Description

Die Erfindung betrifft ein Instrumentarium zum Einsetzen der Gelenkpfanne einer Hüftgelenkprothese, welche Gelenkpfanne parallel gerichtete Zapfen aufweist, die ihre genaue Lage im Beckenknochen festlegen und sichern. Hüftgelenkprothesen mit Gelenkpfannen dieser Art sind bekannt, z. B. aus der FR-PS 48 955 oder der US-PS 29 10 978.
Es sind auch Instrumentarien zum Einsetzen von Gelenkpfannen der obenerwähnten oder anderer Art bekannt. Sie weisen im allgemeinen Häher mit Griffen auf, die ein Aufsetzen der Gelenkpfanne auf das dem Griff gegenüberliegende Ende des Halters gestatten (z.B. DT-Gbm 70 17851 und 71 13621). Mit diesen Haltern lassen sich die Gelenkpfannen in die vorbereiteten Ausfräsungen des Beckenknochens einsetzen und darin festhalten, bis der Zement sich genügend verfestigt hat. Es war jedoch nie dafür gesorgt, durch besondere Vorbereitungen am Beckenknochen die Lage der Gelenkpfanne von vornherein sehr genau zu bestimmen und hiermit sowie durch weitere Maßnahmen die Operation zu vereinfachen und zu beschleunigen.
Es ist das Ziel vorliegender Erfindung, ein besonderes Instrumentarium zum Einsetzen von Gelenkpfannen zu schaffen, die in an sich bekannter Weise parallel gerichtete Verankerungszapfen aufweisen, welches Instrumentarium eine Vereinfachung und Beschleunigung 6S der Operation bei großer Präzision des Einbaus der Gelenkpfanne gestattet.
Das erfindungsgemäße Instrumentarium ist gekennzeichnet durch eine Bohrlehre zum Bohren parallelachsiger Löcher im Beckenknochen zur Aufnahme der Zapfen der Gelenkpfanne., wobei die Bohrlehre ein kugelsektorähnliches Ende aufweist, dessen Durchmesser in etwa demjenigen der Ausnehmung für die Gelenkpfanne entspricht, und das Ende in Längsrichtung Bohrungen aufweist, durch welche mit Spiralbohrern die Löcher in den Beckenknochen gebohrt werden können, ferner gekennzeichnet durch ein Pfanneneinsetzgerät mit einer Griffstange, auf weiche die Gelenkpfanne mit ihren Zapfen parallel zur Griffstange dadurch aufsetzbar ist, daß das Pfanneneinsetzgerät ein kugelförmiges Ende aufweist, dessen Durchmesser demjenigen des Innendurchmessers der Gelenkpfanne entspricht.
Dank der Bohrlehre zum gleichzeitigen Bohren der erforderlichen parallelachsigen Löcher, vorzugsweise zweier Löcher, wird das Vorbereiten des Beckenknochens bei der Operation vereinfacht, rationalisiert und beschleunigt. Die Tatsache, daß ausschließlich parallelachsige Zapfen an der Gelenkpfanne und dementsprechend parallelachsige Löcher zur Aufnahme derselben im Beckenknochen erforderlich sind, erlaubt nicht nur die soeben erwähnte Rationalisierung beim Bohren der Löcher, sondern ergibt auch eine sofortige genaue Positionierung der Gelenkpfanne, sobald ihre Zapfen in die vorbereiteten Löcher eingeführt werden. Es kann also nicht nur sehr rasch und rationell, sondern auch präzis gearbeitet werden. Das Pfanneneinsetzgerät trägt weiter zur Vereinfachung und Beschleunigung der Operation bei.
Die Erfindung wird im folgenden an Hand eines Ausführungsbeispiels näher erläutert. In der Zeichnung zeigt

Die Form der Gelenkpfanne 1 gemäß F i g. 1 und 2 ist soweit als möglich derjenigen der natürlichen Pfanne angeglichen. Als besonderes Merkmal sind die beiden parallel gerichteten Zapfen 2 und 3 zu betrachten, die die Gelenkpfanne 1 in Löchern im. nicht dargestellten Acetabulum genau orientieren und fixieren. Die beiden Zapfen weisen je eine Nut 4 auf, durch welche beim Einzementieren der überflüssige Zement austreten kann. Die sternförmig angeordneten Rippen 5 und der Rand 17 dienen ebenfalls der Fixation der Lage der Gelenkpfanne 1 im Beckenknochen.
Vorzugsweise wird als Pfannenmaterial Metall, beispielsweise eine CR-CO-MO-Legierung, verwendet, und der nicht dargestellte Kugelkopf besteht aus Kunststoffmaterial, beispielsweise aus einem hochmolekularen Niederdruckpolyäthylen. Damit ist es ausgeschlossen, daß die Innenfläche 6 der Gelenkpfanne 1 durch den Gebrauch des Hüftgelenkes in irgend einer Weise beschädigt werden kann. Somit fällt jegliches Auswechseln der einzementierten Pfanne 1 mit all seinen Nachteilen, mit Ausnahme bei Infektionen, dahin.
In F i g. 3 und 4 ist die erfindungsgemäße Bohrlehre zum Bohren der Löcher im Acetabulum dargestellt. Sie weist ein kugelförmiges Ende 7 auf, dessen Form etwa der Form der Gelenkpfanne 1, diesmal aber ihrer äuße-
ren Form, entspricht Nach dem Einführen der Bohrlehre in das vorgefräste Acetabulum wird sie in die richtige Lage (45= zur Körperachse u :d 10 bis 20° Anteversion) gebracht und mit Hilfe der beiden kleinen Stifte 8 und 9 (axial verschiebbar, Spitzen 10 und 11), welche in das Acetabulum eingeschlagen werden, fixiert. Durch die Anschläge 12, 13 ist die Eindringtiefe der Spitzen genau bestimmt In dieser Stellung werden nun mit einem speziellen Spiralbohrer durch die Löcher C der Bohrlehre die beiden Löcher für die Aufnahme der Zapfen 2 und 3 der Gelenkpfanne 1 in das Acetabulum gebohrt Der Spiralbohrer weist ebenfalls einen Anschlag auf, damit die erforderliche Bohrtiefe genau eingehalten werden kann. Somit ist die Lage der Gelenkpfanne 1 genau vorbestimmt
Gegenstand der Erfindung ist ferner ein Gelenkpfanneneinsetzgerät 14 (F i g. 5), das ein kugelförmiges Ende 15 in Form der Gelenkpfanne 1 (Innenfläche 6) aufweist, auf welches Ende 15 die Gelenkpfanne 1 aufgespannt werden kann. Dies geschieht mittels eines e'astischen Halters 16, der sich über den Rand 17 der Gelenkpfanne 1 stülpt. Sobald sich die Gelenkpfanne 1, deren Außenseite vor dem Einsetzen in den Beckenknochen zwischen den Rippen 5 mit Zement gefüllt wird, in ihrer endgültigen Lape befindet, bestimmt durch die beiden Bohrungen im Acetabulum, wird der Griff 18 in Richtung des Beckenknochens, respektive gegen das Ende 15 hin gedrückt, so daß die Gelenkpfanne 1 fest ins Acetabulum gedrückt wird, was zur Folge hat, daß die beiden Zapfen 2 und 3 in die vorbereiteten Löcher des Beckenknochens gelangen. Hierauf wird der Teil 19 entgegen der Wirkung der Feder 20 an den Griff 18 herangezogen. Dadurch wird der elastische Halter 16 vom Ende 15 entfernt, so daß er den Rand 17 der Gelenkpfanne 1 freigibt. Die Gelenkpfanne 1 ist somit in das Skelett eingesetzt, das Einsetzgerät kann zurückgezogen werden.

Claims (3)

Patentansprüche:
1. Instrumentarium zum Einsetzen der Gelenkpfanne einer Hüftgelenkprothese, weiche Gelenkpfanne parallel gerichtete Zapfen aufweist, die ihre genaue Lage im Beckenknochen festlegen und sichern, gekennzeichnet durch eine Bohrlehre (F i g. 3) zum Bohren parallelachsiger Löcher im Beckenknochen zur Aufnahme der Zapfen (Z 3) der Gelenkpfanne (1), wobei die Bohrlehre ein kugelsektorähnliches Ende (7) aufweist, dessen Durchmesser in etwa demjenigen der Ausnehmung für die Gelenkpfanne (1) entspricnt, und das Ende in Längsrichtung Bohrungen (C) aufweist, durch welehe mit Spiralbohrern die Löcher in den Beckenknochen gebohrt werden können, ferner gekennzeichnet durch ein Pfanneneinsetzgerät (14) mit einer Griffstange, auf welche die Gelenkpfanne mit ihren Zapfen (2, 3) parallel zur Griffstange dadurch aufsetzbar ist, daß das Pfanneneinsetzgerät (14) ein kugelförmiges Ende (15) aufweist, dessen Durchmesser demjenigen des Innendurchmessers der Gelenkpfanne entspricht.
2. Instrumentarium nach Anspruch 1, dadurch gekennzeichnet, daß die Bohrlehre mit zwei Stiften (8,
9) im Beckenknochen in der richtigen Lage fixierbar ist.
3. Instrumentarium nach Anspruch 1, dadurch gekennzeichnet, daß das Pfanneneinsetzgerät (14) in der Nähe seines kugelförmigen Endes (15) einen federbelasteten Halter (16) aufweist, der in Längsrichtung verschiebbar auf der Griffstange angeordnet ist und mittels der Feder (20) gegen das kugelförmige Ende (15) gedruckt wird.
