Im Rahmen der Radiologie ist hierbei die interventionelle Radiologie anzusprechen, die wesentlich zur Entwicklung minimalinvasiver Techniken und hierfür notwendiger Geräte und Prothesen aus geeigneten Materialen beigetragen hat. So werden heute kleine Metallgitter als Gefaßendoprothesen, sog. Stents, sowohl von Kardiologen als auch von Radiologen in arteriosklerotische Gefäße eingesetzt, um diese offen zu halten.
Stents, beruhend auf einer Idee von Charles T. Dotter, sind in der Medizin seit 1969 als Gefaßendoprothesen bekannt. Die ersten Stents bestanden aus einer Spirale aus Edelstahl und wurden mittels eines Führungsdrahtes und eines geeigneten Katheters endovaskulär eingeführt. In einer Weiterentwicklung wurde später ein Stent aus Nitinol, einer speziellen Legierung mit „Memory^Verhalten, entwickelt, die, bei Raumtemperatur eingeführt, sich bei Körpertemperatur zur gewünschten, ursprünglich bei der Herstellung vorgegebenen, vollen Größe entfaltet. Während die ersten Einsätze als Tierversuche durchgeführt wurden, begannen in den achtziger Jahren die ersten klinischen Studien mittels einer elastischen selbstexpandierenden Spirale aus zwei Edelstahlbändern (doppelgängige Helix) von

0.10-0.15mm Dicke, die sich bis zu 35mm aufdehnen konnten. Aufgrund ihres großen Durchmessers konnten mit ihnen nur große Gefäße, wie Aorta oder Vena cava, behandelt werden. Da sie jedoch mittels eines relativ großen flexiblen Einführungsinstruments positioniert werden mussten, waren eine chirurgische Arteriotomie bzw. Venotomie hierfür erforderlich. Später wurden kleinere Spiralen (Durchmesser 10- 15mm) entwickelt, die ein kleineres Einführungskit benötigten und perkutan eingeführt werden konnten.
Die heutigen Stents bestehen in der Regel aus Spiralen oder Metallgitterstrukturen verschiedenster Bauart und werden perkutan mittels entsprechendem Katheter eingeführt.
Es lassen sich drei grundsätzliche Stenttypen unterscheiden, Stents mit thermischer Erinnerung (Memory Stents), ballonexpandierende Stents und mechanisch selbstexpandierende Stents. Die Memory Stents und die mechanisch selbstexpandierenden Stents werden auch in einer Gruppe als selbstexpandierende Stents zusammengefasst.
Hinsichtlich ihrer Anwendungsgebiete beschränkt sich die folgende Gegenüberstellung auf periphere Anwendungen und hierfür geeignete Stenttypen.
Stents mit thermischer Erinnerung (Memory Stents)
Diese „Memory Stents" oder genauer „Shape Memory Alloy Stents" bestehen aus Nitinol, einer Nickel-Titan-Legierung, die die Eigenschaft hat, eine Form, die bei hoher Temperatur vorgegeben wurde, nach Abkühlung und Wiedererwärmung auf Körpertemperatur wiederzuerlangen. Dadurch ist es möglich, den Stentdurchmesser für die Einführung in die Schleuse bzw. Gefäße zu reduzieren und dadurch kleinere, weniger traumatische Einführungsschleusen zu verwenden.
In größerem klinischen Umfang eingesetzt wurde dieser Stenttyp, der Rabkin Stent, mit Erfolg zunächst in den Bereichen der Arteria iliaca und renalis. Problematisch hinsichtlich des Offenhaltens der Gefäße war jedoch seine Verwendung in den Femoralsowie Poplitealgefäßen. Er besaß einen spiralförmigen Aufbau der Metallstruktur, die

perkutane Einführung und genaue Positionierung erwiesen sich jedoch als recht schwierig.
Eine Weiterentwicklung dieses Typs, der Cragg® Stent, mit zickzackformiger Metallstruktur, konnte bereits mit gutem Erfolg bei stenosierenden Läsionen der Iliakalgefäße eingesetzt werden. Dieser Stenttyp zeichnet sich aufgrund der aus Polypropylen hergestellten Verknüpfungen zwischen den einzelnen Maschen durch eine gute longitudinale Flexibilität aus und war damit wesentlich einfacher zu handhaben.
Ein weiterer, heute oft benutzter Stent, der Memotherm® Stent, ein bei der Herstellung mittels Laserstrahl aus einem Nitinolrohr herausgearbeiteter Stent mit rombenförmiger Gitterstruktur, zeichnet sich durch eine sehr hohe Flexibilität, sehr niedriger Komplikationsrate (Embolien) und einfache Handhabung aus. Erfolgreiche Anwendung findet er bei der Behandlung von stenosierenden Gefäßverschlüssen. Hauptanwendungsgebiete sind hierbei die Iliakal- und die Femoralgefäße. Außerdem ist hier auch der Symphony® Stent zu nennen, der dem Memotherm Stent ähnelt, aber in der Herstellung einfacher ist. Hierbei wird ein zickzackformiger Draht zu einer Gitterstruktur verschweißt.
Ballonexpandierende Stents
Stents dieses Typs werden auf einen Ballonkatheter montiert und im Gefäß auf den gewünschten Durchmesser expandiert. Die zu behandelnden Stenosen werden in der Regel vorab angioplastisch erweitert, um das Risiko während der Stenteinführung zu veringern. Nach durchgeführter Stentpositionierung wird der Ballonkatheter entfernt. Zur Zeit werden folgende Stenttypen in verstärktem Umfang benutzt.
Der Palmaz® Stent besteht aus Edelstahl mit einer zunächst rechteckigen Gitterstruktur ohne Schweißpunkte, die beim Einbau eine rombenförmig Gestalt annimmt. Stents dieses Typs sind nicht flexibel und können daher nicht in stark geschlängelten Gefäßen positioniert werden. Die Technik des „Cross-over", d.h. Vorschieben des Stents von einer Arterie iliaca über die Aortenbifurkation in die gegenüberliegende Arteria iliaca, ist mit diesem Stenttyp nicht möglich. Hauptanwendungsgebiete sind Stenosen der Arteria iliaca und renalis, seltener der Arteria femoralis und carotidea.

Der Strecker® Stent wurde erstmals 1988 bei der Behandlung von iliakalen Arterien bekannt. Er besteht aus einem einzigen, sehr engmaschigen dünnen Gitterwerk (0,10-015mm) aus Tantalum mit großer Elastizität und Flexibilität sowohl in radialer als auch longitudinaler Richtung. Er wird auf einem Ballonkather mittels einer sehr dünnen Silikonhülle befestigt. Die Anwendungsgebiete entsprechen denen des Palmaz Stents.
Mechanisch selbstexpandierende Stents
Eng zusammengedrückt mit Hilfe einer Kunstoff hülle, die während der Positionierung zurückgezogen wird, expandieren diese Stents langsam von selbst auf den vorherbestimmten Durchmesser. Gefaßstenosen werden abhängig von ihrer Lokalisation und Stenosegrad direkt gestentet oder ggf. vordilatiert. Der endgültige Durchmesser des aufgeweiteten Gefäßes wird erst nach Tagen ereicht in Abhängigkeit von der Verformbarkeit der Gefaßläsion und der radialen Ausdehnungskraft („Radial force") des Stents.
Zwei Stenttypen sind hier zu nennen, der Gianturco Expandable Wire Stent (GEWS)® oder auch Z-Stent und der Wallstent®. Der GEWS mit einer zickzackförmigen Gitterstruktur aus Edelstahl, Drahtdurchmesser 0,45mm, wurde zunächst bei der Behandlung von Verschlüssen der Vena cava eingesetzt. Eng zusammengedrückt wird er durch einen entsprechenden Katheter mit Teflonhülle eingeführt und dann positioniert durch Zurückziehen der Hülle. Die Expansionskraft hängt ab vom Durchmesser des Katheters, Anzahl und Form der Maschen sowie der Länge der Endoprothese. Wegen der relativ großen Steifigkeit erfolgt ein Einsatz dieser Stents im wesentlichen nur bei großkalibrigen Gefäßen wie Aorta und Vena cava. Der Wallstent, ein Stent aus Edelstahl mit rombenförmigen Maschen ohne Verschweißungen, zeichnet sich durch gute Zusammendrückbarkeit und guter longitudinaler Flexibilität aus. Er besitzt ein spezielles Einfuhrungskit und wird zur Zeit eingesetzt für periphere Gefäßstenosen (Arteria iliaca, renalis und carotidea). Als Folge seiner relativ großen radialen Ausdehnungskraft tendiert der Stent zur longitudinalen Verkürzung.

Die hier beschriebene Endoprothese, ein Doppelhelixstent, zeichnet sich dadurch aus, dass sie aufgrund ihrer besonderen Formgebung besonders flexibel ist und gleichzeitig eine sehr starke Radialkraft entwickelt. Dadurch ist sie besonders geeignet für den Einsatz in kleinen geschlängelten Gefäßen, wie zum Beispiel bei Coronargefäßen. Die Formgebung der einzelnen Helix ist dadurch ausgezeichnet, dass sich jedes Element der Helix sowohl in Quer- als auch in Längsrichtung verformen kann. In Längsrichtung sind die Doppelhelices an bestimmten Stellen beweglich miteinander verbunden.
Diese besondere Formgebung verleiht dem Stent eine hohe Beweglichkeit in Längsund Querrichtung, Anpassung an gekrümmte Hohlorgane und gleichzeitig eine hohe radiale Stabilität. Dabei weist der Stent aufgrund der Beweglichkeit in Längsrichtung eine im Vergleich zu anderen Stents geringere Verkürzung auf als herkömmliche Spiralstents.
Die besondere Formgebung von Stentanfang und -ende gewährleistet außerdem einen geschlossenen geraden Abschluss.

Stentform die sich auszeichnet durch in Längsrichtung hintereinandergeordnete Doppelhelices, die über Stentstreben an bestimmten Punkten miteinander verbunden sind.
Hierbei liegen jeweils zwei Heilixbänder spiegelbildlich einander gegenüber und sind über kurze Übergangsstücke gelenkförmig miteinander verbunden, siehe Abbildung. Die Doppelhelices bilden verformbare Rauten, die spiralförmig übereinander liegen. In Längsrichtung sind die Doppelhelices durch doppel-S ähnliche Verbindungselemente miteinander verbunden.
Diese Formgebung, siehe Fig. 1, gewährleistet, dass sich jedes Element der Doppelhelix sowohl in Quer- als auch in Längsrichtung verformen kann und sich der Stent insgesamt jeder Gefaßkrümmung anpassen kann.

Anmerkung:
Die Formgebung der Gitterstruktur ist dimensionsabhängig, d.h. bei unterschiedlichen Dimensionen (kürzer oder länger, kleiner oder größerer Durchmesser) ergeben sich dimensionsabhängige Veränderungen, ohne dass das grundsätzliche Konstruktionsprinzip verändert wird.
Diese besondere Formgebung verleiht dem Stent eine hohe Beweglichkeit in Längsund Querrichtung, Anpassung an gekrümmte Hohlorgane und gleichzeitig eine sehr hohe radiale Stabilität durch Ausbildung von Rauten im entfaltetem Zustand. Dabei weist der Stent aufgrund der Beweglichkeit in Längsrichtung eine im Vergleich zu anderen Stents geringere Verkürzung auf als bei herkömmlichen Spiralstents. Die doppel-S ähnlichen Verbindungsstücke schmiegen sich im zusammen gedrücktem Zustand in gebogener Form schräg zwischen die Elemente der Doppelhelix, so dass sich

der Stent optimal und enger auf den Ballonkatheter montieren lässt, wobei jedoch die Beweglichkeit erhalten bleibt.
Der Stent besteht aus einer Gitterstruktur, die aus einem Kreiszylinder mit einem Durchmesser von 1,0-30 mm (je nach Einsatzort) mittels Lasertechnik geschnitten wird. Der Kreiszylinder kann aus verschiedensten Materialen bestehen, wie zum Beispiel Edelstahl, Tanatalum, Nitinol, Kupfer-Goldlegierung, anderen Metallen, Legierungen, Verbundwerkstoffen, Polylactiden und anderen Polymeren. Die Wandstärke des Zylinders variiert zwischen 0,06 und 0,4 mm (je nach Einsatzort). Die Länge der Endoprothese variiert zwischen 0,5 cm und 15 cm (je nach Einsatzort).
Zur Verbesserung der Bioverträglichkeit können die Implantate beschichtet werden. Als Beschichtung kommen in Frage Polymere wie z.B. Parylene oder Edelmetalle u. deren Legierungen.
Claims (46)
1. Stent für den Einsatz im menschlichen Körper, dessen Formgebung dadurch gekennzeichnet ist, dass der Stent aus in Längsrichtung hintereinandergeordnete Doppelhelices, die über Stentstreben an bestimmten Punkten miteinander verbunden sind, besteht. Hierbei liegen jeweils zwei Heilixbänder spiegelbildlich einander gegenüber und sind über kurze Übergangsstücke gelenkförmig miteinander verbunden. Die Doppelhelices bilden verformbare Rauten, die spiralförmig übereinander liegen. In Längsrichtung sind die Doppelhelices durch doppel-S ähnliche Verbindungselemente miteinander verbunden.
