技术领域
本发明涉及称为支架的管腔内用支架器件。具体地说,本发明涉及一种支架,该支架具有可使之易于弯皱到一传送器件上的几何形状的螺旋状元件。
背景技术
支架是一些用来植入躯体内部的管道内腔中用以支撑管道壁的假内支架。支架的结构支承作用在血管成形的操作中特别重要。通常将支架植入血管系统中用以增强那些局部阻塞、破裂、削弱变或非正常扩张的血管。更一般地说,支架可用在例如包括动脉、静脉、胆管、尿道、消化道、气管支气管树、脑导水管或者生殖泌尿系统之类的任何生理管道中。支架可用于人体和动物躯体。
通常有两种类型的支架:自扩张支架和气囊式可扩张支架。自扩张支架一旦释放即可自动扩张而呈伸展扩张状态。气囊式可扩张支架采用一可充气的气囊导管来扩张。气囊充气以使支架塑性变形。气囊式可扩张支架可通过将处于非扩张或称弯皱状态的支架安装在一导管的气囊分段上而被植入。在将弯皱的支架置于其导管后再将导管插过血管壁上的穿刺口而在血管中移动穿过,直到使它定位于需要修整的血管部分位置为止。接着通过对气囊导管充气使支架扩张并安置血管的内壁。更明确地说,支架通过气囊充气而塑性变形,以致其直径增大且保持在一增大的状态下。在某些场合,植入了支架的血管可以在支架扩张时由支架本身来使之扩张。
在Patrrck W.Serreys等人所著的“冠状支架手册”(1998年Martin Duniz有限公司出版)中揭示Palmaz-Schatztm支架是气囊式可扩张支架的一个例子,它已被植入到成千上万的病人体内。Palmaz-Schatztm支架,象其他已知的支架一样,都具有一定的局限性。这些局限性包括(但并不只限于):(i)低的支架与血管之比的均匀性,(ii)在弯皱和展开状态下伸缩管较大的刚性,以及(iii)有限的柔性而使之难以在狭小的血管中传送和放置。支架与血管之比通常涉及到血管壁在其扩张状态下的支架的支承程度,且最好应在支架的整个长度上均匀一致。而且,由于Palmaz-Schatztm支架包含有将许多连续的开槽管子连接起来的一个或多个搭接桥,因而,在伸缩管扩张后,在血管中会有许多个裸区。这些不足区域对于许多支架来说都是常见的。参见同一手册的第36点处。
发明内容
本发明涉及一种可扩张的支架(stent),它具有可使之易于弯皱到一气囊传送器件上的几何形状。在一实施例中,该支架可包括多个相对于支架的纵轴线成第一螺旋角的多个第一螺旋状分段以及成一第二螺旋角的多个第二螺旋状分段。螺旋状分段能够在周向地扩张或收缩,也就是说,它们可沿支架的圆周扩张或收缩。在该实施例中,当支架弯皱时,一第一螺旋状分段的至少一个部分与一第二个第一螺旋状分段的至少一个部分叠嵌在二个分离的第二螺旋状分段的两个相同的部分之间。
在本发明的一实施例中,支架包括多个第一可扩张元件和多个第二可扩张元件。第一可扩张元件可具有一叠嵌在该第一可扩张元件的另一分段中的分段。在某些实施例中,第一可扩张元件由撑条连接在一起形成第一螺旋状分段,而第二可扩张元件由撑条连接在一起形成第二螺旋状分段。第一和第二螺旋状分段可具有不同的螺旋角或不同的间距。在某些实施例中,第一和第二螺旋状分段共有公共的撑条。
在本发明的某些实施例中,支架可由多个单元所组成。每个单元可由交互连接在一起的第一和第二元件(即每一第一元件和两个第二元件向连接、每一第二元件和两个第一元件相连接而形成一封闭折线)所组成。封闭折线可以是不定形的或者也可具有给定的形状。当支架弯皱时,各构成单元的第一元件的一个部分叠嵌在该单元的第二元件的诸部分之间。在某些实施例中,当支架弯皱时,第一和第二元件可以相互接触。多个撑条连接诸单元形成一伸缩体。此外,第一元件的诸部分可叠嵌在该第一元件的其他部分之中,而第二元件的一个部分也可叠嵌在该第一元件的一个部分之中。

具体实施方式
本发明涉及一种可扩张的支架,它具有很适合于将其弯皱到一传送器件之上的几何形状。在本发明的某些场合(但并非是所有实施例),支架可具有为其弯皱直径的3-6倍的扩张直径。此外,在某些实施例(但也并非所有实施例)中,支架与血管之比可大于15%。
在本发明的一实施例中,如图1和图2所示,支架含有具有一纵轴线1000的主体部分100。图1所示的支架安装在一支承件616上。主体包括多个第一螺旋状分段120a和120b以及多个第二螺旋状分段150a和150b。第一螺旋状分段相对于支架的纵轴线1000形成螺旋角α,结果在第一螺旋状分段形成一第一间距。第二螺旋状分段150a和150b相对于纵轴线1000形成一螺旋角θ,结果在第二螺旋状分段中形成一第二间距。在某些实施例中α角可在20°-50°之间变化,而θ角可在20°-90°之间变化。第一螺旋状分段120a和120b以及第二螺旋状分段150a和150b沿周向可扩张,即它们可沿着与支架的圆周200相平行的方向扩张。当支架弯皱时,螺旋状分段120a、120b、150a和150b就沿周向收缩。
如以下进一步阐述的那样,在某些实施例中,第一螺旋状分段120a和120b可含有多个细丝段,同样地,第二螺旋状分段150a和150b也可含有多个细丝段。在某些实施例中,构成第一螺旋状分段的所有细丝段的合成总长度可以比构成第二螺旋状分段的细丝段总长度要长。在某些例子中,第一和第二螺旋状分段可共有公共的细丝段。
如图2所示,第一螺旋状分段120a和120b系由多个第一可扩张元件300所组成,而第二螺旋状分段一系由多个第二可扩张元件350组成。二个或多个第一可扩张元件300由多个撑条400来连接在一起,以形成各第一螺旋状分段120a和120b。这些撑条400也连接两个第二可扩张分段350,以形成个第二螺旋状分段150a和150b。撑条400可为第一或第二可扩张元件的整体成形的一部分,或者同时为第一和第二可扩张元件的整体成形的一部分。
如图3所示,在某些实施例中,第一可扩张元件300由多个邻接的细丝段700a、700b、710a、710b和720所组成。在一个实施例中,细丝段700a,710a和720连接在一起,以形成一大体为R形的结构730(参见图4)。形成R的头部的细丝,即细丝710a或710b,可为弯曲的并具有一半径r。半径r可为多种数值,包括(但并不只限于)大约0.015英寸。如图3、4a和4b所示,第一可扩张元件300可由多个R形结构730a和730b所组成,这些R形结构的方向相反并共有公共的细丝段720。
在某些实施例中,如图5所示,第二可扩张元件可由多个邻接的细丝段770a、770b、775a、780和775b所组成,并且,例如在某些实施例中可形成Z形结构。例如,如图所示,细丝770a可相对于细丝780置成β角,而分段770a和770b可由弯曲分段775a和775b连接于单个分段780。在某些(但并非全部)实施例中,770a和770b具有相同的尺寸而780可短些。β角也可有很大变动,例如,在一实施例中,可在30°-40°之间的范围内。
如图1所示,本发明的某些实施例可具有跨于主体100上的端区600和610。端区可有方形外边605和615。端区可用多个第二撑条450附接于主体100(参见图2)。第二撑条可有与其他撑条400不同的方向。例如,第二撑条450可与支架的圆筒体轴线1000相平行,而撑条400的方向可与该轴线成一角度。
本发明的支架具有一可改善其弯皱性的几何形状。例如,本发明的一实施例可具有一小于2.0毫米的弯皱直径及6-12毫米或以上的扩张直径。支架可弯皱在一直径为1.50毫米的PTA气囊上,它可用一直径大约为0.03-0.50英寸的管子来制成。当然也可采用其他尺寸的管子。支架可制成为在大范围内变化的尺寸以用于广泛变化的应用场合。
在本发明的一实施例中,当支架弯皱时,第一可扩张元件300的第一部分叠嵌在该第一可扩张元件的另一部分中。例如,如图6所示,细丝部分710a和720叠嵌在细丝700b的凹部中。同样,细丝部分710b和720叠嵌在细丝700a的凹部中。
在本发明的某些实施例中,当支架弯皱时,第二可扩张元件350的一个部分350叠嵌在第一可扩张元件的一个部分300中。例如,如图7所示,第二可扩张元件的一个部分350叠嵌在元件300的一个部分中。更明确地说,在仅用来说明而未穷尽本发明的此实施例中,细丝770b和775b的一个部分叠嵌在细丝710a和700a的凹部中。此例说明了本发明的某些叠嵌特征,但并不是全部的。
在本发明的某些实施例中,当支架弯皱时,从两个分离的第一可扩张元件300的诸部分可安置在两个分离的第二可扩张元件350的这些部分之间。如图10所示,第一可扩张元件的部分,即细丝710a和第二个第一可扩张元件710b,都具有R形结构730a和730b的头部(参见图4a和4b),它们安置在细丝775a和775b之间,细丝775a和775b是分离的第二可扩张元件350的各个部分。图10说明了本发明的几何形状的某些安置特性,但并不是全部的。
如图10、3、4a、3b和3所示,在某些实施例中,不仅细丝775a是一第二可扩张元件350的部分,第二可扩张元件350接着又是一第二螺旋状分段150a的一部分,而且细丝710a也是一第一可扩张元件300的部分,第一可扩张元件300接着又是一第一螺旋状分段120a的部分。同样,细丝775b是不同的一个第二可扩张元件350的部分,第二可扩张元件350接着又是一第二个第二螺旋状分段150b的部分,并且,细丝710b是第二个第一可扩张元件300的部分,第一可扩张元件300接着又是另一个螺旋状分段120b的部分。这样,在本发明的一实施例中,一第一螺旋状分段的部分和另一个第一螺旋状分段的部分,当支架弯皱时,安置在两个分离的第二螺旋状分段之间。
如图8a所示,本发明的支架在某些实施例中可含有多个单元500。在某些实施例中,这些单元500可由撑条400连接在一起,每个单元500可由第一元件300和第二元件350所组成。在一个实施例中,如图8b所示,每个第一元件300与两个第二元件350相连,且每个第二元件350与两个第一元件300相连。这就构成一封闭折线,该折线可有多种形状,或也可为不定形的。如图2和8a所示,单元可连在一起,而使所形成的支架具有多个螺旋状分段,其中至少一个螺旋状分段相互贯穿。(例如参见图2)。
单元的几何形状可做成使每个单元以一相对恒定的速率扩张。例如,在图8a所示的实施例中,每个单元系由多个第一可扩张元件300和多个第二可扩张元件350所组成。每个第一元件300接下来又由多个R形元件730a和730b所组成。在该图示的实施例中的第二可扩张元件350大体为Z形。在扩张过程中,R形元件730a和730b以一个比Z形元件慢的初始速率扩张。通过沿周向交错或交替排列第一元件300和第二元件350,支架就可沿周向以均匀的方式扩张,因为每个单元沿周向均匀扩张,而不经受元件350比元件300更快的扩张。
如图9所示,当本发明的支架弯皱时,每个单元沿周向收缩。在该仅在这里用于说明而不穷尽本发明的实施例中,当支架弯皱时,第一可扩张元件的一个部分(即至少细丝710a和720的部分)叠嵌在该第一可扩张元件的另一部分(例如至少细丝700b的部分)中,而两个分离的第一可扩张元件300的部分(例如细丝710a和710b)安置在两个分离的第二可扩张元件350之间。当支架扩张后,每个单元沿200线均匀扩张,该线是支架的圆周尺寸。(参见图1)。第二可扩张元件350以比第一可扩张元件300为快的速度打开,但因第一可扩张元件如同第二可扩张元件那样,沿对角线方向,单元500的右边部分2000以与单元500的左边部分3000同样的速率扩张。(参见图9)。
上述的实施例和说明是用来说明本发明的广泛范围内的多种多样的特点,而不是用来限制本发明的实质内容和范围的。本发明可以除了以上述具体描述的例子以外的多种形式来实施。例如,并不限制,第一元件300和第二元件500也可采取与上述所示不同的多种形式和形状。这可使第一螺旋状分段具有比第二螺旋状分段更长或更短些的细丝总长度。另外,本发明的支架也可以用已有技术中已知的技术的材料来制成,诸如可用激光来切割由适宜的支架材料所制成的管子来制造。因此,尽管以上的实施例已就该支架不同元件和分段作了说明,但熟悉本发明领域的技术人员都懂得:本发明的支架可以用材料的单个的连续件来构成,或者也可以用众所周知的技术连接在一起的多个不同材料的细丝或分段件来组成。
Claims (25)
1.一种可弯皱在一传送器件上的、可扩张的支架,它包括具有一圆周的主体,该主体包括:
多个第一螺旋状分段,它们具有当该支架扩张时增大而当该支架弯皱时缩小的周向尺寸,以及
多个第二螺旋状分段,它们具有当该支架扩张时增大而当该支架弯皱时缩小的周向尺寸;
其中,当支架弯皱时,一第一螺旋状分段的一个部分和另一第一螺旋状分段的一个部分叠嵌在一第二螺旋状分段的一个部分和另一第二螺旋状分段的一个部分之间;以及
其中,当支架弯皱时,一第二螺旋状分段的诸部分叠嵌在一第一螺旋状分段的诸部分中。
2.如权利要求1所述的支架,其特征在于,它还包括一第一端区和一第二端区,该二个端区跨在主体上,其中,当支架受到径向扩张力时,两端区和第一、第二螺旋状分段以不同的速率扩张。
3.如权利要求2所述的支架,其特征在于,端区具有方形外边。
4.如权利要求2所述的支架,其特征在于,第一螺旋状分段形成一第一螺旋角,而第二螺旋状分段形成一不同的第二螺旋角。
5.如权利要求4所述的支架,其特征在于,第一螺旋状分段包括多个第一可扩张元件,而第二螺旋状分段包括多个第二可扩张元件,并且,第二可扩张元件以较第一可扩张元件更快的速率扩张。
6.如权利要求5所述的支架,其特征在于,当支架受到径向扩张力时,主体以一均匀的速率扩张。
7.如权利要求6所述的支架,其特征在于,支架具有小于2.0毫米的弯皱直径和大于12毫米的扩张直径。
8.如权利要求6所述的支架,其特征在于,第一可扩张元件包括多个基本上呈R形的结构。
9.如权利要求8所述的支架,其特征在于,诸R形元件共有一公共的细丝元件,且它们相互方向相反。
10.如权利要求9所述的支架,其特征在于,第二可扩张元件包括被弯曲段连接在一起的多个直线段。
11.如权利要求10所述的支架,其特征在于,第二可扩张元件基本上呈Z形。
12.一种气囊式可扩张的圆筒状支架,它具有一可使支架弯皱在一传送器件上的几何形状,该支架包括:
多个第一可扩张元件,它们沿周向可扩张,其周向与支架的圆周平行;
多个第二可扩张元件,它们也沿周向可扩张;
多个第一撑条,用于将第一可扩张元件连接在一起以构成多个第一螺旋状分段,并用于将第二可扩张元件连接在一起以构成相反的第二螺旋状分段,每个第一撑条连接两个第一可扩张元件,并连接两个第二可扩张元件;
其中,当支架弯皱时,一第一可扩张元件的一个部分叠嵌在另一个同样的第一可扩张元件的一个部分中。
13.如权利要求12所述的支架,其特征在于,当支架弯皱时,两个第一可扩张元件的诸部分叠嵌在一第二可扩张元件的一个部分和另一第二可扩张元件的一个部分之间。
14.如权利要求13所述的支架,其特征在于,第一可扩张元件的定向成一第一螺旋角,而第二可扩张元件定向成一不同的第二螺旋角。
15.如权利要求13所述的支架,其特征在于,第一可扩张分段具有与第二可扩张分段的总细丝长度不同的总细丝长度。
16.如权利要求13所述的支架,其特征在于,它还包括方形外端。
17.一种气囊式可扩张的支架,它具有可弯皱到一传送器件上的几何形状,该支架包括:
形成多个沿周向可扩张的螺旋状分段的多根邻接的细丝,其中,当支架弯皱时,至少一螺旋状分段具有一叠嵌在该螺旋状分段的另一部分中的一第一部分、以及一叠嵌在另两个螺旋状分段之间的一第二部分。
18.如权利要求17所述的支架,其特征在于,支架具有一弯皱直径以及一为弯皱直径3至6倍的扩张直径。
19.如权利要求18所述的支架,其特征在于,支架在其扩张状态下具有一大于15%的支架与血管之比。
20.一种支架,它包括:
多个单元,每个单元包括:
多个第一元件;
多个第二元件,其中,每个第一元件与两个第二元件相连接,且每个第二元件与两个第一元件相连接,从而形成一封闭折线,其中,当支架弯皱时,至少一第一元件的一部分叠嵌在两个第二元件的诸部分之间;以及
多个将一单元和另一单元相连接起来的撑条。
21.如权利要求20所述的支架,其特征在于,第二元件以比第一元件快的速率扩张,并且其中,诸单元周向均匀地扩张。
22.如权利要求21所述的支架,其特征在于,第一元件至少具有一大体呈R形的部分。
23.如权利要求21所述的支架,其特征在于,第二元件大体呈Z形。
24.如权利要求22所述的支架,其特征在于,诸单元通过撑条连接在一起,用以构成:
多个包括多个第一元件的第一螺旋状分段;以及
多个包括多个第二元件的第二螺旋状分段。
25.如权利要求24所述的支架,其特征在于,撑条是第一和第二元件的整体成形的部分。
