PFANNE FÜR TOTALHÜFTGELENKENDOPROTHESE
Die Erfindung betrifft eine Pfanne für Totalhuftgelenkendoprothesen aus Keramik, insbesondere aus dichter Alp0~ - Keramik, für die zementfreie Implantation, die ohne weitere Hilfsmittel implantiert werden kann und bei der Vorkehrungen getroffen sind zum festen Einwachsen dieser Pfanne in den Hüftknochen.
Pfannen für Huftgelenkendoprothesen sind bereits für die Implantation mit Hilfe von Kunststoffzementen bekannte Auch Pfannen aus Stahl sind für den gleichen Zweck bereits vorgeschlagen worden. Diese Pfannen sind rund und im wesentlichen

auf ihrer Rückseite domförmig ausgeführt. Sie tragen zum Teil Nuten und Riefen, die zur Verankerung dieser Pfannen in dem Kunststoffzement dienen. Es sind auch schon Pfannen vorgeschlagen worden, die Zapfen auf ihrer Rückseite tragen, mit deren Hilfe sie im Zement verankert werden.
Es sind auch schon Pfannen aus verschiedenen Metallegierungen für die zementfreie Implantation vorgeschlagen worden. Zu ihrer Befestigung dienten Schrauben oder auch lange Zapfen, die in den angrenzenden Knochenraum eingeschraubt bzw. eingetrieben werden.
Es hat sich jedoch gezeigt, daß die Biokompatibilität von Kunststoffzementen, Kunststoffen und Metallen nicht sehr günstig ist. Es besteht daher die Gefahr der Pfannenlockerung nach längerem Verbleib der einzementierten Pfanne im Körper. Diese Lockerungsgefahr ist besonders groß bei den zementfrei implantierten metallischen Pfannen.
Seit einiger Zeit ist bekannt, daß gewisse Keramiksorten und einige Arten von Glaskeramik eine gegenüber den Metallen und Kunststoffen erheblich günstigere Biokompatibilität besitzen. Dies trifft besonders für AIpO - Keramik zu, wobei unter Kl1S)-, - Keramik eine Keramik verstanden wird, die 95 % Aluminiumoxid oder mehr enthält, insbesondere 99 % Al 0 . Umfangreiche Tierversuche und erste Ergebnisse von zementfreien Implantationen von Teilen von Gelenk-

endoprothesen aus dichter AIpO - Keramik zeigen, daß eine feste Verankerung derartiger Teile ohne die Hilfe von Zement möglich ist. Unter geeigneten Bedingungen, z. B. nach einer Ruhigstellung des betreffenden Gelenkes für einige Wochen bis Monate, bildet sich zwischen dem Implantat und dem umliegenden Knochengewebe eine mechanisch sehr feste Verbindung.
Für die zementfreie Implantation von Hüftgelenkpfannen für Totalendoprothesen aus AIpO-, - Keramik sind auch bereits Pfannen verschiedener Form vorgeschlagen und benutzt worden: die eine Pfannenkonstruktion dieser Art weist an ihrer Rückseite eine im wesentlichen kugelkalottenförmige Gestalt auf, in die tiefe Nuten eingearbeitet sind. Sie trägt an ihrer Kuppe einen Zapfen, der rund ist und ebenfalls tiefe Nuten aufweist. Diese Pfannenkonstruktion ist an bekannte Pfannen aus Kunststoff angelehnt und hat bei der zementfreien Implantation den Nachteil, daß sie keinen echten Verdrehungsschutz aufweist. Bis zum festen Anvrachsen von Gewebe an die Oberfläche bzw. Einwachsen von Knochengewebe in die Nuten und Riefen besteht daher die Gefahr einer Verdrehung dieser Pfanne bei auch leichten Bewegungen in dem Gelenk, wodurch gerade dieses Ein- und Anwachsen verzögert oder gar verhindert werden kann. - Es ist auch bereits eine Pfanne aus dichter Alp0 - Keramik für die zementfreie Implantation vorgeschlagen worden,

die in den Hüftknochen eingeschraubt wird. Bei dieser Pfannenkonstruktion ist ein Verdrehungsschutz ausdrücklich vorgesehen. Für die Implantation dieser Pfanne wird jedoch ein spezielles Instrumentarium benötigt, das sicher nicht überall zur*Verfügung steht. Das Einsetzen dieser Pfanne erfordert darüberhinaus eine besondere Sorgfalt und einen etwas höheren Zeitaufwand.
Die erfindungsgemäße Pfanne für Totalhüftgelenkendoprothesen aus dichter A1„O - Keramik für die zementfreie Implantation weist eine eckige Gestalt ihrer Rückseite auf. Sie kann drei-, vier-, fünf- oder sechseckig sein. Sie trägt an dieser eckigen Rückseite Nuten und Riefen, von denen ein Teil auch schwalbenschwanzförmige oder ähnliche Hinterschneidungen aufweisen können, in die Knochengewebe zur besseren Verankerung der Pfanne einwachsen kann.
Die Form der erfindungsgemäßen Pfanne wird in Figur 1 am Beispiel einer viereckigen Pfanne näher erläutert: die in Figur 4 dargestellte Pfanne besteht aus dem Pfannenkörper 1, der in seinem Inneren den ungefähr halbkugelförmigen Hohlraum 2 enthält, in dem nach der Implantation der entsprechend geformte Kopf des Oberschenkelteiles der Totalendoprothese lagert. Die Rückseite 3 dieser Prothese ist viereckig ausgeführt, wie die in Figur 2 dargestellt Draufsicht zeigt. Die Pfanne trägt an ihrer Seite Nuten 4. Auch in die Rückseite

sind Nuten und Vertiefungen eingearbeitet, die in Figur 2 mit 5 und 6 bezeichnet sind. Ein Teil dieser Nuten, in den Figuren 1 und 2 die mit 6 bezeichneten Nuten, weisen Hinterschneidungen auf. Derartige Nuten, Riefen oder anderweitige Vertiefungen dienen dazu, die Pfanne durch einwachsendes Gewebe fest im angrenzenden Knochenraum zu verankern. Zur Beschleunigung dieses Einwachsprozesses können diese Nuten, Riefen und Vertiefungen während der Operation und vor dem Einsetzen der Pfanne in den Hüftknochen mit Knochenspänen vom gleichen Patienten gefüllt werden.
Bei den vier-, fünf- oder sechseckigen Pfannen wird mindestens eine Ecke an der Vorderseite der Pfanne abgeschrägt, um ein Scheuern des Iliopsoas zu vermeiden. Diese Abschrägung ist für eine viereckige Pfanne im oberen Teil der Figur 1 mit 7 bezeichnet.
Die vorstehend beschriebene erfindungsgemäße Pfanne weist gegenüber den bisher bekannten Pfannen aus AIpO- - Keramik den Vorteil auf, daß sie ohne besonderes Instrumentarium eingesetzt werden kann. Der für die Pfanne im Hüftknochen benötigte Raum läßt sich mit den in der Orthopädischen Chirurgie üblicherweise gebräuchlichen Werkzeugen ausarbeiten und vorbereiten. Implantationen von Pfannen dieser Art in Großtiere,z. B. Schafe, hatejgezeigt, daß diese Pfannen unmittelbar nach der Implantation

einen so festen Sitz haben, daß sie auch bei sofortiger Belastung des Gelenkes nicht locker werden, sondern mit dem umliegenden Knochengewebe eine mechanisch feste Verbindung entsteht.

    Ci)) Pfanne für TotaIhUftgelenkendoprothesen aus Al 0 - Keramik für die zementfreie Implantation, dadurch gekennzeichnet., daß die Pfanne drei-, vier-, fünf- oder sechseckig ausgeführt ist und auf der Rückseite Nuten und Vertiefungen zum Einwachsen von Knochengewebe aufweist.
  2. 2) Pfanne nach Anspruch 1, dadurch gekennzeichnet, daß die vier-, fünf- oder sechseckigen Pfannen über mindestens eine der Ecken an der Vorderseite eine Abschrägung aufweisen.
