Description

  • Instrumentarium zur Erstellung einer Hüftgelenkprothese Die vorliegende Erfindung betrifft ein Instrumentarium zur Erstellung einer Hüftgelenkprothese, bestehend aus einer Schenkelkopfprothese mit Schenkelschaft und Kugelkopf, und einer Gelenkpfanne.
  • Hüftgelenkprothesen haben ganz allgemein die Aufgabe, natürliche Hüftgelenke zu ersetzen, um dem Menschen mit einem erkrankten Hüftgelenk seine Bewegungsfreiheit zurückzugeben.
  • Seit dem Jahre 1948 sind Prothesen für den Ersatz des Hüftgelenkes bekannt. Vorerst wurde nur die Kugel am Schenkelhals ersetzt.
  • Erst später folgten beide Huftgelenksteile, das heisst, die Pfanne im Becken und die Kugel am Schenkelhals. Operationen, die nur den Ersatz der Gelenkkugel verlangen, wurden und werden auch heute noch mit einer Schenkelkopfprothese durchgeführt. Diese Schenkelkopfprothese weist dabei einen Stiel zum Einführen in die Medullarhöhle des Femurschaftes auf, wobei der Stiel in der Nähe seines einen Endes eine Verbreiterung mit einer Auflagefläche zum Anliegen auf dem Femoralhalsrest und an diese anschliessend einen Zapfen zur Aufnahme einer Gelenkkugel aufweist.
  • Die Gelenkpfanne wird üblicherweise in das Becken einzementiert.
  • Dabei ist es bereits bekannt, an der Aussenseite der Pfanne vorstehende Zapfen vorzusehen, welche in Vertiefungen im Beckenknochen eingreifen (BR-PS 1 170 295). Durch diese, in verschiedenen Richtungen ragenden Zapfen wird zwar der Halt der Pfanne erheblich verbessert, aber der Einbau der Pfanne wird eher erschwert, d.h., es ist schwierig, die Pfanne in einer genau vorbestimmten, den anatomischen Gegebenheiten und insbesondere der vorgesehenen Gestaltung der Kugelkopfprothese optimal angepasster Lage einzubauen.
  • Der Erfindung liegt nun die Aufgabe zugrunde, ein Instrumentarium zu schaffen, welches den Einbau der Gelenkpfanne erheblich erleichtert und zugleich eine sehr hohe Präzision in der Vorbestimmung der Lage derselben erlaubt. Das erfindungsgemässe Instrumentarium ist dadurch gekennzeichnet, dass die Gelenkpfanne zwei parallel gerichtete Zapfen aufweist, die ihre genaue Lage im Beckenknochen festlegen und sichern, und dass eine Bohrlehre zum Bohren parallelachsiger Löcher im Beckenknochen zur Aufnahme der Zapfen vorhanden ist. Die parallel gerichteten Zapfen erlauben die Verwendung einer sehr einfachen Bohrlehre, die in einem Arbeitsgange die erforderlichen Verankerungslöcher zu bohren gestattet, wobei durch die Ausrichtung der Bohrlehre während des Bohrens die spätere Lage der Gelenkpfanne genau vorbestimmt werden kann. Dank der Erstellung beider Verankerungslöcher in einem Arbeitsgang wird die Operationszeit in sehr erwünschter Weise verkürzt.
  • Die Erfindung wird im folgenden anhand eines Ausführungsbeispiels näher erläutert. In der Zeichnung zeigen Fig. 1 einen Schnitt durch eine Gelenkpfanne, Fig. 2 eine Aufsicht auf die Gelenkpfanne nach Fig. 1, Fig. 3 einen Schnitt durch eine Bohrlehre zum Bohren der beiden Löcher ins Acetabulum, in welche die Zapfen der Gelenkpfanne eingreifen, Fig. 4 eine Draufsicht auf die Bohrlehre nach Fig. 3 und Fig. 5 einen Schnitt durch ein Gelenkpfanneneinsetzgerät mit aufgesetzter Gelenkpfanne.
  • Die Form der erfindungsgemässen Gelenkpfanne 1 gemäss Fig. 1 und 2 ist soweit als möglich derjenigen der natürlichen Pfanne angeglichen. Als besonderes Merkmal sind die beiden parallel gerichteten Zapfen 2 und 3 zu betrachten, die die Gelenkpfanne 1 in Löchern im nicht dargestellten Acetabulum genau orientieren und fixieren. Die beiden Zapfen weisen je eine Nut 4 auf, durch welche beim Einzementieren der überflüssige Zement austreten kann.
  • Die sternförmig angeordneten Rippen 5 dienen ebenfalls der Fixation der Lage der Gelenkpfanne 1 im Beckenknochen.
  • Vorzugsweise wird als Pfannenmaterial Metall, beispielsweise eine Cr-Co-Mo-Legierung verwendet und der nicht dargestellte Kugelkopf besteht aus Kunststoffmaterial, beispielsweise aus einem hochmolekularen Niederdruckpolyäthylen. Damit ist es ausgeschlossen, dass die Innenfläche 6 der Gelenkpfanne 1 durch den Gebrauch des Hüftgelenkes in irgend einer Weise beschädigt werden kann. Somit fällt jegliches Auswechseln der einzementierten Pfanne 1, mit all seinen Nachteilen, mit Ausnahme bei Infektionen, dahin.
  • In Fig. 3 und 4 ist die erfindungsgemässe Bohrlehre zum Bohren der Locher im Acetabulum dargestellt. Sie weist ein kugelförmiges Ende 7 auf, dessen Form etwa der Form der Gelenkpfanne 1, diesmal aber ihrer äusseren Form, entspricht. Nach dem Einführen der Bohrlehre in das vorgefräste Acetabulum wird sie in die richtige Lage (450 zur Körperachse und 100 - 200 Anteversion) gebracht und mit Hilfe der beiden kleinen Stifte 8 und 9 (axial verschiebbar, Spitzen 10 und 11), welche in das Acetabulum eingeschlagen werden, fixiert. Durch die Anschläge 12, 13 ist die Eindringtiefe der Spitzen genau bestimmt. In dieser Stellung werden nun it einem speziellen Spiralbohrer durch die Löcher C der Bohrlehre die beiden Löcher für die Aufnahme der Zapfen 2 und 3 der Gelenkpfanne 1 in das Acetabulum gebohrt. Der Spiralbohrer weist ebenfalls einen Anschlag auf, damit die erforderliche Bohrtiefe genau eingehalten werden kann. Somit ist die Lage der Gelenkpfanne 1 genau vorbestimmt.
  • Weiter wird mit Vorteil ein Gelenkpfanneneinsetzgerät 14 (Fig. 5) verwendet, das ein kugelförmiges Ende 15 in Form der Gelenkpfanne 1, respektive deren Innenfläche 6 aufweist, auf welches Ende 15 die Gelenkpfanne 1 aufgespannt werden kann. Dies geschieht mittels eines elastischen Halters 16, der sich über den Rand 17 der Gelenkpfanne 1 stülpt. Sobald sich die Gelenkpfanne 1, deren Aussenseite vor dem Einsetzen in den Beckenknochen zwischen den Rippen 5 mit Zement gefüllt wird, in ihrer endgültigen Lage befindet, bestimmt durch die beiden Bohrungen im Acetabulum, wird der Griff 18 in Richtung des Beckenknochens, respektive gegen das Ende 15 hin gedrückt, so dass die Gelenkpfanne 1 fest ins Acetabulum gedrückt wird, was zur Folge hat, dass die beiden Zapfen 2 und 3 in die vorbereiteten Löcher des Beckenknochens gelangen. Hierauf wird der Teil 19 entgegen der Wirkung der Feder 20, an den Griff 18 herangezogen. Dadurch wird der elastische Halter 16 vom Ende 15 entfernt, so dass er den Rand 17 der Gelenkpfanne 1 freigibt.
  • Die Gelenkpfanne 1 ist somit in das Skelett eingesetzt, das Einsetzgerät kann zurückgezogen werden

Claims (8)

  1. PATENT#NSPRUECHE: 1) knstrumentarium zur Erstellung einer Hüftgelenkprothese, bestehend aus einer Schenkelkopfprothese mit Schenkelschaft und Kugelkopf, und einer Gelenkpfanne, dadurch gekennzeichnet, dass die Gelenkpfanne (1) zwei parallel gerichtete Zapfen (2, 3) aufweist, die ihre genaue Lage im Beckenknochen festlegen und sichern, und dass eine Bohrlehre (Fig. 3) zum Bohren parallelachsiger Löcher im Beckenknochen zur Aufnahme der Zapfen (2, 3) vorhanden ist.
  2. 2) Instrumentarium nach Anspruch 1, dadurch gekennzeichnet, dass die Bohrlehre ein kugelsektorähnliches Ende (7) aufweist, dessen Durchmesser in etwa demjenigen der Gelenkpfanne entspricht.
  3. 3) Instrumentarium nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Bohrlehre in Längsrichtung zwei Bohrungen (C) aufweist, durch welche mit einem Spiralbohrer die beiden Löcher in den Beckenknochen gebohrt werden.
  4. 4) Instrumentarium nach Anspruch 3, dadurch gekennzeichnet, dass die Bohrlehre mit zwei Stiften (8, 9) im Beckenknochen in der richtigen Lage fixiert werden kann.
  5. 5) Instrumentarium nach Anspruch 1, dadurch gekennzeichnet, dass ein Pfanneneinsetzgerät (14) vorgesehen ist, auf welches die Gelenkpfanne mit ihren Zapfen (2, 3) parallel zur Griffstange des Pfanneneinsetzgerätes aufsetzbar ist.
  6. 6) Instrumentarium nach Anspruch 5, dadurch gekennzeichnet, dass das Pfanneneinsetzgerät (14) ein kugelförmiges Ende (15) aufweist, dessen Durchmesser demjenigen des Innendurchmessers der Gelenkpfanne entspricht.
  7. 7) Instrumentarium nach Anspruch 5 oder 6, dadurch gekennzeichnet, dass das Pfanneneinsetzgerät (14) in der Nähe seines kugelförmigen Endes (15) einen federbelasteten Halter (16) aufweist, der in Längsrichtung verschiebbar auf einer Griffstange angeordnet ist.
  8. 8) Instrumentarium nach Anspruch 7, dadurch gekennzeichnet, dass der Halter (16) mittels einer Feder (20) gegen das kugelförmige Ende (15) gedrückt wird.
