Description
- In der Vergangenheit wurden verschiedene Arten chemischer Verbindungen verwendet, um zahlreiche Tumore des Menschen zu verzögern oder zu hemmen. Zur Verwendung in der Krebstherapie sind in zahlreichen Ländern mehr als 30 Verbindungen anerkannt, wobei jedoch die Erlangung des therapeutischen Erfolgs ein Plateau erreicht hat und die Suche nach Antitumormitteln in verschiedenen Richtungen weitergeht.
- 1967 berichteten Weitzel und Mitarbeiter in der Zeitschrift für Physiologische Chemie 348, 433-442, daß Hydrazinacetat und -sulfat in vivo das Wachstum von Aszites- Carcinoma und Sarcoma 180 der Maus und Walker-Carcinosarcoma der Ratte hemmen. Es ist weithin bekannt, daß die von niederen Tieren erhaltenen Ergebnisse für den Menschen nicht extrapoliert werden können. Wie aus der folgenden Tabelle zu entnehmen ist, zeigen Erfahrungen in den US National Institutes of Health, daß mehr als 200 neue Chemotypen mit Antitumoraktivität in Tieren keine klinisch verwendbare Anticanceraktivität bei Menschen haben: (Tabelle I wurde aus verschiedenen Veröffentlichungen des US National Cancer Instituts zusammengestellt). - Darüber hinaus haben hunderte von Analogstoff en neuer und alter Chemotypen trotz einer guten Antitumoraktivität bei Tieren keine anticancerogene Wirkung bei Menschen gezeigt. In den letzten 25 Jahren haben im Gegensatz zu dem Vorgenannten ledig etwa 5 neue Chemotypen als anticancerogene Medikamente den Markt erreicht. Daher genügten die ersten Berichte über Antitumoraktivität von Hydrazinsulfat bei Tieren nicht zur Vorhersage, daß es auch bei Menschen eine anticancerogene Aktivität aufweisen könnte.
- Wegen der schlechten Vorhersagbarkeit von Tiermodellen hat das National Cancer Institute of the U.S. National Institutes of Health nach 25 Jahren unproduktiver Versuchsreihen das Mausmodell jetzt zurückgezogen und konzentriert ein neues in vitro-Programm zur Erforschung neuer Antitumormedikamente ein (E. Eckholm, New York Times, 23. Dezember 1986, S. Cl).
- In Tabelle II wird ein in den Vereinigten Staaten zum Vertrieb anerkanntes Gesamtregister von Krebsmedikamenten zusammengestellt, aus dem zu entnehmen ist, daß die meisten von ihnen Analogstoffe anderer Medikamente sind. Tabelle III zeigt, daß mit einer Ausnahme alle neueren "New Drug Applications", die als anticancerogene Medikamente angemeldet wurden, von Seiten der U.S. Food and Drug Administration zu nichtgenehmigungsfähigen Bewertungen führten. Tabelle IV zeigt, daß der letzte neue Chemotyp, der im klinischen Bereich Erfolg hatte, vor mehr als 20 Jahren entdeckt wurde. - Eine Übersicht über den "FDA's New Drug Evaluation - Statistical Report" (März 1986) zeigt, daß bei der FDA keine Zulassung eines neuartigen anticancerogenen Medikaments anhängig ist. Tabelle III Nachfolgend sind die von 1980 bis 1984 angemeldeten Antineoplastica zusammengestellt: Angemeldet: nicht genehmungsfähig Antineoplasticum
- Tabelle IV ist eine Zusammenstellung der anticancerogenen Medikamente, die in den Vereinigten Staaten genehmigt wurden. Das letzte genehmigte, nichthormonelle anticancerogene Mittel war 1983 das Etoposid.
- Dahinter sind die Jahre der Entdeckung der wichtigsten anticancerogenen Medikamente auf dem US-Markt angegeben (unter der willkürlichen Annahme von einem Jahr vor der ersten Veröffentlichung): Tabelle IV Etoposid 1966 Adriarnycin 1966 Bleomycin 1966 Cisplatin 1965 Mitomycin C 1965 Vincristin 1961 5- Fluorouracil 1957 Cyclophosphamid 1957 Methotrexat 1949
- In den vergangenen 20 Jahren wurde damit kein neuer Chemotyp von cytotoxischen anticancerogenen Medikamenten entdeckt. Demzufolge bleibt ein nichtbefriedigter Bedarf nach zusätzlichen anticancerogenen Medikamenten für den klinischen Einsatz gegen Tumore bei Menschen.
- Bis zum gegenwärtigen Zeitpunkt wurde im allgemeinen nicht erkannt, daß ein spezifisches Antikachexiemittel (aufgrund seiner Fähigkeit solche spezifischen thermodynamischen Stoffwechselprozesse zu unterbrechen, die zu Krebskachexie führen) ein Antitumorpotential besitzt, und zwar aufgrund einer systemischen thermodynamischen Wechselbeziehung zwischen Tumorprogression (Zuwachs der Tumorenergie) und Krebskachexie (Verlust der Wirtsenergie); dieses lehrt die wissenschaftliche Literatur seit 1974 (J. Gold, Cancer Cachexia and Gluconeoganesis, Ann. N.Y. Acad. Sci. 230, 103-110 (1974). Obgleich es richtig ist, däß jedes beliebige Antitumormittel ein Antikachexiepotential haben kann, sofern es heilsam ist, ist es damit auch richtig, daß ein spezifisches Antikachexiemittel ein Potential zur erhöhten Überlebensfähigkeit des Patienten aufweist. Nach dem Stand der Technik ist es weder offensichtlich noch vorhersagbar, daß Hydrazinsulfat dieses Potential besitzt.
- 1978 wurde dem Anmelder das US-P-4 110 437 zur Behandlung von Krebskachexie mit Hydrazinsulfat erteilt. Es wurden auch Untersuchungen durchgeführt, um festzustellen, ob Hydrazinsulfat Tumorwachstum bei Menschen verzögern kann. Diese ersten Untersuchungen waren allerdings unzureichend und versagten hinsichtlich des statistischen Nachweises der Antitumoraktivität.
- Eine Gruppe bei "Sloan-Kettering" kam nach einer Versuchsreihe zu der Schlußfolgerung: "die in diesem Bericht aufgezeichneten klinischen Beobachtungen stützen nicht die Bedeutung von Hydrazinsulfat als ein anticancerogenes Mittel. Wir kommen zu der Schlußfolgerung, daß sein klinischer Einsatz gegenwärtig nicht gewährleistet ist und planen keine weiteren Versuchsreihen." (Ocha et al., Cancer Chemotherapy Reports, Teil 1, Bd. 59, Nr. 6, Nov./Dez. 1975; S. 1151-1154)
- Ferner heißt es in einer Veröffentlichung einer Gruppe an der University of Virginia: "Hydrazinsulfat, das in diesen Versuchsreihen verabreicht wurde, zeigte keinerlei objektive oder subjektive Antitumoraktivität, und es sind gegenwärtig keine weiteren Versuchsreihen geplant." (Lerner und Regelson, Cancer Treatment Reports, Bd. 60, Nr. 7, Juli 1976, S. 959-966) . Eine weitere Veröffentlichung von Regelson et al. führt aus: "Schlußfolgernd sind wir der Ansicht, daß das in der vorliegenden Untersuchung vorgegebene Hydrazinsulfat eine inaktive Verbindung ist." (Cancer Chemother. Pharmacol. 3, 121-124; 1979).
- Damit besagt die bekannte Lehre, daß Hydrazinsulfat in bezug auf primäres Tumorwachstum in Menschen inaktiv zu sein scheint.
- Die US-P-4 110 437 ist auf die Antikachexiewirkungen von Hydrazinsulfat gerichtet. So wird in Spalte 2, Zeilen 17 ... 29 berichtet, daß der Gewichtsverlust bei menschlichen Krebspatienten, die mit Hydrazinsulfat behandelt wurden, aufhörte und das Gewicht danach zunahm. Obgleich die Erhaltung des Körpergewichts bei an Krebs Leidenden eindeutig wünschenswert ist, hat die Fähigkeit des Hydrazinsulfats in bezug auf die Auslösung dieses Infekts niemanden zu der Schlußfolgerung gebracht, daß es bei der Behandlung der ersten Stadien von Krebs verwendbar sein könnte. Sicher ist, daß es keine Lehre gibt, aufgrund der man zu der Schlußfolgerung kommen könnte, daß es zur Zubereitung eines Medikaments verwendet werden könnte, welches das Leben von an Krebs Leidenden tatsächlich verlängern würde, wenn es ihnen in den ersten Stadien von Krebs verabreicht werden würde. Kachexie ist in den ersten Stadien von Krebs normalerweise nicht ausgeprägt. Eine Lehre, daß ein spezielles Medikament eine Antikachexiewirkung hat, führt daher nicht zu der Schlußfolgerung, daß es bei der Frühbehandlung von Krebs eingesetzt werden sollte. Konventionelle Chemotherapien bei menschlichen Krebspatienten war auf die Verabreichung von cytotoxischen Medikamenten gerichtet.
- Vor diesem Hintergrund war für den Fachmann nicht zu erwarten, daß ein Medikament, welches nach der Lehre Antikachexiewirkungen hat und in späten Stadien der Krebsbehandlung verabreicht wird, bei der Behandlung in den Anfangsstadien von Krebs verwendbar sein würde. Aus dem, was über die Antikachexiewirkungen von Hydrazinsulfat bekannt war, war nicht offensichtlich, daß die Modifikation des vom Hydrazinsulfat beeinflußten Wirtsmechanismus zu der Möglichkeit einer Verlängerung des Lebens von an Krebs Leidenden führen würde, wenn sie in den ersten Stadien der Erkrankung behandelt werden würden.
- Die vorliegende Erfindung beruht auf der Entdeckung, daß Hydrazinsulfat, bei parenteraler oder oraler Verabreichung in wirksamen nichttoxischen Mengen an Menschen mit Tumoren der Lunge, Prostata, Brust, Ovarien, Schilddrüse, Pancreas, Lymphe, Cervix, gastrointestinaler Trakt und anderen Stellen das Überleben der Patienten signifikant verlängert, während gleichzeitig die Lebensqualität des Patienten verbessert wird.
- Die Dosierungen des in der vorliegenden Erfindung eingesetzten Hydrazinsulfats können zwischen 1 bis 5 mg/kg täglich variieren, was weit unterhalb der LD50 liegt und wovon festgestellt wurde, daß es bei der Mehrzahl von Patienten, die so über eine Dauer von bis zu 4 Jahren behandelt wurden, gut toleriert wurde.
- Vorzugsweise wird einem Therapieplan mit einer 60 mg- Kapsel Hydrazinsulfat täglich für die ersten drei Tage gefolgt, danach zwei derartige Kapseln täglich für die nächsten drei Tage und danach drei 60 mg-Kapseln jeden weiteren Tag.
- Bei der Ausführung des Verfahrens schlugen bei Patienten mit einem Gewicht von über 130 Pound ((1 pound = 0,454 kg)) drei oder vier 60 mg-Kapseln täglich gut an, während Patienten mit einem Gewicht von weniger als 100 Pound normalerweise auf eine Dosis von zwei- oder dreimal täglich verabreichten 30 mg ansprachen. Zur Erlangung der besten Ergebnisse sollten die Blutwerte von Hydrazinsulfat an diesen Patienten bestimmt werden, um die wirksamste nichttoxische Dosis zu ermitteln.
- Ich habe festgestellt, daß die erfindungsgemäße Behandlung von Patienten in den ersten Stadien der Erkrankung vorteilhaft ist. Wesentliche Vorteile können insbesondere durch Behandlung mit Hydrazinsulfat vor der Entwicklung von Kachexie erzielt werden.
- Hydrazinsulfat-Therapie kann vorteilhaft kombiniert werden mit anderen Formen der Krebsbehandlung wie Chemotherapie, Immuntherapie, Bestrahlung und Operation.
- Hydrazinsulfat ist normalerweise bei Selbstverabreichung eine oder zwei Stunden vor den Mahlzeiten in Form einer Gelatinekapsel am wirksamsten. Nach Wunsch kann das Sulfat aufgelöst oder in steriler, wäßriger isotonischer Salzlösung suspendiert und oral und parenteral gegeben werden. In ähnlicher Weise kann Hydrazinsulfat mit festen Trägerstoffen zubereitet werden, wie beispielsweise Talcum, Getreidestärke oder Stearinsäure, und zu Tabletten für die orale Verabreichung gepreßt werden. Derartige Tabletten können mit Schellack oder Celluloseacetatphthalat in einer Weise magensaftresistent beschichtet werden, wie sie dem Fachmann in der Pharmazie bekannt ist.
- Die Wirksamkeit von Hydrazinsulfat zur Verlängerung des Überlebens bei Menschen wurde jetzt erstmalig in einem Placebo-kontrollierten Doppelblindversuch mit einer statistisch signifikanten Zahl von Patienten nachgewiesen.
- In diesem Versuch zur Bestimmung, ob eine Hydrazinsulfat-Behandlung im Zusammenhang mit einem Gewinn für das Überleben steht (R.T. Chlebowski et al., "Influence of Hydrazine Sulfate on Survival in Non-Small Cell Lung Cancer: A Randomized Placebo-Controlled Trial", vorgetragen auf der Jahrestagung der American Society for Clinical Oncology, 17.-19. Mai 1987, Atlanta, Georgia), wurden 65 Patienten mit nichtresezierbarem, Nichtkleinzellen- Lungenkrebs und ohne vorherige Chemotherapie randomisiert, um eine Kombinations-Chemotherapie entweder mit Hydrazinsulfat oder Plecebozusatz für eine Dauer bis zu 4 Jahren zu erhalten. Alle erhielten alle 28 Tage eine Platinol/Velban/Blenoxan (PVB)-Chemotherapie, bestehend aus 100 mg/m² Platinol, 4 mg/m² Vilban an den Tagen 1 und 2, sowie 10 Einheiten alle 8 Stunden für drei Dosisgaben Blenoxan. Nach den ersten drei Zyklen wurde das Blenoxan abgesetzt und die Platinol-Dosis auf 50 mg/m² reduziert.
- Faktoren der Vor-Chemotherapie, umfassend Alter, Geschlecht, Leistungsstatus (PS), vorheriger Gewichtsverlust und Umfang der Erkrankung, waren in zwei Gruppen in bezug auf Vor-Chemotherapie-Leistungsstatus (0 bis 1 vs. 2) und vorherigem Gewichtsverlust (> 10 %) vergleichbar, die in der Folge die Gesamtüberlebensrate (p< 0,05) beeinflussen. Die Toxizität kam wie erwartet von PVB bei drei Patienten, bei denen wegen hinzukommender Nausea mit Hydrazinsulfat nicht fortgefahren wurde. Die Überlebensrate der Gruppen mit Hydrazinsulfat oder Plecebo betrug: Gruppe der Behandlung mittlere Überlebensrate alle Patienten nach Patientengruppen PS 0-1 Patienten Hydrazinsulfat Tage
- Die statistische Analyse der Daten erfolgte mit Hilfe der verallgemeinerten Methode nach Wilcoxon (Breslau) . Der Vergleich der Gruppen mit Hydrazinsulfat und Plecebo zeigte bei einem Wert von *P< 0,01 Signifikanz Alle mit einem Gewinn der Überlebensrate mit Hydrazinsulfat befanden sich in der Leistungsstatusgruppe 0-1. Bei den Patienten mit Leitungsstatus (PS) 2, deren Zustand schlecht war, waren die Überlebensraten kurz (ein Mittelwert von 132 Tagen) und eng vergleichbar, ob auf Placebo oder Hydrazinsulfat. Damit richtete sich die Hydrazinsulfat-Zugabe als ein Antikachexiemittel hauptsächlich auf das Korrigieren von anormalem Wirtsstoffwechsel und wirkte sich als ein Mittel, daß einen gewissen Antitumoreffekt besitzen kann, sowohl auf die Patientenüberlebensrate als auch auf Patienten mit Nichtkleinzellen-Lungenkrebs aus.
- Es wurden mehrere Patienten mit Tumoren der Prostata, Lunge, Brust, Ovarien, Lymphe, Cervix, Schilddrüse, Pancreas und anderen Tumorstellen entsprechend dem bevorzugten Therapieplan mit Hydrazinsulfat behandelt, wie er bereits ausgeführt worden ist.
