Description
- Wenn menschliche Zwischenwirbelscheiben in einer von einer Vielzahl von Weisen degenerieren, können sie in einer Vielzahl von Weisen behandelt werden.
- Ein Verfahren zur Behandlung der Degenerierung der Scheiben ist ein chemisches und wird gewöhnlich als Chemonukleolyse bezeichnet. Bei diesem Verfahren wird/werden Chymopapain oder anderes) Enzyme) in die degenerierte Scheibe eingespritzt, so daß Gewebe aufgelöst wird. Ein anderes Verfahren zur Behandlung der Degenerierung von Scheiben ist die Diskektomie, bei der etwas Scheibenmaterial entfernt wird. Auch ist ein weiteres Verfahren zur Behandlung der Degenerierung von Scheiben die Fusion.
- Bei einem alternativen Verfahren zur Behandlung der Degenerierung von Scheiben wurden Implantate aus Polymerhydrogel entwickelt. Diese Implantate sind vollständig in dem USA-Patent Nr. 5,047,055 von Bao offenbart, das durch Verweis darauf hier einbegriffen ist.
- Um solche aus Hydrogelen bestehenden Implantate am Wirbelsäulenkern implantieren zu können, muß zuerst etwas Wirbelsäulenscheibenmaterial entfernt werden. Danach werden dann das Scheibenimplantat oder die Implantate eingesetzt.
- Ein solches Verfahren zum Einsetzen dieser Scheiben ist beschrieben in dem USA-Patent Nr. 5,562,736, abgetreten an Raymedica. Bei diesem Verfahren wird ein Einschnitt in die Wirbelscheibe ausgeführt, so daß ein Lappen zurückbleibt; dann wird das Implantat eingesetzt, und der Lappen des Anulus wird wieder angenäht. Das kann in Bezug auf die Wirbelsäule des Patienten von hinten her erfolgen.
- Es wurde nach einem alternativen Verfahren zum Einsetzen eines synthetischen Wirbelsäulenkernimplantats gesucht.
- Der Erfindung liegen die Aufgaben zugrunde, ein Verfahren und eine Vorrichtung zum Einsetzen eines Prothesen-Rückenmarkskernimplantats zu schaffen, das im Operationssaal bequem, schnell und sicher verwendet werden kann.
- Weitere Aufgaben der Erfindung sind ein Verfahren und eine Vorrichtung, die bei verschiedenen chirurgischen Vorgehensweisen gegenüber dem Patienten verwendet werden können, und die bewirken, daß sich der Patient rasch erholt, und daß die Gefahr des Abstoßens des Implantats vermindert wird, da nur ein kleiner Einschnitt in die Scheibe vorgenommen wird.
- Weitere Aufgaben der Erfindung sind ein Verfahren und eine Vorrichtung, die mit den chirurgischen Instrumenten, die gewöhnlich bei der Diskektomiechirurgie verwendet werden, kompatibel sind.
- Weitere Aufgaben der Erfindung sind ein Verfahren und eine Vorrichtung zum schnellen Einsetzen eines elastischen Scheibenimplantats, beispielsweise des Implantats, das in dem USA-Patent Nr. 5,047,055 von Bao offenbart ist, durch eine sehr kleine Öffnung des Anulus hindurch, so daß sich das Implantat nicht dauerhaft verformt.
- Zusammenfassung der Erfindung
- Diese und weitere Aufgaben werden durch die Einsetzvorrichtung zum Einsetzen eines elastischen Prothesen-Rückenmarkskerns in einen Zwischenwirbelraum nach der Definition in dem anliegenden Anspruch 1 erfüllt.

1 ist eine Querschnittsdarstellung einer sich verjüngenden Kanüle gemäß der Erfindung, in der sich innerhalb oder nahe an der sich verjüngenden Kanüle ein aus elastischem Material bestehender Prothesen-Rückenmarkskern befindet und die sich verjüngende Kanüle auch ein kraftübertragendes Element enthält, das als Prothesenkern wirkt und selbst durch einen (schematisch dargestellten) Krafterzeuger betätigt wird. -

- In den Figuren ist in
1 eine (allgemein mit10 bezeichnete) Einsetzvorrichtung zum Einsetzen eines elastischen Prothesen-Rückenmarkskerns12 in einen Zwischenwirbelscheibenraum einer menschlichen Wirbelsäule zum Teil im Querschnitt und zum Teil schematisch gezeigt und umfaßt eine sich verjüngende Kanüle14 und ein kraftübertragendes Element16 , das auf den innerhalb der sich verjüngende Kanüle14 befindlichen elastischen Prothesen-Rückenmarkskern12 wirkt, so daß der elastische Prothesen-Rückenmarkskern12 in einem sehr kurzen Zeitraum aus der sich verjüngenden Kanüle14 ausgespritzt wird; was weiter unten beschrieben wird. Das in1 bei einer Ausführungsform gezeigte kraftübertragende Element16 ist ein Kolben18 , von dem ein Teil auch in3 gezeigt ist. Bei einer bevorzugten Ausführungsform wird auch ein Dichtungselement20 verwendet, das eine Kombination von einem Ballon22 und von O-Ringen ist. - Der Kolben
18 besteht aus einem (in3A ,3B und3C vergrößert gezeigten) Abschnitt26 und einem zweiten Abschnitt28 . - Im Innern einer Leitung
40 befindet sich ein inkompressibles Medium42 , das bei einer bevorzugten Ausführungsform ein biokompatibles Fluid ist, beispielsweise Salzlösung. Die Leitung40 verbindet einen Krafterzeuger44 mit dem kraftübertragenden Element16 und weist bei einer bevorzugten Ausführungsform ein Ventil46 auf, das ein- und ausgeschaltet werden kann. -

14 weist vorzugsweise eine Form auf, die zylindrisch ist, jedoch kann die Querschnittsform der sich verjüngenden Kanüle14 bei Bedarf jede beliebige Form besitzen. Die sich verjüngende Kanüle14 weist eine gerade Zone48 auf, bei dem ein Ende zu dem in2 gezeigten Befestigungsmittel36 benachbart angeordnet ist. Der Querschnittsbereich der geraden Zone48 kommt dem Querschnittsbereich des elastischen Prothesen-Rückenmarkskerns12 nahe, der in die sich verjüngende Kanüle14 eingesetzt werden soll. Die sich verjüngende Kanüle14 weist eine sich verjüngende Zone (oder sich verjüngenden Abschnitt)34 auf, der sich zu einer proximalen Öffnung50 hin verjüngt. Das Verhältnis des Querschnittsbereichs der geraden Zone48 , geteilt durch den Querschnittsbereich der proximalen Öffnung50 , ist ein Verhältnis innerhalb des Bereichs von etwa 3 bis etwa 8. - Bei einer bevorzugten Ausführungsform ähnelt die sich verjüngende Kanüle
14 einer Blaspistole (wie in2 gezeigt). - Wie in
1 gezeigt, wirkt das kraftübertragende Element16 (das ein erforderliches Element der Vorrichtung gemäß der Erfindung ist) zuerst an dem distalen Ende13 des elastischen Prothesen-Rückenmarkskerns12 , wenn der Kern12 innerhalb der geraden Zone48 der die sich verjüngenden Kanüle14 positioniert ist. Das kraftübertragende Element16 bewirkt, daß sich der Kern12 zuerst in den sich verjüngenden Bereich34 hinein und dann aus der proximalen Öffnung50 heraus bewegt. Die von dem kraftübertragenden Element16 übertragene Kraft verändert sich direkt mit dem Verhältnis zwischen dem Querschnittsbereich der geraden Zone48 und dem Querschnittsbereich der proximalen Öffnung50 , so daß durch die Geschwindigkeit, mit welcher der Kern12 durch die proximale Öffnung50 hindurchgeht, eine dauerhafte Verformung des elastischen Prothesen-Rückenmarkskerns12 vermieden wird. - Das kraftübertragende Element
16 kann ein inkompressibles biokompatibles Medium42 sein, das die sich verjüngende Kanüle14 unter Druck setzt. Bei einer bevorzugten Ausführungsform ist das inkompressible biokompatible Medium42 eine Salzlösung. - Alternativ kann das kraftübertragende Element
16 ein Kolben (der ein Gleitstück, das sich mittels Kraft bewegt) sein, beispielsweise ein Tauchkolben oder Druckkolben. - Alternativ kann das kraftübertragende Element
16 eine Kombination von einem Kolben und eines inkompressiblen biokompatiblen Fluids42 sein. - Zum Antreiben des kraftübertragenden Elements
16 wird ein Krafterzeuger44 verwendet. - Der Krafterzeuger
44 kann jede beliebige Art eines mechanischen (manuellen oder kraftunterstützten), gasförmigen oder hydraulischen Krafterzeugers sein, der ausreicht, um eine Geschwindigkeit für das in den Hohlraum der Scheibe zu injizierende Implantat12 innerhalb eines Zeitraums innerhalb des Bereichs von etwa 10 Millisekunden bis etwa 5 Sekunden zu liefern. - Beispielweise kann der Krafterzeuger
44 Druckluft sein, die verwendet werden kann, um das inkompressible biokompatible Medium42 (die Flüssigkeit) durch einen Kolben zu treiben. Jedoch kann Luft aus dem Grunde nicht als kraftübertragendes Element16 verwendet werden, daß das kraftübertragende Element16 ausschaltbar oder anhaltbar sein muß, nachdem das Implantat injiziert ist. Das ist ein erforderliches Merkmal des kraftübertragenden Elementes16 . - Bei einer Ausführungsform, bei der das kraftübertragende Element
16 ein inkompressibles biokompatibles Medium42 ist, das die sich verjüngende Kanüle14 unter Druck setzt, sollte das fluide Medium42 mit einem Krafterzeuger44 (einer Fluidinjektionsvorrichtung) eingeführt werden, der/die in weniger als 5 Sekunden zwischen etwa 3 und etwa 10 cm3 des Fluids injiziert. Wenn durch den Kolben Fluid in die Kanüle eingeführt wird, wird hoher Hydraulikdruck (etwa 3 bis etwa 10 MPa) in dem Raum erzeugt, der durch das elastische Kernimplantat und den Kolben mit dem Abdichtungselement abgedichtet wird. Um den Kolben mit der Kanüle zu verriegeln, damit sich der Kolben nicht rückwärts bewegt, ist ein Kopplungsmechanismus vonnöten. Durch diesen Hydraulikdruck wird eine hohe Schubkraft geschaffen, durch die das elastische Kernimplantat aus der Kanüle ausgespritzt wird. - Der Krafterzeuger
44 wird vorzugsweise verwendet, um ein inkompressibles biokompatibles Medium42 durch das kraftübertragende Element16 zu injizieren. Bei einer Ausführungsform kann der Krafterzeuger44 eine manuell betätigte Kolbenpumpe sein, die ausreicht, damit das Implantat12 eine Geschwindigkeit innerhalb des Bereichs von etwa 10 Millisekunden bis etwa 1 Sekunde erreicht. Eine andere geeignete Art eines Krafterzeugers44 ist eine kraftunterstützte Pumpe zum Antreiben des Kolbens18 oder Druckkolbens. Das kann beispielsweise ein Kugelspindelsteuerelement sein. - Eine weitere geeignete Art eines Krafterzeugers
44 , die verwendet werden kann, ist eine Metallfeder, um die Tätigkeit eines Druckkolbens zu erzwingen, der dann auf das inkompressible Medium42 einwirkt. - Ein Fluidinjektionsvolumen, das hoch ist, weist auf Grund des Stroms der sehr schnellen Flüssigkeit einen ballistischen Effekt auf. Eine kleinere Flüssigkeitsmenge würde nicht ausreichen, um das Implantat aus der Kanüle heraus zu drücken. Bei einer typischen Anwendung beträgt das Flüssigkeitsinjektionsvolumen zwischen etwa 3 und etwa 10 Kubikzentimeter.
- Bei der Umsetzung der Erfindung muß ein Dichtungselement
20 als Teil des kraftübertragenden Elementes16 verwendet werden, so daß sich ein ausreichender Druck aufbaut und dann auf den elastischen Prothesen-Rückenmarkskern12 ausgeübt wird. Das Dichtungselement20 kann ein Ballon22 sein, oder es können O-Ringe 24 oder ein Stopfen oder eine Gummikappe21 oder eine Kombination von einem Ballon22 und von O-Ringen24 sein. Ein sehr wichtiges Erfordernis bei der Umsetzung der Erfindung ist, daß alle Luft aus dem System entfernbar sein (d. h. das System vollfüllbar sein) muß. - Bei der Umsetzung der Erfindung ist es wichtig, daß die Geschwindigkeit, mit welcher der elastische Prothesen-Rückenmarkskern
12 durch die proximale Öffnung50 geht, derart ist, daß der elastische Prothesen-Rückenmarkskern12 sich nicht dauerhaft verformt. Bei den (weiter unten beschriebenen) elastischen Materialien, die sich für die Praxis der Erfindung eignen, liegt die Geschwindigkeit, mit der elastische Prothesen-Rückenmarkskern12 durch die proximale Öffnung50 geht, generell im Bereich innerhalb des Bereichs von etwa 10 Millisekunden bis etwa 5 Sekunden. - Es ist wichtig, daß sich das Material, aus dem der elastische Prothesen-Rückenmarkskern
12 ausgebildet ist, daran erinnert, wie seine ursprüngliche Form aussah. Bei der Umsetzung der Erfindung besteht der elastische Prothesen-Rückenmarkskern12 aus einem Material, das nach der Verformung seine Größe und Form zurückgewinnt. Das Material zum Ausbilden des elastischen Implantats12 wird derart gewählt, daß das Implantat12 nur einen kleinen Verformungsbetrag aufweist, vorzugsweise von weniger als etwa 10 Prozent. - Ein Beispiel für ein geeignetes elastisches Material zur Verwendung bei der Umsetzung der Erfindung ist ein Elastomer nach der Definition in dem USA-Patent Nr. 5,047,055 (Bao), jedoch kann das Elastomer Material zur Verwendung bei der Umsetzung der Erfindung jedes geeignete Hydrogel oder Nichthydrogel oder eine Mischung derselben sein.
- Der Druck oder die Kraft, der/die erforderlich ist, um den elastischen Prothesen-Rückenmarkskern
12 durch die sich verjüngende Kanüle14 zu drücken, muß derart sein, daß das Implantat12 die bevorzugte Geschwindigkeit in der oben beschriebenen Weise erreicht. Deshalb sollte dieser Druck bei einem Implantat der geeigneten Größe, die zwischen 1,5 cm3 und etwa 5 cm3 liegt, größer als etwa 3 MPa sein. Jedoch muß bei dem zu verwendenden Maximaldruck auch die Festigkeit der Kanüle in Betracht gezogen werden. - Ein bevorzugtes Hydrogelmaterial zur Verwendung bei der Umsetzung der Erfindung ist offenbart in Bao, USA-Patent Nr. 5,047,055 und besitzt einen Wassergehalt innerhalb des Bereichs von etwa 30 bis etwa 90 Gew.-%. Am meisten bevorzugt wird ein Hydrogel mit einem Wassergehalt innerhalb des Bereichs von etwa 60 bis etwa 70 Gew.-% verwendet.
- Es werden generell ein oder mehrere Stücke des Materials gemäß der Erfindung unter der Voraussetzung verwendet, daß die Stücke zusammen die allgemeine Form des Hohlraums in der Scheibe bilden, bevor das Material komprimiert wird.
- Die Menge und die Art des bei der Umsetzung der Erfindung zu verwendenden Materials werden derart gewählt, daß das daraus gefertigte Implantat durch eine kleine Öffnung in dem Anulus hindurch, die einen Durchmesser innerhalb des Bereichs von etwa 2 bis etwa 6 mm aufweist, in die Scheibe eingedrückt werden kann. Dieser Durchmesser ist von etwa der gleichen Größe wie das Loch in der Scheibe, in die das Implantat eingesetzt werden soll.
- Im allgemeinen muß dann, wenn das Formgedächtnis des Materials, das zur Ausbildung des elastischen Prothesen-Rückenmarkskerns
12 verwendet wird, schlecht ist oder die Steifheit des Materials hoch ist, das Verhältnis des Querschnittsbereichs der geraden Zone48 zu dem Querschnittsbereich der proximalen Öffnung50 verkleinert werden, oder die Einsetzgeschwindigkeit für das Implantat muß durch Anwendung einer größeren Kraft erhöht werden. Wenn dagegen das Formgedächtnis des Materials gut ist oder die Steifheit des Materials geringer ist, kann das Verhältnis des Querschnittsbereichs (wie oben beschrieben) vergrößert werden, oder die Einsetzgeschwindigkeit für das Implantat12 kann durch Anwendung einer größeren Kraft vermindert werden. - Die Innenseite der sich verjüngenden Kanüle
14 sollte stark poliert sein oder kann bei Bedarf beschichtet werden, so daß die Reibungskraft zwischen dem Implantat12 und der sich verjüngenden Kanüle14 vermindert wird, und so daß der Betrag des zum Eindrücken des Implantats12 in die Wirbelsäulenscheibe erforderlichen Drucks vermindert wird. Die Beschichtung kann je nach Bedarf entweder metallisch oder nichtmetallisch sein. Die sich verjüngende Kanüle14 muß ausreichend fest sein, um so den Druck oder die Kraft auszuhalten, der/die zum Eindrücken des Implantats12 in die Wirbelsäulenscheibe verwendet wird. - Beispiel 1
- Es wurde eine sich verjüngenden Kanüle mit einem Durchmesser von 11,0 mm (Querschnittsbereich A = 95 mm2) im Abschnitt der geraden Zone und mit einer Verjüngung bis hin zu einer Öffnung mit einem Durchmesser von 5,0 mm (Querschnittsbereich a = 19,6 mm2) verwendet, um vier Hydrogelkernimplantate von verschiedener Größe, jedoch mit dem gleichen Wassergehalt von 70% (und mithin dem gleichen Kompressionsmodul oder der gleichen Steifheit) und aus dem gleichen Material bestehend, das Polyvinylalkohol war, und mit der gleichen Form, die zylindrisch war, zu injizieren. Der Anulus wies ein Verhältnis A/a von 4,8 auf. Die Innenseite der Kanüle war mit Dicronite® (einer Art von Wolframdisulfid) beschichtet, um die Reibungskraft zu vermindern. Die Injektionsgeschwindigkeit betrug etwa 0,5 Sekunden und war bei allen diesen vier Injektionen die gleiche. Alle Variablen wurden mit Ausnahme der Größe des Implantats und des zum Injizieren eines Implantats mit der gegebenen Injektionsgeschwindigkeit von 0,5 Sekunden benötigten Spitzendrucks konstant gehalten. Dieser Druck nahm zu, wenn die Größe des Implantats zunahm, wie im folgenden zusammengefaßt ist.
- Beispiel 2
- Dieses Beispiel zeigt, daß sich der Spitzendruck, wenn das Verhältnis A/a höher wurde, für das Implantat der gleichen Größe höher wurde. Es wurden zwei Kanülen, die beide einen Durchmesser der geraden Zone von 11 mm, jedoch verschiedene sich verjüngende Öffnungsdurchmesser von 5,0 mm und 4,6 mm aufwiesen, zum Injizieren von Hydrogelkernimplantaten der gleichen Größe verwendet. Beide Kanülen waren mit Dicronite® beschichtet, und die Verhältnisse A/a betrugen 4,8 bei der größeren sich verjüngenden Öffnung bzw. 5,7 bei der kleineren sich verjüngenden Öffnung. Die zwei Kernimplantate wogen 2,36 g, wiesen den gleichen Wassergehalt von 70 w/o auf und besaßen die gleiche Form, die zylindrisch war.
- Beispiel 3
- Um zu untersuchen, wie durch die Art der Beschichtung die Reibungskraft zwischen dem elastischen Implantat und der Innenseite der Kanüle vermindert wird, wurden zwei im wesentlichen identische Kanülen verwendet. Eine Kanüle war mit Dicronite® beschichtet, und die andere war unbeschichtet. Die Implantate bestanden aus Polyvinylalkohol und wiesen die gleiche Form und Größe und gleichen Wassergehalt auf.
Claims (9)
- Einführungsgerät für das Einführen eines elastischen Prothesen-Rückenmarkskerns (
12 ) in einen Bandscheibenzwischenraum, wobei das Gerät Folgendes umfasst: (a) eine sich verjüngende Kanüle (14 ), die eine gerade Zone (48 ) und eine sich verjüngende Zone (34 ) aufweist, die sich von der geraden Zone (48 ) aus auf eine proximale Öffnung (50 ) zu verjüngt, wobei der Querschnittsbereich der geraden Zone (48 ) geteilt durch den Querschnittsbereich der proximalen Öffnung (50 ) im Verhältnis von 3 zu 8 liegt und (b) ein kraftübertragendes Element (16 ), dadurch gekennzeichnet, dass die durch das kraftübertragende Element (16 ) übertragene Kraft sich direkt mit dem Verhältnis des Querschnittsbereichs der geraden Zone (48 ) zu dem Querschnittsbereich der proximalen Öffnung (50 ) ändert. - Gerät nach Anspruch 1, wobei das kraftübertragende Element (
16 ) ein inkompressibles biokompatibles Medium (42 ) ist, das die sich verjüngende Kanüle (14 ) unter Druck setzt. - Gerät nach Anspruch 2, wobei es sich bei dem Medium (
42 ) um physiologische Kochsalzlösung handelt. - Gerät nach Anspruch 1, wobei das kraftübertragende Element (
16 ) ein Kolben ist. - Gerät nach Anspruch 1, wobei das kraftübertragende Element (
16 ) eine Kombination eines inkompressiblen biokompatiblen Mediums (42 ) und eines Kolben (18 ) ist. - Gerät nach Anspruch 2, das auch einen Krafterzeuger (
44 ) umfasst, der ein Fluidinjektionsgerät (44 ) ist, der das Medium (42 ) unter Druck setzt und der ca. 3 bis ca. 10 cm3 des Mediums (42 ) in weniger als 5 Sekunden unter Bildung eines Drucks von mehr als 3 MPa injiziert. - Gerät nach Anspruch 1, wobei das kraftübertragende Element (
16 ) ein Dichtungselement (20 ) umfasst, das die Bildung eines hydraulischen Drucks gestattet, wenn inkompressibles Fluid (42 ) aus dem kraftübertragenden Element (16 ) in die sich verjüngende Kanüle (14 ) injiziert wird. - Gerät nach Anspruch 1, wobei der elastische Prothesen-Rückenmarkskern (
12 ) mit einer Geschwindigkeit durch die proximale Öffnung (50 ) hindurchgeht, die innerhalb des Bereichs von 10 Millisekunden bis 5 Sekunden liegt. - Gerät nach Anspruch 1, wobei der elastischen Prothesen-Rückenmarkskern (
12 ) aus polymerem Hydrogel hergestellt ist.
