Description
- GEBIET DER ERFINDUNG
- Die vorliegende Erfindung betrifft eine antifungale Kombinationstherapie, welche die Verwendung bekannter antifungaler Mittel, wie z. B. die Azole oder Polyene, in Kombination mit einem Pneumacandin-Derivat als antifungales Mittel umfaßt. Insbesondere betrifft die Erfindung eine antifungale Kombinationstherapie, welche die Verwendung von Azolen, wie z. B. Fluconazol (hierin nachfolgend als FCZ bezeichnet), Voriconazol, Itraconazol, Ketoconazol, Miconazol, ER 30346, SCH 56592; Polyenen, wie z. B. Amphotericin B (hierin nachfolgend als AmB bezeichnet), Nystatin oder Liposomal- oder Lipidformen davon, wie z. B. Abelcet, AmBisome und Amphocil; Purin- oder Pyrimidinnukleotidinhibitoren, wie z. B. Flucytosin; oder Polyoxinen, wie z. B. Nikkomycine, insbesondere Nikkomycin Z oder andere Chitininhibitoren, Elongationsfaktorinhibitoren, wie z. B. Sordarin und Analoga davon, Mannaninhibitoren, wie z. B. Predamycin, bakteriziden/ permeabilitätsinduzierenden (BPI, baktericidal/permeability-inducing) Eiweißprodukten, wie z. B. XMP.97 oder XMP.127, oder komplexen Kohlenhydraten als antifungale Mittel, wie z. B. CAN-296, in Kombination mit einem Pneumocandin-Derivat der Struktur oder einem pharmazeutisch annehmbaren Salz davon oder einer anderen pharmazeutisch annehmbaren Formulierung davon umfaßt.
- Diese Kombinationstherapien erwiesen sich als geeignet gegen opportunistische Krankheitserreger wie Cryptococcus spp., Candida spp., Aspergillus spp., Histoplasma spp., Coccidioides spp., Paracoccidioides spp., Blastomyces spp., Fusarium spp., Sporothrix spp., Trichosporon spp., Rhizopus spp., Pseudallescheria spp., Dermatophyten, Paeciliomyces spp., Alternaria spp., Curvularia spp., Exophiala spp., Wangiella spp., Penicillium spp., Saccharomyces spp., Dematiaceous-Pilze und Pneumocystis carinii.
- HINTERGRUND DER ERFINDUNG
- Es besteht ein steigender Bedarf nach Mitteln, die gegen opportunistische mykotische Infektionen durch Erreger wie Cryptococcus spp., Candida spp., Aspergillus spp., Histoplasma spp., Coccidioides spp., Paracoccidioides spp., Blastomyces spp., Fusarium spp., Sporothrix spp., Trichosporon spp., Rhizopus spp., Pseudallescheria spp., Dermatophyten, Paeciliomyces spp., Alternaria spp., Curvularia spp., Exophiala spp., Wangiella spp., Penicillium spp., Saccharomyces spp., Dematiaceous-Pilze und Pneumocystis carinii wirksam sind. Die derzeitigen Behandlungen, d. h. Polyene, wie z. B. Amphotericin B, verursachen schwere Nebenwirkungen, und Azole, wie z. B. Fluconazol, sind nur fungistatisch. Die Pneumocandine, die mit den Echinocandinen verwandt sind, sind cyclische Hexapeptide, welche die Zellwand-1,3β-D-Glucan-Synthese inhibieren. Die Pneumocandine haben eine starke In-vivo-Wirkung gegen Candida spp., Pneumocystis carinii, Aspergillus spp. sowie gegen die anderen oben aufgeführten fungalen Krankheitserreger gezeigt. Die Pneumocandine alleine besitzen jedoch gegen Cryptococcus spp. eine geringe Wirkung.
- Die Kombinationstherapie mit antifungalen Arzneistoffen kann zusätzliche Möglichkeiten zur Behandlung von Cryptococcus und anderen fungalen Krankheitserreger liefern.
- Frühere Studien haben die Wirksamkeit anderer Pneumocandin-Derivate gegen Cryptococcus neoformans in Kombination mit Amphotericin B und Fluconazol (Abruzzo et al., Antimicrob. Agents Chemo. 1995, 39: 1077–1081 und Bartizal et al., Antimicrob. Agents Chemo. 1995, 39: 1070–1076) untersucht. Keine dieser Studien hat jedoch die Ergebnisse erbracht, die bei Verwendung von Verbindung I als das Pneumocandin-Derivat erzielt wurden.
- DETAILLIERTE BESCHREIBUNG DER ERFINDUNG
- Die vorliegende Erfindung betrifft eine antifungale Kombinationstherapie, welche die Verwendung bekannter antifungaler Mittel, wie z. B. die Azole oder Polyene, in Kombination mit einem Pneumacandin-Derivat als antifungales Mittel umfaßt. Insbesondere betrifft die Erfindung eine antifungale Kombinationstherapie, welche die Verwendung von Azolen, wie z. B. Fluconazol, Voriconazol, Itraconazol, Ketoconazol, Miconazol, ER 30346, SCH 56592; Polyenen, wie z. B. Amphotericin B, Nystatin oder Liposomal- oder Lipidformen davon, wie z. B. Abelcet, AmBisome und Amphocil; Purin- oder Pyrimidinnukleotidinhibitoren, wie z. B. Flucytosin; oder Polyoxinen, wie z. B. Nikkomycine, insbesondere Nikkomycin Z oder andere Chitininhibitoren, Elongationsfaktorinhibitoren, wie z. B. Sordarin und Analoga davon, Mannaninhibitoren, wie z. B. Predamycin, bakteriziden/ permeabilitätsinduzierenden (BPI, baktericidal/permeability-inducing) Eiweißprodukten, wie z. B. XMP.97 oder XMP.127, oder komplexen Kohlenhydraten als antifungale Mittel, wie z. B. CAN-296, in Kombination mit einer Verbindung der Struktur oder einem pharmazeutisch annehmbaren Salz davon oder einer anderen pharmazeutisch annehmbaren Formulierung davon umfaßt.
- Diese Kombinationstherapie erwies sich insbesondere als geeignet gegen opportunistische Krankheitserreger wie Cryptococcus spp., Candida spp., Aspergillus spp., Histoplasma spp., Coccidioides spp., Paracoccidioides spp., Blastomyces spp., Fusarium spp., Sporothrix spp., Trichosporon spp., Rhizopus spp., Pseudallescheria spp., Dermatophyten, Paeciliomyces spp., Alternaria spp., Curvularia spp., Exophiala spp., Wangiella spp., Penicillium spp., Saccharomyces spp., Dematiaceous-Pilze und Pneumocystis carinii.
- Die Verbindung I ist in dem US-Patent Nr. 5 378 804 offenbart. Ihre Herstellung ist in diesem Patent und auch in US-Patent Nr. 5 552 521 beschrieben.
- Es wird vermutet, daß andere Pneumocandin-Derivate, wie z. B. diejenigen, die in dem US-Patent Nr. 5 516 756 und in den anhängigen Anmeldungen 07/936 558, 07/936 561, 08/058 657, 08/055 996, 08/378 687 und 60/006 505 offenbart sind, bei der Kombinationstherapie geeignet sein würden.
- Das Azol, das Polyol oder das andere antifungale Mittel kann oral oder parenteral verabreicht werden. Verbindung I wird vorzugsweise parenteral verabreicht, ohne jedoch auf diesen Weg eingeschränkt zu sein, und sie kann auch durch andere Wege, wie z. B. oral, intramuskulär oder subkutan, verabreicht werden.
- Wie nachstehend gezeigt, führt die Kombinationstherapie zu verstärkten Wirkungen, wobei von allen Mitteln subinhibitorische Konzentrationen verwendet werden. Diese Wirkungen können in vitro und in vivo durch Verwendung klinischer und Umweltstämme von C. neoformans, C. albicans und A. fumigatus veranschaulicht werden.
- Die Erfindung wird in Verbindung mit den folgenden nichtlimitierenden Beispielen weiter beschrieben.
- BEISPIELE
- Es wurde gefunden, daß eine Kombinationstherapie von Verbindung I mit AmB und FCZ gegen C. neoformans zu einer erhöhten Wirksamkeit gegen C.-neoformans-Stämme in vitro führt. Es wurde ebenfalls gefunden, daß eine Kombinationstherapie von Verbindung I mit AmB gegen C. albicans und A. fumigatus zu einer erhöhten In-vitro-Wirkung führt. Dies wurde durch Anwendung eines Bouillon-Mikrodilutionsverfahrens gezeigt, welches das von der NCCLS (Protokoll M27-T) vorgeschlagene Standardverfahren zum Test der antifungalen Suszeptibilität ist. Subinhibitorische Konzentrationen von Verbindung I in Kombination mit subinhibitorischen Konzentrationen von AmB und subinhibitorischen Konzentrationen von FCZ wurden eingesetzt. Die minimalen inhibitorischen Konzentrationen (MICs) für AmB und Verbindung I wurden als die niedrigsten Arzneistoffkonzentrationen, bei denen kein Wuchs auftrat, definiert. Die FCZ-MIC wurde als die niedrigste Arzneistoffkonzentration, die zu einer sichtbaren Trübung von weniger oder gleich 80% Inhibierung, verglichen mit der, die von der Kontrolle ohne antifungales Mittel erzeugt wurde, definiert.
- Die Ergebnisse für die Tests der antifungalen Suszeptibilität zeigen, daß kolonienbildende Einheiten (CFUs) deutlich verringert wurden, wenn Amphotericin B in bestimmten Konzentrationen (0,0075, 0,015 und 0,03 μg/ml) mit Verbindung I in bestimmten Konzentrationen (4, 8 und 16 μg/ml) kombiniert wurde. Zusätzlich steigerte die Verabreichung von Verbindung I deutlich die Wirksamkeit von Fluconazol durch Verringerung der CFU-Anzahl bei bestimmten Konzentrationen (0,25, 0,50 und 1,0 μg/ml).
- Ein zusätzlicher In-vitro-Test der Arzneistoff-Kombination wurde durchgeführt, um die Kombinationen von Verbindung I mit AmB und FCZ gegen klinische Isolate zu untersuchen. Es existierte kein offenkundiger Antagonismus zwischen Verbindung I und AmB gegen C. albicans, A. fumigatus und C. neoformans. Die fraktionellen inhibitorischen Indizes (FIC) lagen bei etwa 0,50 oder niedriger, was auf additive oder synergistische Wirksamkeit hindeutet. Die Ergebnisse legen nahe, daß Verbindung I die Wirkung von FCZ und AmB erhöhen kann, und sie deuten auf eine potentielle Rolle für Verbindung I in Kombinationsregimes gegen jene Pilze hin, die auf Verbindung I, wenn sie alleine verwendet wird, weniger empfindlich oder unempfindlich sind.
- Arzneistoff-Wechselwirkungs- und -Wirksamkeitsstudien mit Verbindung I, kombiniert entweder mit AmB oder FCZ, gegen disseminierte Candidiasis, Aspergillosis und Cryptococcosis wurden durchgeführt. Die Ergebnisse zeigten keine nachteiligen Auswirkungen der Kombinationen bei hohen, Anwendungs- und niedrigeren Konzentrationen und keinen Antagonismus der Wirksamkeit, wenn entweder AmB oder FCZ mit Verbindung I kombiniert wurde. Gegen C. albicans schienen Dosen von Verbindung I mit 0,03 mg/kg und niedriger plus 0,03 mg/kg und niedriger von AmB wirksamer zu sein, als wenn jedes Mittel alleine verabreicht wird. Bei FCZ wurden ähnlich Ergebnisse erhalten bei FCZ-Dosen von 0,31 mg/kg und niedriger, kombiniert mit 0,03 mg/kg von Verbindung I. Gegen A. fumigatus wurden bedeutende Verbesserungen der Wirksamkeit (10 bis > 800fach bei den ED-Werten) festgestellt, wenn Verbindung I zwischen 0,03 und 2 mg/g und AmB zwischen 0,03 und 0,5 mg/kg titrierte. Eine bedeutende Verbesserung bei der Überlebensrate wurde beobachtet, wenn 0,008 mg/kg von Verbindung I mit 0,12 mg/kg von AmB kombiniert wurde, verglichen mit den alleine verabreichten Verbindungen.
- Wie man bei In-vitro-Studien von Arzneistoffkombinationen mit Verbindung I und AmB feststellte, lagen die FIC-Werte bei etwa 0,50, was eine additive oder synergistische Wirksamkeit in vivo nahelegt. Außerdem wurde kein Antagonismus der Wirksamkeit mit FCZ in Kombination mit Verbindung I bei Studien gegen Cryptococcosis bei Mäusen beobachtet. Nützliche Auswirkungen auf die Wirksamkeit wurden gegen C. neoformans mit Kombinationen aus Verbindung I und FCZ bei bestimmten Konzentrationen beobachtet.
Claims (13)
- Die Verwendung eines Pneumocandin-Derivats und eines weiteren antifungalen Mittels, ausgewählt aus der Gruppe, bestehend aus Azolen, Polyenen, Purinnukleotidinhibitoren, Pyrimidinnukleotidinhibitoren, Mannaninhibitoren, Proteinelongationsfaktorinhibitoren, bakteriziden/ permeabilitätsinduzierenden Eiweißprodukten oder Polyoxinen, bei der Herstellung eines Medikaments zur Behandlung von Pilzinfektionen, wobei das Pneumocandin-Derivat oder ein pharmazeutisch annehmbares Salz davon ist.
- Die wie in Anspruch 1 beanspruchte Verwendung, wobei das Pneumocandin-Derivat zur parenteralen Verabreichung formuliert ist.
- Die wie in Anspruch 1 oder Anspruch 2 beanspruchte Verwendung, wobei das Azol, Polyen oder andere antifungale Mittel zur oralen oder parenteralen Verabreichung formuliert ist.
- Die wie in irgendeinem der Ansprüche 1 bis 3 beanspruchte Verwendung, wobei das Azol ausgewählt ist aus der Gruppe, bestehend aus Fluconazol, Voriconazol, Itraconazol, Ketoconazol, Miconazol, ER 30346 und SCH 56592, das Polyen ausgewählt ist aus der Gruppe, bestehend aus Amphotericin B, Nystatin oder liposomalen und Lipidformen davon, der Purin- oder Pyrimidinnukleotidinhibitor Flucytosin ist, der Mannaninhibitor Predamycin ist, der Proteinelongationsfaktorinhibitor Sordarin und Analoga davon ist und das Polyoxin Nikkomycin Z ist.
- Die wie in irgendeinem der Ansprüche 1 bis 3 beanspruchte Verwendung, wobei das andere antifungale Mittel ein Polyen ist.
- Die wie in irgendeinem der Ansprüche 1 bis 3 beanspruchte Verwendung, wobei das andere antifungale Mittel ein Azol ist.
- Die wie in Anspruch 5 beanspruchte Verwendung, wobei das Polyen Amphotericin B ist.
- Die wie in Anspruch 6 beanspruchte Verwendung, wobei das Azol Fluconazol ist.
- Die wie in irgendeinem der Ansprüche 1 bis 8 beanspruchte Verwendung, wobei die Infektion von einem fungalen Krankheitserreger, ausgewählt aus Cryptococcus spp., Candida spp., Aspergillus spp., Histoplasma spp., Coccidioides spp., Paracoccidioides spp., Blastomyces spp., Fusarium spp., Sporothrix spp., Trichosporon spp., Rhizopus spp., Pseudallescheria spp., Dermatophytes, Paeciliomyces spp., Alternaria spp., Curvularia spp., Exophiala spp., Wangiella spp., Penicillium spp., Saccharomyces spp., Dematiaceous-Pilz oder Pneumocystis carinii, verursacht wird.
- Die wie in Anspruch 9 beanspruchte Verwendung, wobei der fungale Krankheitserreger ausgewählt ist aus Cryptococcus spp., Candida spp. oder Aspergillus spp.
- Die wie in Anspruch 7 oder Anspruch 8 beanspruchte Verwendung, wobei der fungale Krankheitserreger ein klinischer oder Umweltstamm von Cryptococcus neoformans ist.
- Die wie in Anspruch 7 beanspruchte Verwendung, wobei der fungale Krankheitserreger ein klinischer oder Umweltstamm von Candida albicans oder Aspergillus fumigatus ist.
- Die wie in Anspruch 6 beanspruchte Verwendung, wobei das Azol SCH 56592 ist.
