Description
Die Erfindung geht aus von einem Endoskop nach dem Oberbegriff des Patentanspruches 1.
Ein derartiges Endoskop ist aus der DE 93 02 891 U1 bekannt. Das bekannte Endoskop
dient als Uretero-Renoskop, bei welchem Schaftquerschnitte zum Einsatz kommen, die
möglichst großlumige Spül- und Instrumentenkanäle realisierbar machen. Ferner ist der
Schaft des Renoskopes starr auszubilden. Die im Schaft vorhandenen Kanäle werden
durch separate ineinanderliegende Rohre gebildet.
Aus der US 4,263,897 ist ein Endoskop mit einer Kanüle bekannt, in deren Hohlraum ein
Lichtleiter sich erstreckt, der durch eine flüssigkeitsdichte Durchführung von außen in
den Kanülenhohlraum geführt ist. Mit Hilfe einer Zuleitung wird im rechten Winkel zur
axialen Ausdehnung der Kanüle eine Fusionsflüssigkeit zugeleitet. Das bekannte Endo
skop dient zur Untersuchung von Körperhöhlen, wie Harnblase oder Gebärmutter. Für
die Anwendung in engen Gewebekanälen des Körpers, wie beispielsweise den Tränen
kanal eines Auges, ist ein derartiges Endoskop nicht geeignet.
Aus der DE 36 21 053 A1 ist ein Okularendoskop mit elliptischem Querschnitt der En
doskopsonde bekannt, um die Kontur der Sonde einem vom Chirurgen gemachten
Schnitt am Auge anzupassen. In der verformbaren halbsteifen Endoskopsonde werden
ein erstes Bündel optischer Fasern zur Beleuchtung eines zu untersuchenden Berei
ches und ein zweites Bündel optischer Fasern zur Betrachtung des beleuchteten Berei
ches geführt. Durch Zusetzen des vorderen Endes der Endoskopsonde kann die Bild
qualität beeinträchtigt werden.
Der Einsatz eines Endoskops ist in vielen Anwendungsfällen begrenzt durch den Au
ßendurchmesser der Kanüle. Der Außendurchmesser der Kanüle wird bestimmt durch
die im Hohlraum der Kanüle geführten Komponenten, wie Lichtleiter und Laserstrah
lungsleiter, welche für die Diagnostik und Therapie zum Einsatz kommen.
Aufgabe der Erfindung ist es, ein Endoskop zu schaffen, bei dem der Außendurchmes
ser der Kanüle auf Bruchteile eines Millimeters, insbesondere bis zu 0,3 mm, bemessen
werden kann, wobei von der zu untersuchenden Körperstelle über den durch den Kanü
lenhohlraum geführten Lichtleiter eine gute Bildqualität vermittelt wird.
Diese Aufgabe wird erfindungsgemäß durch die kennzeichnenden Merkmale des Pa
tentanspruches 1 gelöst.
Der Lichtleiter, welcher durch den Hohlraum der Kanüle geführt ist, ragt bis zum vorde
ren Ende der Kanüle, um in einem Betrachtungsfeld, das durch die Spülflüssigkeit frei
gehalten wird, von den zu untersuchenden Körperstellen Bildinformationen für Bildauf
zeichnungsgeräte, beispielsweise Kamera, Monitor und dergl., weiterzuleiten.
Durch die ständige Umspülung des Lichtleiters wird ein Zusetzen des Lichtleiters, insbe
sondere an seinem vorderen Ende, vermieden. Das Betrachtungsfeld wird für die Aus
leuchtung des von Lichtfasern zugeleiteten Lichts und für die Erfassung von Bildinforma
tionen durch die Bildfasern infolge der Spülwirkung freigehalten. Es ist nicht erforderlich,
daß für die Spülflüssigkeit eine zusätzliche Spülflüssigkeitsleitung im Kanüleninneren
vorgesehen ist. Es reicht aus, wenn der im Kanülenhohlraum frei gebliebene Raum für
das Hindurchströmen der Spülflüssigkeit ausgenützt wird.
Dies ist insbesondere dann von Bedeutung, wenn zusätzlich zu dem Lichtleiter für eine
Therapie ein Laserstrahlungsleiter durch den Kanülenhohlraum zum vorderen Ende der
Kanüle geleitet wird. Der die Licht- und Bildfasern enthaltende Lichtleiter und der Laser
strahlungsleiter haben einen etwa kreisrunden Umfang. Der im zylindrischen Hohlraum
verbleibende restliche Raum wird dann von der Spülflüssigkeit durchströmt, wodurch
das schon erwähnte Zusetzen des Lichtleiters und des Laserstrahlleiters, insbesondere
an deren vordere Enden, welche zur Übertragung von Licht- und Bildinformationen ei
nerseits und zur Behandlung von Körpergewebe andererseits eingesetzt werden, ver
mieden wird. Bei der Spülflüssigkeit kann es sich um eine isotonische Kochsalzlösung,
z. B. 0,9%-ige physiologische NaCl-Lösung, handeln.
Bei der Erfindung ist es möglich, ein Mikroendoskop zu schaffen, das einen Außen
durchmesser von 0,3 oder 0,5 mm hat, wenn das Endoskop für den diagnostischen Be
reich eingesetzt wird. Für den Lichtleiter können in herkömmlicher Weise zu einem
hochflexiblen Bündel geordnete Bild- und Lichtfasern und für den Laserstrahlungsleiter
ebenfalls herkömmliche Laserstrahlfasern, beispielsweise Laser-Saphir-Fasern ver
wendet werden.
In bevorzugter Weise wird der Lichtleiter durch ein komprimierbares Dichtungsmittel in
den Kanülenhohlraum hindurchgeführt. Das Dichtungsmittel kann beispielsweise ein aus
elastischem Material, z. B. Silikongummi bestehender Stopfen sein, welcher im entspann
ten Zustand einen Einfädelkanal für den Lichtleiter freiläßt und welcher im komprimierten
Zustand einen Quetschverschluß bildet und den Lichtleiter flüssigkeitsdicht umfaßt und
im Kanülenhohlraum fixiert.
Für die reine Diagnoseanwendung ist es von Vorteil, den Lichtleiter in axialer Richtung in
den Kanülenhohlraum einzusetzen. Die Spülflüssigkeit wird seitlich zugeführt und tan
gential oder im spitzen Winkel in den Kanülenhohlraum eingeleitet. Die seitliche Zufüh
rung der Spülflüssigkeit wird auch dann angewendet, wenn zusätzlich zum Lichtleiter ein
Laserstrahlungsleiter in den Kanülenhohlraum eingesetzt ist.
Bei Verwendung des zusätzlichen Laserstrahlungsleiters wird dieser in vorteilhafter Wei
se axial in den Kanülenhohlraum eingesetzt. Der Laserstrahlungsleiter kann für die the
rapeutische Behandlung in axialer Richtung in den Kanülenhohlraum verschiebbar sein.
Der Lichtleiter wird bei diesem Ausführungsbeispiel ebenfalls seitlich in den Kanülen
hohlraum eingeleitet. Die Einleitung erfolgt ebenfalls im spitzen Winkel bzw. tangential
zur axialen Richtung des Kanülenhohlraums.
Ein bevorzugtes Anwendungsgebiet des Endoskops ist die Diagnose und Therapie in
engen Gewebekanälen des Körpers. Eine bevorzugte Anwendung findet das Endoskop
bei der Diagnostik und Therapie am Auge, insbesondere in den Tränenwegen des Au
ges sowie auch im Schlemmschen Kanal. Das Kanülenmaterial besteht in bevorzugter
Weise aus V2A-Stahl. Zur Anpassung an den Gefäß- bzw. Kanalverlauf im Körpergewe
be kann die Kanüle biegbar ausgebildet sein.
Anhand der Figuren wird an Ausführungsbeispielen die Erfindung noch näher erläutert.

Für das Einleiten der Spülflüssigkeit durch die Spülleitung 12 ist am äußeren Ende der
Spülleitung 12 ein Bajonettverschlußstück 17 vorgesehen. Dieses Bajonettverschluß
stück kann Teil eines Luer-Locks sein. Über diesen Anschluß erfolgt die Verbindung mit
dem Vorratsbehälter für die Spülflüssigkeit.
Bei dem in der Fig. 2 dargestellten Ausführungsbeispiel ist im Kanülenhohlraum 4 fer
ner ein Laserstrahlungsleiter 7 vorgesehen. Der Laserstrahlungsleiter ist in axialer
Richtung durch eine Verschlußeinrichtung 8 in das Innere der Kanüle 1 eingeführt. Der
Laserstrahlungsleiter 7 erstreckt sich bis zum vorderen Ende der Kanüle 1. Der Laser
strahlungsleiter 7 kann in axialer Richtung im Kanülenhohlraum 4 verschoben werden,
so daß er in das Kanüleninnere zurückgezogen werden kann und auch außerhalb des
Kanülenhohlraums am vorderen Ende der Kanüle 1 zum Einsatz gebracht werden kann.
Der Laserstrahlungsleiter 7 ist an eine Laserstrahlquelle gekoppelt und überträgt deren
Strahlung auf das vordere Ende des Laserstrahlungsleiters 7. Mit Hilfe des vom vorde
ren Ende des Laserstrahlungsleiters 7 abgestrahlten Strahlung können chirurgische und
therapeutische Behandlungen an den jeweiligen Körperstellen vorgenommen werden.
Das Beobachtungs- und Behandlungsfeld an der vorderen Spitze der Kanüle werden
durch die Spülflüssigkeit freigehalten.
In bevorzugter Weise kommt das Endoskop als diagnostisches und therapeutisches
Mittel in Gewebekanälen und Gefäßen, insbesondere in den Tränenwegen des Auges,
zum Einsatz. Es kann auch in engen Gefäßen für die Diagnostik und Therapie zum Ein
satz kommen, beispielsweise im Schlemm-Kanal am Auge.
Bei dem in der Fig. 2 dargestellten Ausführungsbeispiel ist am rückwärtigen Kanüle
nende 11 ein dem Bajonettverschlußstück 17 in der Fig. 1 entsprechendes Verschluß
stück vorgesehen. Auf dieses Verschlußstück kann ein erstes Verschlußteil 9 wie bei
einem Bajonettverschluß in axialer Richtung fixiert werden. Am ersten Verschlußteil 9 ist
ein zweites Verschlußteil 10 in axialer Richtung verschiebbar geführt. Die Verschiebung
kann in Stufen erfolgen, wozu entsprechende Arretiereingriffe 15 am ersten Verschluß
teil und am zweiten Verschlußteil 10 vorgesehen sind. Der Laserstrahlungsleiter 7 ist in
eine axiale mittlere Bohrung des zweiten Verschlußteiles 10 fest eingesetzt. Bei der
axialen Verschiebung des zweiten Verschlußteiles 10 gegenüber dem an der Kanüle 4 in
axialer Richtung festgelegten ersten Verschlußteil 9 wird somit eine axiale Verschiebung
des Laserstrahlungsleiters 7 in der Kanüle 4 in axialer Richtung erreicht.
Bei dem in Fig. 1 dargestellten Ausführungsbeispiel ist eine Kanüle 1 vorgesehen.
Durch einen Hohlraum 4 der Kanüle 1 ist in axialer Richtung ein Lichtleiter 2 hindurchge
führt. Der Lichtleiter 2 ragt bis zum vorderen Ende der Kanüle 1. Am rückwärtigen Kanü
lenende 11 ist der Lichtleiter 2 durch eine Durchführung 3 hindurchgeführt, welche im
einzelnen in Fig. 3 dargestellt ist. Der Lichtleiter 2 kann an ein geeignetes Aufzeich
nungsgerät, wie beispielsweise einen Monitor, eine Kamera, in bekannter Weise ange
koppelt werden.
Seitlich endet in den Kanülenhohlraum 4 eine Spülleitung 12. Durch die Spülleitung 12
wird in den Kanülenhohlraum eine Spülflüssigkeit, beispielsweise isotonische Kochsalz
lösung, eingeleitet. Die Spüllösung strömt durch den im Kanülenhohlraum verbliebenen
Raum zur vorderen Spitze der Kanüle 1 und umspült das vordere Ende des Lichtleiters
2. Hierdurch wird ein Zusetzen des vorderen Endes der Kanüle 1 im Bereich des vorde
ren Endes des Lichtleiters 2 auch bei geringen Durchmessern der Kanüle des Kanülen
hohlraums und des Lichtleiters verhindert. Der Lichtleiter kann somit Bildinformationen
ständig dem angeschlossenen Bildaufzeichnungsgerät zuleiten. Beim dargestellten
Ausführungsbeispiel mündet die Spülleitung 12 im spitzen Winkel zur axialen Ausdeh
nung der Kanüle 1 in den Kanülenhohlraum 4 ein.
Um zu verhindern, daß Spülflüssigkeit am hinteren Ende der Kanüle austritt, ist die
Durchführung 3 flüssigkeitsdicht ausgebildet. Wie insbesondere die Fig. 3 zeigt, besitzt
das hintere Kanülenende 11 eine zylindrische Fassung 13, in die ein Dichtungsmittel 5,
beispielsweise ein Silikongummistopfen, eingesetzt ist. Der Silikongummistopfen hat im
entspannten Zustand einen axialen Kanal 14, durch welchen der Lichtleiter 2 in den Ka
nülenhohlraum 4 eingeführt ist. Am Außenumfang der Fassung 13 befindet sich ein
Außengewinde, auf welches ein Innengewinde der Feststellschraube 6 aufschraubbar
ist. An der Innenseite besitzt die Feststellschraube 6 einen axial sich erstreckenden Vor
sprung 16, der beim Festziehen der Feststellschraube das Dichtungsmittel 5 kompri
miert, so daß der Lichtleiter 2 im Kanal 14 eingequetscht wird. Auf diese Weise erreicht
man einen Quetschverschluß, der flüssigkeitsdicht ist. Dieser Quetschverschluß kann
jedoch durch Lösen der Feststellschraube 6 wieder gelöst werden, so daß der Lichtleiter
in axialer Richtung bewegt bzw. entfernt oder wieder eingesetzt werden kann. Der
Lichtleiter 2 ragt durch eine Bohrung im Vorsprung 16 und eine Öffnung in der Feststell
schraube 6, so daß er, wie schon erläutert, an ein Aufzeichnungsgerät angekoppelt wer
den kann.
Der Lichtleiter 2 wird bei dem Ausführungsbeispiel der Fig. 2 ebenfalls seitlich in den
Kanülenhohlraum 4 eingeführt. Dies kann unter spitzem Winkel oder tangential zur
axialen Richtung des Kanülenhohlraumes 4 geschehen. Zur Führung des Lichtleiters 2
ist eine Lichtleiterführung 18 vorgesehen. Diese Lichtleiterführung hat, wie beim Ausfüh
rungsbeispiel der Fig. 1, eine flüssigkeitsdichte Durchführung 3, die ebenfalls zur Fixie
rung des Lichtleiters 2 in der Kanüle 4 dient.
Der Lichtleiter weist für die Belichtung des auszuleuchtenden und zu untersuchenden
Raumes Lichtfasern, beispielsweise 200 Lichtfasern, auf. Ferner sind zur Bildübertra
gung des ausgeleuchteten Raumes bzw. Gewebes Bildfasern im Lichtleiter 2 vorgese
hen. Es können hierzu 6000 Bildfasern zum Einsatz kommen. Die Lichtfasern und die
Bildfasern sind zu einem hochflexiblen Bündel mit kreisförmigem Querschnitt geordnet
und durch den Kanülenhohlraum 4 hindurchgeleitet. Zur Ausleuchtung des Betrach
tungsfeldes wird in bevorzugter Weise Xenonlicht über die Lichtfasern dem Betrach
tungsfeld zugeleitet. Die Bildinformationen werden von den Bildfasern an Aufzeich
nungsgeräte (Kamera, Monitor und dergl.) geleitet. Es ist auch möglich, die Bilder bei
spielsweise mit Hilfe eines Recorders aufzuzeichnen und zu dokumentieren. Mit Hilfe
eines Videoprinters können Ausdrucke von Einzelbildern gemacht werden. Auch digitale
Archivierungssysteme können angeschlossen werden.
Insbesondere die in der Fig. 1 dargestellte Ausführungsform kann auch zum Absaugen
von Gewebe verwendet werden. Hierzu wird der Lichtleiter 2 aus der Kanüle 1 entfernt,
nachdem die Feststellschraube 6 gelockert worden ist und das Dichtungsmittel 5 sich
entspannt hat. Nach dem Entfernen des Lichtleiters 2 wird die Feststellschraube 6 wie
der festgezogen, so daß der axiale Kanal 14 im Dichtungsmittel 5 beim Komprimieren
wieder verschlossen wird. Der Kanülenhohlraum 4 ist dann am hinteren Ende wieder
abgedichtet. Durch Anschluß einer Saugvorrichtung an das Bajonettverschlußstück 17
im Ausführungsbeispiel der Fig. 1 kann dann durch den Kanülenhohlraum und die
Spülleitung 12 ein Absaugen erfolgen.
Bei dem in der Fig. 2 dargestellten Ausführungsbeispiel wird der Laserstrahlungsleiter
7, welcher bevorzugt aus einer Saphir-Laserfaser besteht, zuerst in die Kanüle einge
setzt. Anschließend erfolgt das seitliche Einführen des Lichtleiters 2 durch die Lichtleiter
führung 18. Wie aus der Fig. 5 zu ersehen ist, verbleibt im Kanülenhohlraum 4 genü
gend Platz für das Durchströmen des Spülmittels.
An die Bildfasern kann in bevorzugter Weise eine Kamera mit hoher Verschlußge
schwindigkeit bis zu 1/2000000 sec und elektronischer Blendensteuerung verwendet
werden. Bei zu hoher Lichtintensität am Bestrahlungsort erreicht man damit ebenfalls
eine gute Bildqualität.
Claims (15)
1. Endoskop mit einer Kanüle (1), in deren Hohlraum ein Lichtleiter (2) sich zu ei
nem vorderen Kanülenende erstreckt und der Lichtleiter durch eine flüssigkeits
dichte Durchführung (3) von außen in den Kanülenhohlraum geführt ist, und mit
einer im spitzen Winkel zur axialen Ausdehnung der Kanüle liegenden Spüllei
tung (12) für die seitliche Zuführung von Spülflüssigkeit,
dadurch gekennzeichnet,
daß der Außendurchmesser der biegbar ausgebildeten Kanüle (1) weniger als
einen Millimeter beträgt, daß der im Kanülenhohlraum angeordnete Lichtleiter (2)
Lichtfasern zum Ausleuchten des zu untersuchenden Raumes und Bildfasern zur
Bildübertragung des ausgeleuchteren Raumes aufweist, welche zu einem hoch
flexiblen Faserbündel mit kreisförmigem Querschnitt geformt sind, und daß die
Spülflüssigkeit im spitzen Winkel zur Kanülenachse in den verbleibenden Kanü
lenhohlraum (4) eingeleitet ist.
2. Endoskop nach Anspruch 1, dadurch gekennzeichnet, daß der Lichtleiter (2) in
der Durchführung (3) durch ein komprimierbares Dichtungsmittel (5) geführt ist,
das im komprimierten Zustand die flüssigkeitsdichte Durchführung des Lichtlei
ters (2) in den Kanülenhohllraum (5) und die ortsfeste Fixierung des Lichtleiters
(2) im Kanülenhohlraum (4) bildet.
3. Endoskop nach Anspruch 2, dadurch gekennzeichnet, daß das Dichtungsmittel
(5) durch eine Feststellschraube (6) komprimierbar ist.
4. Endoskop nach Anspruch 2 oder 3, dadurch gekennzeichnet, daß der Lichtleiter
(2) bei entspanntem Dichtungsmittel (5) aus der Kanüle (1) entfernbar oder in die
Kanüle einsetzbar ist.
5. Endoskop nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß
durch den Kanülenhohlraum (4) zusätzlich zum Lichtleiter (2) ein Laserstrahlleiter
(7) geführt ist.
6. Endoskop nach Anspruch 5, dadurch gekennzeichnet, daß der Laserstrahl

und in axialer Richtung verschiebbar ist.
7. Endoskop nach Anspruch 5 oder 6, dadurch gekennzeichnet, daß der Laser
strahlleiter (7) mittels einer Verschlußeinrichtung (8) am hinteren Ende der
Kanüle (1) in den Kanülenhohlraum (4) eingesetzt ist und daß die Verschlußein
richtung (8) aus zumindest zwei Verschlußteilen (9, 10) besteht, von denen das
erste Verschlußteil (9) mit der Kanüle in axialer Richtung fest verbindbar und das
zweite Verschlußteil (10), welches mit dem Laserstrahlungsleiter (7) verbunden
ist, gegenüber dem in axialer Richtung an der Kanüle (1) festgelegten Verschluß
teil (9) in axialer Richtung verschiebbar ist.
8. Endoskop nach Anspruch 7, dadurch gekennzeichnet, daß die Verschlußeinrich
tung (8) durch Drehen des ersten Verschlußteils (9) mit dem hinteren Kanülen
ende (11) verbindbar ist.
9. Endoskop nach einem der Ansprüche 5 bis 8, dadurch gekennzeichnet, daß der
Lichtleiter (2) seitlich in den Kanülenhohlraum (4) einmündet.
10. Endoskop nach einem der Ansprüche 5 bis 9, dadurch gekennzeichnet, daß der
Lichtleiter (2) tangential in den Kanülenhohlraum (4) einmündet.
11. Endoskop nach einem der Ansprüche 5 bis 9, dadurch gekennzeichnet, daß der
Lichtleiter (2) in spitzem Winkel in den Kanülenhohlraum (4) einmündet.
12. Endoskop nach einem der Ansprüche 5 bis 11, dadurch gekennzeichnet, daß der
Lichtleiter (2) nach dem Laserstrahlleiter (7) in den Kanülenhohlraum (4) ein
schiebbar ist.
13. Endoskop nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß der
Lichtleiter (2) in axialer Richtung von außen in den Kanülenhohlraum (4) einsetz
bar ist.
14. Endoskop nach einem der Ansprüche 1 bis 13, dadurch gekennzeichnet, daß der
Lichtleiter (2) an eine Kamera mit einer bis zu 1/2000000 sec steuerbaren Be
lichtungszeit ankoppelbar ist.
15. Verwendung eines Endoskops nach einem der Ansprüche 1 bis 14 bei der Trä
nenwegkanaldiagnostik und Tränenwegkanaltherapie am Auge.
