Abstract
Die vorliegende Erfindung betrifft ein Mikronadelsystem (kurz: MNS) zur intradermalen Applikation in einer kontrollierten Freisetzung von Glukagon-ähnlichen-Peptid Analoga.
Description
- Die vorliegende Erfindung betrifft ein Mikronadelsystem (kurz: MNS) zur intradermalen Applikation in einer kontrollierten Freisetzung von Glukagon-ähnlichen-Peptid Analoga.
- Das Glukagon-ähnliche-Peptid GLP-1 (glucagon like peptide-1) gehört zu den intestinalen Hormonen und wird mit GLP-2 in Darmzellen gebildet. GLP-1 ist ein insulinotropes Peptid und reguliert den Blutzuckerspiegel. Seine physiologische Aufgabe besteht in der Aufrechterhaltung einer normalen Glucose-Homöostase und einer ausgeglichenen Energiebilanz. GLP-1 hemmt die Glucagonsekretion und verzögert die Magenentleerung. GLP-1 eignet sich nicht als Therapeutikum, da es über eine extrem kurze Halbwertszeit von ca. einer Minute verfügt.
- Wird die Struktur von GLP-1 jedoch geringfügig geändert, bleibt die abbauende Peptidase DPP IV unwirksam. Bereits marginale Moleküländerungen wie der Austausch einer Aminosäure machen das Molekül gegenüber der Peptidase resistent. Die bislang entwickelten GLP-1-Analoga wirken zum Teil stärker als das körpereigene GLP-1 und werden als Antidiabetika verwendet, müssen jedoch subkutan appliziert werden.
- Solche Antidiabetika auf Basis von Glukagon-ähnliche-Peptid Analoga oder Antagonisten - auch Inkretin-Mimetika genannt - sind insbesondere Exenatide, Liraglutide und Lixisenatide (nachstehend: Glukagon-ähnliche-Peptid Analoga) und binden an den GLP-1 Rezeptor. Solche Glukagon-ähnliche-Peptid Analoga weisen bis zu 50 Aminosäuren und eine Molmasse bis 5 kDa auf. Die Halbwertszeit beträgt zumeist mehr als 12 Stunden.
- Exanetide (C184H282N50O60S, Mr = 4186.6 Da) ist ein synthetisches Peptid, das aus 39 Aminosäuren besteht.
- Liraglutid oder γ-L-Glutamoyl(N-α-hexadecanoyl)-Lys26, Arg34-GLP-1 (7-37) ist ein verzweigtkettiges Peptid (C172H265N43O51, Mr = 3751.2 Da), das aus 39 Aminosäuren besteht.
- Lixisenatide (C215H347N61O65S, Mr= 4858.49 Da) ist ein synthetisches Peptid, das aus 44 Aminosäuren besteht.
- Exanetide ist erfindungsgemäß bevorzugt.
- Die Haut besteht aus mehreren Schichten. Die äußerste Hautschicht, das Stratum Corneum, weist bekannte Sperreigenschaften auf, um ein Eindringen von Fremdsubstanzen in den Körper und ein Austreten von Eigensubstanzen aus dem Körper zu verhindern. Das Stratum Corneum, das eine komplexe Struktur aus kompaktierten keratotischen Zellresten mit einer Dicke von ungefähr 10-30 Mikrometern ist, bildet hierzu eine wasserdichte Membran zum Schutz des Körpers aus. Die natürliche Undurchlässigkeit des Stratum Corneum verhindert das Verabreichen der meisten pharmazeutischen Mittel und anderer Substanzen durch die Haut im Rahmen einer intradermalen Applikation. Besonders schwierig ist das Einbringen von Proteinen durch die Haut.
- Mikronadelsysteme (MNS), die aus einem Mikronadelarray (MNA) und gegebenenfalls weiteren Komponenten bestehen, können mittels einer Druckkraft die Mikronadeln (auch: Hautdurchdringungselemente) des Arrays (MNA) gegen die Applikationsstelle auf die Haut pressen, um das Stratum Corneum zu durchdringen und auf diese Weise einen Fluidkanal herzustellen, so dass ein Wirkstoff intradermal appliziert werden kann. Solche Mikronadelarrays (MNA) in Mikronadelsystemen (MNS) als auch deren Herstellung sind im Stand der Technik beschrieben (
WO2004000389A1 ). - Ebenfalls ist bekannt, dass MNS zur kontrollierten Freisetzung verwendet werden können. Insbesondere für Glukagon-ähnlichen-Peptid Analoga besteht ein hohes Bedürfnis der kontrollierten Freisetzung, insbesondere für mehr als 12 Stunden (kurz: h) oder gar 24 h, so dass eine antidiabetische Wirkung für einen Tag erreicht oder unterstützt werden kann.

US9,320,878B2 US9320878B2 - Allerdings ist PVP zur Beeinflussung der Freisetzung von Wirkstoffen gegenüber PVA zu bevorzugen und weist hierzu eine höhere Eignung auf.
- Daher ist es Aufgabe der vorliegenden Erfindung eine intradermale Applikation in einer kontrollierten Freisetzung von Glukagon-ähnlichen-Peptid Analoga mittels einem MNS enthaltend ein MNA auf Basis einer geeigneten Formulierung zu ermöglichen.
- Diese Aufgabe wird erfindungsgemäß durch ein Mikronadelsystem (MNS) enthaltend ein MNA nach Anspruch 1 gelöst, wobei die Glukagon-ähnlichen-Peptid Analoga samt dem PVP Gegenstand der Formulierung des MNA ist und folglich dem MNA inhärent ist bzw. mit dem MNA eine Einheit bildet. Das MNA kann hierzu einförmig bzw. einteilig ausgebildet sein.
- Gegenstand der Erfindung ist daher ein Mikronadelsystem enthaltend ein MNA zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung umfassend oder bestehend aus einer Formulierung aus PVP und mindestens einem Glukagon-ähnlichen-Peptid Analoga.
- Daher umfasst die Erfindung ein Mittel aufweisend ein Mikronadelarray umfassend oder bestehend aus einer Formulierung aus PVP und mindestens einem Glukagon-ähnlichen-Peptid Analoga zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung.
- Ein solches Mittel ist beispielsweise ein Arzneimittel oder Antidiabetikum umfassend ein vorstehendes Mikronadelarray zur kontrollierten Freisetzung von mindestens einem Glukagon-ähnlichen-Peptid Analoga in einer intradermalen Applikation. Das besagte Arzneimittel dient zur Prophylaxe und Behandlung von Diabetes, Diabetes Typ II, Insulinresistenz, Bluthochdruck, Fettleibigkeit (Adipositas, Obesitas), metabolisches Syndrom.
- Der Begriff „intradermale Applikation (synonym: „intrakutane Applikation“) beschreibt erfindungsgemäß die Verabreichung von Stoffen, hier: Glukagon-ähnlichen-Peptid Analoga aus dem MNA in die Haut und erfordert ein Einstechen der Mikronadeln in die Haut.
- Der Begriff „kontrollierte Freisetzung“ im Rahmen dieser Erfindung bedeutet, dass der Wirkstoff, hier: Glukagon-ähnlichen-Peptid Analoga, über eine Zeitspanne von mehr als 2 h, vorzugsweise mehr als 12 h, insbesondere 24 h kontinuierlich freigesetzt bzw. abgegeben wird. Es erfolgt keine schlagartige bzw. sofortige Freisetzung (immediate release) der Wirkstoffe.
- Daher betrifft die Erfindung ein Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung, wobei die Zeitspanne der kontrollierten Freisetzung mehr als 2 h, vorzugsweise mehr als 12 h, insbesondere 24 h beträgt.
- Besonders vorteilhaft kann eine Dosis von bis zu 50 µg/MNA an Glukagon-ähnlichen-Peptid Analoga am Menschen appliziert werden.
- Dies erlaubt die vorteilhafte einmalige Applikation einer Tagesdosis an Glukagon-ähnlichen-Peptid Analoga.
- Gegenstand der Erfindung ist ebenfalls ein Verfahren zur kontrollierten Freisetzung von mindestens einem Glukagon-ähnlichen-Peptid Analoga in einer intradermalen Applikation aufweisend ein Mikronadelarray umfassend oder bestehend aus einer Formulierung aus PVP und mindestens einem Glukagon-ähnlichen-Peptid Analoga.
- Gegenstand der Erfindung ist ebenfalls ein Verfahren zur Durchführung einer intradermalen Applikation zur kontrollierten Freisetzung von mindestens einem Glukagon-ähnlichen-Peptid Analoga umfassend
- a) Fixierung eines erfindungsgemäßen Mikronadelsystems an der Haut,
- b) Penetration eines Mikronadelarrays umfassend eine Formulierung aus PVP und mindestens einem Glukagon-ähnlichen-Peptid Analoga in die Haut.
- In einer weiteren Ausführungsform kann die Formulierung aus einem beliebigen PVP bestehen mit einer relativen Molekülmasse von mehr als 10.000 Da. Der Anteil an PVP (Gewichts-%) in der Formulierung kann 50-99 %, insbesondere 70-90 %, vorzugsweise 75-80 % betragen. Insbesondere ist der PVP Gehalt vom Restwasser abhängig. Typische Restwassergehalt können 5-20 %, insbesondere 12-18 % betragen. Bei einem Restwassergehalt von 16 % ergibt sich ein PVP-Gehalt von ungefähr 76 % oder ggfs. niedriger.
- Weiterhin kann die Formulierung Hilfs- und Zusatzstoffe enthalten, solche wie
- a.) Bindemittel oder Emulgatoren, z.B. Carboxymethylcellulose, Alginate, Gelatine,
- b.) Feuchthaltemittel, z.B. Glycerin, Harnstoff, Trehalose,
- c.) Netzmittel, z.B. Cetylalkohol, Glycerinmonostearat,
- d.) Antioxidantien, wie Ascorbinsäure, Vitamin E.
- Bevorzugt sind jedoch Poloxamere, Alginate und Polysorbate (Tween).
- Der Anteil an Poloxamer in der Formulierung kann 2-7 %, betragen. Der Anteil an Alginat in der Formulierung kann 2-7 % betragen. Der Anteil an Polysorbaten in der Formulierung kann 0,1 - 1 % betragen.
- Die folgenden Beispiele und Figuren sollen die Erfindung näher erläutern, ohne jedoch diese zu beschränken.

1 zeigt in-vitro Ergebnisse für Formulierung F1, F2 und F3, welche eine verlangsamte, zeitlich kontrollierte Freisetzung des Wirkstoffs Exenatide über ein Zeitintervall von bis zu 24 Stunden belegen. Anspruch 2 zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung zur Prophylaxe und Behandlung von Diabetes, Diabetes Typ II, Insulinresistenz, Bluthochdruck, Adipositas, metabolisches Syndrom. - Mikronadelarray oder Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass die Glukagon-ähnlichen-Peptid Analoga ausgewählt sind aus der Gruppe Exenatide, Liraglutide, Lixisenatide.
- Mikronadelarray oder Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass die Zeitspanne der kontrollierten Freisetzung mehr als 2 h, mehr als 12 h, 24 h beträgt.
- Mikronadelarray oder Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass eine Formulierung bis 99 % Polyvinylpyrrolidon und weitere Hilfs- und Zusatzstoffe, insbesondere Emulgatoren enthält.
- Mikronadelarray oder Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung nach
Anspruch 6 , dadurch gekennzeichnet, dass in der Formulierung 2 - 7 % Poloxamer oder 2 - 7 % Alginate oder 0,1 - 1 % Polysorbate vorliegen. - Mikronadelarray oder Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung nach einem der
Ansprüche 6 und7 , dadurch gekennzeichnet, dass eine Formulierung 50-99 %, 70-90 %, insbesondere 75-80 % Polyvinylpyrrolidon enthält. - Mikronadelarray oder Mittel zur Verwendung in der intradermalen Applikation zur kontrollierten Freisetzung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass eine Dosis bis 50 µg/MNA Glukagon-ähnlichen-Peptid Analoga appliziert wird.
- Verfahren zur Durchführung einer intradermalen Applikation zur kontrollierten Freisetzung von mindestens einem Glukagon-ähnlichen-Peptid Analoga in einer intradermalen Applikation aufweisend ein Mikronadelarray umfassend eine Formulierung aus PVP und mindestens einem Glukagon-ähnlichen-Peptid Analoga.
- Verfahren zur Durchführung einer intradermalen Applikation zur kontrollierten Freisetzung von mindestens einem Glukagon-ähnlichen-Peptid Analoga nach
Anspruch 10 umfassend a.) Fixierung eines erfindungsgemäßen Mikronadelsystems an der Haut, b.) Penetration eines Mikronadelarrays umfassend eine Formulierung aus PVP und mindestens einem Glukagon-ähnlichen-Peptid Analoga in die Haut.
