Abstract
Patentgegenstand ist ein neuartiger Wundverband, bestehend aus einem flexiblen und mit einer Haftschicht versehenen folienartigen Material, welches eine integrierte Vorrichtung zur Erzeugung eines Unterdruckes bzw. zum Absaugen von Wundsekret hat.
Description
Die Aufgabe eines Wundverbandes besteht im Schutz der Wunde vor Verschmut
zung und Kontamination von außen und Gestaltung eines für die Wundheilung
günstigen Milieus. Dabei ist es günstig, wenn überschüssige Wundsekretionen
aus der Wunde entfernt werden, da diese durch Substratakkumulation einen guten
Nährboden für eine Infektion darstellen und Wundinfektionen fördern. - Eine wei
tere Funktion eines Wundverbandes ist es dabei, die Wunde möglichst vor chemi
schen und mechanischen Einflüssen zu schützen und andererseits eine Ver
schmutzung der Umgebung durch Wundsekret zu verhindern. -
Eine sehr große Anzahl unterschiedlicher Wundverbände wurden dabei im Laufe
der Zeit entwickelt, angefangen von osmotisch aktiven Verbänden, wie Honig oder
Perubalsam, die dann evtl. mit einem saugfähigen Material abgedeckt wurden, bis
hin zu dem derzeit wohl modernsten Konzept der Wundabdeckung mit einer semi
permeablen Membran, die mit einem Adhäsiv versehen ist. Nachteile der absor
bierenden Verbände ist dabei die Tatsache, daß diese Verbände die Wunde aus
trocknen können, was für die Reepithelisierung und Wundheilung von Nachteil ist.
Im Gegensatz dazu haben semi-permeable oder feuchtigkeits-undurchlässige Ver
bände den Vorteil, daß sie eine Reepithelisierung der Wunde fördern, andererseits
kommt es oft zu der oben erwähnten Akkumulation von Wundsekret mit darauffol
gender Infektionsbildung. Eine in jüngerer Zeit angewandte Methode der Wund
behandlung löst dieses Problem durch Abdecken der Wunde mit einer Membran
mit darunterliegendem Saugschlauch, der entweder durch einen Stichkanal durch
gesunde Haut oder aber zwischen der Haut und der Membran von der Wunde
wegführt. Diese Methode, bei der die Wundheilung wahrscheinlich außerdem
auch durch den auf die Wunde einwirkenden Unterdruck gefördert wird, hat jedoch
den Nachteil, daß entweder gesundes Gewebe verletzt und dem Risiko einer Infek
tion ausgesetzt wird, oder bei Ableitung zwischen der r Haut und dem Wundver
band ein luft- und sekretdichter Abschluß meistens nur extrem schwer zu errei
chen ist.
Dieses Problem wird erfindungsgemäß nach Anspruch 1 dadurch gelöst, daß eine
Absaugung für Wundsekret in den Verband integriert wird. Durch einen Ventilme
chanismus kann außerdem ein Reflux des Wundsekrets mit aufsteigender Infektion
verhindert werden, während der Drainagebehälter gewechselt wird.
Darüber hinaus besteht nach Anspruch 8 auch die Möglichkeit, in den Verband
auch einen kompletten Absaugmechanismus für Wundsektret zu integrieren.
Zu weiteren Ausgestaltungen und Merkmalen der Erfindung wird auf die übrigen
Unteransprüche sowie auf die nachfolgende Beschreibung und auf die zugehöri
gen Zeichnungen von erfindungsgemäßen Ausführungsmöglichkeiten bezugge
nommen. In den Zeichnungen zeigen, und zwar jeweils in einem Längsschnitt:
Fig. 1 den prinzipiellen Aufbau der Erfindung und seine typische Anwendungs
applikation mit externem Absaugsystem.
Fig. 2 den prinzipiellen Aufbau des Verbandes mit komplett integrierter Absau
gung von Wundsekret ohne externes Absaugsystem.
Der Wundverband besteht aus einer luftdichten Membran 1, die an ihrer Unter
seite mit einer Adhäsivschicht oder einem Gel 2 versehen ist. In der Mitte dieser
Membran befindet sich ein vorzugsweise aus einem weichen Elastomer gefertigter
Drainagekörper 3, welcher an seiner Unterseite multiple Perforationen 4 aufweist.
Beide Elemente 1 und 3 sind miteinander funktionell fest verbunden. Die Membran
ist mit dem Adhäsiv 2 auf der Hautoberfläche 5 luftdicht befestigt. Wundsekret 6,
welches sich unter der Membran ansammelt tritt durch die Perforationen 4 in den
Drainagekörper 3 ein. Über das Ausleitungsstück 10, welches ein integriertes
Ventil 7 haben kann, ist der Drainagekörper 3 mit einem Schlauch 8 mit einem ex
ternen, nicht dargestellten Unterdrucksystem verbunden. Dieses besteht typi
scherweise aus einem Reservoir für Wundsekret und einer regelbaren Unter
druckpumpe oder aus einem Unterdruck erzeugenden Behälter.
Bei der Anwendung wird der beschriebene Wundverband über der Wundfläche 9
angebracht, und randdicht mit der Hautoberfläche 5 verklebt. Nach Anschluß an
eine Unterdruckquelle wird Wundsekret 6 durch die Perforationen 4, den Draina
gekörper 3, das Ausleitungsstück 10, sowie das Ventil 7 und den Schlauch 8 gezo
gen. Bei Ausfall des Unterdrucks oder bei Wechsel des Schlauches 8 verhindert
das Ventil 7, daß Wundsekret oder Luft in die Wunde zurückgezogen wird.
Fig. 2. zeigt ein Beispiel für einen Wundverband mit komplett integrierter Absau
gung wobei jedoch der Unterdruck nicht wie in Fig. 1 durch eine externe Saugung
erzeugt wird. Der Unterdruck kommt hierbei durch die Expansionstendenz eines
zuvor komprimierten Schwammes zustande.
Über der Membran 1, die im Zentrum eine Öffnung 20 besitzt, befindet sich ein
vorkomprimierter Schwamm 21, der von einer expandierbaren luftundurchlässi
gen Membran 22 umgeben ist. Unter der Öffnung 20 ist eine weitere wasserlösli
che Membran 23, vorhanden, welche eine vorzeitige Expansion des Schwammes
vor der Anwendung verhindert. Nach Aufbringen des Wundverbandes auf die
Wunde 9 löst sich die wasserlösliche Membran 23 auf, wodurch der durch den
Schwamm 21 erzeugte Unterdruck auf die Wundfläche wirken kann. Gleichzeitig
wird Wundsekret 6 in den Schwamm 21 hinein gesaugt.
In einem ähnlichen, nicht dargestellten Ausführungsbeispiel erfolgt die Kompres
sion des Schwammes 21 erst bei der Applikation des Verbandes, so daß die Ver
wendung einer löslichen Membran 9 nicht nötig ist. Hierbei wird beim Anlegen
des Verbandes der Schwamm dadurch komprimiert, dass er über der Wundfläche
durch Druck auf die expandierbare Membran 22 zusammengedrückt wird.

Claims (11)
1. Verband bestehend aus einer flexiblen, luftundurchlässigen mit einem
Adhäsiv 2 versehenen elastischem membranartigen Material 1, dadurch gekenn
zeichnet, daß dieser Verband in seinem Zentrum oder auch peripher mit einer in
tegrierten Vorrichtung für Drainage des Wundsekretes oder zur Erzeugung eines
Unterdruckes versehen ist, bzw. über diese an ein peripheres Absaugsystem ange
schlossen werden kann.
2. Wundverband nach Anspruch 1, dadurch gekennzeichnet, daß das
Drainagesystem mit einem Ventil 7 versehen ist.
3. Wundverband nach Anspruch 1 und 2, dadurch gekennzeichnet, daß
die Adhäsion zur Wundfläche durch eine Gelschicht erzeugt wird, die ein leichte
res Entfernen desselbigen ohne Schädigung des Epithels erlaubt.
4. Wundverband nach Anspruch 1 bis 3, dadurch gekennzeichnet, daß
das Drainagesystem mit einer gerinnungshemmenden Substanz versehen ist
und/oder die Wände eine besondere Beschichtung aufweisen, welche Anhaften von
Partikeln verhindert.
5. Wundverband nach Anspruch 1 bis 4, dadurch gekennzeichnet, daß
der Wundverband mit 2 oder mehr Drainagesystemen versehen ist, die unterein
ander verbunden sein können.
6. Wundverband nach Anspruch 1 bis 5, dadurch gekennzeichnet, daß
er an der zur Wunde liegenden Seite in seinem Zentrum mit einem porösen oder
absorbierenden Material verbunden ist.
7. Wundverband nach Anspruch 1 bis 6, dadurch gekennzeichnet, daß
er aus mehreren Teilen besteht, die erst kurz vor der Applikation vom Anwender
zusammengebaut werden.
8. Wundverband nach Anspruch 1 bis 7, dadurch gekennzeichnet, daß
die gesamte Saugvorrichtung in den Verband integriert ist, z. B. in Form eines
vorkomprimierten, sich expandierenden Schwamm, welcher gegebenenfalls von
einer wasserlöslichen Membran 23 im komprimierten Zustand gehalten werden
9. Wundverband nach Anspruch 1-8, dadurch gekennzeichnete daß der
Ableitungsteil 10 über einen Ansatzstutzen zur Applikation von therapeutisch
wirksamen Substanzen wie Medikamenten oder Spülflüssigkeiten verfügt.
10. Wundverband nach Anspruch 1-9, dadurch gekennzeichnet, daß
der Verband ein mechanisches federndes Element enthält, welches einen Un
terdruck erzeugt. (Saugglockenprinzip).
11. Wundverband nach Anspruch 1-9, dadurch gekennzeichnet, daß
der Verband ein mechanisch stabiles Element enthält, welches einer Anlegung an
die Wundoberfläche eine Kraft entgegensetzt wodurch eine Formgebung der
Wunde ermöglicht wird.
