auch geschichtete Teile enthalten, die zum Beispiel (a) eine flexible Oberflächenschicht, welche die Belastbarkeit des Implantats verbessert und/oder Medikamente oder andere bioaktive Substanzen freisetzt und/oder als Hydrolysesperre wirkt, und (b) eine steife Innenschicht aufweisen.
- Chirurgische Implantate gemäß der Erfindung können aus bioabbaubaren Polymeren und aus geeigneten bioabbaubaren Verstärkungsfasern mit Hilfe verschiedener Verfahren hergestellt werden, die in der Kunststofftechnologie angewendet werden, wie zum Beispiel Spritzformen, Extrudieren und Faserung und damit verbundenes Formen (siehe zum Beispiel US Patent Nr. 4,968,317), oder mit Hilfe von Druckformen, wobei die Implantatteile durch Anwendung von Wärme und/oder Druck aus dem Rohmaterial geformt werden. Auch eine mechanische Bearbeitung (z.B. Schneiden, Bohren, Drehen, Schleifen usw.) kann in diesem Zusammenhang verwendet werden.
- Es ist auch möglich, erfindungsgemäße Implantate unter Verwendung der oben erwähnten polymerischen Rohmaterialien herzustellen und zumindest einen Teil des Polymers in einem geeigneten Lösungsmittel aufzulösen, oder das Polymer mit Hilfe des Lösungsmittels aufzuweichen, und dann das Polymer unter Anwendung von Druck und/oder leichter Wärme in ein Implantatteil einzudrücken, wobei das aufgelöste oder auf geweichte Polymer geklebt wird, um ein makroskopisches Implantatteil zu bilden, aus dem das Lösungsmittel durch Verdunstung entfernt wird.
- Selbstverständlich können die erfindungsgemäßen Implantate auch verschiedene Additive zur Erleichterung der Verarbeitbarkeit des Materials (z.B. Stabilisatoren, Antioxidantien oder Weichmacher) oder zur Veränderung seiner Eigenschaften (z.B. Weichmacher oder Keramikpulvermaterialien oder biostabile Fasern, wie z.B. Kohlenstofffasern) oder zur Vereinfachung seiner Bearbeitung (z.B. Färbemittel) enthalten.
- Gemäß einer vorteilhaften Ausführungsform der Erfindung enthält das Implantat (und/oder seine Oberflächenschicht) einen oder mehrere bioaktive Wirkstoffe, wie zum Beispiel Antibiotika, chemotherapeutische Wirkstoffe, die Wundheilung aktivierende Wirkstoffe, Wachstumsförderer, knochen-morphogenetische Proteine, Gerinnungshemmer (wie z.B. Heparin), usw. Solche bioaktiven Implantate sind besonders vorteilhaft bei der klinischen Anwendung, da sie zusätzlich zu ihrer mechanischen Wirkung auch biochemische, medizinische und andere Wirkungen in verschiedenen Geweben haben.
Claims (13)
- Chirurgisches Implantat zur Fixierung eines Knochenblocks in einem Bohrloch (
3 ) in einem Knochen, wobei das Implantat mindestens einen länglichen Körper (1 ), mindestens ein Greifelement (2 ) auf dem länglichen Körper (1 ) zum Festklemmen des Implantats im Bohrloch (3 ), und eine Auflagefläche (5 ) auf dem länglichen Körper (1 ) zur Aufnahme des Knochenblocks aufweist, dadurch gekennzeichnet, dass der längliche Körper (1 ) aus bioabsorbierbarem Polymer, Copolymer, Polymer-Legierung oder -Verbundwerkstoff hergestellt ist, und dass die Auflagefläche (5 ) in den länglichen Körper (1 ) versenkt ist, um einen Schlitz zu bilden. - Chirurgisches Implantat nach Anspruch 1, wobei das mindestens eine Greifelement (
2 ) mindestens einen Vorsprung aufweist. - Chirurgisches Implantat nach Anspruch 1, wobei das mindestens eine Greifelement (
2 ) mindestens ein Gewinde aufweist. - Chirurgisches Implantat nach Anspruch 1, wobei das mindestens eine Greifelement (
2 ) mindestens eine Querrippe aufweist. - Chirurgisches Implantat nach Anspruch 1, wobei das mindestens eine Greifelement (
2 ) mindestens einen Stachel (9 ) aufweist. - Chirurgisches Implantat nach Anspruch 1, das weiter Arretiermittel (
8 ) aufweist, um das Gleiten des Knochenblocks aus dem Bohrloch (3 ) zu verhindern. - Chirurgisches Implantat nach Anspruch 1, wobei das Arretiermittel (
8 ) mindestens einen Vorsprung aufweist. - Chirurgisches Implantat nach Anspruch 1, wobei das Arretiermittel (
8 ) mindestens einen Stachel aufweist. - Chirurgisches Implantat nach Anspruch 1, wobei das Arretiermittel (
8 ) mindestens eine Schwelle aufweist. - Chirurgisches Implantat nach einem der vorhergehenden Ansprüche, wobei das Arretiermittel (
8 ) weiter eine Nut (8' ) aufweist. - Chirurgisches Implantat nach Anspruch 1, wobei der längliche Körper (
1 ) von zylindrischer Form ist. - Chirurgisches Implantat nach Anspruch 1, wobei der längliche Körper (
1 ) porös ist. - Chirurgisches Implantat nach Anspruch 1, wobei das Implantat in der Lage ist, ein Medikament oder eine andere bioaktive Substanz freizusetzen.
