Description
- Die Erfindung betrifft die Verwendung von Isochinolinon-Derivaten für die Behandlung von Hyperlipämie. Genauer gesagt betrifft die Erfindung Antihyperlipoproteinämika, die eine wirksame Menge dieser Isochinolinon-Derivate oder deren pharmazeutisch verträglicher Salze umfassen.
- US-Patente 4,129,565 und 4,517,365 offenbaren, daß die Isochinolinon- Derivate der Formel
- wobei R&sub1; Wasserstoff oder eine Alkylgruppe ist und R&sub2; eine Alkylgruppe ist und deren pharmazeutisch verträgliche Salze β-adrenerge Blockierungsaktivität besitzen und verwendbar sind für die Behandlung von Herzkrankheiten wie Hypertonie, Angina pectoris und Arrhythmie.
- U.S.-Patent 5,061,714 offenbart auch, daß die Isochinolinon-Derivate der obigen Formel verwendbar sind für die Behandlung von Glaukomen oder erhöhtem Augendruck.
- Es existiert jedoch kein Bericht über andere pharmazeutische Verwendungen dieser Isochinolinon-Derivate.
- 4-[3-(Tert-butylamino-2-hydroxypropoxy]-N-methylisocarbostyrilhydrochlorid reduziert vaskuläre Läsionen und Nephrosklerosis. (J. Pharmacobiodyn., 1985, 8(2), 134-41). EP-A-0 326 386 offenbart eine andere Klasse von Isochinolinon-Derivaten, die die Konzentrationen von Cholesterin und Low-Density-Lipoproteinen im Blut senkt.
- Angesichts dieser Situation haben die Erfinder andere medizinische Verwendungen der Isochinolinon-Derivate der oben angegebenen Formel untersucht und gefunden, daß sie eine potente antihyperlipämische Aktivität besitzen und verwendbar sind für die Behandlung von Hyperlipämie.
- Daher ist die vorliegende Erfindung gerichtet auf die Verwendung einer Verbindung der Formel (I):
- wobei R&sub1; Wasserstoff oder eine C&sub1;-C&sub6;-Alkylgruppe ist, und R&sub2; eine C&sub1;-C&sub6;- Alkylgruppe ist, oder eines pharmazeutisch verträglichen Salzes davon für die Herstellung eines Medikaments für die Behandlung von Hyperlipämie bei einem Säuger.
- In der obigen Formel (I) schließt die C&sub1;-C&sub6;-Alkylgruppe bevorzugt geradkettige oder verzweigte Alkylgruppen von 1 bis 4 Kohlenstoffatomen ein, z.B. Methyl, Ethyl, n-Propyl, Isopropyl, n-Butyl, Isobutyl und tert-Butyl.
- Repräsentative Beispiele der Verbindungen der Formel (I) sind im folgenden dargestellt.
- 4-(3-tert.-Butylamino-2-hydroxypropoxy)-1-isochinolinon, 4-(3-tert.-Butylamino-2-hydroxypropoxy)-2-methyl-1-isochinolinon, 4-(3-Isopropylamino-2-hydroxypropoxy)-1-isochinolinon, 4-(3-Isopropylamino-2-hydroxypropoxy)-2-methyl-1-isochinolinon, 4-(3-Ethylamino-2-hydroxypropoxy)-1-isochinolinon, 4-(3-Ethylamino-2-hydroxypropoxy)-2-methyl-1-isochinolinon, und 4-(3-Ethylamino-2-hydroxypropoxy)-2-ethyl-isochinolinon.
- Die pharmazeutisch verträglichen Salze die Salze der Isochinolinonverbindungen mit anorganischen oder organischen Säuren wie Salzsäure, Schwefelsäure, Salpetersäure, Bromwasserstoffsäure, Oxalsäure, Maleinsäure, Fumarsäure, Zitronensäure, Weinsäure und Äpfelsäure ein.
- Wenn die Isochinolinon-Verbindungen der Formel (I) oder deren pharmazeutisch verträgliche Salze für die Herstellung eines Antihyperlipoproteinämikums verwendet werden, können sie zu pharmazeutischen Präparaten in verschiedenen Dosierungsformen formuliert werden. Diese Präparate können oral in Form von Tabletten, zuckerüberzogenen Tabletten, Hartkapseln, weichen Kapseln oder Flüssigkeiten wie Lösungen, Emulsionen oder Suspensionen gegeben werden. Alternativ können die Präparate parenteral in Form von Injektionen gegeben werden.
- Die pharmazeutischen Präparate können hergestellt werden durch bekannte Prozesse unter Verwendung von im Stand der Technik gut bekannten Additiven wie Excipienten, Bindern, Verdünnern, Stabilisatoren, Konservierungsmitteln, Lösungsvermittlern, Benetzungsmitteln, Emulgatoren, Schmiermitteln, Süßstoffen, Farbstoffen, Geschmacksstoffen, Puffern und Antioxidantien. Darüber hinaus können die pharmazeutischen Präparate der Isochinolinon-Verbindungen mit einem Stabilisator in einer solchen Weise geformt werden, wie in US-Patent 4,666,919 beschrieben, das durch Verweis hier einbezogen wird.
- Für die vorliegenden Verbindungen umfassende pharmazeutische Präparate variiert die Dosis in Abhängigkeit der Art der Verabreichung, der speziellen behandelten Krankheit und ihrer Schwere, der Gesamtgesundheit und dem Gesamtzustand des Patienten und der Beurteilung des verschreibenden Arztes, im allgemeinen beträgt eine tägliche Dosis jedoch oral 10 bis 1200 mg für einen Erwachsenen.
- Darüber hinaus haben die vorliegenden Verbindungen eine sehr niedrige Toxizität, wie im später beschriebenen Beispiel 2 gezeigt.
- Die vorliegende Erfindung wird weiter durch die folgenden Beispiele illustriert.
- In diesem Beispiel werden die Wirkungen der vorliegenden Verbindung auf die Wand eines Blutgefäßes, das Serumlipid und dem Blutglucosespiegel bei Versuchs-Sklerose-Kanninchen aus der Bestimmung der arteriellen Sklerose-Wirkung, des Assay der die Blutgefäßwand bildenden Komponenten und der Übergangsanalyse des Schadens der Blutgefäßwand im Laufe der Zeit untersucht.
- Männliche japanische weiße Kanninchen (Alter: 25 Monate) wurden nach dem Zufallsprinzip in drei Gruppen aufgeteilt. Eine Gruppe wurde mit einer normalen Diät ernährt (im folgenden "gesunde Gruppe" genannt). Die verbleibenden beiden Gruppen wurden mit 100 g/Tag einer Sklerose- Diät mit hoher α-Stärke, hohem Salz und niedrigem Protein ernährt, enthaltend 0,3 % Cholesterin "hergestellt von Funabashi Farm), und erhielten simultan Norepinephrin (3 mg) intramuskulär und wurden während 150 Tagen so gehalten. Eine Gruppe von diesen wurde anschließend "Sklerose-Gruppe" genannt. Der verbleibenden Gruppe wurde oral bei einer Dosis pro Tag einer Kapsel, die mit 3 mg (±)-4-(3-tert.- Butylamino-2-hydroxypropoxy)-2-methyl-1(2H)-isochinolinon-Hydrochlorid gegeben, wobei die Gruppe im folgenden "Arzneimittelverabreichungs- Gruppe" genannt wird. Alle 50 Tage wurde die Aortenpulswellengeschwindigkeit (PWV, Aortic Pulse Wave Velocity), gemessen, um die mittlere Härte der Aorta der Kanninchen zu bestimmen. Nach 5 Monaten und unmittelbar vor der Schlachtung der Kanninchen wurde Blut entnommen zur Bestimmung von Cholesterol im Blut (T-Cho), Triglycerid (TG), β-Lipoprotein (β-LP), HDL-Cholesterol und des Blutglucosespiegels. Nach dem Schlachten wurde der fünfte interkostale Pars der Thoraxaorta entnommen und dann Formalinfixierung und Einbettung in Paraffin unterworfen. Anschließend wurde jede der Komponenten Glattmuskelzelle (SMC), Elastin (EL), Collagen (CL), Glycosaminoglycan (GAGS), PAS-positive Substanz (PAS), Calcium (Ca) und Cholesterin (Cho) histochemisch beobachtet und bestimmt.
- Die Ergebnisse sind in den folgenden Tabellen dargestellt. Tabelle 1 ΔPWV (m/Sek.) - PWV steigt mit zunehmendem Wachstum, Reifung und Altern. Daher ist ein Anstieg des PWV-Werts ein Hinweis für arterielle Sklerose (vergleiche Arterial Sclerosis, Band 9, Nr. 1 April 1980, Seiten 91-95). Tabelle 1 zeigt eine Änderung des PWV mit der Zeit (Δ PWV) für jede Gruppe, und deutet an, daß der PWV der Arzneimittelverabrelchungs-Gruppe signifikant in 150 Tagen verbessert wurde, verglichen mit der Sklerose- Gruppe. Tabelle 2 zeigt, daß eine bemerkenswerte Verbesserung der arteriellen Sklerose vorlag.
- 4-(3-tert.-Butylamino-2-hydroxypropoxy)-2-methyl-1-isochinolinon- Hydrochlorid und 4-(3-tert.-Butylamino-2-hydroxypropoxy)-1- isochinolinon-Hydrochlcrid wurden jeweils oral männlichen und weiblichen Mäusen zur Bestimmung des LD&sub5;&sub0;-Werts gegeben. Die Ergebnisse sind in Tabelle 4 dargestellt. Tabelle 4 Geschlecht LD&sub5;&sub0; männlich weiblich Durchschnitt
- Die folgenden Beispiele illustrieren die pharmazeutischen Präparate der vorliegenden Verbindung.
- (±)-4-(3-tert.-Butylamino-2-hydroxypropoxy)- 2-methyl-1(2H-isochinolinon-Hydrochlorid 20 mg
- Magnesiumsilicat 20 mg
- Lactose 98 mg
- Hydroxypropylcellulose 7,5 mg
- Magnesiumstearat 1 mg
- gehärtetes Pflanzenöl 3 mg
- Gesamt 150 mg
- Die obige Isochinolinon-Verbindung, Magnesiumsilicat und Lactose wurden gemischt und die Mischung wurde mit einer Lösung aus 5 % Hydroxypropylcellulose in Ethylalkohol verknetet, gefolgt von Granulieren der Mischung zur Herstellung von Granulaten geeigneter Partikelgrößen. Dieses Granulat wurde getrocknet und gesiebt und dann mit Magnesiumstearat und gehärtetem Pflanzenöl gemischt, so daß das Granulat gleichmäßig mit der Mischung aus Magnesiumstearat und Öl überzogen wurde. Das Granulat wurde mit einer Rotationstablettiermaschine zu Tabletten von jeweils 7,0 mm Durchmesser formuliert, die 100 mg wogen und eine Härte von 6 kg hatten.
- (±)-4-(3-tert.-Butylamino-2-hydroxypropoxy)- 2-methyl-1(2H)-isochinolinon-Hydrochlorid 10 mg
- Magnesiumoxid 40 mg
- Calciumhydrogenphosphat 38 mg
- Lactose 10 mg
- Hydroxypropylcellulose 20 mg
- Die obigen Komponenten außer Hydroxypropylcellulose wurden gemischt, und die Mischung wurde mit einer Lösung aus 5 % Hydroxypropylcellulose in Ethylalkohol verknetet. Die verknetete Mischung wurde durch Extrusionsgranulierung granuliert. Das Granulat wurde getrocknet und gesiebt und ergab Granulat mit einer Partikelgröße von 12-48 Mesh.
- (±)-4-(3-tert.-Butylamino-2-hydroxypropoxy)- 2-methyl-1(2H)-isochinolinon-Hydrochlorid 1.000 g
- Saccharose 30.000 g
- D-Sorbitol 70 w/v % 25.000 g
- Ethylparaoxybenzoat 0.030 g
- Propylparaoxybenzoat 0.015 g
- Geschmacksstoffe 0.200 g
- Glycerin 0.150 g
- 96 % Ethanol 0.500 g
- Destilliertes Wasser q.s.
- Gesamt 100 ml
- Saccharose, D-Sorbitol, Methylparaoxybenzoat, Propylparaoxybenzoat und die obige Isochinolinon-Verbindung wurden in 60 g warmen Wasser gelöst. Nach dem Abkühlen wurden Glycerin und eine Lösung der Geschmacksstoffe in Ethanol zugegeben. Zu der Mischung wurde dann Wasser zur Auffüllung auf 100 ml gegeben.
- (±)-4-(3-tert.-Butylamino-2-hydroxypropoxy)- 2-methyl-1(2H)-isochinolinon-Hydrochlorid 10.0 mg
- Natriumchlorid 81.0 mg
- Natriumhydrogencarbonat 8.40 mg
- Destilliertes Wasser zur Injektion q.s.
- Gesamt 10.0 ml
- Natriumhydrogencarbonat, Natriumchlorid und die Isochinolinon- Verbindung wurden zum destillierten Wasser gegeben und darin gelöst zur Herstellung von 10.0 ml.
- (±)-4-(3-tert.-Butylamino-2-hydroxypropoxy)- 2-methyl-1(2H)-isochinolinon-Hydrochlorid 2 g
- Polyethylenglycol 4000 20 g
- Glycerin 78 g
- Gesamt 100 g
- Die Isochinolinon-Verbindung wurde in Glycerin gelöst. Zu der Lösung wurde Polyethylenglycol 4000 gegeben, und die Mischung wurde zu einer Lösung erwärmt. Die Lösung wurde in eine Suppositorienform gegossen und durch Kühlen zur Herstellung von Suppositorien eines Gewichts von 1,5 g pro Stück verfestigt.
Claims (3)
1. Verwendung einer Verbindung der Formel (I)
wobei R&sub1; Wasserstoff oder eine C&sub1;-C&sub6; Alkylgruppe ist und R&sub2; eine
C&sub1;-C&sub6; Alkylgruppe ist oder von deren pharmazeutisch verträglichem
Salz für die Herstellung eines Medikaments zur Behandlung von
Hyperlipämie bei einem Säuger.
2. Verwendung aus Anspruch 1, wobei R&sub1; Wasserstoff ist und R&sub2; eine
C&sub1;-C&sub4; Alkylgruppe ist.
3. Verwendung aus Anspruch 1, wobei R&sub1; eine C&sub1;-C&sub4; Alkylgruppe ist und
