Abstract

Description
本申请是1997年9月15日提交的USSN 60/058,933的部分继续申请,后者在此全文引入作为参考。本发明主要涉及将外源遗传物质传递进细胞的方法和组合物。具体地说,本发明涉及一种受体介导的基因传递进细胞的技术。与一种特殊类型的靶细胞相匹配的基因传递系统由外源遗传物质、一种载体和一种非必需的载剂组成。然后复合物在允许受体介导的胞饮作用发生的情况下与细胞作用,导致外源遗传物质被功能性摄取或转导。本方法不仅可用于体外,也可用于体内将基因传递进抗原呈递细胞,具体地说是通过皮肤将基因传递进皮肤朗氏细胞。当外源遗传物质是一种免疫原,例如编码与一种感染性病毒有关的抗原和颗粒的一个主要部分的DNA,并且不希望进行注射传递时,本技术特别适用于预防性或治疗性基因免疫。
发明背景
动物免疫系统具有两种不同但相关的应答,即细胞免疫应答和体液免疫应答。细胞免疫应答产生能杀伤表面带有外源识别标记的细胞的T淋巴细胞。在表面带有这种识别标记的细胞被认为“呈递”一种抗原,并被称为抗原呈递细胞(APCs)。此外,T淋巴细胞还通过辅助性B细胞刺激体液免疫,B细胞是浆细胞的前体。
体液免疫导致浆细胞产生抗体,抗体作用于溶液中的特殊分子。抗体(或免疫球蛋白)是动物对外源物质的存在产生应答合成的蛋白。它们由浆细胞分泌,浆细胞由B淋巴细胞(B细胞)衍生而来。这些可溶性蛋白是体液免疫应答的识别元件。每种抗体对刺激其合成的外源物质具有特异亲和性。也就是说,抗体具有一个被称为抗原结合位点的片段或位点,该片段或位点能与外源物质结合。一种能刺激针对其自身的抗体产生的外源大分子被称为抗原。蛋白和脂多糖通常是有效的抗原。抗体不是针对整个大分子抗原有特异亲和性,而是针对抗原上被称为抗原决定簇或表位的一个特定位点。抗体能识别溶液中和膜上的外源分子而不管其分子环境。体液免疫应答在多细胞环境中抵抗细菌和病毒最为有效。(体液一词来源于描述流体或液体的拉丁词)。一种给予抵抗疾病的免疫的策略是将个体和与一种病毒或细菌相关的一种或多种抗原作用,而不是使用活的病毒或细菌。这样的疫苗被称为亚单位疫苗,它非常适合于刺激抗体产生。
T细胞介导细胞免疫应答。与体液免疫应答不同,细胞免疫应答破坏病毒感染的细胞、寄生虫和癌细胞。T细胞表面含有被称为T细胞受体的跨膜蛋白,T细胞受体能识别其他细胞表面携带的外源分子。也就是说,T细胞识别抗原呈递细胞(APCs)。T细胞受体不识别单独的外源分子。外源单位必须定位于一种细胞的表面,并必须由一种特殊的膜蛋白呈递给T细胞,这种特殊的跨膜蛋白由被称为主要组织相容性复合物(MHC)的一个高变异性的宿主染色体区域编码。MHC编码三类跨膜蛋白。MHC Ⅰ类蛋白几乎在所有的细胞中表达,它将外源表位呈递给细胞毒T细胞。MHC Ⅱ类蛋白表达于免疫系统细胞和巨噬细胞,它将抗原呈递给辅助性T细胞。MHC Ⅲ类蛋白是被称为补体级联反应过程中的组分。
T细胞有多种,包括细胞毒性T淋巴细胞(CTL或杀伤性T细胞),细胞毒性T细胞能破坏能够展示一种外源表位与一种MHC蛋白结合的细胞。当外源表位加MHC蛋白与T细胞受体结合时,T细胞分泌含有穿孔素的颗粒,穿孔素能多聚化形成跨膜孔道,使细胞打开或诱导细胞裂解。其他种类的T细胞被称为辅助性T细胞,它们分泌被称为淋巴因子的肽和蛋白。这些激素样分子能指导其他细胞的移动和活性。一些例子包括白介素-2(IL-2)、白介素-4(IL-4)、干扰素、粒细胞巨噬细胞集落刺激因子(GM-CSF)和肿瘤坏死因子(TNF)。T细胞也参与补体级联反应,补体级联反应是精确调控的、复杂的一系列事件,导致微生物和感染细胞的破坏。超过15种可溶性蛋白相互协作形成多单位的抗原-抗体复合物,接着在细胞质膜上形成大的孔道。
在抗原呈递细胞(APC)中表达外源基因可用来在动物中产生有效的CTL应答。因此,APC的基因转移和遗传修饰具有产生针对不同疾病包括病毒感染和癌症的有效疫苗和治疗方法的潜力。表达不同外源抗原的活重组病毒载体,例如痘病毒、腺病毒和逆转录病毒,都能通过模仿病毒感染用于诱导体液和细胞免疫应答。而且,活的减毒(或弱化)病毒也被建议用作疫苗。DNA疫苗免疫策略也正在开发。不同的病毒基因已被克隆到质粒DNA中,并注射到肌肉、皮肤或皮下。这些结构物能表达蛋白并激发细胞和体液免疫应答。
有人认为基因免疫可能对病毒性疾病有效。已知某些细胞如树突细胞能捕捉抗原,并从机体组织迁移到淋巴组织。在那里,这些细胞在淋巴器官中呈递抗原:也就是说,它们展示与MHC蛋白结合的外源表位。这样的抗原呈递细胞(APCs)是免疫应答机制的一个已知部分。如果细胞例如树突细胞(DC)获得修饰,使之含有编码一种病毒的DNA,并且这种病毒是感染性的但不能有效复制,那么细胞将不仅能以经典方式呈递抗原,而且能被操作来产生或表达病毒颗粒和多种病毒蛋白。在代理人案卷号RGT-100A,USSN 08/803,484“用于保护性和治疗性基因免疫的方法和组合物”中已经介绍了一种新的技术,该文献在此全文引入作为参考。该文献公开了以下内容:一种无复制能力的病毒的基因能整合到抗原呈递细胞中,后者接着迁移到淋巴器官中,并产生一整套病毒抗原和病毒颗粒,因而激发体液和细胞免疫应答。该文献指出,淋巴器官中的DC可能接着表达所有的病毒抗原并产生“看起来真正”的病毒颗粒。这些病毒颗粒将因此在其他免疫应答的产生过程中起核心作用。
该参考文献在实施例13中介绍了细胞包括APC的“体内转导”。在该实施例中,举出了几种众所周知的方法包括病毒和非病毒基因传递系统作为例子。在实施例14中,介绍了细胞包括APC的“体内转导”。它们使用了(1)直接DNA注射;(2)含有DNA的脂质体或病毒体注射;(3)4类痘病毒的脾间直接注射;以及(4)携带基因传递载体的直肠和阴道栓剂。但是,该文献没有详细介绍体内和体外基因传递的方法。这正是本发明的主题。
有证据表明,APC的遗传修饰可对新生和成年的人和动物进行有效的免疫接种。Ridge et al.(Science 271:1723-1726,1996)将从另一动物中分离的表达外源抗原的DC静脉注射给小鼠。注射了这些DC的新生和成年小鼠均能产生能非常好的杀伤靶细胞的CTL。这些实验还证明,表达外源抗原的DC能诱导保护性的细胞介导的免疫应答,后者能在病毒感染时消灭被感染的细胞。此外,这些实验还证明,新生动物可能有DC介导的免疫。但这些实验没有使用遗传修饰的细胞,也没有像本发明一样,使用几种外源抗原或一种病毒。
Sarzotti et al.(Science 271:1726-128,1996)证明,用低剂量的病毒接种会导致一种保护性的免疫应答(Th1型),这种应答能产生CTL应答;而高剂量的接种将导致一种非保护性的免疫应答(Th2型),这种应答主要产生抗体。这种CTL应答能持续较长时间,并且还可在新生儿中产生。高剂量的病毒能在DC激活T细胞前,战胜T细胞,使T细胞失去作用。这再次说明,接种的途径,即不通过注射而通过DC呈递,非常重要。这些发现与其他一些显示接触低剂量的病毒主要激活细胞免疫应答(Th1型)结果相一致。在猕猴中,低剂量的SIV引发没有抗体产生的Th1型应答,并使受保护的动物抵抗高剂量的攻击(Clerici et al.AIDS 8:1391-1395,1994)。Rowland-Jones et al.在人体内也证明了相似的结果(Nature Med1:59-64,1995)。
修饰细胞使之携带外源遗传物质的过程被称为基因转移、转染或转导。这里引用的文献均未显示将基因有效转移至抗原呈递细胞的证据,不管是体内还是体外。作为将基因转移进抗原呈递细胞如DC的背景知识,有几种“低效率”的体外方法已有介绍,这包括脂质体介导的基因转移、电转化和逆转录病毒载体和腺病毒载体介导的基因转移(Arthur,J.F.et al.Cancer Gene Therapy.4:117-21,1997,Song E.S.et al.Proc Natl Acad Sci U S A 94:5,1943-8,1997)。所有这些体外方法都需要分离大量的细胞,并且这些细胞都要在实验室内用一种基因传递载体处理。所有在人或动物中的应用都需要将这些遗传修饰的细胞回输。因此,体外基因传递方法不适宜于对大量个体进行免疫接种或治疗。已知的体内方法包括重组病毒载体的皮内或肌肉内注射以及质粒DNA的皮内、皮下和肌肉内注射。这些方法都没有显示能有效地将基因转移进抗原呈递细胞如树突细胞,通过皮肤将基因转移进朗氏细胞更少。

发明简述

发明详述
本发明最主要涉及将外源遗传物质传递进细胞的方法和组合物。提高基因转移进参与免疫系统的特定细胞如抗原呈递细胞(APCs)的效率对提高基因免疫的效率特别有用。虽然本发明的目的是将遗传物质传递进细胞,但发明人预计,任何适当大小和构型的分子都能用本发明的方法传递进细胞。因此,其他物质如药物或蛋白也能用这里介绍的技术传递进靶细胞。
本发明利用了某些动物中可用的天然途径。
已知,例如,特殊蛋白可通过被称为受体介导的胞饮作用的过程进入细胞。在此过程中,特殊蛋白或配体与一种细胞的胞质膜上的特殊受体结合。膜围绕蛋白形成一个小泡或包裹团,并最终使配体内在化。也就是说,使蛋白进入细胞。然后,内体通常将这些复合物传递至溶酶体,在那里它们被消化成组成成分、肽。在表达MHC的细胞中,肽-MHC复合物在溶酶体中聚集,然后通过一个被称为抗原呈递的过程到达细胞表面。受体介导的胞饮作用是将大分子如基本代谢物、激素和生长因子传递进细胞的方法。它也是许多病毒和毒素用来进入细胞的途径,并还参与免疫应答。例如,巨噬细胞具有能够使其摄取抗原抗体复合物的受体。
与抗体如IgG或补体如C3b或者两者复合的颗粒可有效地通过受体介导的胞饮作用和吞噬作用进入细胞。抗体或免疫球蛋白含有不同的部分来使它们进行不同的功能。例如免疫球蛋白G(IgG)是一种Y型分子,具有两个带有抗原结合位点的Fab片段和一个介导效应物功能的Fc片段。多价抗原能与抗体结合并形成免疫复合物。这些免疫复合物的大小由抗原和抗体相对浓度决定。
胞饮作用可被一种称为调理作用的过程增强。调理作用是一种抗体包被抗原的过程,它提供了一种免疫系统的其他组分识别并对抗原产生应答的方式。具有合适Fab位点的免疫球蛋白可用来包被抗原颗粒,然后,表达相应Fc受体的细胞能识别被调理抗原的Fc部分,并容易地胞饮它们。补体如C3b、C4b和C3bi也具有调理活性。较大的免疫复合物比较小的免疫复合物能更有效地被表达针对免疫球蛋白分子Fc部分的受体的细胞如B细胞、单核吞噬细胞、粒细胞、嗜中性粒细胞和树突细胞吞噬。
IgG抗体可通过注射带有或不带有佐剂的抗原在动物中制备。而且,抗体可用分子遗传技术克隆和人源化。其他受体通常位于免疫系统细胞的膜上,这包括,例如,转铁蛋白、甘露糖和脱唾液酸糖蛋白受体,它们也可容易地用于免疫细胞的转导。抗体的Fc受体部分也可使用分子遗传技术来用其他的受体结合结构域取代。对发明人的现实目的来说,Fc受体和相应的IgG分子是方便的,因为它们对将基因转移进树突细胞的进一步功能有用。
一旦一种分子或颗粒通过胞饮作用或吞噬作用被摄取到一种细胞中,它们就被包含在一种被称为内体的蛋白-受体复合物中。内体是细胞内酸性小室,具有分类的功能。吞噬体由吞噬作用产生,是一种大的(10x-20x)的内体,内体接着与溶酶体融合,在那里,物质被消化成较小的产物如肽、核苷酸和糖。在本发明中,载体的作用是将外源基因送至细胞,并避免基因的降解。也就是说,载体必须能使内体破裂,并将基因释放至细胞内液体、胞质、或核内。已知多种颗粒能在受体介导的胞饮作用后是内体破裂,这包括病毒基因传递颗粒如腺病毒载体、逆转录病毒载体、痘病毒载体和SV-40病毒。非病毒基因传递颗粒包括DNA与聚赖氨酸、聚乙烯亚胺及其衍生物的结合物、脂质体、病毒体和能够提高内体中pH的化合物如氯奎。靶细胞
本发明可使用任何能够进行受体介导的胞饮作用或巨噬细胞的细胞。靶细胞表达一种受体位点,该受体位点能通过与一种互补分子结合,将所需的分子带入内体或吞噬体。就本发明的目的来说,这样的细胞优选是参与免疫应答的细胞。它们包括能进行受体介导的抗原胞饮和吞噬的细胞。这样的细胞包括诸如B细胞、单核吞噬细胞、粒细胞和树突细胞。这些细胞表达针对免疫球蛋白或补体受体或两者的Fc部分的受体。树突细胞和巨噬细胞特别优选,因为它们能有效地呈递外源抗原,因而能够激发细胞免疫应答或CTL应答。细胞也可通过其他受体如转铁蛋白和甘露糖受体作为靶细胞。
树突细胞定居于淋巴样组织如脾、扁桃体和淋巴结中,但它们也在血液、表皮、粘膜和其他外周组织中被发现。这些细胞摄取抗原,并携带抗原迁移至淋巴样组织中。在皮肤内,被称为朗氏细胞的树突细胞可在表皮中发现。当它们胞饮一种抗原后,它们迁移至局部淋巴结内。在淋巴结中,它们被称为交错突细胞,它们将抗原呈递给幼稚T-细胞,激发细胞免疫应答。
为提高基因转移效率,可以使用的树突细胞数目必须最大。位置的选择就成为一个因素。在诸如皮肤和粘膜如口、阴道和直肠中发现高浓度的树突细胞。组织中不成熟的DC能有效地进行胞饮作用,因此它们是基因传递复合物通过受体介导的胞饮作用传递基因的很好的靶细胞。但是,为更有效地表达MHC分子和进行抗原呈递,DC也必须被激活。在体外,不成熟的DC可用GM-CSF和IL-4从外周血中产生或用GM-GFS从骨髓前体中产生。这些不成熟DC的活化可用细菌产物如脂多糖和TNFα在体内或体外诱导(Watts C.Nature 338:724-725,1997)。
树突细胞可通过一个包含免疫过程的事件如细胞或组织损伤吸引到特定位置并活化。就本发明的目的来说,抗原呈递细胞包括树突细胞的吸引和活化可用一种与接种或病毒感染无关的免疫应答来介导。一个例子是接触敏感化合物如药物和毒素、化妆品和环境抗原产生的皮疹,如果一种化学刺激剂涂在皮肤上,在处理后24-48小时后通常就会出现皮疹或病灶。病灶是由于化合物与朗氏细胞表面蛋白结合产生的新抗原引起的。新抗原共价修饰了“正常”蛋白(如磷酸化),新抗原能被抗体识别。在该位点,可发现比正常数量多的树突细胞,它们更可能已活化,也就是说,更容易立即接受一种抗原的胞饮作用。刺激剂的选择依赖于吸引DC的效率及其副作用。在本发明的另一个实施方案中,制造了一种免疫复合物介导的损伤。在该场合,同时含有抗原和抗体组分的免疫组合物被用来活化B细胞和结果组织损伤的补体级联反应。
因为细胞是通过一种特殊类型的受体作为靶,本发明的一个优点是基因传递复合物能针对特殊靶细胞。如果基因传递复合物用IgG或一种用淀粉或糖修饰的聚乙烯亚胺制备,它将主要被抗原呈递细胞摄取。这在以基因为基础的疫苗发展中将是一个巨大的进步。针对其他表达如补体受体或转铁蛋白受体的细胞如上所述也是可行的。基因传递复合物
本发明的基因传递复合物可用于体外或体内将基因传递进带有一种给定受体的细胞。基因传递复合物由两个部分组成:遗传物质和一个传递颗粒,并且可进一步含有一种载剂(见图1)。在一个实施方案中,遗传物质衍生于一种减毒的HIV病毒,而传递颗粒是非病毒载体。
在另一个实施方案中,相同或不同的遗传物质可与一种病毒载体和一种载剂组合。在这种情况下,载剂优选是一种抗体,该抗体带有一种与靶细胞表面展示的一种受体互补的位点,并带有一种针对所需传递颗粒的抗原结合位点。这样的载剂对细胞和传递颗粒都具有特异性。对基因免疫来说,一般选择针对基因传递颗粒的人IgG。如果没有商业化的抗体,它可用本领域的技术人员熟知的技术制备。如果传递颗粒是复制缺陷型HIV-1,人IgG可用作载剂,并大量用于被动免疫治疗。基因传递颗粒可与抗体通过在室温下共同温育5分钟来复合。基因传递颗粒与抗体的相对数量可根据是否需要对基因传递颗粒进行调理来确定。遗传物质
遗传物质,不管是DNA还是RNA,都通过传递复合物来携带。一种或多种基因可在质粒DNA的一条链、双链DNA或在RNA上编码。此外,如果重组病毒被用作基因传递颗粒,遗传物质可整合在重组病毒中。如果基因转移的目的是诱导一种免疫应答,则遗传物质必须表达一种或多种免疫原蛋白。转导细胞必须随后表达足够的免疫原蛋白(不同病毒抗原和产生足够真实的病毒颗粒)来激发足够的免疫应答(如保护个体不受野生型病毒的感染)。
基因传递颗粒的选择将根据疾病和选择转移的基因来进行选择。当需要构建一种针对逆转录酶依赖型病毒如HIV的疫苗时,DNA优选编码至少一种复制缺陷型或整合缺陷型病毒的一个主要部分,或编码复制缺陷型或整合缺陷型病毒自身。例子包括但不限于双养型原始分离株如HIV-1/LW的整合酶阴性突变体,及其在gag基因的蛋白酶切位点具有一个删除的衍生物。见1997年2月20日提交的USSN08/803,484“用于保护性和治疗性基因免疫的方法和组合物”,该文献在此全文引入作为参考。当需要构建一种针对癌症的疫苗时,免疫原优选是编码一种或多种肿瘤抗原的DNA。其他DNA结构物可以是编码复制缺陷型人乳头瘤病毒(引起宫颈癌)、复制缺陷型肝炎A、B和C病毒(引起肝炎和肝癌)的DNA,以及编码复制缺陷型动物病毒如牛白血病病毒或猫免疫缺陷病毒的DNA。用来包含外源遗传物质的传递颗粒的选择可包括:(a)复制缺陷型HIV或其他逆转录病毒;(b)重组腺病毒;(c)与PEI或一种PEI衍生物复合的质粒或线形DNA或RNA;(d)含有任何DNA或RNA的病毒体;(e)含有DNA或RNA的脂质体;(f)质粒DNA-聚赖氨酸-病毒复合物;(g)与任何DNA或RNA复合的糖。传递系统
为有效地将基因转移进细胞,基因传递系统必须含有要转移的基因或基因群,并还必须能破坏内体(或吞噬体),而不是转移给溶酶体或孤立于细胞的外表面,成为破坏的靶。而且,基因传递系统应便于外源遗传物质整合到细胞的遗传物质中。
基因传递系统可包括一种病毒或非病毒载体。病毒型基因传递系统包括重组病毒载体如腺病毒载体、逆转录病毒载体、痘病毒载体、突变病毒(如上文介绍)和病毒体。非病毒基因传递系统包括与糖、聚赖氨酸、聚乙烯亚胺、聚乙烯亚胺衍生物和脂质体结合的DNA以及它们的衍生物。
优选使用非病毒基因传递系统如那些使用糖、糖衍生物、脂质体、脂质体衍生物和聚乙烯亚胺或聚乙烯亚胺衍生物的系统。其中,适合于以免疫系统细胞的甘露糖受体为靶的糖和聚乙烯亚胺衍生物最为优选。
非病毒基因传递系统较病毒基因传递系统有几个优势:1)首先,非病毒载体不被免疫系统识别,因此不会针对它产生免疫应答。这样,在重复免疫时,用最后的疫苗处理的个体将会对其耐受,并产生足够的免疫应答;2)非病毒系统可能比病毒系统更安全,因为系统不可能以不可预料的方式产生突变;3)非病毒系统可大量化学合成,因而可能更便宜。
优选的实施方案以一种阳离子型聚合物---聚乙烯亚胺(PEI)为基础。PEI与DNA复合,形成复合物。PEI-DNA复合物能通过脱唾液酸糖蛋白受体介导的胞饮作用进入皮肤抗原呈递细胞---朗氏细胞的内体。然后,该复合物的PEI成分利用内体的缓冲和膨胀作为一种逃逸机制进入胞质(Pollard H;Remy JS;Loussouarn G;DemolombeS;Behr JP;J Bio1 Chem 1998 Mar 27;273(13):7507-11).PEI也可被修饰来靶向其他受体。例如,一种PEI衍生物如一种糖修饰的PEI能够获得相似的结果,除了它通过细胞的甘露糖受体被摄取。这样的衍生物可在实验室中制备。例如,一种异硫氰酸根合苯基苯基甘露糖衍生物可被偶联至PEI 25 kDa,产生一种配体(或对甘露糖受体有较低亲和力的甘露糖残基,1mM)。另一个可能是使用线形的PEI 22kDa衍生型mannotenaose配体。(这些物质由Dr.Jean-Paul Behr,Laboratoire de chimie Genetique,Faculte de Pharmacie,CNRS-UMR 7514 74 Route du Rhin 67401 Illkirch,France慷慨提供)。
甘露糖受体是一种175kDa的跨膜糖蛋白,该蛋白特异性地表达于巨噬细胞和朗氏细胞的表面。甘露糖受体的胞外域含有8个碳氢识别结构域。甘露糖受体识别修饰多种细菌、寄生虫、酵母、真菌的糖谱和甘露糖酸化的配体。(Takahashi K;Donovan MJ;Rogers RA;Ezekowitz RA,Cell Tissue Res 1998 May;292(2):311-23)。与Fc受体相对,甘露糖受体在释放其携带物后能重新组成其自身(Stahl et al.Cell 1980 19:207)。因此,它能以与转铁蛋白受体相似的方式连续地使配体内在化,提供持续的抗原俘获能力(Goldstein,et al.1985,Annu Rev Cell Biol.1:1)。最近发现,通过甘露糖受体介导的抗原摄取将抗原呈递给T细胞比通过流动相方式的抗原内在化有效约100倍(Engering et al.1997,Eur.J.Immunol.27:2427-2425)。这种抗原呈递提高是因为通过甘露糖受体的更高效率的抗原摄取。由于这些原因,我们相信,靶向甘露糖受体可能产生对抗原细胞的特异性,并同时提高复合物进入内体的功能性摄取的效率。载剂
本发明的载剂是基因传递复合物的一个部分,它与一种细胞受体一起参与基因传递系统。在一个实施方案中,载剂是一种免疫球蛋白G(IgG)。IgG是一种Y型分子,带有两个具有抗原结合位点的Fab片段和一个能与被称为Fc受体的细胞受体结合的Fc片段。免疫系统细胞如B细胞、单核吞噬细胞、粒细胞和树突细胞都带有Fc受体。当IgG用作载剂时,它特异性地靶向带有Fc受体的细胞。
为改变载剂靶向其他细胞的特异性,抗体的Fc部分可用其他受体结合结构域如补体、糖或转铁蛋白取代。
当载剂是一种抗体,组成大的复合物时,载剂和基因传递系统优选以等量部分组合。当需要对颗粒进行调理时,载剂的数量大大超过基因传递颗粒的数量。在大的复合物和调理的颗粒时,胞饮作用和吞噬作用都被增强。基因传递系统优选用载剂进行调理。当用一种抗体与一种整合有外源遗传物质传递颗粒组成的调理的基因传递复合物对个体进行给药时,细胞免疫应答对比体液免疫应答将最大。树突细胞将被调理的复合物活化,胞饮作用将更有效。而且,多价抗体将封闭传递颗粒的抗原决定簇(表位)。因此,不会产生针对传递颗粒的直接抗体应答。而且,有些与抗原复合的抗体将与B细胞的Fc受体位点结合,进一步抑制它们的抗体应答。但是,细胞免疫将被激活,因为复合物将被各种抗原呈递细胞包括树突细胞和巨噬细胞胞饮或吞噬。
在一个优选的实施方案中,载剂是与非病毒基因传递系统共价结合的载剂。PEI可用糖(如甘露糖、葡萄糖、半乳糖等)化学修饰。在这种情况下,载剂是糖配体,它能被甘露糖受体识别。为改变载剂的特异性,糖可用其他受体结合结构域取代。体内基因传递
基因传递复合物可直接注射到血液、皮肤、或其他与载剂的结合特异性相应的细胞定居的地方,复合物可直接施用于皮肤或粘膜表面。在这种情况下,优选表面的朗氏细胞被活化。活化可通过受体激活(例如,甘露糖受体)、毒素激活(霍乱毒素)、一种组织或细胞损伤如炎症完成,并可以是另一种抗原刺激的结果。
对人或动物的成年个体或新生儿,复合物可使用一种儿科喂饲管经口、阴道或直肠进行输入。新生儿可能比成年个体对经口给药的反应好。此外,基因传递复合物可包裹入一种栓剂,并插入阴道或直肠。
当使用一种病毒传递颗粒时,传递颗粒可经过两个单独的程序在存在或不存在佐剂的情况下直接注射进受体的肌肉或皮肤,以获得高滴度的体内抗体产生。第一次注射将主要产生体液免疫应答。即,获得产生大量抗体的能力。当获得能足以对传递颗粒进行调理的IgG抗体浓度时,(这可用实验来测量或评估),接着可按上面1-3介绍的方法进行第二次给药。第二次给药的部位必须仔细选择,以保证能吞噬或胞饮调理抗原的细胞存在。活动感染的处理
本发明的疫苗还可用作治疗活动期HIV感染的方法。HIV复制量大、突变迅速,也就是说,当免疫系统能够针对给定类型的HIV颗粒产生有效应答时,该颗粒的足量新突变体已被产生,继续领先于免疫系统。如果在免疫系统被实质性破坏前或时间长到足以让免疫系统恢复,野生型病毒的复制能被压制,本发明的疫苗可用来提高免疫系统识别新病毒突变体的能力,因而为已被感染的个体提供一种控制病毒复制的方法。
能够有效地至少暂时抑制HIV复制的药物组合已经知道。发明人已经显示,药物组合包括羟基脲、一种或多种逆转录酶抑制剂以及可选用的一种或多种蛋白酶抑制剂特别有效。而且,对某些病人,可在延续期内停止药物治疗。见1998年4月8日提交的USSN 09/056,691“组合使用羟基脲、一种核酸类似物和蛋白酶抑制剂抑制HIV的方法”和1998年3月26日提交的USSN 09/048,753“使用羟基脲和逆转录酶抑制剂体外抑制HIV的方法”以及1998年3月26日提交的USSN09/048,756“体内使HIV群体不能复制的方法”,上述文献均在此全文引入作为参考。本发明包括用合适的药物组合对患有活动性HIV感染的病人进行处理,直到血液中携带的病毒达到合适的低水平,少于每毫升50000拷贝,优选少于每毫升10000拷贝,更优选少于每毫升200-500拷贝。然后在药物组合抑制野生型病毒复制时,使用本发明对病人进行免疫接种。
提供下面实施例的目的是说明本发明的应用。它们并不限制本发明或后面的权利要求。
实施例
1.在培养的DC中表达编码一种复制缺陷型HIV的质粒DNA
有几种DC来源,DC可从骨髓CD34+造血祖细胞中分离。骨髓单个核细胞用Ficoll-Hypaque梯度离心分离。这些细胞用结合了人CD34抗体的磁珠(Dynal Detachabeads)进行阳性选择,CD34+细胞可用针对CD34单克隆抗体的高亲和性多克隆抗体从磁珠上取代下来。当这些细胞与干细胞生长因子、GM-CSF和TNFα温育时,它们会分化成DC(Canque,B.,M.Rosenzwajg,et al.(1996)“人免疫缺陷病毒感染对树突细胞分化和功能的体外影响”B1ood 88(11):4215-28)。
单核细胞来源的DC在存在GM-CSF和IL-4时从外周血单个核细胞中产生(Bender,a.,M.Sapp,et al.(1996)“从人血中未分增殖的祖细胞中产生树突细胞的改进方法”J Immunol Methods 196(2):121-35)。在第4天,细胞用复合了编码HIV-1/Lwint-(USSN08/803,484中介绍的一种整合和复制缺陷型HIV)的质粒DNA的脂转染胺进行转染。脂转染胺是一种用作转染试剂的商业化阳离子型脂质体(可从Gibco BRL Life Technology,PM.Gathersburg,Md.,US获得)。48小时后,洗涤并分析细胞。DC的纯度用测量表面标记的荧光激活细胞分选仪(FACS)分析为90.6%。用FACS分析,DC细胞类型为CD3-、CD19-、CD56-、CD14-和HLA DR+。通过细胞中的HIV-1Gag和Env和Tat蛋白的表达也用FACS进行了测量。同位素对照Ig的非特异性结合的水平在对照转导和特异性质粒转导时是相同的。我们在三个独立的实验中发现,25-37%的HIV/Lwint-转导的DC表达Env、Gag和Tat蛋白。也就是说,在转导样品中25-37%的细胞表达HIV蛋白。转导的和对照细胞样品也用p24和B7-2抗体双染来证明,是DC而不是巨噬细胞正在表达抗原。这些结果令人惊奇的好,因为用另一种质粒DNA(CMV驱动的流感病毒的血凝素基因)使用相同的方法仅有5-8%的转染细胞表达蛋白,这些结果证明,缺陷型HIV可被转染的DC有效表达。
2.编码复制缺陷型病毒的DNA是比编码一种或多种蛋白更有效的抗
原
在一个独立的实验中,我们比较了两种不同HIV质粒---HIV-1/Lwint-和LTR-tat在DC中的表达。两种结构物都由相同的启动子---HIV-1-LTR驱动,并且两种结构物的表达均依赖Tat的反式激活。按实施例1的介绍进行转染,48小时后,Tat蛋白的表达用FACS进行分析。我们发现,32%的HIV-1/Lwint-质粒转染的DC表达Tat蛋白,与此相对,只有10%的LTR-tat转染的DC表达相同的Tat蛋白。这个结果令人惊奇,因为在这个比较实验中,我们期望两种不同结构物有相同的效率。复制缺陷型病毒无疑具有形成病毒颗粒的能力,病毒颗粒可从细胞中释放。因为抗原呈递依赖DC中的基因表达,这个实验清楚地证明,编码缺陷型病毒的DNA比编码一种或多种蛋白的DNA是更有效的抗原。
转导的培养DC体外激活幼稚T细胞
转导后,DC与自体T细胞按1∶10的比例一起温育。7天后,这些T细胞用作效应细胞来裂解(杀死)一种来自同一供体的单核细胞/巨噬细胞群体,该细胞群体已用一种通常用作标记的HIV蛋白---p55进行了刺激。这种CTL活性用Cr释放试验进行测量。如图5证明,转导的DC诱导的CTL能特异性地和有效地裂解靶细胞。因为体外CTL的产生比体内困难得多,这些实验显示,经过基因免疫的细胞能激活幼稚CTL,使它们能在体内有效地裂解感染细胞。而且,实验进一步证明,编码一种感染性病毒的DNA不仅能有效表达HIV基因,而且能产生有效的免疫应答。
用转导的DC进行体内基因免疫
为证明在猴中基因免疫的体内效果,树突细胞从40ml短尾猕猴的外周血中制备。细胞使用聚乙烯亚胺用LW/int-质粒按实施例5的介绍进行转染。洗涤转染的DC,并在转染36-48小时后注射给幼年的短尾猕猴。一部分转染的DC经皮下注射,另一部分经静脉注射。4周后,并且只进行一次免疫,一只猴已经能够显示CTL应答(图6),这说明体外结果可在动物的体内重复。与此相对,目前已被批准进行Ⅲ期人临床试验的HIV亚单位疫苗并不能显示产生任何的CTL应答,即使经过多次免疫努力后也不能。
5.聚乙烯亚胺介导的基因转移进培养的DC细胞
按实施例1的介绍用编码HIV-1/Lwint的质粒转导树突细胞,除了用聚乙烯亚胺(PEI由Dr.Behr惠赠)代替脂转染胺。按实施例1对细胞进行检测,用聚乙烯亚胺转导的树突细胞中超过60%的细胞能表达HIV-1蛋白,而与此相对,用脂转染胺转导的细胞只有25-37%。因为到目前为止,脂转染胺是将质粒DNA导入DC的最好基因转移方法,此实验证明,PEI是将基因转移进DC的最有效的非病毒传递系统。但是,PEI和脂转染胺都显示对树突细胞有毒性,这可用锥虫蓝染色进行测量。
6.通过甘露糖受体特异性地靶向DC
不成熟的DC按上面实施例1中的介绍制备,并用编码一种绿色荧光蛋白(GFP)的DNA转染。我们将此DNA用作基因表达的标记,因为表达绿色荧光蛋白的细胞在荧光激活后显绿色。因此,转染细胞可用荧光显微镜和流式细胞仪(FACS)进行观察。
选择不同糖修饰的PEI来靶向树突细胞表面的甘露糖受体,因为甘露糖受体能识别细菌、寄生虫、酵母和真菌表面的所有糖谱。将DNA与PEI和荷载不同糖的PEI(可从Dr.Jean-Paul Behr,Laboratoirede Chimie Genetique,Faculte de Pharmacie,CNRS-UMR 7514 routedu Rhin 67401 Illkirch,France订购)复合。将2微克DNA与溶于150 mM NaCl中的PEI不同衍生物(10∶1,N∶P比例)在室温下一起温育约5分钟。然后DC用复合物转导6小时,洗涤,48小时后分析绿色荧光蛋白细胞。我们发现,最有效的PEI-糖修饰物是PEI-甘露糖(表1)表1用于体外DC转导的不同复合物

7.体外PEI-甘露糖介导的基因转移进培养的树突细胞
荷载甘露糖的聚乙烯亚胺(PEI-man)是一种异硫氰酸根合苯基苯基甘露糖衍生物,与PEI 25 kDa偶联,从而产生的一种配体(或与甘露糖受体有较低亲和力的甘露糖残基,1 mM)。以前已经证明,通过脱唾液酸糖蛋白受体(被PEI使用)进入细胞需要复合物带电荷。也就是说,需要使用比DNA更多的PEI。当复合物被中和时,即PEI被DNA中和时,复合物不能通过脱唾液酸糖蛋白受体进入。(ZantaMA;Boussif 0;Adib A;Behr JP;Bioconjug Chem 1997 Nov-Dec;8(6):839-44)。这些研究者发展了一种肝细胞定向的复合物,它包括几种被认为有利于向肝脏转移基因的关键特征:1)静电中性颗粒,能防止与其他细胞的非特异性结合,和2)防止脱唾液酸糖蛋白受体介导的胞饮作用。该系统以荷载5%半乳糖的聚乙烯亚胺(PEI-gal)聚合物为基础,该聚合物与质粒DNA一起浓缩至中性。
我们发现,使用PEI-man-DNA复合物,与使用PEI比较,需要较少DNA来中和PEI-man。使用不同N∶P比例的PEI-DNA复合物进行的凝胶电泳实验证明,5∶1(N∶P)的PEI-man:DNA复合物为中性电荷,不同于3∶1(N∶P)PEI-DNA复合物。(用DNA中和PEI依赖于N(氮)∶P(磷)比例;1微克DNA=3×109摩尔P,而1 mM PEI=109摩尔N/微升。这意味着以10∶1的比例为例是3微升10 mM的PEI和1微克DNA的混合物。
人DC按上文的介绍分离。用FACS分析,DC的纯度超过99%。通过用锥虫蓝染色测量细胞的存活率,我们发现,PEI-man比PEI的毒性要小得多。我们还发现,PEI和PEI-man都能将DNA导入DC,但PEI-甘露糖更有效。在5∶1(N∶P)的比例时,PEI-man-DNA复合物是中性的,因此复合物只能通过甘露糖受体进入DC。在这种条件下,30%的DC表达绿色荧光蛋白。与此相对,5∶1(N∶P)比例的PEI-DNA复合物带有电荷,复合物通过脱唾液酸糖蛋白受体进入。在这种条件下,14%的DC表达绿色荧光蛋白(图4)。
8.体内基因转移进皮肤朗氏细胞
在皮肤中,能胞饮甘露糖酸化配体的惟一细胞是朗氏细胞。因此,主要的问题是PEI-(man)-复合物是否能穿透皮肤,并体内转染朗氏细胞。PEI-(man)复合物用编码绿色荧光蛋白(GFP)的质粒DNA复合。按图3的介绍用不同的基因传递复合物进行实验。复合物含有50微克DNA和8.25微升溶于5-10%(最好为8%)葡萄糖溶液的100 mM PEI-man。将BALB/c小鼠麻醉,剪去小鼠背部的毛。按表2的指示将0.1 ml复合物涂于皮肤上1小时,或皮下施用。免疫6小时后处死小鼠,将皮肤样品置于补加了10%胎牛血清和抗生素的DMEM培养基中。在这种条件下,细胞包括朗氏细胞从皮肤上迁出。1天后,收集迁出的细胞,用流式细胞仪(FACS)进行分析,因为这种分析能识别表达绿色荧光蛋白的细胞。在我们的分析中,只分析大的和致密的细胞群体,因为已知树突细胞和朗氏细胞都是大的致密的细胞。表2皮肤朗氏细胞的体内转导

这些实验证明,1)经皮基因转移比皮下转移产生更有效的基因转移进朗氏细胞(比较实验2&4或3&5)。这很重要,因为目前使用的最有效的免疫接种技术之一使用皮下注射。2)通过甘露糖受体进入比脱唾液酸糖蛋白受体进入对体内转染朗氏细胞来说更有效(比较实验4&6)。这些体内实验证明了我们的体外实验(见图4)。因此,优选使用糖修饰的基因传递系统来转导抗原呈递细胞。
9.转导的朗氏细胞迁移至淋巴结
BALB/c小鼠按实施例8的介绍准备,将0.1 ml的PEI-man-DNA复合物样品施用于皮肤1小时,2天后,将动物处死,取出淋巴结(LN)。检查附属LN,因为它们是背部的流经LN,迁移的朗氏细胞可能在那里发现。将LN冷冻、切片并在荧光显微镜下检查。将实验小鼠的LN与一只对照小鼠的LN比较。我们能在来自实验LN的样品中检测到15个绿色荧光细胞,而在对照LN中没有检测到一个。这些结果证明,复合物进入位于皮肤的细胞,细胞能迁移到LN,并表达绿色荧光蛋白。这些绿色细胞的形态类似DC的形态:它们是大的细胞,而且绿色荧光的位置呈现“波动”的图谱,这是DC的特征。(其他细胞如293细胞在胞质中显示扩散的绿色图谱)。此外,皮肤中惟一能够摄取抗原并迁移至LN的细胞型是朗氏细胞。这些细胞是皮肤中仅有的带有甘露糖受体来摄取复合物并且在激活后已知能迁移到流经LN的细胞。
这些实验显示,PEI-man-DNA复合物能穿透皮肤,并将DNA转移进朗氏细胞。朗氏细胞被激活并迁移进流经的LN,并在LN中表达由DNA结构物编码的基因。已知培养的DC重新注射进体内后能迁移进LN并产生有效的免疫应答。本发明证明体外分离DC对基因转移进朗氏细胞或在淋巴器官中表达基因来说并不需要。我们还证明,在DC中表达一种复制缺陷型病毒能在体内和体外有效诱导CTL应答(见上文)。因此,我们已显示,使用复合物(如PEI-man-DNA)的经皮转移系统能用于产生针对DNA编码的蛋白的免疫应答。
10.糖-DNA复合物转导皮肤朗氏细胞
表2描述的实验结果提供以下证据,一种糖-DNA复合物在没有PEI-man的情况下,能够体内转导朗氏细胞。没有PEI的糖与DNA的复合物比DNA与PEI的复合物在皮下和经皮方法中使用时更为有效(见表2,实验3&5)。这是一个令人惊奇的结果。这显示糖(如这些实验中的8%的葡萄糖)也能与DNA复合并通过甘露糖受体将DNA传递进朗氏细胞。重要的是,最有效的体内转移进朗氏细胞的基因传递系统是经皮途径中使用的糖-DNA复合物。
我们期望用糖-DNA复合物进行的免疫接种也能使朗氏细胞迁移至流经淋巴结。原因是使用了相同的进入朗氏细胞的机制:甘露糖受体介导的摄取。在目前使用的免疫接种技术中使用糖作为佐剂的优点是,高百分比的朗氏细胞将参与免疫应答的产生。这可期望来明显提高现有免疫接种策略的效率。例如,将疫苗与糖混合来进行DNA和蛋白抗原的皮下、皮内和肌肉内注射。
11.经皮免疫的意义
本技术对免疫接种技术来说是一次革命,因为不需要使用针。所介绍的经皮免疫是一种非常简单的技术,它可用来在发达和不发达国家进行便宜的免疫接种。
方法学上的简单以及抗原呈递细胞能有效转导这一事实,使我们能使用任何能产生一种免疫原蛋白的DNA序列。因此,广谱的疾病可成为免疫的目标,如感染性疾病和癌症。
消除感染性疾病如HIV感染、肝炎、疟疾的惟一途径是简单便宜的免疫接种。
不需要针的疫苗对幼儿的父母来说将特别受欢迎。
本发明的应用将允许发展更安全的疫苗。经典疫苗的少数副反应在某种程度上是由于对疫苗生产过程中的副产品产生过敏反应。当这样的多余物质(例如卵清蛋白)被消除时,副反应可随之降低。
它可以与或不与化学疗法组合来免疫接种以治疗疾病。
Claims (22)
1.一种包含外源遗传物质和一种非病毒载体的基因传递复合物,其中载体具有对外源遗传物质有特殊亲和性的第一个位点,以及对一种抗原呈递细胞上的受体有特殊亲和性的第二个位点。
2.权利要求1的基因传递复合物,其中外源遗传物质选自RNA和DNA。
3.权利要求1的基因传递复合物,其中外源遗传物质编码一种逆转录酶依赖的病毒或一种突变的逆转录酶依赖病毒。
4.权利要求3的基因传递复合物,其中外源遗传物质是编码一种复制缺陷型人免疫缺陷病毒的至少一个主要部分的DNA。
5.权利要求3或4的基因传递复合物,其中外源遗传物质是编码一种整合缺陷型人免疫缺陷病毒的至少一个主要部分的DNA。
6.权利要求5的基因传递复合物,其中外源遗传物质是编码一种人免疫缺陷病毒的双养型原代分离株的整合酶阴性突变体的至少一个主要部分的DNA。
7.权利要求6的基因传递复合物,其中DNA进一步在整合酶基因的阅读框中含有一个或多个终止密码子。
8.权利要求1的基因传递复合物,其中复合物选自糖、聚乙烯亚胺、聚乙烯亚胺衍生物的DNA结合物或它们的混合物。
9.权利要求8的基因传递复合物,其中复合物是糖修饰的聚乙烯亚胺的DNA结合物。
10.权利要求8的基因传递复合物,其中复合物是葡萄糖的DNA结合物。
11.权利要求8的基因传递复合物,其中复合物对甘露糖受体有特殊亲和性。
12.权利要求1的基因传递复合物,其中抗原呈递细胞是朗氏细胞。
13.权利要求1的基因传递复合物,其中抗原呈递细胞是树突细胞。
14.权利要求1的基因传递复合物,其中受体是甘露糖受体。
15.用外源遗传物质转导细胞的方法,包括使细胞与权利要求1-10任一项的基因传递复合物作用的步骤。
16.制备一种基因传递复合物的方法,包含以下步骤:(a)选择一种具有一种受体位点的靶细胞,(b)选择权利要求1-10任一项中的一种基因传递复合物,(c)在适合于受体介导的胞饮作用的条件下使细胞与基因传递复合物作用。
17.通过使动物与权利要求1-10任一项的基因传递复合物作用来对动物进行预防性和治疗性免疫接种的方法。
18.对动物进行针对逆转录酶依赖型病毒感染的预防性和治疗性免疫接种的方法,包括以下步骤:从动物中分离树突细胞,使细胞与权利要求17的基因传递复合物作用,并将细胞回输至动物的机体。
19.对动物进行针对逆转录酶依赖型病毒感染的预防性和治疗性免疫接种的方法,包括以下步骤:使动物与权利要求17的基因传递复合物在皮肤或粘膜表面作用。
20.治疗活动性病毒感染的方法,包括以下步骤:在需要时用一种适合于抑制病毒负荷的药物组合处理病人,用权利要求1-10任一项的基因传递复合物对病人进行免疫接种,并继续用一种适合于抑制病毒负荷的药物组合进行处理,直到免疫应答建立,并监测病人的病毒负荷的反弹,在病毒负荷反弹时,重新启动药物组合或用另一种适合于抑制病毒负荷的药物组合处理,并用权利要求1-10任一项的基因传递复合物对病人进行免疫接种。
21.权利要求20的方法,其中活性病毒感染是一种HIV感染。
22.经皮基因免疫的方法,包括以下步骤:用选自糖、聚乙烯亚胺、聚乙烯亚胺衍生物及其混合物的一种非病毒载体与外源遗传物质复合,并将复合物施用于动物的皮肤或粘膜表面。
